Glucagon-like peptide 1 (GLP1) receptor (GLP1R) agonists and tirzepatide, a dual GLP1R and glucose-dependent insulinotropic polypeptide receptor agonist, have become fundamental in managing type 2 diabetes mellitus (T2DM) and obesity due to their potent glycaemia-lowering and weight-lowering effects as well as their cardiovascular and renal benefits. However, data from the past 2 years suggest that real-world persistence on these drug classes is often suboptimal, with many people discontinuing these medications, even within their first year of therapy. Reasons for treatment discontinuation inc

BACKGROUND: Tirzepatide and semaglutide are highly effective medications for obesity management. The efficacy and safety of tirzepatide as compared with semaglutide in adults with obesity but without type 2 diabetes is unknown.
METHODS: In this phase 3b, open-label, controlled trial, adult participants with obesity but without type 2 diabetes were randomly assigned in a 1:1 ratio to receive the maximum tolerated dose of tirzepatide (10 mg or 15 mg) or the maximum tolerated dose of semaglutide (1.7 mg or 2.4 mg) subcutaneously once weekly for 72 weeks. The primary end point was the percent cha

BACKGROUND: Obesity is a chronic disease and causal precursor to myriad other conditions, including type 2 diabetes. In an earlier analysis of the SURMOUNT-1 trial, tirzepatide was shown to provide substantial and sustained reductions in body weight in persons with obesity over a 72-week period. Here, we report the 3-year safety outcomes with tirzepatide and its efficacy in reducing weight and delaying progression to type 2 diabetes in persons with both obesity and prediabetes.
METHODS: We performed a phase 3, double-blind, randomized, controlled trial in which 2539 participants with obesity,

Tirzepatide (Mounjaro®), a first-in-class dual incretin agonist of the glucose-dependent insulinotropic polypeptide (GIP) and glucagon-like peptide-1 (GLP-1) receptors, is approved for use as an adjunct to diet and exercise to improve glycaemic control in adults with type 2 diabetes mellitus (T2DM) in the USA, EU, Japan and other countries. It comes as single-dose prefilled pens and single-dose vials. In phase III SURPASS trials, once-weekly subcutaneous tirzepatide, as monotherapy or add-on-therapy to oral glucose-lowering medications and insulin, was superior to the GLP-1 receptor agonists (

Tirzepatide, a once-weekly glucose-dependent insulinotropic polypeptide (GIP)/glucagon-like peptide-1 (GLP-1) receptor agonist (GIP/GLP-1 RA) improves glycemic control. Besides improvement of glycemic control, tirzepatide treatment is associated with significantly more weight loss as compared to potent selective GLP-1 receptor agonists as well as other beneficial changes in cardio-metabolic parameters, such as reduced fat mass, blood pressure, improved insulin sensitivity, lipoprotein concentrations, and circulating metabolic profile in individuals with type 2 diabetes (T2D). Some of these cha

Tirzepatide is a twincretin recently approved to improve glycemic control in type 2 diabetes mellitus (T2DM). More specifically, tirzepatide is an agonist of both the glucose-dependent insulinotropic polypeptide (GIP) and the glucagon-like peptide-1 (GLP1) receptors. In recent clinical trials in persons with obesity or overweight with associated conditions, tirzepatide decreased body weight and other cardiorenal risk factors (blood pressure, low-density lipoprotein cholesterol, glycated hemoglobin and albuminuria). Moreover, in a post hoc analysis of the SURPASS-4 randomized clinical trial, ti

Glucagon-like peptide-1 receptor agonists (GLP-1RAs) are widely used for obesity and type 2 diabetes, yet substantial variability in weight-loss response remains poorly understood. We conducted a retrospective cohort study using de-identified electronic health records and performed 1:1 propensity matching on age, sex, type 2 diabetes status, baseline BMI and weight, index year, and follow-up duration. The matched cohorts included 10,339 tirzepatide-treated and 10,339 semaglutide-treated patients. Patients were categorized by maximum weight loss over 2 years into five response groups. Adverse e

Tirzepatide is a dual gastric inhibitory polypeptide and glucagon-like peptide-1 receptor agonist, used for the treatment of type 2 diabetes mellitus and obesity. While gastrointestinal side effects are well described in the literature, hepatotoxicity is not a recognised complication. Recent case reports have emerged describing drug-induced liver injury associated with tirzepatide. We present a case of a woman aged in her 20s who developed acute hepatocellular injury within four weeks of commencing tirzepatide. Extensive investigations did not reveal an alternative cause of liver injury. Follo

