Successful Tracheostomy Decannulation: Why Body Mass Index Alone Is Not Enough.

BackgroundObesity is increasingly common among critically ill patients. The association between body mass index and outcomes of tracheostomy including eventual decannulation are not well understood. This study aimed to evaluate whether body mass index (BMI) is associated with time to tracheostomy decannulation among surgical critical care patients undergoing tracheostomy. We hypothesized that increasing BMI would be associated with longer duration from tracheostomy placement to decannulation in surgical patients.MethodsAdult participants undergoing tracheostomy creation by the general surgery

Outcomes of Tracheostomy Decannulation: Study Protocol for a Longitudinal Observational Study.

BACKGROUND: Tracheostomy management aims to achieve optimal decannulation outcomes in tracheostomized patients. Successful removal of the tracheal cannula (decannulation) is key from a physical and psychological health perspective. Decannulation success is commonly defined as the absence of physical complications leading to re-cannulation within a few hours or days post-decannulation. However, this definition does not account for complications occurring weeks or months later, or for (serious) complications that require actions other than re-cannulation. Additionally, the definition neglects th

Predictive Factors of Successful Decannulation in Tracheostomy Patients: A Scoping Review.

Background/Objective: Tracheostomy (TCT) creates an artificial airway, essential for overcoming obstructions or enabling long-term ventilation. Decannulation represents a critical step in recovery, with its success strongly influenced by the underlying indication for tracheostomy and the patient's clinical profile. Successful decannulation requires careful assessment of multiple factors, including respiratory function and underlying pathology. This scoping review aims to identify and categorize these predictive factors, crucial for optimizing decannulation protocols and patient outcomes. Metho

Safety evaluation of a stepwise tracheostomy decannulation program in pediatric patients.

PURPOSE: In the event of failed tracheostomy decannulation, patients might have a tragic course of events. We retrospectively evaluated our stepwise tracheostomy decannulation program and examined its safety. METHODS: A 12-year retrospective study of pediatric patients was conducted. The decannulation program was performed on patients who had airway patency by laryngobronchoscopy and whose cannula could be capped during the day. A stepwise decannulation program was performed: continuous 48-h capping trial during hospitalization (Phase 1), removal of the tracheostomy tube for 48 h during hospi

Prognostic factors for tracheostomy early decannulation in acquired brain injury patients.

After severe brain injuries, a tracheostomy tube is usually inserted for respiratory support. This study aimed to clarify the prognostic factors for tracheostomy early decannulation in patients with acquired brain injuries. We retrospectively reviewed the medical records of inpatients with acquired brain injuries who underwent successful tracheostomy decannulation between March 2021 and June 2022. Fifty-six patients were included; median age was 68 (59-72) years; 28 (50%) were men; 28 (50%) underwent tracheostomy due to stroke. The median time to decannulation was 47 days. The patients were di

Single-Stage Bronchoscopy-Guided Protocol for Tracheostomy Decannulation in Adult Patients.

INTRODUCTION: Tracheostomy decannulation is a routine procedure in airway management. There is no standard decannulation method; however, the two commonly practiced approaches are tracheostomy downsizing and intermittent capping, which are both accompanied by multiple visits to the clinic and increase patient discomfort. Herein, we explore fiberoptic bronchoscopy application in a novel single-stage decannulation protocol. METHODS: We conducted a retrospective study on tracheostomy patients eligible for decannulation. Fiberoptic bronchoscopy was performed on patients with spontaneous ventilati

Difficult tracheostomy decannulation related to nasogastric tube syndrome: A case report.

INTRODUCTION AND IMPORTANCE: Nasogastric tube syndrome (NGTS) is a rare but potentially life-threatening complication. Patients receiving both tracheostomy and indwelling nasogastric tube (NGT) are not uncommon, however difficult tracheostomy decannulation due to NGTS has not been reported. CASE PRESENTATION: A 65-year-old woman was hospitalized with cervical spine stenosis and cervical spinal cord injury after a fall. The surgeon planned neck surgery, but unanticipated tracheotomy had to perform due to emergency airway during anesthesia induction. She then suffered from acute respiratory dis

Tracheostomy Tube Manometry Can Predict Capping Test Success in Subjects Undergoing Decannulation.

