Clinical Significance of Helicopter Transportation for Patients With Acute Illness Admitted to Medical Intensive Care Unit.

BACKGROUND: We aimed to elucidate the characteristics and prognosis of patients admitted to a medical intensive care unit (ICU) by helicopter emergency medical service (HEMS). METHODS AND RESULTS: Among 11,140 patients transported via the institute's HEMS from January 2012 to December 2021, 459 were admitted to the medical ICU and included as the HEMS group. The 3,810 patients who were transported by other means to the medical ICU comprised the non-HEMS group. Multivariable logistic regression analysis revealed that cardiovascular etiologies other than acute heart failure (AHF), as well as yo

National Database Study of the effect of mode of transportation on trauma patients injured in snowmobile accidents.

BACKGROUND: Snowmobile accidents occur in areas that may be difficult to reach by ground Emergency Medical Services (GEMS) vehicles. Helicopter EMS (HEMS) can potentially be useful in these situations, but there are few available data assessing HEMS-associated survival improvement in patients injured in snowmobile accidents. This study evaluates the national trauma database (NTDB) to identify snowmobile-related injury cases to determine if HEMS transport was associated with improved outcome, and whether any improvement in outcome is associated with worse functional status as indicated by dispo

Dynamics of Intracranial Pressure and Cerebrovascular Reactivity During Intrahospital Transportation of Traumatic Brain Injury Patients in Coma.

BACKGROUND: Intrahospital transportation (IHT) of patients with traumatic brain injury (TBI) is common and may have adverse consequences, incurring inherent risks. The data on the frequency and severity of clinical complications linked with IHT are contradictory, and there is no agreement on whether it is safe or potentially challenging for neurocritical care unit patients. Continuous intracranial pressure (ICP) monitoring is essential in neurointensive care. The role of ICP monitoring and management of cerebral autoregulation impairments in IHT of patients with severe TBI is underinvestigated

Double the Trouble: Successful Cannulation and Air Transportation of Two Obese Trauma Patients Requiring Extracorporeal Membrane Oxygenation.

Acute Respiratory Distress Syndrome (ARDS) is a known and severe complication of thoracic trauma. Many patients, despite appropriate ventilator and medical support, continue to worsen requiring additional cardiopulmonary support with extracorporeal membrane oxygenation (ECMO). Additionally, obesity adds a layer of complexity in the management of trauma ARDS on ECMO. We describe the first U.S. Military air transportation mission via Critical Care Air Transport (CCAT) involving the cannulation and transportation of 2 obese trauma patients requiring ECMO support. We reviewed a cohort of 2 obese p

How do they get here: Does the method of transportation impact salvage for patients with testicular torsion?

INTRODUCTION: A growing number of patients are arriving at our tertiary care center for evaluation of possible testicular torsion using ambulance or helicopter transport. In many cases the parents arrive by car before the patient arrives. Are these advanced methods of medical transport worth the expense and risk in the case of suspected testicular torsion? OBJECTIVE: We evaluated the total number of patients presenting to our emergency room for suspected testicular torsion to see if the means of transport affected testicular survival. STUDY DESIGN: Retrospective. RESULTS: As shown below in

Comparison of Secondary Ischaemia Incidence, Intracranial Pressure, and Cerebrovascular Reactivity Dynamics During Intrahospital Transportation of Severe TBI Patients.

UNLABELLED: The aims of the study were to evaluate posttraumatic cerebral ischaemia (PTCI) incidence in severe traumatic brain injury (TBI) patients and to assess the intracranial pressure (ICP) and cerebrovascular reactivity dynamics during intrahospital transportation (IT). MATERIALS: A total of 153 severe TBI patients and 182 IT were included. The mean Glasgow Coma Scale (GCS) score was 6.7 ± 2.1. ICP and arterial pressure were invasively monitored, and an improved pressure reactivity index (iPRx) was calculated from the measured parameters. Statistical analysis was done using Student's t-

Transportation of patients on extracorporeal membrane oxygenation: a tertiary medical center experience and systematic review of the literature.

BACKGROUND: Utilization of extracorporeal membrane oxygenation (ECMO) has increased worldwide, but its use remains restricted to severely ill patients, and few referral centers are properly structured to offer this support. Inter-hospital transfer of patients on ECMO support can be life-threatening. In this study, we report a single-center experience and a systematic review of the available published data on complications and mortality associated with ECMO transportation. METHODS: We reported single-center data regarding complications and mortality associated with the transportation of patien

Intercontinental LVAS patient transport.

Mechanical circulatory support is currently indicated for patients with cardiac insufficiency as a bridge to transplantation or as a bridge to recovery. These systems continue to evolve and improve, and many patients (after they are stabilized) are now able to be discharged from the hospital. This article reports our experience with the intercontinental transportation of a patient while being supported with a Novacor left ventricular assist system (WorldHeart Corp, Ottawa, Canada). While in Japan, the Canadian patient suffered a myocardial infarction and despite coronary artery bypass grafting

Collective Critical Care Ambulance: an innovative transportation of critical care patients by bus in COVID-19 pandemic response.

BACKGROUND: During the COVID-19 pandemic, as the number of available Intensive Care beds in France did not meet the needs, it appeared necessary to transfer a large number of patients from the most affected areas to the less ones. Mass transportation resources were deemed necessary. To achieve that goal, the concept of a Collective Critical Care Ambulance (CCCA) was proposed in the form of a long-distance bus re-designed and equipped to accommodate up to six intensive care patients and allow Advanced Life Support (ALS) techniques to be performed while en route. METHODS: The expected benefit o

Patient referral and transportation to a regional tertiary ICU: patient demographics, severity of illness and outcome comparison with non-transported patients.

A descriptive analysis and comparison of critically ill transported patients with non-transported patients will assist in selecting the appropriate referral and transportation process and subsequent incorporation into the critical care services of receiving hospitals. A retrospective review of transported and non-transported patients admitted to the same Intensive Care Unit was conducted. Patient demographics, disease categories, source of admission to ICU, APACHE II scores, predicted and actual hospital mortality, hospital and ICU length of stay were examined. Of all ICU admissions, 16% were
