At what frequency of vaccination do the vaccinated potentially pose an equal risk to the unvaccinated for transmission of SARS-CoV-2 inside restaurants in New York City?

From August 2021 to 7 March 2022, New York City prohibited indoor dining in restaurants selectively for persons who had not received a Coronavirus disease 2019 (COVID-19) vaccine. However, vaccinated persons may also be actively infected and potentially transmit severe acute respiratory syndrome coronavirus 2 (SARS-CoV‑2). Based on assuming a 7:1 ratio of COVID-19 cases in New York State for the unvaccinated versus the vaccinated, it can be estimated that when 87.5% of adults in New York City are vaccinated, the rate of unsuspected SARS-CoV‑2 infections (asymptomatic or minimally symptomatic)

A longitudinal cohort study of childhood MMR vaccination and seizure disorder among American children.

BACKGROUND: Measles (rubeola) is a highly contagious infectious disease with significant morbidity/mortality. Measles-Mumps-Rubella (MMR) is a live-attenuated vaccine used in the United States (US) to prevent measles. This retrospective longitudinal cohort study evaluated childhood MMR vaccination and the risk of a seizure episode and seizure disorder. METHODS: The Independent Healthcare Research Database (IHRD) composed of records prospectively generated from Florida Medicaid was analyzed using SAS to identify persons continuously enrolled from birth for 120 months. Two cohorts were examined

Breakthrough infections and incomplete vaccine efficacy drive pathogen immune escape.

INTRODUCTION: While vaccines effectively reduce disease severity and mortality, their consequences for pathogen immune escape remain poorly understood. Vaccines provide protection through three distinct mechanisms: prevention of infection, reduction of symptoms, and limitation of transmission. When vaccine efficacy is incomplete across multiple dimensions, breakthrough infections in vaccinated individuals may paradoxically increase pathogen escape pressure. This study investigates how vaccine efficacy profiles and coverage levels jointly affect escape pressure. MATERIALS AND METHODS: We devel

Reemergence of pertussis in the highly vaccinated population of the Netherlands: observations on surveillance data.

We analyzed pertussis reporting, death, hospitalization, and serodiagnostic data from 1976 to 1998 to help explain the cause of the 1996 pertussis outbreak in the Netherlands. The unexpected outbreak was detected by an increase in pertussis reporting and by other surveillance methods. In 1996, according to reporting and serologic data, the increase in pertussis incidence among (mostly unvaccinated) children less than 1 year of age was similar to the increase in hospital admissions. Among older (mostly vaccinated) persons, the increase in hospital admissions was relatively small. The increase i

Vaccination Ameliorates Cellular Inflammatory Responses in SARS-CoV-2 Breakthrough Infections.

BACKGROUND: Data on cellular immune responses in persons with severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection following vaccination are limited. The evaluation of these patients with SARS-CoV-2 breakthrough infections may provide insight into how vaccinations limit the escalation of deleterious host inflammatory responses. METHODS: We conducted a prospective study of peripheral blood cellular immune responses to SARS-CoV-2 infection in 21 vaccinated patients, all with mild disease, and 97 unvaccinated patients stratified based on disease severity. RESULTS: We enrolled

Reduced Odds of Mpox-Associated Hospitalization Among Persons Who Received JYNNEOS Vaccine - California, May 2022-May 2023.

The effectiveness of 1 dose of JYNNEOS vaccine (modified vaccinia Ankara vaccine, Bavarian Nordic) against hospitalization for mpox (caused by Monkeypox virus), has been demonstrated; however, the impact of 2 doses on hospitalization risk, especially among persons infected with HIV, who are at higher risk for severe disease, is an important factor in evaluating vaccine effectiveness against mpox disease severity and Monkeypox virus infection. Surveillance data collected by the California Department of Public Health were used to evaluate whether receipt of 2 doses of JYNNEOS vaccine reduced the

Evaluating risk factors associated with COVID-19 infections among vaccinated people early in the U.S. vaccination campaign: an observational study of five states, January-March 2021.

