Urinary stress incontinence and overactive bladder symptoms in older women.

Urinary incontinence - the accidental leaking of urine - is a major problem in Australia and has a significant impact on quality of life, affecting the social, psychological, physical and financial aspects of living. The prevalence of urinary incontinence is significantly higher in women than in men. While urine loss itself is a devastating symptom, older women are also likely to suffer lower urinary tract symptoms associated with the overactive bladder syndrome such as urgency, frequency, mixed incontinence and nocturia which are reported more frequently in older women. These symptoms disrupt

Sex and Gender Differences in the Pharmacology of the Overactive Urinary Bladder.

Dysfunction of the lower urinary tract in general and the overactive bladder syndrome (OAB) in particular are prevalent and have major impact on the quality of life of the afflicted patients and their partners. We concisely review sex and gender differences in patients and animal models in physiological bladder function, its alterations in disease (mostly OAB), and its responses to treatment. Women appear to have a smaller functional bladder capacity and, therefore, must void more often than men. On the other hand, men have a greater bladder outlet resistance, which is partly attributed to a l

Impact of Lower Urinary Tract Symptoms on Daily Life in Individuals With Co-Existence Overactive and Underactive Bladder, Overactive Bladder, and Underactive Bladder.

OBJECTIVES: To explore the particular lower urinary tract symptoms (LUTS) that impact daily life in co-existent overactive and underactive bladder (COUB), overactive bladder (OAB), and underactive bladder (UAB) as symptom syndromes. METHODS: Using data from a nationwide, cross-sectional internet survey of adults aged ≥ 20 conducted in Japan, participants were categorized into COUB, OAB alone, UAB alone, and non-OAB/UAB groups according to the OAB symptom score and the provisional UAB definition of the International Continence Society working group. Participants were asked what symptoms affect

Urinary ATP may be a dynamic biomarker of detrusor overactivity in women with overactive bladder syndrome.

BACKGROUND: Nowadays, there is a considerable bulk of evidence showing that ATP has a prominent role in the regulation of human urinary bladder function and in the pathophysiology of detrusor overactivity. ATP mediates nonadrenergic-noncholinergic detrusor contractions in overactive bladders. In vitro studies have demonstrated that uroepithelial cells and cholinergic nerves from overactive human bladder samples (OAB) release more ATP than controls. Here, we compared the urinary ATP concentration in samples collected non-invasively from OAB women with detrusor overactivity and age-matched contr

Urinary nerve growth factor levels in overactive bladder syndrome and lower urinary tract disorders.

Overactive bladder (OAB) is a syndrome based on self-reported symptoms of urgency and frequency with or without urge incontinence. Although urgency is the core symptom of OAB, patients might have difficulty to distinguish urgency from the urge to void. Urodynamic study is a useful diagnostic tool to discover detrusor overactivity (DO) in patients with OAB; however, not all OAB patients have DO. Therefore, a more objective and non-invasive way to diagnose and assess OAB including DO is needed. Recent research has focused on urinary biomarkers in assessment of OAB. Urinary nerve growth factor (N

Solifenacin objectively decreases urinary sensation in women with overactive bladder syndrome.

PURPOSE: To evaluate the test-retest reliability of a device that measures urinary sensation during cystometry, and to use that device to determine whether treatment of overactive bladder syndrome (OAB) with solifenacin is associated with a change in urinary sensation. METHODS: Overall 15 women were recruited for this study, ten women with OAB and urodynamically demonstrated detrusor overactivity and five controls without OAB underwent filling cystometry twice with contemporaneous, continuous recording of urinary sensation. Women with OAB received solifenacin 10 mg daily during the weeks betw

Can urinary nerve growth factor be a biomarker for overactive bladder?

The clinical diagnosis of overactive bladder (OAB) greatly varies and is based on subjective symptoms. A more objective method to diagnose and assess therapeutic outcome in OAB patients, especially for health care providers not trained in urology, needs to be found. Evidence has shown that urinary proteins such as nerve growth factor (NGF) and prostaglandin E(2) levels increase in patients with OAB, bladder outlet obstruction, and detrusor overactivity. Urinary NGF level increases physiologically in normal subjects at urge to void, but increases pathologically in OAB patients at a small bladde

Urinary Tract Infection in Overactive Bladder: An Update on Pathophysiological Mechanisms.

Overactive bladder (OAB) is a clinical syndrome defined by urinary urgency, increased daytime urinary frequency and/or nocturia, with or without urinary incontinence, that affects approximately 11% of the western population. OAB is accepted as an idiopathic disorder, and is charactersied clinically in the absence of other organic diseases, including urinary tract infection. Despite this, a growing body of research provides evidence that a significant proportion of OAB patients have active bladder infection. This review discusses the key findings of recent laboratory and clinical studies, provi

[Allergic inflammation as one of the factors of pathogenesis of overactive urinary bladder].

