The introduction of a mid-urethral stent for hypospadias surgery in toilet-trained children.

PURPOSE: To address the unique challenges presented by hypospadias repair in toilet-trained boys, we propose a modification to the standard stenting technique: implementation of a mid-urethral stent (MUS) extending beyond the urethroplasty, terminating distally to the sphincter mechanism. This modification upholds continence while facilitating normal voiding. METHODS: Toilet-trained boys undergoing hypospadias repair from 2009 to 2020 were retrospectively assessed. Patients were allocated into one of two groups: "Continent" drainage (a short stent was placed across the urethroplasty) or "inco

Assisted Infant Toilet Training and Bladder and Bowel Health: A Global Integrative Review.

BACKGROUND: Toilet training practices vary across cultures and time. Assisted Infant Toilet Training (AITT) is commonly used in low- and middle-income countries. OBJECTIVES: To synthesise the literature on AITT, including timing of initiation and completion, infant elimination signalling, and associations with bladder and bowel dysfunction. METHODS: An integrative review methodology was employed. Comprehensive searches of Scopus, Medline, CINAHL, Web of Science, PsycINFO, and Google Scholar identified relevant studies. Two reviewers independently screened and appraised studies using GRADE an

Pelvic-floor therapy and toilet training in young children with dysfunctional voiding and obstipation.

OBJECTIVE: To analyse experience in treating young children (4-5 years old) with urodynamically confirmed voiding dysfunction, using a noninvasive training programme. PATIENTS AND METHODS: Between January 1996 and October 1997, 20 children (all < 5 years old, mean age 4.45 years, 18 girls and two boys, mean ages 4.44 and 4.5 years, respectively) with voiding dysfunction were treated. Three children showed filling phase dysfunction alone (bladder instability), six emptying phase dysfunction alone (dysfunctional voiding) and 11 showed both filling and emptying phase dysfunction. Sixteen childre

Association Between Toilet Training Timing and Prognosis of Nocturnal Enuresis in Children: A Retrospective Study.

AIMS: Toilet training (TT) is a critical developmental milestone with potential implications for lower urinary tract function. However, its association with the prognosis of nocturnal enuresis (NE) remains unclear. We aimed to evaluate whether the timing of TT initiation and completion affects 1-year treatment outcomes in children with NE. METHODS: We conducted a retrospective cohort study of 163 children diagnosed with NE between April 2020 and March 2023. Patients were classified into complete response (CR) and non-CR groups based on 1-year treatment outcomes. Data on TT timing, constipatio

Do disposable diapers reduce urination elimination signals in non-toilet-trained children?

UNLABELLED: Elimination signals play a key role in facilitating toilet training. This pilot study assessed whether disposable diapers affect urination elimination signals in typically developing children aged 18 to 36 months who are not yet toilet trained. A prospective observational pilot study with a within-subject design was conducted under two conditions, presented in a fixed order: first diaper-free, then with a diaper. Three trained researchers conducted real-time and video-recorded observations, using a urination elimination signals checklist. Of the 28 children enrolled, complete data

The role of bladder function in the pathogenesis and treatment of urinary tract infections in toilet-trained children.

Urinary tract infections (UTIs) are a common reason for referral to pediatric specialists and the risk profile of these children is influenced by age, sex, and underlying urinary tract abnormalities. UTIs in toilet-trained children represent a different entity than confirmed, febrile UTIs that occur in infants, impacted by suboptimal bladder habits, bladder dysfunction, constipation, or a combination of these factors. A comprehensive literature search was conducted using PubMed and MEDLINE and search terms included recurrent UTI, VUR, bladder and bowel dysfunction (BBD), constipation, lower ur

Impaired toilet training and bladder and bowel dysfunction in children with developmental coordination disorder - an underreported issue.

BACKGROUND & OBJECTIVE: Children with Developmental Coordination Disorder (DCD) face significant, yet often overlooked, challenges beyond motor impairments, including difficulties with toilet training and bladder and bowel dysfunctions (BBD). This study aims to explore whether children with DCD exhibit greater difficulties in these areas compared to typically developing children (TDC). METHOD: This cross-sectional case-control study included 84 children aged 5-8 years (42 DCD, 42 TDC), matched by school-grade and sex (33 boys and 9 girls per group). Parents completed the Vancouver Symptom Sco

Nonstented tubularized incised plate urethroplasty with Y-to-I spongioplasty in non-toilet trained children.

INTRODUCTION: This study was designed to evaluate the supportive role of spongioplasty during tubularized incised plate (TIP) urethroplasty repair of hypospadias. METHODS: All non-toilet trained children who underwent TIP repair for primary hypospadias by 1 surgeon over a 30-month period were included in our study. The divergent spongiosa was mobilized off the corpora cavernosa and was rotated toward the midline to wrap the neourethra. A dartos flap was used to cover the neourethra. The neourethra was calibrated immediately after surgery in all patients. A urethral stent was left in place onl

Bowel and bladder function in infant toilet training (BABITT) - protocol for a randomized, two-armed intervention study.

BACKGROUND: In the last decades, the average age for toilet training has increased in the western world. It is suggested that the postponed initiation of toilet training is a contributing factor to problems related to bowel and bladder control. Functional gastrointestinal and urinary tract disorders are prevalent in childhood, causing suffering in affected children and for their families, and consuming healthcare resources. To evaluate whether assisted infant toilet training can prevent functional gastrointestinal and urinary tract disorders in young children, we are conducting a randomized in

[Toilet training in mental retardation; approach to diurnal enuresis in a 12-year-old boy with hemiparesis].

A 12-year-old boy with hemiparesis, severe mental retardation, reduced mobility and behavioural problems was not yet toilet trained. He was successfully trained using a behavioural treatment. The training program was based on gradual prolongation of urine retention, the introduction of behaviour restrictions, rewards for using the toilet and overcorrection using repeated exercises if diurnal enuresis occurred. A third of 4- to 18-year-olds with a cerebral palsy do not have diurnal bladder control at the age of 6, in contrast to 1-3% in the general population. An important cause of this differe

Elevated postvoid residual urine volume predicting recurrence of urinary tract infections in toilet-trained children.

BACKGROUND: The aim of this study was to examine whether toilet-trained children with a history of febrile urinary tract infection (fUTI) and elevated postvoid residual (PVR) urine volume according to a recently published PVR nomogram were at greater risk of UTI recurrence. MATERIALS AND METHODS: One month after recovery from febrile UTI, constipation was diagnosed according to the Rome III criteria, and lower urinary tract (LUT) function was evaluated with two sets of uroflowmetry and PVR by ultrasonography. For children aged ≦ 6 and ≧ 7 years, elevated PVR is defined as >20 and >10 ml, resp

Antibiotics for urethral catheterization in children undergoing cystography: retrospective evaluation of a single-center cohort of pediatric non-toilet-trained patients.

Antibiotics are often used to prevent post-catheterization urinary tract infections (UTIs) in patients undergoing cystography. However, there is no universally accepted protocol and the literature data are conflicting. We aimed to determine prevalence and risk factors of post-cystography UTIs in non-toilet-trained children. We retrospectively evaluated 216 non-toilet-trained children not assuming antibiotics when undergoing cystography. Only the patients with dilated vesico-ureteral reflux (VUR) assumed antibiotics just after the exam (within 15 min). One hundred eleven patients without vesico
