Sympathetic components in left and right human cervical vagus nerve: implications for vagus nerve stimulation.

Cervical vagus nerve stimulation is in a great variety of clinical situations indicated as a form of treatment. It is textbook knowledge that at the cervical level the vagus nerve contains many different fiber classes. Yet, recently, several reports have shown that this nerve also may contain an additional class of potentially noradrenergic fibers, suggested to denote efferent sympathetic fibers. As such, the nature and presence of these fibers should be considered when choosing a stimulation protocol. We have studied human vagus material extracted from dissection room cadavers in order to fur

[Malignant peripheral nerve sheath tumor. Vagus nerve sarcoma].

A young man was seen for a mass on the left side of the neck. Computed tomography revealed a round mass suggesting a lymph node metastasis with a necrotic center. Fine needle aspiration found mesenchymal cells. The tumor excised from its location on the Xth left cranial nerve was found to be a malignant peripheral nerve sheath tumor. Treatment was complemented by regional irradiation. After concluding complementary treatment in April 1996, the patient currently is disease-free. This type of soft-tissue sarcoma is reviewed. Survival depends on tumoral size and cellular differentiation.

The vagus nerve in myocardial infarction.

Myocardial infarction can result from and leads to sympathetic activation, whereas vagal engagement provides a counter-regulatory influence that can limit ischaemic injury. This article reviews the organization of vagal sensory and efferent pathways, their transmitters and receptors, and their targets within the myocardium, coronary and systemic vasculature, and other organs including the intestine and spleen. Atrial mechanosensitive afferents mediate the Bainbridge reflex, resulting in tachycardia, whereas activation of ventricular mechano- and chemosensitive afferents trigger the Bezold-Jari

Development and regeneration of the vagus nerve.

The vagus nerve, with its myriad constituent axon branches and innervation targets, has long been a model of anatomical complexity in the nervous system. The branched architecture of the vagus nerve is now appreciated to be highly organized around the topographic and/or molecular identities of the neurons that innervate each target tissue. However, we are only just beginning to understand the developmental mechanisms by which heterogeneous vagus neuron identity is specified, patterned, and used to guide the axons of particular neurons to particular targets. Here, we summarize our current under

Optogenetic Manipulation of the Vagus Nerve.

The vagus nerve plays a pivotal role in communication between the brain and peripheral organs involved in the sensory detection and the autonomic control of visceral activity. While the lack of appropriate experimental techniques to manipulate the physiological activity of the vagus nerve has been a long-standing problem, recent advancements in optogenetic tools, including viral vectors and photostimulation devices, during the late 2010s have begun to overcome this technical hurdle. Furthermore, identifying promoters for expressing transgenes in a cell-type-specific subpopulation of vagal neur

Mesenchymal chondrosarcoma of the vagus nerve.

Mesenchymal chondrosarcoma is a rare, aggressive, malignant neoplasm, which arises from extraskeletal sites in 30-40 per cent of cases. It is extremely rare in children. We present a novel case of childhood mesenchymal chondsarcoma arising from the vagus nerve in the neck, resulting in paralysis of the right vocal fold. The clinicopathologic features and management of this case are described along with a brief discussion on the aetiology of vocal fold paralysis in this age group. Current literature on extraskeletal presentation of mesenchymal chondrosarcoma is reviewed.

[Myeloarchitectonics of the vagus nerve in man].

In serial electively stained transversal sections of the vagus nerve trunk taken at the levels beginning from its exit out of the brain, up to terminal branches in the abdominal cavity (10 corpses of mature persons) myelin fibers of three classes have been counted. Along the course of the nerve trunk in the craniocaudal direction the total number of the myelin fibers decreases. Only about 10% of the initial amount of the myelin conductors revealed at the level of the intracranial part of the vagus nerve get into the abdominal cavity. Simultaneously, the ratio of the myelin fibers belonging to

Distribution of the vagus nerve to the stomach.

Examination of the distribution of vagus nerve fibers to the stomach in 100 cadavers revealed considerable individual variation. No single pattern shown in textbooks or published papers can be accepted as correct for all individuals. The variations encountered are discussed from the viewpoint of the surgeon who wishes to perform highly selective vagotomy.

Internal senses of the vagus nerve.

