Morphologie : bulletin de l'Association des anatomistes2007fre
The number of carpal bones may be increased or decreased by the fact of anatomical variants or true congenital anomalies. Numerical increment arises from additional or from split bones. Over twenty accessory carpal bones have been described but the commonest are the os centrale carpi, the os radiale externum, the triangular bone and the styloideum bone. Additional carpal bones usually result from a failure of fusion of their ossification centers. A congenital origin is not clearly established for all these ossicles. The scaphoid and lunate may split into two or three bones and several cases of

Although radioscapholunate (RSL) and lunotriquetral (LT) fusions are less commonly performed than other limited carpal fusions, they can relieve pain, correct deformity, and restore function for appropriately selected patients with wrist arthritis or instability. Distal scaphoid excision and triquetrum excision may improve range of motion in patients undergoing RSL fusion. Although complete LT coalitions are often asymptomatic, LT fusion may be considered to eliminate motion at the pseudoarthrosis site and relieve pain for symptomatic patients. Arthroscopic approaches may improve visualization

CASE: We describe the case of a 40-year-old man with Minnaar type-III congenital synostosis of the lunate and the triquetrum who presented with Lichtman stage-I Kienböck disease. Surgical treatment consisted of capitate shortening with a capitate-hamate fusion.
CONCLUSION: This case demonstrates the tenuous regional nature of the vascularity to the lunate, even in the setting of a complete lunotriquetral synostosis.

In fifteen cadaver wrists and ten foetus wrists palmar ligaments were studied. As a result, the palmar scapho-triquetral ligament was presented by means of gross anatomical study, MR imaging, and histological study. A kinematic study was conducted on three cadaver wrists to analyze patterns of motion on the proximal carpal row bones based on the helical axis concept. Motion analysis showed that the scaphoid has a set of helical axes bundled at the radial side of the wrist. On the contrary, the triquetrum had axes bundled on the ulnar side, approximately in the same position as the lunate. That

Because of their low incidence, studies about carpal fractures are rare. The aim of the present study was to analyze epidemiology and treatment of fractured carpal bones. We retrospectively analyzed data of 178 patients admitted to our emergency room with carpal fractures over 6 years. More males than woman were injured. In 91%, a CT scan was performed. The most commonly affected bone was the triquetrum followed by the scaphoid. Almost all triquetral fractures were treated conservatively as opposed to perilunate dislocations that were all operated on. Half of all patients with scaphoid fractur

The primary goal in the treatment of carpal fractures is the preservation of a painless wrist function. Scaphoid fractures are the most common carpal fractures and when such a fracture is clinically suspected CT or MRI scans are usually advisable. Only stable and non-displaced scaphoid fractures can be treated conservatively, all other fractures require internal fixation with restoration of normal anatomy. Second most common are fractures of the triquetrum which can occur as chip avulsions of the dorsal rim and are usually treated symptomatically. Fractures of the body of the triquetrum should

Fractures of the hamate are rare, and a clear treatment algorithm does not exist. Nonetheless, surgical treatment is generally recommended for displaced fractures using a dorsal approach. There is also a lack of data on hamate malunion. We present a case of a 28-year-old female with a coronal malunion of the hamate and hamate hook fracture. Because the triquetrum prevented direct access to the fracture, we planned and undertook a transtriquetral coronal osteotomy based on three-dimensional computed tomography imaging data. After removing the bone callus, reduction was possible with subsequent

Carpal fractures in children are rare, especially with a combination of scaphoid, capitate, and triquetrum fractures. We report one such case in a 10-year-old boy who was successfully treated with Kirschner wires.

A 17-year-old man had chronic pain and recurrent radial dislocation of the pisiform bone following a fall on his outstretched arm. The bone was excised, along with a 3 x 4-mm osteocartilaginous fragment from the distal triquetrum. Pisotriquetral joint instability should be considered in the differential diagnosis of palmar-ulnar wrist pain.

