Vaginal birth after cesarean section: a reappraisal of risk.

A three-year retrospective study was performed to assess morbidity associated with an attempted vaginal birth after cesarean section at a tertiary level military obstetric hospital. Examination of delivery records revealed a 61.16% success rate for attempted vaginal births after cesarean sections. Mean birth weights differed significantly between successful and unsuccessful vaginal births after cesarean sections. Infants with birth weights greater than 3720 gm were less likely to be vaginally delivered. Those gravidas with a successful vaginal birth after cesarean section had a significantly h

Are prediction models for vaginal birth after cesarean accurate?

BACKGROUND: The use of trial of labor after cesarean delivery calculators in the prediction of successful vaginal birth after cesarean delivery gives physicians an evidence-based tool to assist with patient counseling and risk stratification. Before deployment of prediction models for routine care at an institutional level, it is recommended to test their performance initially in the institution's target population. This allows the institution to understand not only the overall accuracy of the model for the intended population but also to comprehend where the accuracy of the model is most limi

Evaluating professional society guidelines on vaginal birth after cesarean.

The objective was to compare national guidelines regarding vaginal birth after cesarean. Along with the American College of Obstetricians and Gynecologists practice bulletin, guidelines from the Royal College of Obstetricians and Gynaecologists and the Society of Obstetricians and Gynecologists of Canada were reviewed and compared. Although the 3 organizations agree on most of the risk factors for uterine rupture and failed vaginal birth after cesarean (VBAC), there were some variances in the recommendations to women with 2 previous cesareans and those who required oxytocin augmentation. Morbidity associated with failed vaginal birth after cesarean section.

We investigated morbidity and factors associated with failed vaginal birth after cesarean delivery (VBAC). In a retrospective cohort study maternal and neonatal outcomes of women who underwent VBAC in three Nigerian University Teaching Hospitals were reviewed. Univariate, followed by multivariate analyses, were conducted. VBAC was successful in 683 of 1,013 women (67.4%), whereas 330 (32.6%) had failed VBAC. Failed VBAC was associated with higher incidence of chorioamnionitis, postpartum hemorrhage, blood transfusion, uterine rupture, hysterectomy, and composite major neonatal morbidities. You

Obstetric Anal Sphincter Injury: Vaginal Birth After Cesarean Versus First Vaginal Delivery.

IMPORTANCE: Modern data regarding the relationship between vaginal birth after cesarean (VBAC) and obstetric anal sphincter injury (OASI) are minimal with mixed results. OBJECTIVE: The aim of the study was to determine if VBAC is associated with an increased risk of OASIs. STUDY DESIGN: This was a retrospective cohort study of liveborn deliveries from 2018 to 2022 within a large, multihospital academic health system. The primary outcome was the proportion of OASIs compared between patients who underwent a first vaginal delivery (primiparous vaginal delivery group) versus patients who underwe

Comparison of maternal and neonatal outcomes between vaginal birth after cesarean section and normal vaginal birth.

OBJECTIVE: This study aimed to compare the outcomes of vaginal birth after cesarean (VBAC) with those of normal vaginal birth (NVB) in a tertiary hospital in China. METHODS: This retrospective cohort study analyzed 1,024 women who birthed vaginally between January 2019 and December 2020. The VBAC group (n = 512) included women with one previous cesarean, while the NVB group (n = 512) had no previous caesareans. All women used epidural analgesia. We assessed maternal and neonatal complications using descriptive statistics, chi-square tests, and logistic regression. Statistical analysis was per

Vaginal birth after cesarean section versus elective repeat cesarean delivery: Weight-based outcomes.

OBJECTIVE: The study was undertaken to compare infectious morbidity and trial of labor (TOL) success stratified by weight in women. STUDY DESIGN: Vaginal birth after cesarean section (VBAC) candidates were divided into groups based on prepregnancy weight: group I, 70 (<200 pounds); group II, 70 (200-300 pounds); and group III, 69 (>300 pounds). RESULTS: The TOL success rate was 81.8% in group I compared with 57.1% in group II and 13.3% in group III (P =.001). The overall infectious morbidity was significantly greater in the obese women 39% (P =.001) compared with the average women at 11.4% a

FIGO good practice recommendations for vaginal birth after cesarean section.

