The trabecular meshwork in glaucoma: An inflammatory trabeculopathy?

Glaucoma is an optic neuropathy in which the primary risk factor is increased intraocular pressure (IOP), attributed to increased resistance to trabecular outflow of aqueous humor (AH). This resistance is believed to result from trabecular degeneration secondary to chronic oxidative stress and cellular senescence but may also involve inflammatory mechanisms whose roles are little known. In fact, inflammatory processes play a major role in the pathophysiology of glaucoma to varying degrees, affecting all structures of the eye, including the ocular surface, the anterior and posterior segments, a

Trabecular meshwork in normal and pathological eyes.

PURPOSE: The impact of glycosaminoglycans on intraocular pressure in glaucoma patients and in healthy young or aging subjects is explored. MATERIALS AND METHODS: Thirty small autoptic samples were harvested from the tissue localized around the iridocorneal angle of the eye, taking care not to cause aesthetic damage. The samples came from three groups (young, old, and subjects with glaucoma). All samples were divided in two fragments and both were used for morphological and biochemical analyses. Quantitative data were obtained from image analysis to correlate with biochemical values. All resul

Steroid response after complete trabecular meshwork removal.

INTRODUCTION: Steroid-induced hypertensive response is a common, yet underdiagnosed complication of steroid therapy. Certain patients, referred to as high responders, can experience a rapid and severe rise in intraocular pressure (IOP) potentially reaching sight-threatening levels. This report presents two high-responder patients with multiple risk factors. METHODS: Medical records of two patients who underwent 360° angle surgery and experienced postoperative IOP spikes were reviewed, and clinical data was extracted. RESULTS: Two male patients aged 8 and 22 years old with juvenile open angle

Transcriptome Profiling of Trabecular Meshwork Progenitor Cells.

The loss and dysfunction of trabecular meshwork (TM) cells are implicated in aging and primary open-angle glaucoma. TM progenitor cells (TMPCs) contribute to the population and function of the TM, but their identity is not well elucidated. This study aimed to identify the expression profile of differentially expressed genes (DEGs) in human TM cell cultures, TM-derived spheres, and their differentiated progeny. Primary normal human TM cells (PTM) from three donors were cultured, de-differentiated into spheres, and re-differentiated into TM cells (DTM). RNA-Seq was performed using Illumina NGS,

Trabecular Meshwork Stem Cells for Glaucoma Treatment.

Elevated intraocular pressure (IOP) is the most important risk factor for primary open-angle glaucoma (POAG) and currently is the only effective treatment target for glaucoma to prevent vision loss. In POAG patients, the trabecular meshwork (TM) cellularity is reduced which might be the main pathologic reason for the conventional outflow pathway dysfunction leading to elevated IOP. Stem cell-based therapy has been shown promising to reduce IOP and preserve retinal ganglion cells and their function in animal models. In this chapter, we describe the method details on TM stem cell cultivation and

Autophagic dysregulation in glaucomatous trabecular meshwork cells.

Primary open angle glaucoma (POAG) is a degenerative disease commonly associated with aging and elevated intraocular pressure (IOP). Higher resistance to aqueous humor (AH) outflow through the trabecular meshwork (TM) generates the elevated IOP in POAG; unfortunately the underlying molecular mechanisms responsible for elevated resistance are unknown. It is widely accepted, however, that differences between normal and POAG TM tissues are presumably a consequence of cellular dysfunction. Here, we investigated the autophagic function and response to chronic oxidative stress in TM cells isolated f

sCD44 internalization in human trabecular meshwork cells.

PURPOSE: To determine whether soluble CD44 (sCD44), a likely biomarker of primary open-angle glaucoma (POAG), is internalized in cultured human trabecular meshwork (TM) cells and trafficked to mitochondria. METHODS: In vitro, 32-kD sCD44 was isolated from human sera, biotinylated, and dephosphorylated. TM cells were incubated for 1 hour at 4°C with biotinylated albumin (b-albumin), biotin-labeled sCD44 (b-sCD44), or hypophosphorylated biotin-labeled sCD44 (-p b-sCD44) in the presence or absence of unlabeled sCD44, hyaluronic acid (HA), and a selected 10-mer HA binding peptide. Trabecular meshwork cellularity. Differences between fellow eyes.

The difference in trabecular meshwork cellularity between fellow eyes was determined in a series of seven pairs of normal human eyes. Variations in cellularity between different quadrants of individual eyes also were determined. Characterization of these differences and development of appropriate sampling techniques to determine trabecular cellularity has become important in light of recent studies that find a decrease in trabecular cell numbers with age and with primary open-angle glaucoma, and the apparent stimulation of trabecular cell division by laser trabeculoplasty. The difference in ab

Nanotubules and Cellular Communication in Trabecular Meshwork Cells.