BACKGROUND: Decannulation represents the final phase of temporary tracheostomy, started once clinical criteria for cannula removal are met. A key indicator of decannulation readiness is the patient's ability to tolerate breathing through a capped uncuffed cannula (capping test). Since tracheal pressure manometry can be of help during placement of speaking valves and plugs, it is likely that it can also discriminate between patients who tolerate or do not tolerate a capping test. This study explores how tracheal pressure measurement during tidal breathing through a plugged cannula can be predic

Tracheostomy decannulation readiness: A cross sectional study comparing standardised evaluation for tacheostomy decannulation to flexible endoscopic evaluation of swallowing examination.

INTRODUCTION: Tracheostomy is a procedure commonly performed in neurocritical and mechanically ventilated patients in the intensive care unit. Dysphagia and impaired airway protection are the main causes for a delay in tracheostomy decannulation in patients with neurological disorders. Endoscopic evaluation is an objective examination of readiness for tracheostomy decannulation with flexible endoscopic evaluation of swallowing (FEES) as the most commonly used method, yet it requires special expertise and is heavily dependent on its operator in assessing the parameters. A relatively new method

Cough flows as a criterion for decannulation of autonomously breathing patients with tracheostomy tubes.

BACKGROUND: Adequate cough or exsufflation flow can indicate an option for safe tracheostomy decannulation to noninvasive management. Cough peak flow via the upper airways with the tube capped is an outcome predictor for decannulation readiness in patients with neuromuscular impairment. However, this threshold value is typically measured with tracheotomy tube removed, which is not acceptable culturally in China. The aim of this study was to assess the feasibility and safety of using cough flow measured with tracheostomy tube and speaking valve (CFSV) > 100 L/min as a cutoff value for decannula

[Modified tracheostomy management: a protocol for the application of stoma buttons in difficult decannulations].

Current approaches to decannulation management often fail to account for patients with combined swallowing and respiratory deficits. We expanded our existing weaning and decannulation protocol by adding an optional 3-day decannulation trial to evaluate readiness for decannulation. If a patient meets predefined test-decannulation criteria a tracheostomy button is inserted during a laryngoscopic examination and left in situ for up to 3 days. Before, during and after button insertion the patient's respiratory function and saliva management are closely monitored before the decision for or against

Factors and Outcomes Attributable to Weaning and Decannulation from Ventilation after Tracheostomy.

INTRODUCTION: The goal of tracheostomy in patients with long-term tracheal intubation is to facilitate weaning from mechanical ventilation (MV), achieve decannulation, and ultimately enable discharge to home. In this study, we investigated the factors influencing withdrawal from MV after tracheostomy and cannulation in patients undergoing long-term tracheal intubation. We also examined tracheostomy status (whether the tracheostomy tube was removed and whether the patient was weaned from MV) and discharge outcomes. METHODS: A total of 199 patients who underwent tracheostomy following long-term

Tracheostomy decannulation to noninvasive positive pressure ventilation in congenital central hypoventilation syndrome.

PURPOSE: Noninvasive positive pressure ventilation (NPPV) may permit tracheostomy decannulation (TD) in patients with congenital central hypoventilation syndrome (CCHS) requiring nocturnal positive pressure ventilation via tracheostomy (PPV-T). There is limited evidence on optimal strategies for transitioning patients from PPV-T to NPPV. This study aimed to describe the clinical course and outcome of children with CCHS who underwent TD and transitioned from PPV-T to NPPV. METHODS: Retrospective review was conducted on patients with CCHS using nocturnal PPV-T who underwent TD to NPPV. The resu

Complications of tracheostomy and decannulation in pediatric and young patients with traumatic brain injury.

Tracheostomy is indicated for many patients who are in a prolonged comatose state. The procedure in children is believed to be associated with a higher morbidity and mortality; however, most studies have focused on adult patients. Thus, to better define the incidence, a retrospective study was done on 30 head injured pediatric patients who had tracheostomies at our rehabilitation institution. In 26 patients tracheostomy was indicated for prolonged hyperventilation. The average duration of tracheostomy was 49 days. Twenty-six total complications (86.6%) were reported within seven days of trache