Fractures of the triquetrum: Triquetrum fractures consist of two types: chip fractures of the dorsum and fractures of the body of the bone. Dorsal cortical fractures respond well to a short time splint or cast support. Non-displaced fractures of the body can be treated by immobilization for four to six weeks. Displacement of body fractures is very rare and often the result of perilunate instability. This condition usually requires open reduction, internal fixation and ligament repair. Lunotriquetral ligament injury: Partial lunotriquetral ligament disruption and dynamic instability should be t

UNLABELLED: Fractures of the carpal bones are rare compared to fractures of the metacarpals and phalanges. The triquetrum and scaphoid are the most frequently involved, and the other bones injuries are less common. The aim of the study was to analyze the incidence of carpal fractures other than the scaphoid bone, methods and results of their treatment.
MATERIAL AND METHODS: Between 2021 and 2023, 35 patients, 25 men (71%) and 10 women (29%), at the mean age of 49 years, with isolated fractures of the carpal bones other than the scaphoid, were treated at the author's institution. Fractures of

We describe a case of attritional rupture of a flexor digitorum profundus tendon to the right index finger in a 73-year-old man by an accessory carpal bone located on the palmar side of the capitate and triquetrum. The radial edge of the accessory carpal bone appeared to be the cause of the rupture. Excision of the accessory carpal bone, bridge tendon graft using the palmaris longus tendon, and early controlled mobilization after surgery led to a successful outcome.

PURPOSE: An emerging imaging modality, four-dimensional computed tomography, can provide dynamic evaluation of carpal motion, which allows for a better understanding of how the carpals work together to achieve range of motion. The objective of this work was to examine kinematic motion of the carpus through a flexion/extension arc of motion using four-dimensional computed tomography.
METHODS: A convenience sample of 20 uninjured participants underwent a four-dimensional computed tomography scanning protocol through a complete arc of flexion/extension motion. Kinematic changes in motion were qu

Diagnosis and proper initial management of acute fractures of the carpal bones is critical because of the limited blood supply of many bones of the wrist and the role of the carpus in optimizing hand function. Pathology is correctly diagnosed by a focused history and examination. Injuries may be missed with a cursory examination and routine wrist radiographs. Together, fractures of the scaphoid and triquetrum make up nearly 90% of carpal bone fractures. Relative frequency, mechanism of injury, diagnosis, and management principles are covered for each of the bones of the wrist.

Ulnar-sided wrist pain remains a commonly encountered diagnostic challenge, and its successful management requires a comprehensive understanding of the multiple conditions that can present with this symptom. Midcarpal impaction syndromes in the form of Hamato-lunate and Triquetro-hamate impingement have both previously been reported as rare potential causes of ulnar-sided wrist pain. Despite this, they remain poorly recognized and incompletely understood. This article reviews existing literature that describes the diagnosis and management of these clinical entities.

A patient sustained multiple injuries during a road traffic accident, including closed palmar luxation of the lunate together with dislocation of the triquetrum and hamulus of the hamate bone. Initially the injury was treated closed, but redislocation necessitated closed reduction and stabilisation with K-wires. Three years later movement was painless and slightly restricted despite persisting palmar dislocation of the lunate and triquetral bones as well as ulnar translocation.

Annales de chirurgie de la main et du membre superieur : organe officiel des societes de chirurgie de la main = Annals of hand and upper limb surgery1993fre
The authors report an exceptional case of retrolunate dislocation of the carpus associated with a Schernberg type III scaphoid fracture and a transverse fracture of the triquetrum at the level of its median crest, avulsing all of the upper third of this bone. That describe the emergency treatment and the radiological and operative findings.

INTRODUCTION: A transscaphoid and transtriquetral perilunate fracture dislocation is fairly rare among the known cases of perilunate fracture dislocations, and the details of the initial treatment and outcome of this injury have never been reported.
MATERIALS AND METHODS: A 21-year-old, right-handed man presented with fractures at the proximal third of the scaphoid and at the mid-body of the triquetrum with an associated dorsal perilunate dislocation after a fall onto his outstretched hand. Under general anesthesia, closed reduction was attempted with 3 kg of traction applied by means of fing

A case of closed anterior dislocation of the triquetrum is reported. Because of delayed diagnosis the patient was not operated until four months after the injury, when the bone was excised. At follow-up 19 months later the power of grip was slightly reduced but the movements were unimpaired and the hand was painfree.

PURPOSE: Treatment options for advanced stage Kienböck disease primarily involve lunate excision, either in the context of proximal row carpectomy or in association with partial wrist fusion. This study aimed to investigate the in vivo three-dimensional carpal kinematics following arthroscopic lunate excision.
METHODS: We analyzed carpal translation and rotation of the affected wrist in five patients who underwent arthroscopic lunate excision for stage IIIC Kienböck disease. Our dynamic computed tomography image-based analysis focused on determining the centroid position of the scaphoid, triq
