The clinical significance of antenatal detection of discordant umbilical arteries.

OBJECTIVE: To evaluate the clinical significance of the antenatally detected discordant umbilical arteries (UAs). METHODS: Women with singleton gestations undergoing sonographic evaluations were examined for the presence of discordant UAs. Transverse and longitudinal diameters as well as the area of both UAs were measured. Doppler flow velocity waveforms were recorded from both arteries. Macroscopic and microscopic examination of the umbilical cord was performed after delivery and the area of each artery was measured. Mann-Whitney U test and Spearman rank correlation were used for statistical

Comparative study of umbilical cord cross-sectional area in foetuses with isolated single umbilical artery and normal umbilical artery.

Single umbilical artery (SUA) is one of the most common prenatal diagnoses in cases of foetal abnormality. This prospective study evaluated 77 foetuses with isolated SUAs and 77 healthy foetuses, both at 22-39 gestational weeks. We categorised gestational age into the second and third trimesters, measured the umbilical arterial blood flow parameters and calculated the umbilical vein (UV) area, umbilical artery (UA) area and UV area/UA area ratio. In the second and third trimesters, a higher UA area was obtained in the isolated SUA group than in the control group (p < .01). Furthermore, the iso

[Single umbilical artery (SUA)].

Single umbilical artery (SUA) is the most common abnormality of the umbilical cord. It is associated with an increased incidence of atresia of hollow organs, renal abnormalities, limb reduction defects and spontaneous abortions. The aim of our study is to establish the association of SUA with types of malformations and the corresponding pathology of pregnancy and parturition. Although our investigation is in proof of the association of SUA with anomalies of GIT, CCS and CNS, most of our cases exhibit circulatory disturbances leading to pathology of pregnancy and delivery.

The significance of assessing the umbilical cord on the placental side: A case report on fused umbilical arteries.

This report details the case of a fetus with fused umbilical arteries (FUAs), which exhibited the presence of a single umbilical artery near the site of placental insertion, as well as normal two umbilical arteries at the fetal end of the umbilical cord. In our case, as the pregnancy progressed, there was an occurrence of late-onset fetal growth restriction and oligohydramnios. This is a possibility that FUAs might impact the development of the fetus.

From dual to single umbilical artery: a case of umbilical artery thrombosis with hypercoiling and literature review.

Umbilical artery thrombosis (UAT) is an extremely rare but severe obstetric complication associated with adverse perinatal outcomes, including fetal growth restriction (FGR), fetal distress, and intrauterine fetal demise. This case report highlights the diagnostic challenges of UAT and its potential misdiagnosis as a single umbilical artery (SUA). A 32-year-old woman with a history of uncomplicated vaginal delivery was initially misdiagnosed with SUA at 29 3/7 weeks of gestation. At 32 1/7 weeks, detailed ultrasound examination revealed a single umbilical artery, FGR, and hypercoiling. Followi

Risk factors and adverse fetal outcomes in umbilical artery thrombosis: a comparison with single umbilical artery.

BACKGROUND: Single umbilical artery (SUA) and umbilical artery thrombosis (UAT) are associated with adverse fetal outcomes, yet comparative data on their risk factors and clinical implications are limited. This study aims to evaluate the risk factors and their impact on pregnancy outcomes associated with SUA and UAT, and to assess the applicability of SUA management strategies to UAT. METHODS: This retrospective cohort study investigated pregnancies diagnosed with SUA and UAT at a tertiary hospital from 2017 to 2021. Medical history, birth details, and diagnoses were collected from medical re

The impact of different sides of the absent umbilical artery on fetal growth in an isolated single umbilical artery.

PURPOSE: The aim of this study was to determine whether laterality of an absent umbilical artery (AUA) is associated with fetal growth in fetuses with isolated single umbilical artery (SUA). METHODS: Fifty singleton pregnancies were studied, including 26 cases with a right AUA and 24 cases with a left AUA in isolated SUA, and 200 singleton pregnancies with a three-vessel cord. Delivery data, including gestational age and birth weight and height, were recorded. Compare the birth weight and height in fetuses between the different sides of an AUA and the three-vessel cord by covariance analysis.

Volumetric umbilical artery blood flow: comparison of the normal versus the single umbilical artery cord.

