RATIONALE: Acute generalized exanthematous pustulosis (AGEP) is a rare delayed allergic reaction, which is mostly caused by drugs, but there are few reports of iodixanol in particular. Iotrolan solution at 300 mgI/ml (Isovist 300, Schering AG), was injected into the cerebromedullary cistern in 8 cats (Isovist group), at a volume of 0.5 ml/Kg BW and over one minute. A 33-year-old woman with cryoglobulinaemia developed acute hydrocephalus 20 h after lumbar myelography using the water-soluble contrast medium iotrolan. After a ventricular drainage, she was free of symptoms and the hydrocephalus resolved. The development of acute hydrocephalus after myelography has not been reported previously.

Adverse reactions to radiographic contrast media are relatively rare and occur with a frequency of 0.02-0.04%. We describe a case of isolated pulmonary oedema after computed tomography of the coronary arteries in a 51 year-old man. Initially anaphylaxis was suspected, but due to the clinical picture together with lack of response to treatment with adrenaline and lack of increase in the serum tryptase concentration an IgE mediated mechanism was less likely. The patient responded to non-invasive ventilation over three days. The mechanism behind the reaction is unknown.

OBJECTIVE: To report a rare case of vicarious contrast material excretion (VCME) following iodixanol administration, leading to acute kidney injury (AKI) and multi-organ complications, and to explore the underlying mechanisms.
CASE PRESENTATION: A 71-year-old female underwent lower limb venography with intravenous iodixanol. Post-procedure, she developed AKI, followed by acute pancreatitis, hepatic hemorrhage, and hemoperitoneum. RELATION: Iodide mumps is an uncommon condition, induced by iodide-containing contrast, and is characterized by a rapid, painless enlargement of the bilateral or unilateral salivary gland. At present, the pathogenesis of iodide mumps is not yet clear. It may be related to an idiosyncratic reaction, a toxic accumulation of iodine in the gland duct, or renal function damage leading to an iodine excretion disorder.
