Ulnar nerve injuries are common in young people. When there is a delay in the diagnosis and surgical management, the results are usually poor and can be permanent. Nerve grafting procedures were carried out in 85 patients with ulnar nerve injuries and 30 patients with both ulnar and median nerve injuries between 1981 and 1992. The patients could be divided into three categories according to the level of the trauma. Group A with injuries in the carpal area consisted of 43 cases with ulnar nerve injuries and 30 cases with both ulnar and median nerve injuries. Group B comprised 25 patients with u

The ulnar neuropathy in bicycle riders is a less common occurrence, due to a local damage of the nerve on the level of the ulnar tunnel. It has been described in neurological literature only a few times, and hence its good prognosis is not sufficiently known. Within a short time two patients of this disease were transferred to our clinic for operation. After a local space-occupying growth in the tunnel could be excluded by MR or sonography of the wrist, we discouraged performance of an operative procedure. The patients' complaints and the neurological deficits vanished in a few weeks. Ulnar ne

OBJECT: The palmar cutaneous branch of the ulnar nerve (PCUN) has received little attention in the literature, and to the authors' knowledge, has received no attention in the neurosurgical literature. The present study was performed to help the surgeon minimize postoperative complications of nerve decompression at the wrist.
METHODS: Forty cadaveric upper limbs underwent dissection of the ulnar nerve in the forearm, at the wrist, and in the palm. The PCUN was investigated and when identified, measurements were made and relationships documented between this cutaneous branch and the ulnar arter

Seven patients with tardy ulnar nerve palsy from a posttraumatic cubitus varus deformity were reviewed retrospectively. The severity of symptoms was grade I in 3 patients and grade II in 4 patients according to McGowan's classification. The mean internal rotation angle was 30.7 degrees (range, 25 degrees -45 degrees ). The most prominent feature was dislocation of the nerve anterior to the medial epicondyle and entrapment of the nerve by the fibrous band of the flexor carpi ulnaris muscle. Of these 7 patients, 4 were treated by 3-dimensional osteotomy with ulnar nerve transposition, and 3 were

A 20-year-old right-handed fisherman experienced a severe injury in a high-energy traffic accident. These injuries included a shaft fracture of the right humerus, open comminuted fracture of the right forearm and multiple peripheral nerve palsies involving the musculocutaneous, median and radial nerves. Six months after the initial treatment, the patient was referred to our hand surgeons for further treatment. Several operations for functional reconstruction using only the ulnar nerve and ulnar nerve-innervated muscles were performed: the main procedures included a partial ulnar nerve transfer

Presentation of ulnar nerve entrapment at the wrist varies based on differential anatomy and the site or sites of compression. Therefore, an understanding of the anatomy of the Guyon canal is essential for diagnosis in patients presenting with motor and/or sensory deficits in the hand. The etiologies of ulnar nerve compression include soft-tissue tumors; repetitive or acute trauma; the presence of anomalous muscles and fibrous bands; arthritic, synovial, endocrine, and metabolic conditions; and iatrogenic injury. In addition to a thorough history and physical examination, which includes motor,

Two cases of compression of the ulnar nerve by ganglions in the distal ulnar tunnel are reported. The type of neurologic deficit (motor, sensory, or both) was helpful in predicting both the zone of nerve compression and the nature of the compressing lesion.

A review of 53 dissections associated with injections and radiographs was performed (by the authors) to specify the arteriovenous vascularization of the ulnar nerve of the arm. An arteriovenous pedicle is constantly to be found. The artery "collateralis ulnaris superior" arises from the inferior border of the "pectoralis major" at an average distance of 6,7 cm. The arterial depending vein most often directly rejoins the "vena brachialis" lower than "vena basilica" does, but usually more distally than the arterial emergence level. Thus constituted, the pedicle can be anastomosed, measuring gene

OBJECTIVE: To determine the accuracy of the ultrasonographic measurement of ulnar nerve to cubital tunnel area for diagnosis of ulnar neuropathy at the elbow.
DESIGN: Patients with confirmed ulnar neuropathy at the elbow and normative, healthy volunteers were evaluated with high-resolution ultrasound. The cross-sectional areas (CSAs) of the ulnar nerve and cubital tunnel were measured with the elbow extended and flexed, and results from the 2 groups were compared.
SETTING: Electromyography laboratory and radiology department of a tertiary care center.
PARTICIPANTS: Twenty-seven patients wit

We carried out a prospective randomised study comparing medial epicondylectomy with anterior transposition for the treatment of ulnar neuropathy at the elbow. The mean follow-up period was 4.5 years and we assessed the patients neurologically and orthopaedically. Neither procedure appeared to have a significant effect on elbow function. Our study showed better results after medial epicondylectomy; in particular patient satisfaction was higher than after ulnar nerve transposition. There were no significant differences in motor power or nerve-conduction rates and sensory fibres appeared to be mo

CONTEXT: Acromegalic patients may complain of sensory disturbances in their hands. Cubital tunnel syndrome, the ulnar nerve neuropathy at the cubital tunnel (UCT), in acromegalic patients has never been reported.
OBJECTIVE: To describe and assess the prevalence of UCT in acromegalic patients and the effects of 1 year of therapy on UCT.
PATIENTS: We examined prospectively 37 acromegalic patients with no history of polyneuropathy, acute trauma at the elbow, no diabetes or hypothyroidism with clinical examination, nerve conduction studies (NCS), and high-resolution ultrasound (US). A control gr

Twenty unembalmed intact cadavers had strain measurements taken of the ulnar nerve at 5 positions around the elbow and at varying degrees of elbow flexion. A microstrain gauge was implanted directly into the nerve through a superficial incision. Maximum strains in the ulnar nerve occurred at maximum elbow flexion and directly behind the medial epicondyle; meaningfully less strain was seen at lesser degrees of flexion and positions distal and proximal to the epicondyle. A wide range of maximal strains (0% to 14%) was found. Twenty-five percent of the ulnar nerves showed strains of greater than

BACKGROUND: Tardy ulnar nerve palsy is a common late complication of traumatic cubitus valgus deformity. Whether both problems can be corrected together, safely and effectively, in a single surgical procedure remains unknown.
QUESTIONS/PURPOSES: We therefore reviewed a patient cohort having this combined surgery and compared preoperatively and at a minimum of 24 months postoperatively (1) active elbow ROM; (2) radiographic correction of the cubitus valgus deformity of the preoperative and postoperative humerus-elbow-wrist angles and the medial prominence index; (3) ulnar nerve function throug

INTRODUCTION: We investigated the outcomes of patients with severe ulnar nerve compression who underwent anterior interosseous nerve transfer combined with cubital and ulnar tunnel release.
METHODS: Twenty-eight patients with McGowan IIB or III ulnar neuropathy caused by compression and with compound muscle action potentials of the ulnar intrinsics less than 3 mV were prospectively recruited for outcome analysis. Clinical signs of ulnar palsy, two-point discrimination tests, gripping and pinching force and electrophysiological studies were recorded over a 2 year follow-up.
RESULTS: Significa

A fracture of the distal radius with an associated injury to the ulnar nerve is rare. The management of the ulnar nerve lesion is unclear. We present a patient with a closed distal radius fracture related to an injury to the ulnar nerve associated with diminished sensation and a claw deformity. This was assessed by ultrasonography (US) that showed the nerve to be in continuity without any evidence of compression. The nerve was deviated towards the volar side at the distal end of the ulna and was enlarged at the same point. Open reduction and internal fixation was performed for the fracture. Em
