Complications and Complication Avoidance With Cervical Total Disc Replacement.

Complications with cervical arthroplasty can be generalized to errors in patient selection or surgical technique. Patients with advanced spondylosis or osteophytic disease, severe facet arthropathy, osteoporosis, sagittal deformity, or preoperative instability are poor candidates for arthroplasty and are more prone to complications. Poor surgical technique can result in subsidence, expulsion, and kyphosis, and it can contribute to heterotopic ossification. Additionally, all of the inherent complications from an anterior cervical approach may occur with cervical artificial disc placement. This

The impact of total lumbar disc replacement on segmental and total lumbar lordosis.

BACKGROUND: One of the goals of total lumbar disc replacement is restoration of the physiological sagittal alignment. There is little evidence if this goal is reached in vivo and further affects the clinical outcome. METHODS: In 29 patients segmental lordosis and total lumbar lordosis were measured on X-rays pre- and postoperatively. The functional outcome was evaluated prospectively with the Visuell Analogue Scale, Oswestry Low Back Pain Disability Questionnaire and Short Form 36 Health Survey Questionnaire. FINDINGS: Total disc replacement increased segmental lordosis significantly while t

Preclinical and clinical experience with a viscoelastic total disc replacement.

BACKGROUND: The purpose of this study is to describe the mechanical durability and the clinical and radiographic outcomes of a viscoelastic total disc replacement (VTDR). The human intervertebral disc is a complex, viscoelastic structure, permitting and constraining motion in 3 axes, thus providing stability. The ideal disc replacement should be viscoelastic and deformable in all directions, and it should restore disc height and angle. METHODS: Mechanical testing was conducted to validate the durability of the VTDR, and a clinical study was conducted to evaluate safety and performance. Fifty

The Total Disc Replacement Osteolysis Grading Scale - a simple, reliable and quantifiable tool for assessing, managing and reporting osteolysis after cervical total disc replacement.

PURPOSE: Cervical Total Disc Replacement (CTDR) related periprosthetic osteolysis is inconsistently reported. The purpose of this study is to assess the previously published TDR Osteolysis Grading Scale, a quantitative classification system of CTDR-related osteolysis, and to assess its reliability and utility using Computer Tomography (CT) and X-ray imaging. METHODS: Participants were assigned to Groups A (CT, 20 participants, 27 implants) and B (X-ray, 20 participants, 26 implants). Four blinded raters independently measured osteolytic cysts and both vertebral endplates, calculated the perce

Analysis of adjacent segment reoperation after lumbar total disc replacement.

BACKGROUND: Fusion has long been used for treating chronic back pain unresponsive to nonoperative care. However, potential development of adjacent segment degeneration resulting in reoperation is a concern. Total disc replacement (TDR) has been proposed as a method for addressing back pain and preventing or reducing adjacent segment degeneration. The purpose of the study was to determine the reoperation rate at the segment adjacent to a level implanted with a lumbar TDR and to analyze the pre-TDR condition of the adjacent segment. METHODS: This study was based on a retrospective review of cha

Heterotopic ossification following cervical total disc replacement: iatrogenic or constitutional?

OBJECTIVE: To elucidate etiological factors of heterotopic ossification (HO) by evaluating retrospectively if HO is a unique finding following cervical total disc replacement (CTDR) or a finding observable following an anterior cervical interbody fusion (ACIF). METHODS: The authors had selected 87 patients who underwent anterior cervical surgery (TDR or ACIF), and could be followed up more than 24 months. A cervical TDR was performed using a Bryan disc or a ProDisc-C and an ACIF using a stand-alone cage or fibular allograft with a plate and screws system. The presence of HO was determined by

Lumbar total disc replacement. Seven to eleven-year follow-up.

BACKGROUND: Symptomatic lumbar degenerative disc disease is a challenging entity to treat. The results of arthrodesis may be compromised in the short term by pseudarthrosis and in the long term by pain at the iliac-crest donor site and by junctional degeneration. Total disc replacement has the potential to provide long-lasting relief to these patients. The purpose of this study was to present the clinical and radiographic results assessed seven to eleven years following a Prodisc total lumbar disc replacement. METHODS: Sixty-four patients had single or multiple-level implantation of a total l

Retrospective study on effectiveness of Activ L total disc replacement.

