Smoking and Gambling Disorder: Does Tobacco Use Influence Treatment Outcome?

The purpose of this study was to verify whether tobacco use influenced treatment outcome in a population of treatment seeking individuals with gambling disorder. Gambling disorder is defined as persistent and maladaptive gambling behaviour which meets four or more outlined criteria in the DSM-5. Tobacco use is the most frequent comorbidity with gambling disorder. A total of 676 treatment seeking individuals with gambling disorder were assessed at the National Problem Gambling Clinic in London. We analysed differences in socio-demographic, clinical and gambling variables between smokers and non

Alcohol, tobacco, and drug use disorders and personality disorder symptoms.

In a nonclinical sample of 395 young adults, the authors evaluated the relations between major personality traits, Diagnostic and Statistical Manual of Mental Disorders (4th ed.; DSM-IV; American Psychiatric Association, 1994) personality disorder symptoms, and DSM-IV alcohol use disorders (AUDs). Consistent with previous findings, traits related to disinhibition and negative affectivity were consistently associated with AUDs, as were Cluster B personality disorder symptoms (especially antisocial and borderline disorder symptoms). Multivariate analyses revealed that Cluster B symptoms were sig

Residential Treatment Exclusively for Tobacco Use Disorder: A Narrative Review.

INTRODUCTION: Tobacco use disorder is a major public health issue, and novel smoking cessation approaches are urgently needed. Residential treatment programs have been suggested as a potentially effective treatment for tobacco use disorder. However, there is limited literature on residential treatment programs for patients who are exclusively dependent on nicotine. AIM: The aim of this study is to review the literature on residential treatment programs for patients admitted exclusively for tobacco use disorder. METHODS: A literature search was conducted in PubMed with the aim of identifying

Use and co-use of tobacco and cannabis among couples impacted by alcohol use disorder.

Background: Tobacco and cannabis are common among individuals with alcohol use disorder (AUD); however, co-use is less understood among couples impacted by AUD.Objectives: This study characterized alcohol, tobacco, and cannabis co-use among individuals with AUD (i.e. identified patient; IP) and partners in two dyadic AUD samples separately. It also examined associations within individuals (i.e. intrapersonal) and between partners for the same (i.e. intraclass) and different substances (i.e. interpersonal). Methods: This is a secondary analysis of 96 treatment-seeking couples with AUD (N = 192,

Modeling complex genetic and environmental influences on comorbid bipolar disorder with tobacco use disorder.

BACKGROUND: Comorbidity of psychiatric and substance use disorders represents a significant complication in the clinical course of both disorders. Bipolar Disorder (BD) is a psychiatric disorder characterized by severe mood swings, ranging from mania to depression, and up to a 70% rate of comorbid Tobacco Use Disorder (TUD). We found epidemiological evidence consistent with a common underlying etiology for BD and TUD, as well as evidence of both genetic and environmental influences on BD and TUD. Therefore, we hypothesized a common underlying genetic etiology, interacting with nicotine exposur

Strategies for Prevention or Treatment of Tobacco and Cannabis Use Disorder.

Tobacco and cannabis use in pregnancy are associated with increased adverse perinatal and long-term offspring outcomes. Products for both have evolved with various forms available on the market, challenging accurate counseling of risks and quantification of tobacco and cannabis usage during the perinatal period. Health care providers are recommended to screen for any type of use, provide consistent messaging of harms of tobacco and cannabis use in pregnancy, and offer individualized interventions. The journey to cessation can be complicated by barriers and triggers, lack of social supports, an

Food Addiction and Tobacco Use Disorder: Common Liability and Shared Mechanisms.

As food addiction is being more commonly recognized within the scientific community, parallels can be drawn between it and other addictive substance use disorders, including tobacco use disorder. Given that both unhealthy diets and smoking are leading risk factors for disability and death, a greater understanding of how food addiction and tobacco use disorder overlap with one another is necessary. This narrative review aimed to highlight literature that investigated prevalence, biology, psychology, and treatment options of food addiction and tobacco use disorder. Published studies up to August

N-acetylcysteine for therapy-resistant tobacco use disorder: a pilot study.

INTRODUCTION: N-Acetylcysteine (NAC) may have efficacy in treating tobacco use disorder (TUD) by reducing craving and smoking reward. This study examines whether treatment with NAC may have a clinical efficacy in the treatment of TUD. METHODS: A 12-week double blind randomized controlled trial was conducted to compare the clinical efficacy of NAC 3 g/day versus placebo. We recruited 34 outpatients with therapy resistant TUD concurrently treated with smoking-focused group behavioral therapy. Participants had assessments of daily cigarette use (primary outcome), exhaled carbon monoxide (CO(EXH)

How is Tobacco Use Addressed at US Substance Use Disorder Treatment Facilities? The Importance of Financial Support for Tobacco Cessation Treatment.

Persons in substance use disorders (SUD) treatment report a high prevalence of tobacco use. Often neglected in SUD treatment, some recent state-level policies mandated screening and offering tobacco use treatment or referral. We interviewed 85 providers from 68 SUD treatment facilities across 20 US states, assessed providers' estimates of tobacco use prevalence in their facilities, their perceptions on types of nicotine-containing products used by their service users, tobacco policies that were implemented at their facilities, and whether treatment (counseling or pharmacologic treatment) was o

Developing Competency-Based Nursing Treatment for Persons With Tobacco Use Disorder.