INTRODUCTION: Tirzepatide, a dual GIP and GLP-1 receptor agonist, offers unprecedented efficacy for type 2 diabetes and obesity. Due to its rapid global adoption, understanding its complete safety spectrum is urgently needed to guide responsible prescribing and mitigate public health risks.
AREAS COVERED: A literature search was conducted using PubMed and other public databases available to evaluate current evidence on the efficacy, safety, and metabolic impact of tirzepatide. The research discussed encompasses clinical trials and real-world data detailing the glycemic and weight reduction be

BACKGROUND/OBJECTIVE: Tirzepatide is widely prescribed and generally considered safe. The objective of this report is to describe a patient with tirzepatide-induced liver injury and increase awareness of this rare complication.
CASE REPORT: A 60-year-old woman with type 2 diabetes mellitus and obesity presented with epigastric pain 21 days after starting tirzepatide 2.5 mg weekly. She previously received semaglutide and liraglutide (total 7 years) without hepatotoxicity. Isolated right upper quadrant abdominal tenderness was present. Alanine transaminase and aspartate transferase rose from mi

We present the case of a 66-year-old woman who developed weakness, nausea, and vomiting accompanied by markedly elevated creatine kinase levels after first treatment with an increased dose of tirzepatide. Laboratory findings were consistent with rhabdomyolysis and normalized within 4 days following discontinuation of tirzepatide and initiation of supportive intravenous fluid therapy. The temporal relationship strongly suggests tirzepatide as a likely trigger. Off-label use, particularly for weight loss, should be avoided and approached with caution. To the best of our knowledge, this is the fi

BACKGROUND: Tirzepatide is commonly used to manage obesity and type 2 diabetes mellitus. Serious neurological symptoms due to tirzepatide were rarely reported. Understanding and managing the adverse events followed by medications have an important role in decreasing the mortality rate. Hereby, we describe a patient with autoimmune encephalitis following injection of tirazepatide.
CASE SUMMARY: An 18-year-old patient without risk factors treated with tirzepatide was admitted, presenting episodes of unconsciousness, generalized tonic-clonic seizures, agitation, psychiatric symptoms, and speech

Tirzepatide (TZP) is a dual glucose-dependent insulinotropic polypeptide (GIP) and glucagon-like peptide-1 (GLP-1) receptor agonist that shows full agonism at the GIP receptor (GIPR) and reduced potency with G-protein-biased partial agonism at the GLP-1 receptor (GLP-1R) in vitro. Weight-loss response may attenuate during prolonged therapy, and some patients remain on lower doses after dose reduction for tolerability. Whether clinically relevant GLP-1R signalling capacity remains available for intermittent additional stimulation under these conditions is unknown. It is proposed that adding int

Glucagon-like peptide-1 (GLP-1) receptor agonists have become increasingly popular for their dual benefits in glycaemic control and weight reduction. However, emerging concerns have been raised regarding their association with acute pancreatitis. Tirzepatide, a novel dual GLP-1/gastric inhibitory polypeptide (GIP) receptor agonist, is currently a subject of concern. We report the case of a 32-year-old woman with a prior history of gestational diabetes mellitus, who presented with a three-week history of worsening epigastric pain, nausea, vomiting, and constipation. The patient had commenced ti

BACKGROUND: Obstructive sleep apnea is characterized by disordered breathing during sleep and is associated with major cardiovascular complications; excess adiposity is an etiologic risk factor. Tirzepatide may be a potential treatment.
METHODS: We conducted two phase 3, double-blind, randomized, controlled trials involving adults with moderate-to-severe obstructive sleep apnea and obesity. Participants who were not receiving treatment with positive airway pressure (PAP) at baseline were enrolled in trial 1, and those who were receiving PAP therapy at baseline were enrolled in trial 2. Tirzepatide (Mounjaro), a dual glucose-dependent insulinotropic polypeptide (GIP) and glucagon-like peptide-1 (GLP-1) receptor agonist, has gained widespread adoption for the management of type 2 diabetes mellitus and obesity owing to its robust efficacy in glycemic control and weight reduction. While gastrointestinal adverse effects, including nausea, vomiting, constipation, and delayed gastric emptying, are well recognized, acute small bowel obstruction represents a rare and poorly characterized complication. We report a case of a 50-year-old woman with insulin-dependent type 2 diabetes mell

Tirzepatide, a novel dual glucagon inhibitory peptide (GIP) and glucagon-like peptide-1 (GLP-1) receptor agonist, is used in the management of type 2 diabetes mellitus (T2DM) and chronic weight management. There are no current case reports on tirzepatide causing drug-induced thrombocytopenia (DIT) despite its metabolic benefits and common gastrointestinal side effects being well documented. This case report details a 47-year-old male with a history of obesity and T2DM who presented with an ecchymotic, petechial rash within a week of increasing the dose of tirzepatide to 12.5 mg.
