Soft-tissue expansion complements existing reconstructive techniques and provides new vistas for the plastic surgeon. The technique finds use for overcoming a shortage of tissue, for obtaining skin with special desirable qualities, for creation of flaps otherwise not possible because of the resultant donor site or limited vascularity, for creation of flaps with functioning muscle and overlying soft tissue, and for minimizing flap donor-site problems. Careful planning should include patient counseling, optimum incision placement, and time for a leisurely, complete expansion. The surgery can oft

Tissue expansion has become a method of choice for immediate or delayed breast reconstruction. The compatibility of radiotherapy and tissue expansion is a growing problem. We analyse a series of 50 breast reconstructions by expansion, 24 without radiotherapy and 26 with radiotherapy. For the 24 patients without radiotherapy, very satisfactory cosmetic results were obtained in 71% of cases with 21% of stage III-IV capsular contractures and 8% of failures. For the 26 patients with a history of radiotherapy, very satisfactory cosmetic results were obtained in 65% of cases with 20% of capsular con

The expanded forehead flap is frequently used for nasal reconstruction. When expanding the forehead tissue, the underlying cranial bones are compressed by the inserted expander. Many effects of tissue expansion on the bone have been reported including bone remolding, erosion, displacement, and so forth. In this work, we report a peculiar patient of cranial bone deformity after forehead tissue expansion, which demanded a surgical revision.

OBJECTIVE: To investigate a method of auricular reconstruction with overlapping tissue expansion techniques and without skin graft.
METHODS: Two tissue expanders were implanted subcutaneously at the mastoid. 6 patients with microtia (overlapping group) were treated. After completion of skin expansion, the expanders were removed. The autologous rib cartilage or Medpor scaffolds were implanted. The flap A made by the upper expanded flap was used to cover the upper part of the front and the back of the framework. The flap B made by lower expanded flap was transplanted to cover the lower part of

Elongation of peripheral nerves would facilitate the repair of peripheral nerve injuries. The purpose of this study was to determine the efficacy of nerve expansion and the effect of different levels of intraluminal expander pressure upon nerve conduction. A 2-mL Silastic expander was placed under one sciatic nerve of adult male rats. The other nerve served as a control. Electroneuromyography (ENMG) was done at the time of initial expansion in seven animals. At 40 mm Hg expansion pressure a significant change in latency was found with an increase of 17% +/- 5%. Expansion was carried out for 14

Soft-tissue expansion in the lower extremities is typically well tolerated. The more proximal one is--that is, the closer to the thigh and buttocks--the easier and less complication prone the expansion will be. It is another valuable technique for resurfacing the lower extremity and for reconstructing defects in contour and in skin character. There are limitations to this technique, which generally is most useful in late reconstructions. Intraoperative expansion has no place in lower extremity reconstruction. Soft-tissue expansion may be limited by an unsuitable geometry or the sheer size of d

The present study reviews our clinical experience with tissue expansion in head and neck reconstruction. The clinical series consists of 33 expansions performed in 29 patients between 1983 and 1986. The results of tissue expansion in the head and neck were distinguished by a high complication rate, necessitating some revision in the original treatment plan in 48 percent of patients. Despite this high incidence of complications, sufficient tissue was generated to successfully complete the planned reconstruction in 79 percent of patients. Specific complications were observed with particularly hi

Soft-tissue expansion enjoys ever-wider use, but to date an experience using this technique in the lower extremity has never been presented. We reviewed our first 16 patients to describe the indications and contraindications for the use of tissue expansion in the lower extremity. Guidelines evolved from study of the data. Soft-tissue expansion merits consideration for coverage of problem wounds, in preparation for removal of large benign lesions, and for the repair of contour defects. The operator should know that an open wound below the knee predicts a complication if soft-tissue expansion is

BACKGROUND: Skin defects can reduce functions and can be aesthetically unpleasant. Tissue expansion for reconstruction of skin defects was introduced as a method in the plastic and maxillofacial surgery department, Ullevaal University Hospital, in autumn 2002.
MATERIAL AND METHODS: Patient health records and registration forms for expansion procedures were studied for all the 16 patients (12 adults and 4 children) treated at the department in the period 2002 - 05.
RESULTS: Patients were strictly selected for indication, availability of skin with adequate quality and possibility of cooperatio

In order to shorten the expansion time and reduce complications in tissue expansion, we developed a new method using local delivery of drug during tissue expansion. The procedure commenced with placing an implant syringe between the skin and the expander. While the skin was expanded, the drugs such as prednisolone and procaine were injected under the skin from the implant syringe. We have used the method in 23 cases and compared the results with the control group of 11 cases. It was found that the new method shortened the expansion time and reduced complications. The method had definite effect

Tissue expansion was used successfully to prepare adequate soft tissue for closure following a difficult clubfoot correction. The gradual expansion was done weekly at the outpatient clinics (average 3-4 months). The procedure proved to be useful in severe cases of clubfoot.

