Stop Asking How Much Information Your Patient Wants Before Discussing Serious News.

Discussing serious news is a fundamental communication skill, and many clinicians have been taught to ask their patients how much detail they want to hear before sharing difficult information. Over the past decade, we have taught hundreds of medical students how to discuss serious news and reviewed hundreds of their recorded conversations. We've found that asking how much detail a patient wants to hear often results in confusion and is not an effective way to understand their communication preferences. Instead of asking how much detail your patient wants to hear, we propose an alternative way

When lying changes memory for the truth.

In the legal field, victims and offenders frequently lie to avoid talking about serious incidents, such as past experiences of sexual abuse or criminal involvement. Although these individuals may initially lie about an experienced event, oftentimes these same people eventually abandon their lies and are forthcoming with what truly happened. To date, it is unclear whether such lying affects later statements about one's memory for the experienced event. The impetus of the present review is to compile the current state of knowledge on the effects of lying on memory. Based on existing literature,

[Neonatal pseudohypoaldosteronism: when a denied truth can delay a diagnosis].

A child with neonatal pseudohypoaldosteronism is referred. The diagnosis was delayed and complicated as her parents didn't tell us that her sister had been affected by the same illness. The child was born after premature rupture of membranes at 34 weeks. At birth, her conditions were satisfactory. The general conditions of the child gradually worsened and, though she regularly ate human pastorized bank milk, she didn't gain any weight. After 13 days she appeared dehydrated with marbled dry skin and haloed eyes, hypotonic and hyporeflexic. Her suction became more and more weaker, with frequent

Introduction to special issue. Careful conversations: adolescents managing their parents' access to information.

Stattin and Kerr's (2000; Kerr & Stattin, 2000) seminal work challenged our understanding of parental monitoring, shifted attention to adolescents' agency as information managers, and pushed researchers to focus more on their measures and to think more about the interactional and relational processes that keep, or fail to keep, parents informed. Spurred by this reinterpretation of "parental monitoring", research has shifted in the last decade from a nearly exclusive focus on parents' role in socializing adolescents through monitoring their whereabouts, friendships, and activities, to a broader

"Stop Waking the Dead": Internet Child Sexual Abuse and Perspectives on Its Disclosure.

The phenomenon of Internet child sexual abuse (ICSA) has been receiving growing attention over the last decade, and studies have promoted knowledge with respect to the phenomenon's epidemiology, as well as to characteristics of the victims, perpetrators, and dynamics in these cases. The current retrospective study sought to delve into the disclosure component in cases of ICSA. The sample comprised 52 cases of adolescents who arrived at a child advocacy center (CAC) following ICSA. Analysis of these cases was targeted to capture the multifaceted nature of disclosure using those perspectives tha

Disclosing the diagnosis of terminal-stage cancer to patients: attitudes amongst UAE healthcare providers and universities' students: A cross-sectional study.

Disclosing a cancer diagnosis, particularly to individuals in family-centric cultures, is a complex process that directly impacts patient-centered care. This study explores the factors that influence attitudes toward truth disclosure of a terminal cancer diagnosis in the diverse population of the United Arab Emirates and compares these findings with practices in other nations and regions. A descriptive cross-sectional study was conducted using an online self-administered questionnaire. The questionnaire comprised 4 sections: 9 questions collected demographic data, 6 questions assessed responde

Personal and Network-Related Factors Associated to Diagnosis Disclosure Reactions for Children and Adolescents Living with HIV.

The process of human immunodeficiency virus (HIV) diagnosis disclosure for vertically infected young people living with HIV has proven decisive for acceptance/adherence to treatment. Herein, we present a cross-sectional study aiming at evaluating how individual and network related variables are associated with reactions to HIV disclosure among them. We used the egocentric approach with a structured questionnaire applied to individuals aged 15-25 years in an HIV referral center in Rio de Janeiro, Brazil. Outcome variable referred to adoption or not of risk behavior after diagnostic disclosure,

[Ethics and giving bad news to a patient with Alzheimer's].