BACKGROUND: COVID-19 vaccines are an effective tool to prevent illness due to SARS-CoV-2 infection. However, infection after vaccination still occurs. We evaluated all infections identified among recipients of either the Pfizer-BioNTech or Moderna COVID-19 vaccine in five U.S. states during January-March 2021. METHODS: Using observational data reported to CDC, we compared the incidence of SARS-CoV-2 infection among vaccinated and unvaccinated persons, and the sex, age, and vaccine product received for individuals with vaccine breakthrough infections to those of the vaccinated population using

Evaluation of BCG vaccination in Bab El Shaaria zone--Cairo.

137 cases of pulmonary tuberculosis with positive sputum by direct smear and/or culture were studied as regards BCG vaccination. Among these cases the percentage of vaccinated individuals was 23.3% (32 out of 137) and the percentage of unvaccinated individuals was 76.7% (105 out of 137). This may reveal a beneficial effect of BCG. It also shows that the percentage of vaccination in our zone is very low. In our study there were 7% diseased contacts among the vaccinated and 13.8% among the unvaccinated contacts, so BCG was of high protective value. The pulmonary lesions were less extensive among

Measles--United States, January 1-April 25, 2008.

Measles, a highly contagious acute viral disease, can result in serious complications and death. As a result of a successful U.S. vaccination program, measles elimination (i.e., interruption of endemic measles transmission) was declared in the United States in 2000. The number of reported measles cases has declined from 763,094 in 1958 to fewer than 150 cases reported per year since 1997. During 2000-2007, a total of 29-116 measles cases (mean: 62, median: 56) were reported annually. However, during January 1-April 25, 2008, a total of 64 confirmed measles cases were preliminarily reported to

Vaccinations.

Tetanus-diphtheria toxoid, influenza virus vaccines, and pneumococcal vaccine are recommended for older persons in the United States by the Centers for Disease Control. But most high-risk older persons remain unvaccinated, despite experiencing relatively high rates of tetanus, influenza-related complications, and pneumococcal disease, and having available effective, safe, and low-cost vaccines. Strategies for improving these vaccination rates require an intensive approach focused on (1) identifying older persons with high-risk conditions, (2) improving the delivery of vaccines, (3) improving t

COVID-19 Vaccination and Non-COVID-19 Mortality Risk - Seven Integrated Health Care Organizations, United States, December 14, 2020-July 31, 2021.

By September 21, 2021, an estimated 182 million persons in the United States were fully vaccinated against COVID-19.* Clinical trials indicate that Pfizer-BioNTech (BNT162b2), Moderna (mRNA-1273), and Janssen (Johnson & Johnson; Ad.26.COV2.S) vaccines are effective and generally well tolerated (1-3). However, daily vaccination rates have declined approximately 78% since April 13, 2021†; vaccine safety concerns have contributed to vaccine hesitancy (4). A cohort study of 19,625 nursing home residents found that those who received an mRNA vaccine (Pfizer-BioNTech or Moderna) had lower all-cause

An outbreak of yellow fever with concurrent chikungunya virus transmission in South Kordofan, Sudan, 2005.

From September through December 2005, an outbreak of hemorrhagic fever occurred in South Kordofan, Sudan. Initial laboratory test results identified IgM antibodies against yellow fever (YF) virus in patient samples, and a YF outbreak was declared on 14 November. To control the outbreak, a YF mass vaccination campaign was conducted and vector control implemented in parts of South Kordofan. Surveillance data were obtained from the Sudan Federal Ministry of Health. Clinical information and serum samples were obtained from a subset of patients with illness during the outbreak. Cephalic tetanus progressing to generalized tetanus after facial trauma in an unvaccinated elderly patient: A case report.

A 67-year-old unvaccinated male patient was analyzed. Clinical manifestations, laboratory and imaging findings, treatment, and disease progression were evaluated using hospital records. Eight days after facial trauma from a bicycle accident, the patient developed progressive trismus, dysphagia, neck stiffness, and respiratory discomfort. He was hospitalized on day 10 post-injury. Initial treatment included 500 IU of tetanus immunoglobulin. Two days later, his condition deteriorated with progression to generalized tetanus, requiring ICU admission. Additional 2000 IU of tetanus immunoglobulin, s

Cohort studies may underestimate rare vaccine adverse events: evidence from stroke incidence following COVID-19 vaccination.