The use of tissue expanders having external reservoirs is presented. Twenty-four patients had 36 tissue expanders placed in the scalp, face, neck, upper limbs, chest, and lower limbs. All patients, apart from one, were taken to completion. This patient had erosion of the skin over expander and was only partially completed after removal of the expander. Apart from one instance of deflation at the end of the expansion where the skin could still be used, there were no other complications, in particular, no infections. The advantages of this technique are reduced operating time, ease of injection

Post-traumatic soft tissue loss results in significant functional and esthetic morbidity. Reconstruction is often limited by shortage of soft tissues. Esthetic and functional restorations are possible by generation of well-matched autogenous sensate tissue by tissue expansion. Fifteen (nine females and six males) cases of large and complex soft tissue defects after trauma, age ranging from 7 to 43 years of age, were managed by tissue expansion over a period of 22 months. Indications were post-burn scars of the face (five), post-burn alopecia (three), avulsion injury scalp (one), massive abdomi

BACKGROUND: The authors aim to present a comprehensive review detailing the present state of evidence with regard to complications following tissue expansion in the head and neck.
METHODS: A systematic literature search was conducted to identify all studies reporting complications of tissue expansion in the head and neck between 2000 and 2019. Subgroup comparisons based on expander locations and planes were conducted.
RESULTS: A total of 7058 patients were included. Tissue expansion was associated with an overall complication rate of 8.73% (616/7058). The most common complications were extru

The following guidelines are suggested for the use of tissue expansion in ear reconstruction based on both personal experience and my evaluation of the reported results of expansion in the hands of other surgeons experienced in ear reconstruction: 1) Cases should be carefully selected to avoid those with excess scarring from previous surgery or trauma as well as avoid the other common contraindications to the use of tissue expansion. 2) In most congenital cases the expander should be placed through a remote incision within the postauricular hairline, and excision of the cartilage vestige delay

The authors describes a new device for external tissue extension (ETE) which will be able to replace or complement tissue expanders. The device consists of many ETE units, each unit consisting of a needle and two friction stoppers counted on a silicone string. Application, optimal tension and final surgical procedure are described. The indications are the same as for tissue expanders, e.g. scars, naevi and previous skin grafts, and also concern the closure of acute fasciotomies. The advantages are numerous: very simple technique, application under local anaesthetics, faster cutaneous profits (

Cancer is a clonal disorder derived from a single ancestor cell and its progenies that are positively selected by acquisition of 'driver mutations'. However, the evolution of positively selected clones does not necessarily imply the presence of cancer. On the contrary, it has become clear that expansion of these clones in phenotypically normal or non-cancer tissues is commonly seen in association with ageing and/or in response to environmental insults and chronic inflammation. Recent studies have reported expansion of clones harbouring mutations in cancer driver genes in the blood, skin, oesop

Avoidance of complications in tissue expansion requires careful outpatient observation and consistent follow-up-two factors that are difficult to manage in a city hospital-based population. To determine the complication rate of tissue expanders in a given population, the authors reviewed retrospectively 34 tissue expanders placed in 30 patients at a New York City public hospital over a 7-year period from 1989 to 1996. The mean age of the patients at the time of insertion was 25 years (range, 11 months-65 years). The most common conditions for treatment were nevi (N = 11), burn scars (N = 8), b

OBJECTIVE: Tissue expansion is the preferred technique for the treatment of giant congenital melanocytic nevus (GCMN) located on the face, abdomen, and legs. We believe that the use of multiple tissue expanders in the same surgery is a safe and effective technique for the treatment of GCMN.
MATERIALS AND METHODS: A total of 14 patients were treated between 2013 and 2016 for GCMN removal with placement in the same surgery of 3 or more tissue expanders. We analyzed esthetic results and complications.
RESULTS: The locations of the GCMN on the 14 patients were on the abdomen, thigh, and shoulder
