Neurogenic bladder and chronic urinary retention associated with MDMA abuse.

INTRODUCTION: The use of 3,4-methylenedioxymethamphetamine (MDMA, known as "ecstasy"), a synthetic amphetamine and "club drug," has been associated with acute, transient urinary retention. We report a case of neurogenic bladder and chronic urinary retention associated with MDMA abuse. CASE REPORT: A 21-year-old male presented to the emergency department (ED) because he had abdominal pain and difficulty urinating. He had experienced difficulty in initiating urination over the past 1.5 months, with periods of 24 to 36 hours between voids and large volumes of urine. The patient had a chronic pat

Management of the obstructed urinary tract associated with neurogenic bladder dysfunction.

The child with neurogenic bladder dysfunction may suffer from lower urinary tract obstruction, which, if untreated, can result in upper urinary tract deterioration. It is prudent to identify those newborns and infants who are at risk for upper urinary tract deterioration and advise an appropriate treatment protocol. In order to identify those children at risk, a thorough examination is required, along with appropriate upper urinary tract imaging studies and urodynamic assessment of the bladder. Infants and children at risk for upper tract deterioration are those who have intermittent or contin

Augmentation cystoplasty in the management of neurogenic bladder disease and urinary incontinence.

Vesical augmentation procedures were performed on 15 patients for neurogenic bladder disease and urinary incontinence. Enterocystoplasty with ileum, cecum and sigmoid was used associated with different operations to prevent upper tract deterioration or urinary incontinence. The small bowel stored larger amounts of urine at a lower maximal detrusor pressure at capacity than the large bowel. All ureterointestinal and ureterovesical reimplantations were successful, including 5 ureteroileal with the Camey procedure. The 2 failures, characterized by persistent urinary incontinence, included a male

Catheter-associated urinary tract infections in persons with neurogenic bladders.

This review is based on the author's Donald Munro Lecture given at the annual conference of the Academy of Spinal Cord Injury Professionals in New Orleans, LA. Indwelling catheters play an important role in bladder management following SCI for many individuals with neurogenic bladders. There is an increased risk of UTI compared to other types of bladder management with indwelling urethral catheters but not SP catheters. To minimize the risk of symptomatic UTI, the following steps are essential: prevent bladder wall distention and resulting ischemia, maintain colonization with "good" bacteria,

Exploring urinary microbiome: insights into neurogenic bladder and improving management of urinary tract infections.

The traditional view of sterile urine has been challenged by advancements in next-generation sequencing, revealing that the urinary microbiome significantly influences individual health and various urinary system diseases. Urinary tract infections in patients with neurogenic bladder are highly prevalent, recurrent, and lifelong. If frequent urinary tract infections are not adequately managed, they may ultimately lead to chronic renal failure. The excessive use of antibiotics to prevent and treat urinary tract infections may lead to increased bacterial resistance, limiting future therapeutic op

Neurogenic bladder dysfunction presenting as urinary retention in neuronopathic Gaucher disease.

Neuronopathic Gaucher disease can present as a continuum of clinical findings, including somatic symptoms of anemia, thrombocytopenia, hepatosplenomegaly, and bone disease as well as neurologic sequelae. There is a spectrum of neurologic symptoms ranging from oculomotor apraxia to severe convulsions. The heterozygosity of phenotypes makes it difficult to predict the disease course. We describe an 8-year-old male with neuronopathic type III Gaucher disease who developed bladder dysfunction and was unable to completely void. He also presented with hypertension and acute renal insufficiency, most

Periurethral colonization and urinary leukocytes as markers for bacteriuria in children with neurogenic bladder.

Bacteriuria and associated renal damage is common in children with a neurogenic bladder, but the pathogenesis of urinary tract infection (UTI) is undefined. We examined the association between periurethral bacterial colonization and the presence of urinary leukocytes in 76 catheter urine specimens from children with neurogenic bladders. Although all the children were asymptomatic, 38/76 (50%) of the urine cultures were positive. Periurethral colonization was significantly more common with positive than with negative urine cultures, suggesting a pathogenetic role for periurethral bacteria in in

Advancing Personalized Care for Neurogenic Bladder: The Case for Continent Urinary Diversion.

INTRODUCTION: Neurogenic lower urinary tract dysfunction (NLUTD) with a poorly compliant bladder that is refractory to minimally invasive therapies (MIT) represents an "end-stage" phenotype in which the principal objectives are durable upper urinary tract protection and restoration of continence with acceptable long-term morbidity. For most surgically eligible patients in this setting, continent urinary diversion-most commonly a continent cutaneous catheterizable reservoir (e.g., Indiana pouch) and, in highly selected cases, an orthotopic continent diversion-offers a uniquely definitive strate

Stratification of neurogenic bladder risk in spina bifida using the urinary peptidome.

Neurogenic bladder poses a major morbidity in children with spina bifida (SB), and videourodynamic studies (VUDS) are used to stratify this risk. This small-scale pilot study utilized current mass-spectrometry-based proteomic approaches to identify peptides or proteins in urine that may differentiate children at high risk of developing renal complications from a neurogenic bladder. Twenty-two urine samples of which nine had high bladder pressure storage that put the upper urinary tract at risk, while 13 with a lower risk for renal compromise were analyzed. More than 1,900 peptides across all 2

Evidence for a contribution by purines to the neurogenic response of the guinea-pig urinary bladder.

In order to determine if ATP contributes as an excitatory transmitter in the guinea-pig bladder, experiments were conducted with ANAPP3, a photoaffinity analogue of ATP, which is an antagonist of adenine nucleotides in several other smooth muscles. With or without photoactivation with visible light, ANAPP3 antagonized contractile responses of in vitro strips of bladder to exogenous ATP. The antagonism was specific in that responses to acetylcholine and KCl were not affected by ANAPP3. Responses of strips of bladder to transmural electrical stimulation were not antagonized by ANAPP3 and were re

Behavior therapy for urinary frequency in a patient with neurogenic bladder: a case report.

A 32-year-old female with a seven year history of multiple sclerosis was hospitalized due to an exacerbation of symptoms, and complained of a high frequency of urination and occasional urinary incontinence. A urologic consult was obtained and the patient was diagnosed as having neurogenic (spastic) bladder, resulting in a reflexive voiding whenever the accumulation of urine in the bladder reacher 100 mls. A behavioral management program using feedback and goal-setting was implemented. Whenever she voided, the patient recorded the time of day, and measured the amount by using a graduated urinal

Autovaccine immunoprophylaxis in patients with neurogenic bladder experiencing recurrent urinary tract infections.

INTRODUCTION: Neurogenic Bladder (NB) patients are highly susceptible to recurrent urinary tract infections (UTIs), often requiring repeated hospitalizations and prolonged antibiotic use. Current preventive strategies, including long-term antibiotics, frequently fail due to resistance and limited efficacy. Autovaccination, a form of personalized immunoprophylaxis using inactivated patient-specific bacterial strains, has shown potential in reducing UTI recurrence but has not been well studied in NB populations. METHODS: A prospective, single-center study was conducted at the Hospital Universit
