Travel and vaccination patterns: a report from a travel medicine clinic in northern Sweden.

BACKGROUND: The Travel Medicine Clinic in Umeå is one of Sweden's largest public providers of vaccination and counselling prior to international travel. During the study period it was the only travel medicine clinic in Umeå. This study describes the demography of the visitors to the clinic and travel destinations and durations, as well as vaccinations administered. METHODS: This was a retrospective study for the period January 2005 to April 2008 based on pre-travel consultation questionnaires and on vaccine expenditure data. A 10% sample of 16,735 first visits prior to international travel wa

[Injuries and medical emergencies in adult travel medicine].

Emergencies among adult travelers are numerous and clearly outside the scope of usual pre-travel consultations (vaccinations, infectious and/or tropical disease prophylaxis). No prospective studies have addressed this issue, and rare retrospective studies report trauma in 40 % of cases and non-transmissible medical emergencies in 60 % of cases. In-flight emergencies, traffic accidents, as well as environmental (altitude, carbon monoxide) and behavioral (selfies, alcohol consumption, medical tourism) injuries warrant attention. Finally, at-risk travelers should receive special counseling and be

Rabies: relevance, prevention, and management in travel medicine.

Rabies is a zoonotic viral disease, transmitted only in mammals. Terrestrial rabies, predominantly transmitted by dogs, is the most important rabies cycle threatening humans. The causative neurotropic virus is a negative-stranded RNA virus of the family Rhabdoviridae, genus Lyssavirus. This genus contains several rabies-related viruses. All variants are known or suspected to cause rabieslike diseases. Transmission occurs by the virus entering through the skin or the mucosa after bites, scratches, or preexisting injuries contaminated by the saliva of an infected mammal. Only 51 human rabies cas

Recent Advances and New Challenges in Travel Medicine.

Recent advances in travel medicine include the use of computer resources to obtain information on outbreaks and recommendations to travelers, the introduction of atovaquone/proguanil as chemoprophylaxis and treatment for malaria, the use of azithromycin as an alternative in the self-treatment of traveler's diarrhea, and the combination of hepatitis A and hepatitis B vaccines. At the same time, new challenges continue to appear. Shifts in the distribution of infections, such as West Nile virus and dengue fever, underscore the need for up-to-date information. Well-known infectious diseases, such

Travel medicine: helping patients prepare for trips abroad.

One third of persons who travel abroad experience a travel-related illness, usually diarrhea or an upper respiratory infection. The risk of travelers' diarrhea can be reduced by eating only freshly prepared, hot foods. Combination therapy with a single dose of ofloxacin plus loperamide usually provides relief from travelers' diarrhea within 24 hours. Using a diethyltoluamide (deet)-containing insect repellent and wearing permethrin-coated clothing can reduce the risk of malaria, yellow fever and other diseases contracted from insects. Routine immunizations such as tetanus, measles, mumps and r

Meningococcal vaccines: a neglected topic in travel medicine?

Meningococcal disease is a devastating disease. Regular epidemics, particularly in the African meningitis belt and outbreaks during the Hajj pilgrimage, have caused significant morbidity and mortality. Although meningococcal vaccination is recommended for travelers to these destinations, disease awareness among travelers is generally poor. Case-fatality rates of meningococcal disease are high (10-14%) and the disease can be fatal within 24 h, even when the best healthcare is available. This is particularly relevant to travelers, as gaining timely and accurate diagnosis and treatment is often d

Healthcare seeking during travel: an analysis by the GeoSentinel surveillance network of travel medicine providers.

BACKGROUND: International travellers may seek care abroad to address health problems that arise during their trip or plan healthcare outside their country of residence as medical tourists. METHODS: Data were collected on travellers evaluated at GeoSentinel Network sites who reported healthcare during travel. Both unplanned and planned healthcare were analysed, including the reason and nature of healthcare sought, characteristics of the treatment provided and outcomes. Travellers that presented for rabies post-exposure prophylaxis were described elsewhere and were excluded from detailed analys

Trend Analysis of Travel Medicine Topics Presented at an International Tropical Medicine Conference.

Prior to the COVID-19 pandemic, there was a rapid increase in international travel. Travel medicine is a branch of preventive medicine focusing on risk assessment pre-travel, during travel and post-travel with the aim of promoting health and preventing adverse health outcomes. Travel medicine specialists inform travelers about potential health risks and mitigate infectious disease risks such as travelers' diarrhea, yellow fever, and malaria. Travel medicine topics were popular in the American Society of Tropical Medicine and Hygiene conferences between 2016 and 2020, and now comprise approxima

[Analysis of prevention of diseases in travellers on the basis of latest results in travel medicine].

When we survey the possibilities of prevention in travel medicine, it is soon detected that the "classical" or "infectology-oriented" approach of prevention (vaccination - chemoprophylaxis - pretravel advice) cannot respond to all peritravel medical problems. These newly arisen challenges require a new, complex outlook for travel medicine, mainly in the field of prevention, treatment and management of travel-related illnesses. Although travellers are still exposed to various infectious diseases, which are mostly preventable by the primary prevention methods (biomedical prophylaxis), the majori

Balancing innovation and risk: artificial intelligence in travel medicine.

Artificial intelligence (AI) is increasingly influencing travel medicine, a field traditionally reliant on expert judgement and static guidelines. Machine learning systems, generative AI models and digital surveillance tools have the potential to enhance pre-travel counselling, improve diagnostic pathways and strengthen outbreak detection. However, concerns remain regarding variable performance, data privacy and the absence of clear regulatory frameworks. This Perspective examines the role of AI across the travel health continuum, highlighting both opportunities and limitations. Rather than re

Tick-borne encephalitis: a disease neglected by travel medicine.

Tick-borne encephalitis (TBE) is a vector-borne disease that is primarily transmitted to humans by infected ticks and causes infection of the central nervous system. Clinical presentations range from meningitis to encephalitis with or without myelitis, and infection may result in death or long-term neurological sequelae. TBE is endemic in regions of at least 27 European as well as in some Asian countries. Infection and disease, however, can be averted successfully by tick-bite prevention and active vaccination. The risk of infection has shifted from daily life and occupational exposure to leis

Travel and tropical medicine practice among infectious disease practitioners.

BACKGROUND: Infectious disease specialists who evaluate international travelers before or after their trips need skills to prevent, recognize, and treat an increasingly broad range of infectious diseases. Wide variation exists in training and percentage effort among providers of this care. In parallel, there may be variations in approach to pre-travel consultation and the types of travel-related illness encountered. Aggregate information from travel-medicine providers may reveal practice patterns and novel trends in infectious illness acquired through travel. METHODS: The 1,265 members of the

Needs assessment study for community pharmacy travel medicine services.

BACKGROUND: Community pharmacists in the UK currently provide limited travel medicine services. An enhanced service could offer the traveling public general advice, provision of immunizations, and malaria prophylaxis. The assessment of need for a travel vaccine service from community pharmacies is key to the decision to develop the service. METHOD: The needs assessment used a questionnaire survey of potential travelers recruited from community pharmacies and the regional travel clinic. RESULTS: In total, 151 completed questionnaires were received (response rate 40%); nearly three times as ma
