Uterine packing with chitosan-covered gauze compared to balloon tamponade for managing postpartum hemorrhage.

BACKGROUND: Postpartum hemorrhage (PPH) is a major cause of maternal death worldwide. Management of PPH includes the administration of uterotonics, and intrauterine packing techniques. OBJECTIVE: In this study the effectiveness and safety of chitosan covered gauze versus a balloon tamponade for managing severe PPH should be assessed. STUDY DESIGN: This retrospective cohort study was conducted at the Department of Obstetrics, Charité, university hospital Berlin, between October 2016 and June 2018. Women with PPH were treated according to management guidelines. When bleeding persisted, we appl

Uterine balloon tamponade in combination with topical administration of tranexamic Acid for management of postpartum hemorrhage.

While uterine balloon tamponade is an effective modality for control of postpartum hemorrhage, the reported success rates have ranged from the level of 60% to the level of 80%. In unsuccessful cases, more invasive interventions are needed, including hysterectomy as a last resort. We developed a modified tamponade method and applied it to two cases of refractory postpartum hemorrhage after vaginal delivery. The first case was accompanied by uterine myoma and low-lying placenta. After an induced delivery, the patient had excessive hemorrhage due to uterine atony. Despite oxytocin infusion and bi…

Disseminated intravascular coagulation as a complication of intrauterine balloon tamponade for posthysteroscopic acute uterine bleeding.

Although there is anecdotal evidence of balloon tamponade for posthysteroscopic acute uterine bleeding, a literature review disclosed no articles describing complications of the technique. In our patient, balloon tamponade for acute posthysteroscopic bleeding resulted in disseminated intravascular coagulation. In a second woman the procedure was successful.

Unilateral uterine artery embolization and Bakri tamponade balloon insertion in the treatment of acute puerperal uterine inversion: a case report.

BACKGROUND: Acute puerperal uterine inversion is rare but may cause massive postpartum blood loss due to uterine atony. Therefore, these patients must be diagnosed, and uterine replacement must be performed as soon as possible. However, in some cases, active bleeding due to uterine atony becomes uncontrollable, even though the uterine inversion itself is treated. In these cases, additional treatments, including surgical procedures, are needed. CASE PRESENTATION: A 41-year-old Japanese woman, gravida 1, para 0, was hospitalized for labor induction at 40 weeks and 3 days of gestational age. She

Virtual reality exoskeleton for post-partum uterine tamponade balloon training: Impact on learning and operator satisfaction.

BACKGROUND: Virtual reality's (VR) use in medical education is increasing; however, traditional VR controllers lack real-world dexterity. A post-partum hemorrhage (PPH) is an obstetric emergency with significant maternal morbidity. The use of an intrauterine balloon reduces the need for further surgical interventions, but resident doctors might lack experience with balloon insertion. OBJECTIVES: This study evaluates whether a novel VR exoskeleton (VRE) improves medical students' and residents' uterine balloon insertion technique and learning experience compared to traditional didactic teachin

Efficacy of Intra-Uterine Tamponade Balloon in Post-Partum Hemorrhage after Cesarean Delivery: An Impact Study.

Invasive therapies (surgery or radiological embolization) are used to control severe post-partum hemorrhage. The intra-uterine tamponade balloon is a potential alternative, well documented after vaginal delivery. However, available data on its use after cesarean delivery remain scarce. This study assessed the efficacy of the intra-uterine tamponade balloon during post-partum hemorrhage in a cesarean delivery setting. Using a retrospective impact design, post-partum hemorrhage-related outcomes before ("pre-balloon" period) versus after implementation of intra-uterine tamponade balloon ("post-ba

Successful use of a Bakri Tamponade Balloon in the treatment of puerperal uterine inversion during caesarean section.

Acute puerperal inversion of the uterus is a rare life-threatening obstetric emergency, especially during caesarean section. We present the case of a 30-year-old patient with acute puerperal inversion of the uterus that occurred during placental removal. After a quick reversion of the uterus, an immediate postpartum haemorrhage (PPH) due to massive uterine atony was observed. This atony impacted the whole uterus, with a very thin uterine myometrium. The use of a Bakri Tamponade Balloon use allowed treating extreme uterine atony, immediately stop haemorrhage, and prevent a possible risk of imme

Cost analysis of intrauterine balloon tamponade versus uterine artery embolization in the management of persistent postpartum hemorrhage.

OBJECTIVE: The aim of this study was to compare the cost of a strategy initially resorting to intrauterine balloon tamponade versus a strategy initially resorting to uterine artery embolization in the management pathway of persistent postpartum hemorrhage. METHODS: This was a propensity score-matched cohort study including women who were subsequently treated with (scenario 1) versus uterine artery embolization (scenario 2) in order to control PPH, defined as postpartum bleeding that was resistant to first-line treatment. We compared these two scenarios in terms of cost. These were based on co…

Uterine tamponade balloon for the management of massive hemorrhage during cesarean section due to placenta previa/increta.

OBJECTIVE: Uterine tamponade with the Bakri catheter is effectively used as a treatment in postpartum hemorrhage and as a means to prevent fertility. CASE: We present a case of a 40-year-old pregnant woman who had a massive hemorrhage during cesarean section who was successfully treated with a tamponade balloon. Furthermore, we comment on a similar technique--Logothetopoulos pack--which was first developed and used in our department in the early years of the previous century. CONCLUSION: A conservative technique such as the Bakri catheter is an alternative intermediate step to control postpa

Post-partum haemorrhage from the lower uterine segment secondary to placenta praevia/accreta: successful conservative management with Foley balloon tamponade.

Profuse bleeding from the lower uterine segment secondary to placenta praevia/accreta during caesarean delivery is a challenging problem in obstetrics. We present our experiences using intrauterine Foley balloon tamponade for the conservative management of post-partum haemorrhage from the lower uterine segment. Intraoperative haemostasis was achieved in all women who were unresponsive to other conservative methods. Foley balloon tamponade may be considered in the management of lower uterine segment bleeding at caesarean delivery.

Healthcare providers experiences of using uterine balloon tamponade (UBT) devices for the treatment of post-partum haemorrhage: A meta-synthesis of qualitative studies.

BACKGROUND: Postpartum haemorrhage (PPH) is a leading cause of maternal mortality and severe morbidity globally. When PPH cannot be controlled using standard medical treatments, uterine balloon tamponade (UBT) may be used to arrest bleeding. While UBT is used by healthcare providers in hospital settings internationally, their views and experiences have not been systematically explored. The aim of this review is to identify, appraise and synthesize available evidence about the views and experiences of healthcare providers using UBT to treat PPH. METHODS: Using a pre-determined search strategy,…

Vacuum-assisted hemorrhage control device versus uterine balloon tamponade for refractory atonic postpartum hemorrhage in a protocol-driven care pathway-A retrospective cohort study.

INTRODUCTION: Globally, 1 in 5 maternal deaths are attributable to major postpartum hemorrhage (PPH), despite widespread uterotonic use. Intrauterine tamponade is recommended as second-line therapy for refractory atonic PPH. Uterine balloon tamponade (UBT) achieves hemostasis through intrauterine pressure. Conversely, the vacuum-assisted hemorrhage-control device (VHD) promotes uterine contraction through negative pressure. Limited data compare these modalities. We compared the clinical effectiveness of UBT and VHD in refractory atonic PPH. MATERIAL AND METHODS: This retrospective cohort stud…
