[Compression syndromes of peripheral nerves of the upper extremity (author's transl)].

Among the non-traumatic lesions of peripheral nerves the compression syndromes occupy a numerically leading position. After mentioning the common characteristics the individual compression syndromes of the brachial plexus and peripheral nerves of the upper extremity are described: the compression of the brachial plexus in the posterior triangle of the neck, in the costoclavicular passage and on hyperabduction. The suprascapular notch syndrome ist mentioned. The distal compression syndrome of the median nerve, the carpal tunnel syndrome is described in detail as the most common of the compressi

Ulnar tunnel syndrome.

Ulnar tunnel syndrome could be broadly defined as a compressive neuropathy of the ulnar nerve at the level of the wrist. The ulnar tunnel, or Guyon's canal, has a complex and variable anatomy. Various factors may precipitate the onset of ulnar tunnel syndrome. Patient presentation depends on the anatomic zone of ulnar nerve compression: zone I compression, motor and sensory signs and symptoms; zone II compression, isolated motor deficits; and zone III compression; purely sensory deficits. Conservative treatment such as activity modification may be helpful, but often, surgical exploration of th

The Simultaneous Compression of the Median and Ulnar Nerves by Lipoma: A Case Report and Review of the Literature.

Entrapment syndromes in the hand are very common and usually idiopathic. However, it can be the expression of nerve compression by a nearby structure or tumor. Space-occupying lesions are widely reported as a cause of median or ulnar nerve compression in the wrist. Nevertheless, a simultaneous compression of the median and ulnar nerves by a tumor is a rare condition. To the best of our knowledge, only three cases are described in the English literature. Herein, we present a case of a simultaneous compression in the wrist of the median and ulnar nerves by lipoma in a 79-year-old patient while a

Possible points of compression of the ulnar nerve: Tricks and traps that await clinicians from an anatomical point of view.

The ulnar nerve can be subject to numerous types of compression. The most common are cubital tunnel and ulnar tunnel syndromes, but there are many others with more uncommon etiologies. The existence of additional communicating branches, median nerve involvement, various types of injuries, and unusual anatomical variations can be a challenge for both diagnosis and treatment. This review presents a comprehensive depiction of ulnar nerve entrapment syndromes with particular reference to their anatomical background, risk factors, and clinical evaluation. Even common disorders can result from atypi

Nerve compression syndromes of the hand and forearm associated with tumours of non-neural origin and tumour-like lesions.

Nerve compression syndromes caused by non-neural tumours or tumour-like lesions are rare. We retrospectively reviewed 541 consecutive patients operated on by the same surgeon to study nerve compression syndromes in the forearm and hand. There were 414 due to nerve compression and 127 caused by tumours. Twenty-two patients showed compression neuropathy associated with 17 tumours and six tumour-like lesions, with 13 different pathological types. The most common types were fatty and vascular tumours. Twenty-one tumours were extraneural and one was intraneural. The median nerve was affected in nin

Ulnar Tunnel Syndrome, Radial Tunnel Syndrome, Anterior Interosseous Nerve Syndrome, and Pronator Syndrome.

In addition to the more common carpal tunnel and cubital tunnel syndromes, orthopaedic surgeons must recognize and manage other potential sites of peripheral nerve compression. The distal ulnar nerve may become compressed as it travels through the wrist, which is known as ulnar tunnel or Guyon canal syndrome. The posterior interosseous nerve may become entrapped in the proximal forearm as it travels through the radial tunnel, which results in a pain syndrome without motor weakness. The median nerve may become entrapped in the proximal forearm, which can result in a variety of symptoms. Spontan

Ulnar nerve entrapment at the wrist.

Presentation of ulnar nerve entrapment at the wrist varies based on differential anatomy and the site or sites of compression. Therefore, an understanding of the anatomy of the Guyon canal is essential for diagnosis in patients presenting with motor and/or sensory deficits in the hand. The etiologies of ulnar nerve compression include soft-tissue tumors; repetitive or acute trauma; the presence of anomalous muscles and fibrous bands; arthritic, synovial, endocrine, and metabolic conditions; and iatrogenic injury. In addition to a thorough history and physical examination, which includes motor,

Cubital tunnel syndrome pathophysiology.

Cubital tunnel syndrome is the second most common peripheral compression neuropathy. The unique anatomic relationships of the ulnar nerve at the elbow place it at risk for injury. Normally with elbow range of motion, the ulnar nerve is subjected to compression, traction, and frictional forces. As the elbow is flexed the arcuate ligament elongates producing a decrease in canal volume of 55%. Intraneural and extraneural pressures increase and have been shown to exceed 200 mm Hg with elbow flexion and flexor carpi ulnaris contraction. Because the ulnar nerve courses behind the elbow axis of rotat

[Narrow tunnel and compression syndromes of the hand].

