Abnormal mechanical stress due to excessive femoral torsion is associated with dysplasia of the distal femoral epiphyseal plate and trochlea.

AIMS: The distal femoral epiphysis and epiphyseal plate are essential for skeletal morphogenesis during development. However, it is unclear how these growth mechanisms are affected by distal femoral torsion (DFT) and patellar instability. This study aimed to investigate how DFT development affects epiphyseal plate growth mechanisms. METHODS: This study evaluated CT-based 3D reconstructed images of the distal femoral epiphyseal plates in 98 knees exhibiting trochlear dysplasia (50 patients). Morphological parameters including femoral anteversion, DFT, and the anatomical epicondylar axis-poster

Abnormal torsion and helical flow patterns of the neo-aorta in hypoplastic left heart syndrome assessed with 4D-flow MRI.

BACKGROUND: The Norwood procedure is the first stage of correction for patients with hypoplastic left heart syndrome (HLHS) and may lead to an abnormal neoaortic anatomy. We prospectively studied the neoaorta's fluid dynamics and the abnormal twist of the neoaorta by MRI examinations of HLHS patients in Fontan circulation. This study for the first time investigates the hypothesis that the neoaorta twist is associated with increased helical flow patterns, which may lead to an increased workload for the systemic right ventricle (RV) and ultimately to RV hypertrophy. METHODS: A group of forty-tw

Prevalence and diagnostic accuracy of in-toeing and out-toeing of the foot for patients with abnormal femoral torsion and femoroacetabular impingement: implications for hip arthroscopy and femoral derotation osteotomy.

AIMS: Abnormal femoral torsion (FT) is increasingly recognized as an additional cause for femoroacetabular impingement (FAI). It is unknown if in-toeing of the foot is a specific diagnostic sign for increased FT in patients with symptomatic FAI. The aims of this study were to determine: 1) the prevalence and diagnostic accuracy of in-toeing to detect increased FT; 2) if foot progression angle (FPA) and tibial torsion (TT) are different among patients with abnormal FT; and 3) if FPA correlates with FT. PATIENTS AND METHODS: A retrospective, institutional review board (IRB)-approved, controlled

Ultrasound femoral anteversion (FAV) relative to tibial torsion (TT) is abnormal after school screening for adolescent idiopathic scoliosis (AIS): evaluation by two methods.

In the scoliotic spine, torsion is generally evaluated in relation to axial rotation of the apical vertebra. In the lower limbs, the changes in torsion by age of femoral anteversion (FAV) relative to tibial torsion (TT) have been studied in dried bones, normal growing subjects and adults and subjects with osteoarthritis of the hip or the knee. This paper reports the application of real-time ultrasound to FAV and TT in normal children age 11-18 years and in scoliosis screening referrals with particular reference to how FAV relates to TT as 1) ratios, and 2) tibio-femoral index (TFI) of torsion,

Association between abnormal native tibial torsion and poorer clinical outcomes following robotic-assisted total knee arthroplasty: follow-up of at least two years.

The study aimed to compare clinical outcomes of robotic-assisted total knee arthroplasty (RA-TKA) between patients with tibial rotational malalignment and those with normal tibial rotational alignment. From 2022 to 2023, the retrospective cohort included 382 patients who received RA-TKA using a functional alignment (FA) strategy for end-stage varus knee osteoarthritis (KOA). One surgeon performed all procedures. A tibial torsion angle (TTA) > 30° defined the abnormal tibial torsion (ATT) group, whereas a TTA ≤ 30° defined the normal tibial torsion (NTT) group. Following 1:1 matching, each grou

[Value of spermatic cord sonography in the early diagnosis and treatment of testicular torsion].

OBJECTIVE: To improve the early diagnosis and therapeutic outcomes of testicular torsion. METHODS: We retrospectively reviewed the clinical data of 49 cases of testicular torsion along with the results of their intratesticular color Doppler flow imaging (CDFI) and spermatic cord sonography. RESULTS: Of the 49 cases, 42 showed abnormal intratesticular blood flow, including 3 cases of increased blood flow, while the other 7 presented no obvious abnormality. Morphological abnormality of the spermatic cord was found in 47 cases. Twenty-eight cases underwent testis removal, and the other 21 recei

Perinatal testicular torsion: preoperative radiological findings and the argument for urgent surgical exploration.

Perinatal testicular torsion is an infrequent event, the management of which has been controversial. Occurrence is rare, estimated at 1 in 7500 newborns (Kaplan, G. W., Silber, I.: Neonatal torsion--to pex or not? In: Urologic surgery in neonates and young infants. Edited by King, L.R. Philadelphia: W.B. Saunders Co., 1988; Chapter 20, pp. 386-395). The frequency of bilateral perinatal torsion is up to 22% (J Urol. 2005;174:1579). Here, we describe two cases of bilateral asynchronous perinatal torsion, in which the only presenting abnormality on exam after birth was a unilateral scrotal mass.