Breaking bad news to a patient with Alzheimer's often raises ethical questions: should the person be informed or not when there is a risk he or she will not understand or retain the explanations provided? While on the face of it the truth seems desirable, it is important to carefully analyse what is at stake as this truth can potentially harm a vulnerable patient. The person making the disclosure can find themselves at the centre of a conflict: on the one hand loyalty and a moral duty to inform, on the other a compassionate lie to protect the patient from upsetting news. Being aware of the eth

How parents experience their adolescent's disclosure of previous sexual abuse: a qualitative study.

INTRODUCTION: Parents whom adolescents disclose sexual abuse face both a personal traumatic experience, and the need to support their child who is going through a grueling period and needs them. Many quantitative studies exploring the psychological impact of disclosure on parents have been conducted, but few have used qualitative methods. The objective of this study is to explore parents' experiences of their adolescent's disclosure of sexual abuse during psychiatric care, identify the possible beneficial factors and shortcomings, share this knowledge, and improve interventions for these famil

"It's a secret between us": a qualitative study on children and care-giver experiences of HIV disclosure in Kinshasa, Democratic Republic of Congo.

BACKGROUND: It is estimated that 64,000 children under 15 years of age are living with HIV in the Democratic Republic of Congo (DRC). Non-disclosure - in which the child is not informed about their HIV status - is likely to be associated with poor outcomes during adolescence including increased risk of poor adherence and retention, and treatment failure. Disclosing a child's HIV status to them can be a difficult process for care-givers and children, and in this qualitative study we explored child and care-giver experiences of the process of disclosing, including reasons for delay. METHODS: A

Social desirability response set and systematic distortion in the self-report of adult male gender patients.

This study showed that the "socially desirable" presentation for a heterosexual male gender dysphoric is one that emphasizes traits and behaviors characteristic of "classic" transsexualism. Fifty-one homosexual and 64 heterosexual adult male gender patients were administered the Crowne-Marlowe (1964) Social Desirability Scale as well as eight questionnaire measures that tapped various features of the clinical history commonly given great weight in differential diagnosis. The tendency for a heterosexual subject to describe himself in terms of moral excellence or admirable personal qualities was

Factors associated with delayed disclosure of child sexual abuse: A focus on the victim's having been led to perform sexual acts on the perpetrator.

BACKGROUND: Contextual variables associated with children's willingness to disclose sexual abuse have been identified in previous studies, but further investigation is needed to better understand delayed disclosure of child sexual abuse (CSA). OBJECTIVES: This study aimed to examine the factors associated with the risk of delayed disclosure of CSA. More specifically, it focused on a factor that has rarely, if ever, been looked into: the fact that some victims are led to perform sexual acts on the perpetrator. PARTICIPANTS AND SETTING: A content analysis was performed on 68 transcripts of inv

"We will tell when we are ready": perinatally HIV-infected adolescents and self-disclosure of their status in Eswatini.

Access to antiretroviral therapy (ART) has promoted a significant decrease in mortality of vertically HIV-infected children. As a result, there has been an increasing growth of this population that reaches adolescence. These adolescents face problems such as self-disclosure and the stigma of the disease. This study aimed to determine the process followed by perinatally HIV-infected adolescents in self-disclosing their HIV status to significant others and the barriers and promoters of perinatally HIV-infected adolescents' disclosure of their HIV status to others. Data were collected from 15-19-

What's needed next: a culture of candor.

If there's one thing that the past decade's business disasters should teach us, it's that we need to stop evaluating corporate leaders simply on the basis of how much wealth they create for investors. A healthier yardstick would be this: the extent to which leaders create firms that are economically, ethically, and socially sustainable. The first step toward accomplishing that task is to create a culture of candor. Companies can't innovate, respond to stakeholder needs, or run efficiently unless the people inside them have access to timely, relevant information, point out professors O'Toole, o