Glaucoma causes dysfunction to tissues located in the anterior and posterior eye. In the anterior eye, the trabecular meshwork (TM) is the site of pathogenesis, where decreased TM cell numbers and alterations to the amount and composition of extracellular matrix hinder outflow of aqueous humor fluid from the anterior chamber. This causes intraocular pressure (IOP) elevation. Elevated IOP, a main risk factor for primary open-angle glaucoma, damages the axons of retinal ganglion cells in the posterior eye, which ultimately leads to blindness. Thus, clinical treatment paradigms for glaucoma are f

An Overview of Current Glaucomatous Trabecular Meshwork Models.

PURPOSE: To provide an overview of the existing alternative models for studying trabecular meshwork (TM). METHODS: Literature review. RESULTS: The TM is a complex tissue that regulates aqueous humor outflow from the eye. Dysfunction of the TM is a major contributor to the pathogenesis of open-angle glaucoma, a leading cause of irreversible blindness worldwide. The TM is a porous structure composed of trabecular meshwork cells (TMC) within a multi-layered extracellular matrix (ECM). Although dysregulation of the outflow throughout the TM represents the first step in the disease process, the u

The dysfunction of the trabecular meshwork during glaucoma course.

Primary open angle glaucoma is a multi-tissue disease that targets, in an ascending order, the trabecular meshwork, the optic nerve head, the lateral geniculate nuclei, and the visual cortex. Oxidative stress and vascular damage play major roles in triggering apoptotic cell loss in these tissues. Molecular alterations occurring in the ocular anterior chamber during the early course of glaucoma trigger this cell loss. These molecular events are mainly of endogenous origin and related to the long-term accumulation of oxidative damages arising from mitochondrial failure and endothelial dysfunctio

Matricellular proteins in the trabecular meshwork: review and update.

Abstract Primary open-angle glaucoma (POAG) is a leading cause of blindness worldwide, and intraocular pressure (IOP) is an important modifiable risk factor. IOP is a function of aqueous humor production and aqueous humor outflow, and it is thought that prolonged IOP elevation leads to optic nerve damage over time. Within the trabecular meshwork (TM), the eye's primary drainage system for aqueous humor, matricellular proteins generally allow cells to modulate their attachments with and alter the characteristics of their surrounding extracellular matrix (ECM). It is now well established that EC

The trabecular meshwork in normal eyes and in exfoliation glaucoma.

Trabecular meshwork (TM) and ciliary muscle contraction and relaxation function together to provide control of outflow. The active role the TM plays in the regulation of intraocular pressure (IOP) is mediated by cytoskeletal and contractility mechanisms as well as signal/transduction factors that mediate its response to stressors. This complex system is altered with age and the glaucomas, and it can be difficult to differentiate between the various etiological effects/agents. Factors such as a compromised antioxidant defense system and altered extracellular matrix metabolism are known to contr

Mechanisms of action of unoprostone on trabecular meshwork contractility.

PURPOSE: This study was performed to clarify the possible mechanism behind the ocular hypotensive effect of unoprostone isopropyl (Rescula; Novartis Ophthalmics AG, Basel, Switzerland), a new docosanoid that has been shown to reduce intraocular pressure (IOP) in patients with ocular hypertension or primary open-angle glaucoma. To gain insight into the possible mode of action, the effects of unoprostone on ciliary muscle (CM) and trabecular meshwork (TM) contractility, intracellular calcium levels, and membrane channels were investigated. Ultrastructural changes in the trabecular meshwork of congenital glaucoma.

Trabeculectomy specimens from 10 patients with congenital glaucoma were studied by electron microscopy. In all cases proliferation of fibrous connective tissue was found from the inner wall of Schlemm's canal. Accumulation of collagen fibres, sometimes of varied thickness and irregular striation, and agglomerations of microfibrillar material with the structure of collagen microfibrils were observed. This material formed basement membrane-like structures and 'shaggy' or fingerprint-like patterns. The ultrastructural examination of the trabecular meshwork revealed no significant differences betw

The regulation of trabecular meshwork and ciliary muscle contractility.

Current models of aqueous humor outflow no longer treat trabecular meshwork (TM) as an inert tissue passively distended by the ciliary muscle (CM). Instead, ample evidence supports the theory that trabecular meshwork possess smooth muscle-like properties and is actively involved in the regulation of aqueous humor outflow and intraocular pressure. In this model, trabecular meshwork and ciliary muscle appear as functional antagonists, with ciliary muscle contraction leading to a distension of trabecular meshwork with subsequent reduction in outflow. and with trabecular meshwork contraction leadi

Functional morphology of the trabecular meshwork in primate eyes.

The trabecular meshwork forms most of the resistance to aqueous humor outflow needed for maintenance of a pressure gradient between intraocular pressure of approximately 17 mmHg and venous pressure of approximately 10 mmHg. The composition of the extracellular material in the subendothelial or cribriform layer seems to be mainly responsible for outflow resistance. The aqueous humor pathways through the subendothelial layer can be influenced by ciliary muscle contraction and presumably also by contractile elements recently found both in trabecular meshwork and scleral spur.