The vagus nerve is an indispensable body-brain connection that controls vital aspects of autonomic physiology like breathing, heart rate, blood pressure, and gut motility, reflexes like coughing and swallowing, and survival behaviors like feeding, drinking, and sickness responses. Classical physiological studies and recent molecular/genetic approaches have revealed a tremendous diversity of vagal sensory neuron types that innervate different internal organs, with many cell types remaining poorly understood. Here, we review the state of knowledge related to vagal sensory neurons that innervate

Schwannoma of the cervical vagus nerve.

Nerve sheath tumors arising from the cervical vagus nerve are extremely rare and difficult to diagnose. We report a 14-year-old boy who presented with a several month history of an asymptomatic right cervical swelling. Ultrasound and fine needle aspiration cytology revealed a vascular lesion. He was meticulously evaluated with CT scan and digital subtraction angiography. Surgical excision of the lesion was carried out and histological examination revealed a schwannoma. Schwannoma is a relatively rare tumor and even rarer in children. The incidence of such tumors and the management of our patie

[Bilateral paraganglioma of the vagus nerve].

Paragangliomas are rare, usually benign tumours of the head and neck region, located along the cranial nerves and the big vessels. Vagus paraganglioma represents less than 3% of all cases of paragangliomas of the head and neck. Approximately 10% of vagus paragangliomas are presented as bilateral masses. We present the case of 75 year old female with bilateral paraganglioma, and papillary cancer of the thyroid gland at the time of diagnosis. Diagnostic and therapeutic procedures are discussed with special regard to possible complications.

Awakening the Vagus: Transcutaneous Auricular Vagus Nerve Stimulation-A Narrative Review.

Disorders of consciousness (DoC), including unresponsive wakefulness syndrome/vegetative state (UWS/VS) and minimally conscious state (MCS), remain a major challenge in clinical neurology. Therapeutic options are limited, and prognosis is often uncertain. Transcutaneous stimulation of the auricular branch of the vagus nerve (taVNS) is a non-invasive neuromodulation technique that has gained increasing interest as a potential strategy to promote recovery of consciousness. This narrative review summarizes the anatomical and functional rationale of taVNS, compares invasive and non-invasive vagus

Functional vagotopy in the cervical vagus nerve of the domestic pig: implications for the study of vagus nerve stimulation.

OBJECTIVE: Given current clinical interest in vagus nerve stimulation (VNS), there are surprisingly few studies characterizing the anatomy of the vagus nerve in large animal models as it pertains to on-and off-target engagement of local fibers. We sought to address this gap by evaluating vagal anatomy in the pig, whose vagus nerve organization and size approximates the human vagus nerve. APPROACH: Here we combined microdissection, histology, and immunohistochemistry to provide data on key features across the cervical vagus nerve in a swine model, and compare our results to other animal models

Malignant peripheral nerve sheath tumors of intrathoracic vagus nerve.

The authors report two cases of malignant peripheral nerve sheath tumors arising from the vagus nerve in the mediastinum in patients who had no stigmata of neurofibromatosis (von Recklinghausen's disease). Computed tomography showed homogeneous soft tissue masses, with minimal calcification seen in one patient. T2-weighted magnetic resonance images showed regions of low signal intensity caused by a dense cellular population.

Vagus Nerve Stimulation in Ischemic Stroke.

PURPOSE OF REVIEW: Vagus nerve stimulation (VNS) has emerged as a potential therapeutic approach for neurological and psychiatric disorders. In recent years, there has been increasing interest in VNS for treating ischemic stroke. This review discusses the evidence supporting VNS as a treatment option for ischemic stroke and elucidates its underlying mechanisms. RECENT FINDINGS: Preclinical studies investigating VNS in stroke models have shown reduced infarct volumes and improved neurological deficits. Additionally, VNS has been found to reduce reperfusion injury. VNS may promote neuroprotecti

Vagus nerve stimulation for focal seizures.

BACKGROUND: This is an updated version of the Cochrane Review published in 2015. Epilepsy is a chronic neurological disorder, characterised by recurring, unprovoked seizures. Vagus nerve stimulation (VNS) is a neuromodulatory treatment that is used as an adjunctive therapy for treating people with drug-resistant epilepsy. VNS consists of chronic, intermittent electrical stimulation of the vagus nerve, delivered by a programmable pulse generator. OBJECTIVES: To evaluate the efficacy and tolerability of VNS when used as add-on treatment for people with drug-resistant focal epilepsy. SEARCH MET