BACKGROUND: The effectiveness of lumbar total disc replacement (TDR) with different prostheses for sagittal alignment has been reported previously. However, there are only few reports on Activ L TDR and no specific evidence regarding whether sagittal alignment affects the clinical outcomes. METHODS: Eighty-seven patients who underwent mono- or bi-segmental lumbar TDR with Activ L were studied. The films of the upright anteroposterior and lateral spine in neutral, flexion, and extension positions were obtained before surgery and at 1 month and 1 and 3 years after surgery. Cervical Total Disc Replacement: Food and Drug Administration-Approved Devices.

The first US Food and Drug Administration (FDA) approval for cervical total disc replacement (CTDR) was issued in 2007. Since then, 8 more artificial discs have been granted FDA approval for single-level CTDR. Two of these have also been approved for 2-level CTDR. All devices are indicated for levels C3 to C7 for symptomatic patients with radiculopathy or myelopathy caused by disc herniation or spondylosis unresponsive to conservative management. Trials have shown noninferiority of CTDR compared with anterior cervical decompression and fusion in their overall success. Hybrid surgery and CTDR o

Disc herniation caused by a viscoelastic nucleus after total lumbar disc replacement-a case report.

Degenerative disc disease (DDD) is highly prevalent. If conservative treatment fails, spinal fusion procedures are commonly performed. Total disc replacement (TDR) might be a surgical option for a distinct subset of patients with DDD. Several prostheses have been or are still available. Despite some promising initial clinical results, there is still limited experience with hardware-related adverse events. This report highlights an unreported complication after TDR with a viscoelastic device. Literature about long-term outcome and safety of this particular TDR is scarce. Hence, there exists lim

Total disc replacement using tissue-engineered intervertebral discs in the canine cervical spine.

The most common reason that adults in the United States see their physician is lower back or neck pain secondary to degenerative disc disease. To date, approaches to treat degenerative disc disease are confined to purely mechanical devices designed to either eliminate or enable flexibility of the diseased motion segment. Tissue engineered intervertebral discs (TE-IVDs) have been proposed as an alternative approach and have shown promise in replacing native IVD in the rodent tail spine. Here we demonstrate the efficacy of our TE-IVDs in the canine cervical spine. TE-IVD components were construc

Epidemiology of indications and contraindications to total disc replacement in an academic practice.

BACKGROUND CONTEXT: Given the experience with anterior lumbar cages, the similar enthusiasm for total disc replacement (TDR) and the economic incentives driving use of this new technology, it is important to document reasonable expectations as to the incidence of patients with appropriate indications for this new technology. PURPOSE: To document epidemiological data for the indications and contraindications to total disc replacement (TDR) to guide expectations for use of this new technology. STUDY DESIGN/SETTINGS: Retrospective evaluation of contraindications and indications for lumbar TDR in

Total disc replacement in the lumbar spine: a systematic review of the literature.

The current evidence for total disc replacement was assessed by performing a systematic review of the published literature. This search identified two randomised controlled trials (RCTs), two previous systematic reviews, seven prospective cohort studies, eleven retrospective cohort studies and eight case series. The RCTs involved the use of the Charité artificial disc and the Pro-Disc II total disc replacement. All papers analysed were classified according to their level of evidence as defined by the Centre for Evidence Based Medicine, Oxford, UK (www.cebm). For degenerative disc disease at L4

Postoperative Physical Therapy for Cervical Total Disc Replacement: A Case Report.

BACKGROUND AND PURPOSE: Cervical total disc replacement (CTDR) was developed to decompress nervous structures, preserve spinal biomechanics, and reduce postoperative complications such as nonunion or fracture. There is no published literature regarding the optimal content, timing and duration of postoperative physical therapy (PT) for CTDR. Therefore, the purpose of this case report is to describe the postoperative PT management and one-year outcomes of a subject with CTDR as it relates to pain, function and quality of life. CASE DESCRIPTION: A 37-year-old male and Brazilian Jiu Jitsu athlete

Effect of two-level total disc replacement on cervical spine kinematics.

STUDY DESIGN: Biomechanical study using human cadaver spines. OBJECTIVE: To characterize kinematics of cervical spines implanted with total disc replacement (TDR) at 2-levels referencing the implanted and adjacent levels. SUMMARY OF BACKGROUND DATA: Cervical TDR is an appealing alternative to fusion particularly when treating multilevel disease, where the advantages of maintaining motion and reducing adjacent level stresses with TDR are compelling. To our knowledge there are no biomechanical studies evaluating multilevel cervical TDR. METHODS: Six human cadaveric cervical spine specimens (C
