We report postoperative effects of 23 cases having tapered terminal ileal continent urinary reservoir is reported. The operative technique was characterized by tapering the terminal ileum with stapler to form a continent efferent tract, segments of intestine were cut open and reformed a pouch, and the ureter implantation was performed by means of inserting the end of ureters into the pouch. Ileocolonic pouch wes constructed for 14 patients and colonic pouch for 9 patients. 22 patients were followed up for 1 to 6 years except one died of cerebral hemorrhage 2 weeks after operation. Spontaneous rupture of a continent urinary reservoir is a rare, but clinically important event. The diagnosis of reservoir rupture should be considered in any patient with peritonitis and a history of continent urinary diversion. A pouchogram may confirm the diagnosis, but ultimately a laparotomy is mandatory in the setting of peritonitis and sepsis. Catheter drainage alone has been reportedly successful for patients who meet certain criteria. This case highlights the key steps in evaluation and management of a ruptured continent urinary reservoir.

Urinary stones are a well-established complication of lower urinary tract reconstruction in children and can be managed through an open, percutaneous, or transurethral endoscopic approach for their surgical removal. For children with no urethral access, it is not generally advised to pursue an endoscopic approach through a catheterizable channel. In this study, we describe a safe and effective technique for the endoscopic management of stones through a catheterizable channel in a child with a continent urinary reservoir.

Complications associated with continent urinary reservoirs are well recognised. Fistulae between the pouch and colon are rare but are documented. This uncommon complication is reported in the early post-operative period in patients who have previously received pelvic radiotherapy. Colonic fistulae are normally secondary to malignancy or diverticular disease. We present the first literature report of a late, acutely presenting colo-pouch fistula secondary to neither malignancy nor diverticular disease. We highlight the need for medical staff to be aware that not all stomas are incontinent and t

Urinary diversion by transplantation of both ureters into a reservoir constructed from 60 cm of jejunum was performed in six clinically normal dogs. The reservoir was connected to a stoma on the abdominal wall through a valve constructed from a short segment of intussuscepted bowel to provide continence. The reservoir was emptied three times daily by catheterization. One dog did not survive the surgical procedure and two dogs died within 1 week of surgical complications. The postoperative capacity of the reservoir in surviving dogs was 100 to 200 mL; it increased during the first month to 600

From August 1982 through January 1984, 51 patients underwent urinary diversion that included creation of a continent reservoir from an ileal segment, according to the method described originally by Kock. An important modification included removal of a narrow strip of mesentery for 8 cm. along the afferent and efferent limbs of the pouch to allow adequate ileal intussusception, and fixation to prevent reflux and to ensure continence. Previous urinary diversion was by ureterosigmoidostomy in 3 patients, standard ileal conduit in 7 and suprapubic cystotomy in 1. A total of 39 patients underwent s

Following construction of a continent colonic urinary reservoir, 5 uretero-colonic reimplantations became obstructed at the anastomotic site. In these obstructed units previous percutaneous balloon dilation and stent placement had failed and they were subsequently treated by a new reimplantation procedure. Preoperatively, in all ureteral units a percutaneous ureteral stent was inserted to facilitate intraoperative recognition of the ureteral meatus. The technique included a trans-reservoir approach, which allowed easy localization of the stent and anastomotic site. The ureter was dissected fre

Total cystectomy at the time of radical pelvic surgery for gynecologic malignancies is not uncommon. Many techniques have been developed for urinary diversion including the continent ileocecal urinary reservoir. Twenty-nine patients underwent construction of a continent ileocecal urinary reservoir during the reconstructive phase of an exenterative procedure or for the relief of urinary tract fistula between 1990 and 1993. All procedures were performed using the mechanical stapling devices using metal staples. The right and proximal transverse colon were mobilized to a length of 24 cm. The ileu