Overactive bladder (OAB) is still a serious problem because of its high prevalence in men and women. Endoscopic or open detrusor biopsies were obtained from 28 (24 male and 4 female) patients of 52 to 75 years of age with obstructive or idiopathic OAB of 1 to 10 year duration. The control group included 6 males and 2 females without OAB symptoms. Allergic inflammation with involvement of mast cells, parasympathetic element hyperplasia, ultrastructural changes of myocytes with structural damage to mitochondria were detected in the detrusor of patients with both obstructive and idiopathic OAB. Pharmacological agents for the treatment of urinary incontinence due to overactive bladder.

Although the exact aetiology of overactive bladder is unknown to date, pharmacological therapy has been targeted to both the central and peripheral nervous systems. Potential CNS targets include GABA, opioid, serotonin (5-HT), dopamine and glutaminergic receptors as well as the alpha-adrenoceptors. Potential PNS targets include muscarinic receptors, calcium and potassium channels and alpha- and beta-adrenergic receptors. Since acetylcholine is the primary excitatory neurotransmitter involved in bladder (detrusor) contraction and emptying, anticholinergic agents are the primary compounds used c

Recurrent urinary tract infection in women and overactive bladder - Is there a relationship?

Overactive bladder (OAB) in women has similar symptomatology with other common urologic diseases such as recurrent urinary tract infection (UTI). Recent evidence showed that chronic low-grade bacterial bladder colonization might exacerbate OAB symptoms and could be the etiology of recurrent UTI. The high prevalence of lower urinary tract dysfunction is associated with OAB. Women with urgency urinary incontinence refractory to antimuscarinic therapy had more bacteria and a more diverse urinary microbiome. The bacterial reside in the superficial urothelial cells to form intracellular bacterial c

Recurrent Urinary Tract Infections in Females and the Overlap with Overactive Bladder.

PURPOSE OF REVIEW: There are an estimated 33 million men and women with overactive bladder (OAB) in the USA. Despite the prevalence of OAB, it remains a frequently misdiagnosed condition. OAB has shared symptomatology with other common urologic conditions, namely recurrent urinary tract infections (UTIs). Here, we will review key distinguishing features of OAB that aid in establishing an accurate diagnosis and recent advances in OAB management. RECENT FINDINGS: Recent studies have shown that among women presenting with lower urinary tract symptoms, the majority were diagnosed with UTIs and tr

Differences in the Urinary Microbiome of Patients with Overactive Bladder Syndrome with and without Detrusor Overactivity on Urodynamic Measurements.

INTRODUCTION: It has been hypothesized that the urinary microbiome might play an important role in OAB. Studies have been conducted on the association between OAB symptoms and the microbiome, although a possible causality still has to be determined. MATERIAL AND METHODS: In this study, 12 female patients, ≥18 years of age, with 'OAB DO+' and 9 female patients with 'OAB DO-' were included. Patients were excluded if they met one of the following exclusion criteria: bladder tumors and previous bladder operations; sacral neuromodulation; injection of Botox in the bladder; and TOT or TVT operation

Systematic review of the epidemiology of urinary incontinence and detrusor overactivity among patients with neurogenic overactive bladder.

BACKGROUND: The prevalence and incidence of neurogenic overactive bladder (nOAB) are poorly defined. This systematic literature review identified nOAB epidemiological data and estimated the incidence and prevalence of urinary incontinence (UI) and detrusor overactivity (DO) in patients with multiple sclerosis (MS), spinal cord injury (SCI), Parkinson's disease (PD), stroke and spina bifida. An initial search of MEDLINE, Embase, PubMed, and the Cochrane library was supplemented by an internet search for grey literature and manual searching of the bibliographies of retrieved articles. Urinary Fungal Microbiome and Associations With Overactive Bladder, Antibiotics and Therapy Response.

OBJECTIVE: To characterize and compare the fungal urinary microbiome ("mycobiome") in healthy women and those with overactive bladder (OAB), and secondarily compare the mycobiome by comorbidities, recent antibiotic exposure, and response to OAB therapy. MATERIAL AND METHODS: DNA was isolated from urine of those with and without OAB and underwent internal transcribed spacer (ITS) sequencing to detect fungi. Alpha- and beta-diversity, and relative abundance of fungal taxa were analyzed and compared by patient characteristics including OAB status. Fungal-bacterial interactions were identified.