The rising global rate of cesarean section (CS) has prompted renewed focus on vaginal birth after cesarean (VBAC) as a safe and effective alternative to repeat CS in properly selected women. The FIGO good practice recommendations provide evidence-based recommendations to guide VBAC care. Success rates for VBAC range from 60% to 80%, with the highest likelihood in women with prior vaginal birth, non-recurrent CS indications, interbirth intervals ≥18 months, BMI <30 kg/m2, and spontaneous labor onset. While uterine rupture is the primary concern, its risk remains low at approximately 0.3%-0.7% f

Prediction of vaginal birth after cesarean deliveries using machine learning.

BACKGROUND: Efforts to reduce cesarean delivery rates to 12-15% have been undertaken worldwide. Special focus has been directed towards parturients who undergo a trial of labor after cesarean delivery to reduce the burden of repeated cesarean deliveries. Complication rates are lowest when a vaginal birth is achieved and highest when an unplanned cesarean delivery is performed, which emphasizes the need to assess, in advance, the likelihood of a successful vaginal birth after cesarean delivery. Vaginal birth after cesarean delivery calculators have been developed in different populations; howev

Vaginal birth after cesarean: what's new in the new millennium?

PURPOSE OF REVIEW: This brief review will focus exclusively on very recent developments and controversial aspects of vaginal birth after cesarean. Only papers published in 2001 or 2002 are included. RECENT FINDINGS: Recent studies have addressed the intrapartum management of vaginal birth after cesarean patients and the safety of trial of labor compared with elective repeat cesarean. SUMMARY: The recent trend has been towards a more cautious approach to vaginal birth after cesarean. Some are concerned that this trend may limit childbirth options for those women who wish to avoid repeat cesar

Vesicouterine fistula: a rare complication of vaginal birth after cesarean.

BACKGROUND: Vesicouterine fistula is rare. We report a vesicouterine fistula that was the direct result of a vaginal delivery in a patient with a history of lower uterine segment cesarean delivery. CASE: A woman developed a vesicouterine fistula during vaginal delivery after a previous cesarean. An anterior uterine wall defect was noted immediately after the delivery. Continuous bladder drainage was unsuccessful in managing her fistula, and surgical correction was necessary. CONCLUSION: Although rare, a vesicouterine fistula can occur as a complication of vaginal delivery in patients with a

Factors influencing the practice of vaginal birth after cesarean section.

OBJECTIVES: Vaginal birth after cesarean has been recommended for most women with previous cesarean sections for the past 10 years. This practice, however, has not yet been generalized because high variations can still be observed among countries, hospitals, and physicians. METHODS: A case-control study involving 635 case patients and 2593 control patients was carried out to determine which characteristics of the physician, the patient, or the hospital were important in the adoption of this practice. RESULTS: The results of the multiple stepwise logistic regression analysis indicate a higher

Vaginal birth after cesarean delivery in the West African setting.

OBJECTIVE: To determine the fetal weight beyond which women with one previous cesarean delivery (CD) are most likely to have a repeat CD. METHODS: A retrospective cohort study of 586 women who had one previous CD and were undergoing trial of labor was conducted in Accra, Ghana. Following delivery, the women were allocated to one of three groups according to whether they had a successful vaginal delivery, underwent a CD for cephalopelvic disproportion, or underwent a CD for another indication. The groups were then compared using analysis of variance or Kruskal-Wallis tests. Multiple logistic r

Is vaginal birth after two or more cesarean sections safe?

Fifty-five patients with a history of two or more cesarean sections underwent a trial of labor. Forty-two had had previous uterine incisions of unknown type, 11 had had low cervical transverse incisions, and two had had low vertical incisions. Twenty-five women (45%) had successful vaginal deliveries, and 30 (55%) received oxytocin augmentation of labor. The incidence of vaginal delivery was significantly lower in patients who required oxytocin augmentation (30 versus 64%, P less than .01). Three of the 55 patients had scar separation detected at the time of delivery. Two patients underwent hy