OBJECTIVE: To assess the results of the use of ileocecal continent urinary reservoirs in patients with previous pelvic irradiation.
METHODS: A retrospective analysis for morbidity and clinical outcome was undertaken for 37 female patients with prior therapeutic pelvic irradiation who underwent continent urinary diversion with a detubularized right colonic segment as the urinary reservoir, a plicated ileocecal valve as the continence mechanism, and a tapered distal ileum for efferent catheterization.
RESULTS: Thirty-one patients had persistent or recurrent pelvic malignancies, 17 of whom had

A continent urinary reservoir which consists of 1) detubularized caecum, ascending colon and the right half of transverse colon. 2) anastomosis of ureter and ascending colon with antireflex technique, and 3) plicated terminal ileal segment for continence and catheter insertion was created in 12 patients after radical cystectomy for urothelioma. Our method to create a continent urinary reservoir is simple and technically easy. It can, therefore, provide patients' satisfaction with low incidence of complication. However, such continent urinary reservoir diversion should be performed only for car

Little information exists on the use of continent urinary reservoirs in patients with previous pelvic irradiation. We report the use of the Indiana pouch urinary reservoir in ten women with a history of pelvic irradiation for cervical cancer, of whom eight underwent a total pelvic exenteration for recurrent pelvic tumor and two had diversion for radiation-induced vesicovaginal fistula. All ten women achieved daytime continence, with a median time between catheterizations of 4.5 hours and a median pouch capacity of 500 mL. There was no evidence of leakage from the reservoir or significant urete

A new technique of using the in situ appendix to construct a continent catheterizable stoma is described in a patient who underwent continent urinary diversion, and the technical aspects of this procedure are illustrated in detail. The appendix is remodeled by invaginating its base into the cecum for 1 cm., cecoplicating the middle portion for 2 cm. and bringing the distal end to the skin as a cutaneous stoma. This new technique demonstrates that the in situ appendix can be constructed successfully to provide continence without the need for isolating it from the cecum and implanting the distal

A patient, who underwent total cystectomy and urinary diversion with the Kock pouch, was trained to use the Bladder Assist Device (BLADMAN) to augment the Kock continent ileal urinary reservoir. BLADMAN is a device to provide an intermittent drainage system, which simulates the normal periodic filling and emptying of the bladder of a patient with indwelling catheter. On the 16th day after starting the pouch augmentation with BLADMAN, the pouchgram of the patient revealed a pouch capacity of 450 ml. BLADMAN is considered to be a useful device for the augmentation of Kock continent ileal reservo

Continent diversion of urine via a cecal-right colonic reservoir has been performed at our university hospital since 1977. Several modifications of surgical technique have been devised to prevent problems of urinary leakage and difficulties in catheterization. The current technique, used during the last 3 years on 14 patients, involves use of a detubularized right colonic segment as a reservoir, ileal mesenteric exclusion, fashioning the intussuscepted ileal nipple valve with staples and anchoring of a fascia strip sling around the nipple base to the anterior rectus sheath. Complication from t

Construction of continent colonic urinary reservoir was performed in 44 patients after exenteration for invasive bladder carcinoma or various gynecologic tumors. The distal ileum was tapered over a 14 French red rubber catheter. The ileocecal valve was reinforced with three circumferential silk sutures in a purse-string fashion. Full continence was obtained in all patients (100%). A non-tunneled, non-refluxing ureterocolonic anastomosis was performed in all 88 ureters. No obstruction or reflux was observed in 84 ureters (95%). There were 7 early postoperative complications and 3 of them requir

The Kock continent ileal urinary reservoir (Kock pouch) is a new form of urinary diversion that, due to its advantages over previous techniques of urinary bypass, will probably become widespread in urologic practice. When bone imaging is performed in the presence of the Kock pouch, the unusual configuration of the pouch may obscure or simulate osseous lesions. An understanding of the surgical anatomy as well as the planar and SPECT scintigraphic appearances of the Kock pouch is necessary to avoid errors during interpretation. This series of 51 bone images reports on the variable scintigraphic