BACKGROUND: The self-controlled case series (SCCS) design is widely used for short-term vaccine safety assessment but allows only indirect estimation of absolute risk differences. This study evaluated whether absolute measures should be estimated exclusively using cohort designs by comparing excess stroke incidence following COVID-19 vaccination in a nationwide cohort with previous estimates from an SCCS analysis. METHODS: A nationwide retrospective cohort study was conducted in Qatar between 1 December 2020 and 11 April 2023. Stroke incidence during the 42 days following vaccination was comp

Registry Cohort Study to Determine Risk for Multiple Sclerosis after Vaccination for Pandemic Influenza A(H1N1) with Arepanrix, Manitoba, Canada.

To investigate a potential risk for multiple sclerosis (MS) after vaccination with Arepanrix, the GlaxoSmithKline AS03-adjuvanted influenza A(H1N1)pdm09 vaccine, we used the provincewide immunization registry for Manitoba, Canada, to match 341,347 persons vaccinated during the 2009 pandemic to 485,941 unvaccinated persons on age, sex, address, and a propensity score measuring the probability of vaccination. We used a previously validated algorithm to identify MS cases from provincial hospital, physician, and prescription drug claims databases. After 12 months of follow-up, the age-adjusted inc

Pneumococcal vaccine efficacy in selected populations in the United States.

The efficacy of pneumococcal vaccine in groups of patients in the United States at high risk for pneumococcal disease was estimated by comparing distributions of serotypes of Streptococcus pneumoniae isolated from vaccinated and unvaccinated persons. Between May 1978 and March 1984, 187 blood isolates and 62 cerebrospinal fluid isolates from vaccinated patients, and 1447 blood isolates and 191 cerebrospinal fluid isolates from unvaccinated patients were serotyped at the Centers for Disease Control. The study did not include patients who were less than 2 years old or who had Hodgkin's disease,

Two measles outbreaks after importation--Utah, March-June 2011.

Before licensure of a measles vaccine in 1963, more than 500,000 measles cases on average were reported in the United States each year during 1951-1962. By 1993, through measles vaccination and control efforts, only 312 cases were reported nationwide. In 2000, the last year in which an outbreak had occurred in Utah, measles was declared "not endemic in the United States," but measles importations continue to occur, leading to outbreaks, especially among unvaccinated persons. Many U.S. health-care personnel have never seen a measles patient, which might hamper diagnosis and delay reporting. Dur

Effect of COVID-19 vaccination and SARS-CoV-2 infection status on mortality risk in patients with chronic obstructive pulmonary disease: a nationwide population-based cohort study.

BACKGROUND: The coronavirus disease 2019 (COVID-19) vaccination is suggested to be effective in improving outcomes of chronic obstructive pulmonary disease (COPD). However, real-world evidence on long-term mortality among individuals with COPD, accounting for both vaccination and COVID-19 infection status, remains sparse. METHODS: A retrospective cohort study was conducted using datasets from the Korean National Health Insurance system. Through two-step propensity score matching, 716 individuals with COPD were included in the analysis and classified into four groups according to COVID-19 vacci

Severe rhabdomyolysis and refractory dysautonomia complicating generalized tetanus in an unvaccinated child: A case report.

Tetanus has become rare in settings with established immunization programs; however, severe cases continue to pose complex management challenges, particularly in children. We report a 12-year-old unvaccinated boy from an Indigenous community in southwestern Colombia who developed generalized tetanus seven days after a puncture wound, complicated by early severe rhabdomyolysis (creatine phosphokinase peak >32,000 U/L), acute kidney injury, and refractory autonomic instability. While rhabdomyolysis may occur in severe tetanus, its coexistence with marked dysautonomia created a therapeutic dilemm
