Histochemically differentiated mucins have been studied in the mucosal lining of the esophagus and the stomach. Acid mucin was differentiated from neutral mucin by the alcian blue/PAS technique and sulphomucin by the high iron diamine/alcian blue technique. Neutral mucin secreted normally by the stomach mucosa was replaced by acid mucin in 17 of the 19 mucin secreting adenocarcinomas involving the lower third of the esophagus, and in 24 of the 28 mucin secreting gastric adenocarcinomas studied. Involvement of the upper gastrointestinal tract by Crohn's disease is being recognized with increasing frequency. Gastroduodenal disease is more common, although esophageal disease is now noted infrequently as well. There is nearly always concomitant involvement of the small bowel or colon. Initial phases of the disease are manifested as superficial inflammatory changes of the mucosa but can progress to scarring and stenosis. Optimum double-contrast technique is necessary to detect these early lesions.

Certain dopamine antagonists have gained increasing clinical use because of their effect on the motility of the upper gastrointestinal tract. Dopamine, while acting through specific dopaminergic receptors, inhibits lower oesophageal sphincter pressure (LOSP) and gastroduodenal motility. Bleeding in the upper gastrointestinal (GI) tract is a frequent and complex emergency. There are guidelines for acute medical treatment, established endoscopic treatment as well as surgical and radiological rescue procedures. Similar to other tumor entities, complete tumor removal with an adequate safety margin in all three dimensions (the oral margin, the aboral margins and the tumor bed) must be the primary aim of any surgical approach to carcinoma of the upper gastrointestinal tract. The same goal has to be achieved in the area of the lymphatic drainage. All positive nodes and nodes with a so-called 'microinvolvement' have to be removed together with the primary tumor. The safety margin of lymphadenectomy can be estimated by the lymph node ratio, i.e., the ratio between the number of removed and positive nodes.

Fluid aided ultrasound examination of the neonatal upper gastrointestinal tract has been highly successful in the evaluation of hypertrophic pyloric stenosis. It can diagnose midgut volvulus. We studied the upper gastrointestinal (GI) tract lesions by endoscopy, and evaluated the relationship between the lesions and clinical characteristics in 154 patients with rheumatoid arthritis (RA).