In osteofibrous channels of the hand a disproportion of capacity and contents may cause either a restriction of the gliding of the tendon or a lesion of nerves by pressure. During a 5 year period 362 narrow pass and compression syndromes were operated. Trigger fingers were most frequent with 224 cases, followed by 85 de Quervain's diseases, 49 carpal tunnel syndromes and 4 ulnar nerve compression syndromes. These diseases can be operated in a bloodless field and under regional anaesthesia, mostly under outpatient conditions. The manifold reasons of compression syndromes call for differentiated

Cubital tunnel syndrome: Anatomy, clinical presentation, and management.

Cubital tunnel syndrome is the second most common peripheral nerve compression seen by hand surgeons. A thorough understanding of the ulnar nerve anatomy and common sites of compression are required to determine the cause of the neuropathy and proper treatment. Recognizing the various clinical presentations of ulnar nerve compression can guide the surgeon to choose examination tests that aid in localizing the site of compression. Diagnostic studies such as radiographs and electromyography can aid in diagnosis. Conservative management with bracing is typically trialed first. Surgical decompress

Necrotic Granulomatous Inflammation after Use of Small Intestine Submucosa Matrix for Recurrent Compression Neuropathy.

Various techniques exist for treating recurrent carpal and ulnar tunnel syndrome, but AxoGuard nerve wrap has shown promising results for treatment of compression neuropathies when used in conjunction with neurolysis and tenosynovectomy. Prior results demonstrate no safety concerns, and there have not been any reported cases of infection, persistent inflammation, or recurrent perineural fibrosis. A 41-year-old, right-hand-dominant woman experienced repeated bouts of carpal and ulnar tunnel syndromes, which were treated with a small intestine submucosa matrix wrap around the median and ulnar ne

Ulnar nerve entrapment at the wrist: cases from a hand surgery practice.

BACKGROUND: Eight patients with 11 instances of wrist-level ulnar nerve entrapment, a fairly uncommon compression syndrome, were identified in a hand surgery practice from 1992 through 1996. METHODS: Presentations, causes, and surgical outcomes were examined, and the pertinent literature was reviewed. RESULTS: All eight patients had extrinsic, nonidiopathic compression of the ulnar nerve caused by tumor, vascular disease, anomalous muscle development, or a tight fibrous arch at the origin of the flexor digiti minimi. In all cases, sensory symptoms resolved with removal of the cause of ulnar

Subcutaneous transposition of the ulnar nerve for treatment of cubital tunnel syndrome.

Subcutaneous transposition of the ulnar nerve has been widely reported as a successful surgical treatment for ulnar neuropathy at the elbow attributable to a variety of causes. Accepted indications for anterior transposition include any anatomic lesion that interferes with or impinges on the nerve along its native course. This may include a tumor, ganglion, osteophyte, valgus deformity or instability, or subluxation of the nerve, as listed previously. The surgical technique of ASCT also was described thoroughly earlier in this article. Points that warrant emphasis include thorough decompressio

Ulnar nerve entrapment in Guyon's canal due to recurrent carpal tunnel syndrome: case report.

Guyon's canal syndrome is a compression neuropathy of the ulnar nerve entrapment at the wrist. Compression of the ulnar nerve at the wrist by a ganglion, lipomas, diseases of the ulnar artery, fractures of the hamate and trauma are common etiologcal factors. Unlike Guyon's canal syndrome, carpal tunnel syndrome (CTS) is the most common nerve entrapment of the upper extremity. Although, open (OCTR) or endoscopic carpal tunnel release (ECTR) is highly effective in relieving pain, failure with carpal tunnel release is seldom seen. In this paper, we presented a patient with ulnar nerve entrapment

Magnetic resonance imaging of sports injuries of the elbow.

Many abnormalities seen in the elbow result from trauma, often from sports such as baseball and tennis. Elbow problems are frequently related to the medial tension-lateral compression phenomenon, where repeated valgus stress produces flexor-pronator strain, ulnar collateral ligament sprain, ulnar traction spurring, and ulnar neuropathy. Lateral compression causes osteochondral lesions of the capitellum and radial head, degenerative arthritis, and loose bodies. Other elbow abnormalities seen on magnetic resonance imaging include radial collateral ligament injuries, biceps and triceps tendon inj

Wrist and hand overuse syndromes.

Overuse syndromes in the wrist or hand can occur from repetitive use of the wrist and hand or from recurrent direct trauma to the hand area. Tendinitis syndromes due to overstretching or shear stress are seen commonly on both the extensor and flexor sides of the wrist. Overuse syndromes also can take the form of neurovascular syndromes, resulting in compression syndromes of the median, ulnar, and superficial branch of the radial nerve in the wrist area and trauma to the ulnar and digital vessels supplying the hand. Treatment in most cases involves rest with splinting, icing, and NSAIDs in acut

Nerves of the hand beyond the carpal tunnel.

Imaging studies including ultrasound (US) and magnetic resonance imaging may be required to evaluate the median nerve in patients with suspected carpal tunnel syndrome. However, the radial and ulnar nerves contribute to sensory and motor innervations to the hand as well. Compressive, traumatic, and iatrogenic events may damage the small terminal branches of these nerves. In the hand, US is able to identify injuries of the median, ulnar, radial nerve, and terminal branches. This article presents the role of imaging to evaluate the nerves of the hand with an emphasis on US. Due to its high-resol
