This article focuses on the main patterns of damage that are seen in liver allograft biopsies. As with the interpretation of liver biopsies from the native liver, clinicopathological correlation is very important. The therapeutic implications of the biopsy report should also be considered, in particular whether changes in immunosuppression are indicated. For some conditions, such as liver allograft rejection, histology remains the gold standard for diagnosis. In other cases, a likely cause of graft dysfunction may already have been identified by other methods, but liver biopsy still provides u

Over the past 40 years, heart transplantation has become a routine treatment for end-stage heart failure. Over 3000 heart transplants are performed annually. Despite improvements in short-term outcomes, long-term survival remains limited. Current efforts remain directed at alleviating the shortage of donor organs, developing effective treatments for chronic rejection, and improving mechanical circulatory support for heart failure.

Today, kidney transplantation is the treatment of choice for most patients with end-stage renal failure. However, because of the organ shortage, the limiting supply and increasing demand, needing transplants do not receive them. Chronic renal disease should be efficiently screened and prevented. On the other hand, all ways of using available organs, from living and deceased donors, including those with extended criteria are needed. Finally, strategies targeting factors involved in to chronic allograft nephropathy must be pursued in the effort to improve long-term outcomes after renal transplan

Transplantation is an established therapy for end-stage diseases of the kidney, endocrine pancreas, heart, liver, and lung, and for many hematologic disorders. Improved survival rates have been accompanied by increased recognition of previously neglected long-term complications of transplantation such as fractures and osteoporosis. Pretransplantation bone disease and immunosuppressive therapy result in rapid bone loss and increased fracture rates early after transplantation. Patients should be assessed and pretransplantation bone disease should be treated. Preventive therapy initiated in the i

Liver transplantation represents the first choice treatment for patients with fulminant acute hepatitis and for patients with chronic liver disease and advanced functional failure. Patients in the waiting list for liver transplantation are classified according to the severity of their clinical conditions (evaluated using staging systems mostly based on hematochemical parameters related to liver function). This classification, together with the blood group and the body size compatibility, remains the main criterion for organ allocation. The main indications for liver transplantation are cirrhos

Analysis of the OPTN/SRTR database demonstrates that, in 2002, pediatric recipients accounted for 7% of all recipients, while pediatric individuals accounted for 14% of deceased organ donors. For children fortunate enough to receive a transplant, there has been continued improvement in outcomes following all forms of transplantation. Current 1-year graft survival is generally excellent, with survival rates following transplantation in many cases equaling or exceeding those of all other recipients. In renal transplantation, despite excellent early graft survival, there is evidence that long-ter

Numerous laboratory studies have shown that hepatocyte transplantation may serve as an alternative to organ transplantation for patients with life-threatening liver disease. Because of the successes of experimental hepatocyte transplantation, institutions have attempted to use this therapy in the clinic for the treatment of a variety of hepatic diseases. Unfortunately, unequivocal evidence of transplanted human hepatocyte function has been obtained in only one patient with Crigler-Najjar syndrome type I, and, even then, the amount of bilirubin-UGT enzyme activity derived from the transplanted

Until recently, only life and death situations warranted organ transplantation. Nonvital transplantation, to further a patient s wishes and goals, was not considered justified. It can be argued, however, that this distinction is not morally significant. Patients with kidney failure, for example, can be kept alive by dialysis. But their quality of life would be greatly enhanced by kidney transplant, which is thus considered a justified procedure. So a spectrum of rationales may justify transplantation. Transplantation of the uterus would relieve the anguish of women who greatly desire to concei

Repopulation of the liver with transplanted cells holds significant promise for developing novel therapies. The liver is a most suitable target for treating a variety of genetic, metabolic and acquired diseases. Liver disease, such as chronic viral hepatitis, constitutes an enormous burden worldwide. Advancing liver cell therapy requires insights into mechanisms of cell engraftment and proliferation, as well as unique requirements of specific diseases for correction by cell transplantation. This review highlights recent developments in the area of hepatocyte transplantation. Aspects concerning

Since February 1999, organ transplantation from brain-dead donors has started in Japan. During the more than 3 years since, 77 patients have received organ transplantations from 19 donors (heart in 13, lung in 10, liver in 17, pancreas-kidney in 6, pancreas alone in 1, small intestine in 1, and kidney in 29). As of April 2002, 116 patients were registered with the Japan Organ Transplant Network as heart transplant (HTx) candidates, and 13 and 8 underwent HTx in Japan and abroad, respectively, while 32 died while on the waiting list. Ten of 13 HTx recipients were implanted with a left ventricul

Fat transplantation has been used by physicians for over 100 years; however, this technique has been refined considerably since the development of liposuction in the late 1970s. Fat transplantation can be used for augmentation of subcutaneous defects throughout the body. It continues to be the safest and most versatile form of soft tissue augmentation.

Advances in organ preservation, surgical technique, and postoperative care have permitted the rapid development of liver transplantation in children. Consequently, the applicability of this procedure has gone beyond the treatment of life-threatening complications of chronic liver disease and now includes disabling morbidities and quality-of-life issues. The use of hepatic segments for transplantation with reduced or split cadaveric grafts and living-related donors has decreased the mortality of children awaiting liver transplantation. We are presently armed with a new potent immunosuppressive

Although intensified insulin therapy regimens enable normalization of blood glucose levels and related metabolic parameters, these regimens are associated with an increased incidence of hypoglycemic episodes. Pancreas transplantation has achieved the goal of providing insulin independence with stable and continuous normoglycemia. But because of the associated morbidity and mortality and the need for life-long immunosuppression after transplant, it is difficult to justify pancreas transplantation in diabetic patients at a pre-uremic stage. Pancreas transplantation is therefore performed in conj

The authors present a short overview on the history of clinical renal transplantation and an update of the results of renal transplantation today. Factors contributing to chronic renal allograft failure and immunotherapy-related exposure to malignant diseases complicating life after transplantation are discussed.

Intestinal transplantation is often the only alternative form of treatment for patients dependent on total parenteral nutrition for survival. Although a limited number of intestinal transplantations have been performed, results with FK 506 immunosuppression are comparable to those for other organ transplants. The impact of successful intestinal transplantation on gastroenterology will likely be similar to the impact of kidney and liver transplantation on nephrology and hepatology.

Renal transplantation is the treatment of choice for patients with end-stage renal disease. The authors present a rational approach to the pre-, intra-, and postoperative care required for successful patient management.

As alternative methods of lung transplantation are available, in terminal respiratory or combined cardio-respiratory failure, the surgical procedure can be chosen to suit the needs of the individual recipient in accordance with the philosophy of the transplant centre. At one Scandinavian centre the preferred procedure has been en bloc double lung transplantation with bronchial artery revascularisation, whereas at another centre the preference is for single lung transplantation if considered suitable in view of the pathology resulting in respiratory failure. Short-term results compare favourabl

From more than 900 articles in the medical literature, published between August 1, 1991 and July 31, 1992, 46 were selected for review. During the year of review, major international symposiums were held on a new drug, FK506, and intestinal transplantation. In addition, at least one new attempt at a unifying hypothesis regarding graft adaptation was proposed. Other studies included living related donor transplantation, results with transplantation for alcoholics, and the controversy regarding the role of liver transplantation for the treatment of Budd-Chiari syndrome. Concerns regarding the re