OBJECTIVE: This was a study of the volumetric blood flow in single umbilical artery (SUA) cords as compared to three-vessel cords. HYPOTHESIS: SUA flow will be twice that of an artery in a normal cord. METHODS: We studied 276 patients (24 SUA, 252 normal cord) at 18-40 weeks' gestation utilizing gray-scale and color Doppler. Flow, flow/kg, velocity, artery diameter, Doppler velocimetry indices, estimated fetal weight (EFW) and amniotic fluid index (AFI) were compared. All fetuses were anatomically and cytogenetically normal. RESULTS: Blood flow increased with advancing gestation and the SUA

The cloacal outgrowth orchestrates co-development of the bladder and umbilical arteries.

The urinary bladder is an innovation of eutherian mammals to manage liquid waste. However, bladder development and its embryonic function remain poorly understood. Here, we report that in both humans and mice the bladder emerges from the cloaca as a rostral outgrowth, which is flanked by the umbilical arteries. Arrest of the outgrowth, as seen in Shh-null mouse embryos, causes bladder agenesis and unexpected severe defects of the umbilical arteries, suggesting that the outgrowth epithelial signal Shh coordinates development of both the bladder and the umbilical arteries. The signaling molecule

PDE4 and PDE5 regulate cyclic nucleotides relaxing effects in human umbilical arteries.

Cyclic nucleotides (cAMP and cGMP) are the main second messengers linked to vasodilatation. They are synthesized by cyclases and degraded by different types of phosphodiesterases (PDE). The effect of PDE inhibition and cyclases stimulation on 5-hydroxytryptamine (5-HT; 1 microM) and histamine (10 microM) contracted arteries was analysed. Stimulation of guanylate cyclase or adenylate cyclase relaxed the histamine- and 5-HT-induced contractions indicating that intracellular increase of cyclic nucleotides leads to vasodilatation of the human umbilical artery. We investigated the role of different

Pharmacological analysis of vasoconstrictor responses of isolated and perfused human umbilical arteries.

Using the cannula inserting method, we studied vascular responses of isolated human umbilical arteries to several vasoactive substances. ACh did not produce a vasodilation in non-constricted preparations but induced only a vasoconstriction. Histamine and 5HT produced strong vasoconstrictions in a dose-dependent manner. Epinephrine and norepinephrine in large doses induced only a slight vasoconstriction. The ACh-induced vasoconstriction was markedly suppressed by atropine and slightly, but significantly, suppressed by methylsergide. The vascular responses to ACh were not influenced by removal o

Vasoactive actions of local anaesthetics on human isolated umbilical veins and arteries.

1. An in vitro study, using helical preparations of human umbilical arteries and veins obtained from healthy women at term pregnancy, was designed to determine: (a) whether three local anaesthetics commonly utilized in obstetric anaesthesia (bupivacaine, 2-chloroprocaine, and lignocaine) can induce contraction or relaxation of resting umbilical vessels; (b) whether these agents can induce contraction or relaxation of umbilical vessels which have been previously induced to contract by a known activator, potassium chloride (KCl); and (c) the relative potency of these agents as compared to KCl. 2

Left versus right intra-abdominal umbilical arteries: comparison of their Doppler waveforms.

OBJECTIVES: The purpose of this study was to compare Doppler velocimetric indices between the left and right intra-abdominal umbilical arteries in normally grown and growth-restricted fetuses. METHODS: In this cross-sectional study, Doppler indices of the intra-abdominal segment of the left and right umbilical arteries were measured lateral to the fetal bladder. Measurements, including the systolic to diastolic ratio and pulsatility index, were obtained from both anatomic locations. The Student t test was used to make comparisons. RESULTS: Differences between left and right Doppler indices w

Pre-eclampsia-associated differential expression of proteoglycans in the umbilical cord arteries.

OBJECTIVE: The role of proteoglycans (PGs) of the umbilical cord arteries (UCAs) in the pathomechanism of pre-eclampsia is not known. Therefore we decided to compare the PGs of normal (control) UCAs and those of newborns delivered by mothers with pre-eclampsia. METHODS: PGs were extracted in dissociative conditions, purified by Q-Sepharose anion exchange chromatography and lyophilized. They were analyzed by gel filtration and SDS-PAGE before and after treatment with chondroitinase ABC. RESULTS: It was found that the PG preparation from pre-eclamptic UCAs had a higher amount of sulphated glyc