Oophoropexy to round ligament for recurrent ovarian torsion.

Ovarian torsion is a gynaecological emergency commonly affecting women of reproductive age, often associated with ovarian or paraovarian masses. However, in the absence of these masses, torsion can still occur, potentially indicating an underlying anatomical abnormality that predisposes to recurrent torsion. This report presents a distinctive surgical technique used in a young girl in her early adolescence with recurrent right ovarian torsion. The patient, with a history of torsion managed by detorsion, presented again with symptoms suggestive of recurrence. Emergency diagnostic laparoscopy re

The testes after torsion.

In torsion of the testis a large proportion of testes saved by surgery develop secondary atrophy. External manual reduction may be used as a first treatment, but surgical fixation is mandatory before the patient leaves the hospital. Prophylactic fixation of the contralateral testis should always be performed as 30% of the patients will otherwise develop symptoms. The preventive operation causes no discomfort to the patient and no harm to the testis. The fertility of the torsion patients is reduced. Patients treated for unilateral testicular torsion seem to have bilateral testicular abnormality

Mechanisms of injury in unilateral testis torsion.

We investigated the mechanisms of injury involved in unilateral testis torsion as well as the interventional effects of orchiectomy at 48 h and immunosuppression with corticosteroids in rats. Torsion was associated with abnormal contralateral testis histology and raised antilymphocytes and antisperm antibody titers. Both orchiectomy and steroid administration lessened these findings, suggesting that an immunological process underlies the abnormality seen in this experiment model.

The fate of the human testes following unilateral torsion of the spermatic cord.

In an attempt to explain the oligozoospermia commonly observed after unilateral testicular torsion, 56 patients with acute torsion were investigated prospectively. Blood was taken pre-operatively for antibody studies and a contralateral testicular biopsy was performed at the time of orchiopexy. At review 3 to 6 months post-operatively, late testicular atrophy was assessed and repeat antibody studies were performed together with a hormone profile and seminal analysis. Although the duration of torsion showed a close correlation with the degree of testicular atrophy (P less than 0.001), no such a

Old and new aspects of testicular torsion.

Torsion of the spermatic cord is a common condition that requires emergent treatment. Diagnosis based on history and physical examination is rarely conclusive and, although a number of radiographic studies can be helpful, any clinically suspicious lesion should be treated immediately. Manual detorsion may be helpful, but operative exploration with bilateral orchiopexy is still essential. Prolonged torsion usually results in unilateral testicular atrophy and, in postpubertal animals, an autoimmune injury with consequent contralateral testicular damage is seen. This suggests that, in humans, nec

Laparoscopic cholecystectomy for gallbladder torsion in a 3-year-old child.

BACKGROUND: Gallbladder torsion is a congenital abnormality of the gallbladder and is a rare cause of acute abdomen. Although gallbladder torsion is a life-threatening condition requiring emergency surgery, laparoscopic cholecystectomy for gallbladder torsion in pediatric patients has seldom been reported. CASE PRESENTATION: A 3-year-old boy with abdominal pain and vomiting was admitted to our hospital. He was diagnosed with acute cholecystitis through abdominal ultrasound and computed tomography, and he received antibiotics. Two days after admission, a repeat abdominal ultrasound revealed ex

Ovarian Torsion: A Review of the Evidence.

IMPORTANCE: Ovarian torsion is a gynecological emergency caused by the twisting of the ovary and/or fallopian tube, further resulting in ischemic changes of the adnexa. Early diagnosis is likely to preserve ovarian function. OBJECTIVE: The purpose of this review is to review the current findings of ovarian torsion including clinical presentations, diagnostic criteria, surgical procedures, and prognosis. EVIDENCE ACQUISITION: The literature search is mainly available in PubMed and Web of Science platforms by searching "ovarian torsion" combined with one or several terms including "diagnosis"

Torsion of the testicle: a clinical review of 20 cases.

Early diagnosis of testicular torsion is absolutely mandatory for salvage of a viable organ. The classical clinical presentation is a boy in the second decade with sudden onset of testicular pain accompanied by testicular tenderness and swelling as well as scrotal erythema and edema. The management is emergency exploration of the testicle with either orchiopexy or orchidectomy, depending upon the viability of the testis. In this series of 20 cases 90% had exploration with a salvage rate of 72%. Torsion of the testicle is in most cases due to a congenital abnormality; therefore contralateral or

Fetal Ovarian Torsion Presenting with Fetal Abdominal Solid Mass and Ascites.

Fetal ovarian torsion is quite a rare event during the antenatal period and usually seen because of an ovarian cyst complication. In this case report, we present a case of fetal ovarian torsion without any ovarian cyst or underlying detectable causes. A 27-year-old primigravid woman with no significant past medical history had a routine prenatal ultrasound at 30 weeks' gestation. The ultrasound showed abdominal ascites and a 47×42-cm intraabdominal solid diffuse mass at the left side under the stomach. Doppler examination showed no blood flow on the mass. Paracentesis was performed, cytologica
