Child and Adolescent All-Cause Mortality in the United States vs Peer Nations: 1935-2023.

BACKGROUND: Children in the United States experience higher all-cause mortality than high-income peer nations, but the timing, durability, and age and sex contributions to this gap remain unclear. OBJECTIVES: To characterize the timing, persistence, and age- and sex-specific patterns of the US child and adolescent mortality disadvantage relative to peer nations. METHODS: Time-series comparison using vital statistics from the Human Mortality Database (1935-2023), comparing all-cause mortality among those aged 0 to 19 years in the United States vs those living in 13 high-income peer nations fr

Case of severe acute hepatitis A virus infection in United Nations peacekeepers in South Sudan.

Hepatitis A virus (HAV) is a major cause of acute viral hepatitis, which can lead to severe cases and acute liver failure. A 41-year-old Chinese military officer of the United Nations peacekeeping forces in South Sudan manifested fever, jaundice and coagulation dysfunction. The patient initially had a negative anti-HAV IgM antibody test result but a positive anti-HAV-IgM antibody test result five days after his alanine aminotransferase (ALT) levels peaked. He was given telemedicine consultation and was transferred to a Chinese military hospital specialising in infectious diseases for treatment

[Hepatitis C in prisons : Treatment and barriers to the elimination goals of the United Nations].

BACKGROUND: Despite general advances in the treatment of people with hepatitis C in Germany, it is unclear to what extent this also applies to specific key populations, such as prisoners. METHODS: In a short survey conducted in June 2021, the ministries of justice in the federal states were asked about data collection and diagnostic methods as well as the prevalence of infections caused by the hepatitis C virus (HCV) and HCV treatment among prisoners in Germany. In addition, expert interviews were conducted regarding HCV treatment barriers and missed opportunities in prisons. RESULTS: It is

Oral health problems and treatment needs in Danish military personnel recruited for United Nations service.

A group of 223 men from the Danish Army designated to serve in the United Nations forces in the former Yugoslavia were examined to determine their oral health status and estimate their needs for dental treatment and the related dental treatment time. The population studied consisted of privates (63%), noncommissioned officers (28%), and officers (9%). About 80% of the population was younger than 28 years. Among the persons older than 27 years, 29% had not consulted a dentist within the past 3 years. Subjective symptoms were recorded in 19% of the study population. The average number of teeth p

Epidemiological experience in the mission of the United Nations Transition Assistance Group (UNTAG) in Namibia.

Epidemiological data have rarely been generated during United Nations (UN) missions to Third World countries, even in situations where there is hardly any combat involvement. Continuous surveillance was therefore carried out during the 12-month stay of UN personnel in Namibia in 1989-90. In this population of 7114 persons, mostly young men, the mortality rate was 255 per 100,000; death was mainly due to traffic accidents. Hospitalization was chiefly because of fever of unknown origin or trauma. Repatriation to the country of origin was necessary in 46 patients, frequently for psychiatric reaso

International patterns of childhood chronic myeloid leukemia: comparisons between the United States and resource-restricted nations.

BACKGROUND: Chronic myeloid leukemia (CML) is a rare disease in children and represents approximately 2% of all childhood leukemia. This results in difficulty creating large cohorts of patients for pediatric CML research. The Glivec International Patient Assistance Program (GIPAP) is a patient-access program sponsored by Novartis Oncology and administered by The Max Foundation (MAX) that provides imatinib free of charge to patients in resource-restricted countries who are not able to afford this treatment. PROCEDURES: GIPAP highlights a cohort of children (n = 3,188) with CML that provides no

Achieving the United Nations Sustainable Development Goals in Spinal Surgery: Patient-Centered Care and Social Contribution.

BACKGROUND: In recent years, achieving a sustainable society has become an international challenge. The medical field is also being called upon to contribute to the United Nations Sustainable Development Goals (SDGs). However, awareness of the SDGs in Japan is low, and relevant efforts in the healthcare sector are particularly lagging behind. This study aimed to describe the relationship between spinal surgery and the SDGs and to raise awareness regarding it in Japan. METHODS: 1) Literature review: PubMed was used to search for literature using the 2 keywords "spinal surgery" and "SDGs." Addi

The United Nations Material Assistance to Survivors of Cholera in Haiti: Consulting Survivors and Rebuilding Trust.

INTRODUCTION: In August 2016, the United Nations (U.N.) Secretary General acknowledged the U.N.'s role in the cholera epidemic that has beset Haiti since 2010. Two months later, the Secretary General issued a historic apology to the Haitian people before the U.N. General Assembly, for the organization's insufficient response to the cholera outbreak. These steps are part of the U.N.'s "new approach" to cholera in Haiti, which also includes launching a material assistance package for those most affected by cholera. METHODS: This paper draws on the authors' experience and findings from consultat

The United Nations Convention on the Rights of Persons with Disabilities and Social Work: Evidence for Impact?

The United Nations Convention on the Rights of Persons with Disabilities (hereafter CRPD) has provided a radical imperative for the reform of mental health and capacity legislation around the world. The interpretation of the CRPD has been controversial, ranging from the complete abolition of detention, forcible treatment, and substitute decision-making to accepting that elements of these measures need to be retained based on non-discriminatory criteria, additional safeguards, and a comprehensive shift towards supported decision-making. While the potential effects of the CRPD on mental health s

Orthopedic surgery at a MASH deployed to the former Yugoslavia in support of the United Nations Protection Force.

From November 1992 to April 1993, the 212th Mobile Army Surgical Hospital (MASH) was deployed from Germany to Zagreb, Croatia, to provide medical support for the United Nations Protection Force serving in the former Yugoslavia. A 60-bed deployable medical systems hospital was established. The usual MASH 72-hour evacuation policy was extended to 30 days; the orthopedic equipment inventory and surgical capability were increased significantly. Eighty-three orthopedic surgical procedures were performed during a 5-month period on soldiers from 14 nations. Sixty-two (75%) of these procedures were no

Ten years after the United Nations General Assembly Special Session (UNGASS): assessing drug problems, policies and reform proposals.

In 1998 the United Nations General Assembly Special Session resolved that governments would reduce drug production and consumption greatly within 10 years. With that period now elapsed, there is an interest in reviewing how successful this was and considering how drug policy could be improved. The demand for drugs in the world has stabilized mainly as a result of the interaction of epidemic forces, culture and economic development. Supply has become more concentrated and the menu of drugs has changed surprisingly slowly. Drug policy is shifting to a more explicitly tolerant configuration in Eu

Salivary Antibody Responses to Potentially Waterborne and Environmentally Transmitted Infections Among Two Tribal Nations in the Southwest United States.

PURPOSE: Tribal Nations disproportionately lack access to safe drinking water and can be adversely affected by other water quality and environmental concerns. Such conditions could lead to an increase in the transmission of waterborne, environmental and hygiene related infections. We collected saliva samples from attendees at two Tribal Nation annual festivals and tested them for salivary immunoglobulin G (IgG) responses to selected common infections using an in-house multiplex immunoassay. Antibody responses were compared to responses from a previously conducted study in the midwestern United
