In this study the authors evaluated the outcomes of tracheal resection and anastomosis in postintubation tracheal stenosis. The authors also aimed to identify clinical and radiological factors associated with restenosis. Thirty-four consecutive patients were included in the study. Patients with subglottic stenosis who underwent procedures involving cricoid cartilage and patients with other etiologies were excluded from the study. Univariate analysis was performed to determine the risk factors for restenosis. Receiver operating characteristic (ROC) curves were generated for the diameter of the …

OBJECTIVES: Evaluate the results of a technique for repair of congenital tracheal stenosis by use of a free tracheal autograft.
METHODS: Between January 1996 and July 1997, six infants with congenital tracheal stenosis resulting from complete tracheal rings underwent repair with a free tracheal autograft. Mean age at the time of repair was 4.9 months; mean weight was 5.4 kg. The approach was through a median sternotomy with cardiopulmonary bypass for respiratory support. The trachea was incised anteriorly through the area of stenosis, the midportion of the stenotic trachea was excised, and an…

Management of stenosis of the subglottic larynx remains one of the most challenging problems facing the laryngologist. This report describes our results treating severe subglottic stenosis. Five patients, 2 males and 3 females, with acquired stabilized laryngotracheal stenosis were treated during the period 1981-1991. Ages ranged from 13 to 62 years. According to Cotton's classification the patients were assigned to a grade II-IV. The cause of the stenosis was trauma in two cases and intubation injury in three. All patients had a previously placed tracheotomy. All patients underwent a Laryngo-…

Currently, a single classification of cicatricial tracheal stenosis is absent in national and world surgical practice. This issue is actual considering anatomical variability of tracheal structure in people with different constitutional features and previous complications. The proposed classification ensures individualized approach in surgical treatment of patients with cicatricial tracheal stenosis considering anatomical features in a particular patient.

Postintubation tracheal stenosis (PITS) is a known complication of endotracheal intubation or tracheostomy. It is the most common indication for tracheal resection/reconstructive surgery. Despite technological improvement and skilled patient care in the ICU, PITS still constitutes an important group of iatrogenic sequela after intubation. With increasing number of patients requiring ICU admission and mechanical ventilation in Nigeria, it is important that this complication is prevented from occurring. The care of such patients often is technically challenging. The successful management by rese…

Patients with upper airway obstruction from malignant disease are difficult to manage. A 62 year old patient presented with stridor and was found to have an upper tracheal tumour. Bronchoscopy, dilatation and stenting were performed successfully. The techniques and indications for the use of dynamic airway stent are discussed.

In the Department of General and Thoracic Surgery between 1981 and 1992 497 patients with tracheal stenosis were treated surgically. In 19 (3.8%) cases the stenosis was after previous tracheostomy. In 469 patients (94.4%) the stenosis was caused as the result of compression by goitre and in 9 (1.8%) by neoplastic tumors of mediastinum. Most patients with tracheal stenosis were treated surgically. In 13 cases the stenosis was resected, in 3 cases T-tube was inserted and in 3 cases the stenosis was dilated. In patients with secondary stenosis due to compression by goitre subtotal thyroidectomy w…

After exposure to ammonia, a 63 year old woman experienced dyspnoea and respiratory infections. A stenosis was diagnosed in the proximal part of the trachea. High levels of antibodies against thyroperoxidase suggested thyroiditis. After treatment with corticosteroids both the antibodies and the stenosis decreased and the patient's symptoms improved. A few months later the patient developed therapy-resistant uveitis. Cyclosporin was added to the treatment and the condition was stabilized. The diagnosis was unclear. Tracheal stenosis may have developed secondary to autoimmune thyroiditis, while …

A case of congenital tracheal stenosis is reported. The diagnosis was made on plain X-ray, tomography and computerized axial tomography of the trachea when the child was 8 years old. The operative approach was via a median sternotomy and anaesthesia maintained with dual simultaneous ventilation of the right upper lobe bronchus and distal trachea. Histology of the stenotic area revealed the cartilaginous rings to be completely encircling the trachea.

This is the report on one case of tracheo-stenosis after aspiration of a foreign body which has been diagnosed only after 7 years and successfully treated by surgery. Based on this case, the symptoms of stenosis of trachea or main bronchus are described and it is referred to the difficulties of diagnosis. Anamnesis, therapy-resistant cough and dyspnea, inspiratoric stridor, isolated stenotic murmur in central bronchial sections or a weakened respiratory sound give a hint to the existence of a stenotic process which is to be confirmed by tomography and bronchoscopy. Problems of misinterpretatio

Stenosis of the postlaryngectomy tracheal stoma remains a persistent problem. A retrospective study of 89 patients determined on overall incidence of 22 percent. The use of postoperative radiotherapy was associated with a higher incidence (36 percent) than preoperative radiotherapy (19 percent) and combined preoperative and postoperative radiotherapy (17 percent). Retention of the tracheal cannula for longer than 1 week was associated with a slightly increased incidence (25 percent) of stenosis. The technique of stomal construction was an important determinant of stoma stenosis. A simple circl

For all cases of tracheal obstructions surgery should be considered first. Interventional endoscopic procedures can provide immediate relief. Intraluminally growing tumors can be resected with laser, argon-plasma coagulation, an electrosurgical knife or cryo-probe. Photodynamic therapy of smaller tracheal tumors can be curative. Narrowing from intramural tumor growth or wall destruction requires internal splinting with an airway stent. Scar strictures can be dilated with balloons but the biotrauma may stimulate new scarring. In benign strictures and malacias, tracheal stents should only be pla

From 1969 to 1990, 43 patients with tracheal stenosis were treated at the University of Mississippi Medical Center. Seventy-four percent of these patients (n = 32) had intrinsic tracheal stenosis, most frequently as a complication of prolonged endotracheal trauma. A total of 41 distinctly separate stenotic segments were identified in the 32 patients. The stenoses were considered moderate or severe in 33 (80%) of the 41 cases and the length of the stenotic segment was greater than 1 cm in 23 (56%) of the cases. An overall 70% success rate was achieved following 93 surgical procedures in this gr…

BACKGROUND: To study the role of microecology and metabolism in iatrogenic tracheal injury and cicatricial stenosis, we investigated the tracheal microbiome and metabolome in patients with tracheal stenosis after endotracheal intubation.
METHODS: We collected 16 protected specimen brush (PSB) and 8 broncho-alveolar lavage (BAL) samples from 8 iatrogenic subglottic tracheal stenosis patients, including 8 PSB samples from tracheal scar sites, 8 PSB samples from scar-free sites and 8 BAL samples, by lavaging the subsegmental bronchi of the right-middle lobe. Metagenomic sequencing was performed …

Congenital tracheal stenosis is a rare but life-threatening malformation of the trachea. Surgical reconstruction is high risk, and not frequently performed in neonates born of extreme prematurity and low birth weight. We present the case of an extremely premature 950-gram neonate with severe congenital tracheal stenosis who underwent tracheal reconstruction. Complete repair, with no residual stenosis, was achieved with slide tracheoplasty without the need for cardiopulmonary bypass.