BACKGROUND: Tobacco continues to have a deleterious impact on health outcomes in the United States. Professional nurses at all levels of practice have an opportunity to be a part of the solution. The development of nurse-specific competencies for treating tobacco use disorder (TUD) disorder is long overdue. A task force of American Psychiatric Nurses Association (APNA) subject matter experts was assembled to engage in the process of reviewing the available peer-reviewed literature and additional evidence-based resources (e.g., professional organization position statement, toolkits, national su

Deployment stress, tobacco use, and postdeployment posttraumatic stress disorder: Gender differences.

OBJECTIVE: Epidemiological research has demonstrated that tobacco use and posttraumatic stress disorder (PTSD) frequently co-occur and are highly prevalent among Veterans; research with female Veterans is limited. Given the increasing numbers of women deployed to combat zones in recent conflicts, the objective of the current study was to examine gender-specific associations between deployment stress, tobacco use and postdeployment PTSD symptoms. METHOD: Two thousand thirteen Veterans deployed to Afghanistan and Iraq (50.9% female; mean age = 35.53) completed a postdeployment, mailed survey th

Availability of tobacco cessation services in substance use disorder treatment programs: Impact of state tobacco control policy.

OBJECTIVE: Given the high prevalence of smoking among substance use disorder (SUD) patients, the specialty SUD treatment system is an important target for adoption and implementation of tobacco cessation (TC) services. While research has addressed the impact of tobacco control on individual tobacco consumption, largely overlooked in the literature is the potential impact of state tobacco control policies on availability of services for tobacco cessation. This paper examines the association between state tobacco control policy and availability of TC services in SUD treatment programs in the Uni

DSM criteria for tobacco use disorder and tobacco withdrawal: a critique and proposed revisions for DSM-5.

AIMS: This paper aims to identify appropriate criteria for tobacco dependence assessment, evaluate relevant research and suggest revisions that may be incorporated into DSM-5. METHODS: Desirable conceptual and psychometric features of tobacco dependence assessments were identified, including the types of outcomes against which such assessment should be validated. DSM-IV criteria were matched against these criteria and compared with other dependence measures. RESULTS: DSM-IV criteria were found to be ambiguous, little used in tobacco research, and have relatively low predictive validity. Othe

Treating tobacco use disorder in pregnant women in medication-assisted treatment for an opioid use disorder: a systematic review.

Smoking is associated with adverse effects on pregnancy and fetal development, yet 88-95% of pregnant women in medication-assisted treatment for an opioid use disorder smoke cigarettes. This review summarizes existing knowledge about smoking cessation treatments for pregnant women on buprenorphine or methadone, the two forms of medication-assisted treatment for opioid use disorder indicated for prenatal use. We performed a systematic review of the literature using indexed terms and key words to capture the concepts of smoking, pregnancy, and opioid substitution and found that only three studie

Tobacco and alcohol use disorders: Evaluating multimorbidity.

There is growing interest in applying the multimorbidity model for mental health disorders - i.e. the interactive effects of co-occurring diagnoses. Alcohol use disorder (AUD) and tobacco use disorder (TUD) often occur together, but distinctive correlates of their co-occurrence have not been studied. Veterans treated by the Veterans Health Administration (VHA) nationally in FY 2012 with diagnoses of both AUD and TUD were compared to those with only AUD or only TUD on socio-environmental factors, medical and psychiatric diagnoses, and service use. Multimorbid effects were defined as those in wh

Validity of the DSM-5 tobacco use disorder diagnostics in adults with problematic substance use.

BACKGROUND: DSM-5 tobacco use disorder (TUD) nosology differs from DSM-IV nicotine dependence (ND) by including craving and DSM-IV abuse criteria, a lower threshold (≥ 2 criteria), and severity levels (mild; moderate; severe). We assessed concurrent and prospective validity of the DSM-5 TUD diagnosis and severity and compared validity with DSM-IV ND diagnosis. METHODS: The sample included U.S. adults with current problematic substance use and past year cigarette smoking (N = 396). Baseline assessment collected information on DSM-IV ND and DSM-5 TUD criteria, smoking-related variables, and psy

Tobacco use and gambling disorder: Associations with clinical characteristics, and treatment outcome.

BACKGROUND AND AIMS: Gambling disorder (GD) usually co-occurs with other addictive behaviors. Given shared neurobiological underpinnings, particularly involving dopaminergic pathways, and behavioral similarities such as high reward sensitivity, investigating the link between GD and tobacco use is warranted. Therefore, the present study explored how daily smoking use relates to clinical, personality, and psychopathological profiles, as well as its potential association with treatment outcomes. METHODS: Our sample included 455 individuals with GD, who completed clinical, personality, psychopath

Cross-ancestry genetic investigation of schizophrenia, cannabis use disorder, and tobacco smoking.

Individuals with schizophrenia frequently experience co-occurring substance use, including tobacco smoking and heavy cannabis use, and substance use disorders. There is interest in understanding the extent to which these relationships are causal, and to what extent shared genetic factors play a role. We explored the relationships between schizophrenia (Scz), cannabis use disorder (CanUD), and ever-regular tobacco smoking (Smk) using the largest available genome-wide studies of these phenotypes in individuals of African and European ancestries. All three phenotypes were positively genetically c

Reappraising Choice in Addiction: Novel Conceptualizations and Treatments for Tobacco Use Disorder.

The introduction of alternative nicotine and tobacco products (such as e-cigarettes, heat-not-burn devices, nicotine pouches) warrants an updated framework from which to conceptualize tobacco use disorder (TUD). The following review provides considerations for TUD within the context of novel products. Historically, the tobacco industry falsely claimed that cigarettes were not addictive or harmful and that those who smoked simply chose to do so. This generated an inaccurate lay perception that smoking is a free or informed choice. Research on nicotine pharmacology demonstrates the powerful addi
