Reintervention Burden and Patency of Hemodialysis Vascular Access in a 15-Year Single-Center Cohort.

OBJECTIVE: Maintaining a hemodialysis access typically requires repeated intervention, yet the cumulative reintervention burden-as distinct from patency-is rarely quantified, and configuration-specific estimates from Asian centers are scarce. We evaluated patency and quantified the configuration-specific reintervention burden of upper-extremity arteriovenous accesses over 15 years at a single center. METHODS: We retrospectively analyzed 871 consecutive upper-extremity arteriovenous accesses (648 fistulas; 223 grafts) created in 810 patients between 2009 and 2024. Every subsequent procedure wa

Patency Test of Vascular Anastomosis with Assistance of High-Speed Video Recording in Digit Replantation.

BACKGROUND: Quality assessment of vascular anastomosis primarily depends on the experience of the treating surgeon. This highlights the need for an objective index. The main goal of our study was to establish a method of assessing the quality of vascular anastomosis in digit replantation. METHODS: A total of 182 digits from 141 patients were included in this study. The patients underwent replantation of completely amputated digits between June 1, 2015, and February 1, 2017. Patency tests of arterial and venous anastomoses were conducted for each replantation and recorded on digital video at 1

Nursing and midwifery practice for maintenance of vascular access device patency. A cross-sectional survey.

BACKGROUND: Up to 85% of hospital in-patients will require some form of vascular access device to deliver essential fluids, drug therapy, nutrition and blood products, or facilitate sampling. The failure rate of these devices is unacceptably high, with 20-69% of peripheral intravenous catheters and 15-66% of central venous catheters failing due to occlusion, depending on the device, setting and population. A range of strategies have been developed to maintain device patency, including intermittent flushing. However, there is limited evidence informing flushing practice and little is known abou

Host Cell Membrane-Cloaked Nanomedicine Reprograms Endothelial Surface for Enhanced Patency in Allogeneic Vascular Grafts.

The endothelium serves as the first point of contact following the implantation of allogeneic vascular grafts (AGs) or organs. Rapid endothelial injury triggered by immunocytotoxicity, combined with subsequent endothelial dysfunction during vascular tissue remodeling, collectively exacerbates complications, including thrombosis, inflammation, and intimal hyperplasia, ultimately leading to graft failure. In this study, a nanomedicine, R-SB@PLGA, is developed for use in metabolically guided cell surface reprogramming (CSR) in AGs. R-SB@PLGA is fabricated by encapsulating SB431542 (SB)-loaded pol

Vascular patency of fibular free graft: assessment by Doppler color-flow imager: a case report.

Viability of a vascularized free fibula transfer can be difficult to establish. It is possible, however, to assess patency of the involved microvascular anastomoses and blood flow in the vessels of the transposed fibula. A Doppler color-flow imager can be used to evaluate blood flow and vascular patency. The imager potentially can be employed in the operating room, during the immediate postoperative period, and for long term follow-up. It is non-invasive and produces a permanent record of vascular anatomy and flow characteristics.

Soft hydrophilic interfaces boost endothelial selectivity of bioactive peptides for long-term vascular graft patency.

The surface modification of bioactive molecules is thought to aid endothelial cell adhesion, which is crucial for achieving rapid endothelialization of vascular grafts and thus ensuring long-term patency. However, conventional hydrophilic coatings possess inherent limitations in resisting nonspecific adsorption, making it difficult to maintain selectivity for endothelial cells in complex blood environments. As a result, the deposition of nonspecific proteins and cells on the surface may trigger neointimal hyperplasia and luminal stenosis, ultimately leading to graft failure. This study propose

Gelatin microgel-coated balloon catheter with enhanced delivery of everolimus for long-term vascular patency.

In-stent restenosis (ISR) after percutaneous coronary intervention is a major reason for limited long-term patency due to complex neointimal proliferation caused by vascular injury. Drug-coated balloon (DCB) has been developed to treat various cardiovascular diseases including ISR by providing anti-proliferative drugs into blood vessel tissues. However, a significant proportion of the drug is lost during balloon tracking, resulting in ineffective drug delivery to the target region. In this study, we report an everolimus-coated balloon (ECB) using everolimus-loaded gelatin-hydroxyphenyl propion

A multicenter study of permanent hemodialysis access patency: beneficial effect of clipped vascular anastomotic technique.

OBJECTIVE: There is an urgent and compelling need to reduce the morbidity and expense of maintaining hemodialysis vascular access patency. This large, long-term, retrospective, multicenter study, which compared access patency of autogenous arteriovenous fistulas (AVF) and synthetic bridge grafts (AVG) created with conventional sutures or nonpenetrating clips, was undertaken to resolve conflicting results from previous smaller studies. DESIGN: Patency data for 1385 vascular access anastomoses (clipped or sutured) was obtained from 17 hospitals and dialysis centers (Appendix). Five hundred eigh

Operative management of vascular thoracic outlet syndrome (VTOS) over two decades: patency, complications, and functional outcome.

BACKGROUND: Vascular thoracic outlet syndrome (VTOS), encompassing both venous and arterial subtypes, arises from extrinsic compression of the subclavian vessels within the thoracic outlet, leading to significant morbidity. Surgical decompression remains the definitive treatment to restore vascular patency and relieve neurovascular compromise. OBJECTIVES: This study analyzed the long-term outcomes of surgical management of VTOS over a twenty-year period, focusing on vessel patency, recurrence, functional recovery, and procedure-related complications. METHODS: A retrospective review was conduct

Ultrasonographic evaluation of vascular remodeling and patency after drug-coated balloon treatment for arteriovenous fistula stenosis.

BACKGROUND: Drug-coated balloons (DCBs) have shown promise in prolonging arteriovenous fistula (AVF) patency by inhibiting neointimal hyperplasia. However, few studies have longitudinally evaluated vascular remodeling after DCB treatment using ultrasound. This study aimed to evaluate vascular remodeling after DCB treatment using high-frequency ultrasound. METHODS: In this retrospective single-center study, 50 patients with single AVF stenosis were treated with DCB and followed by ultrasound at four time points: pre-treatment, immediately post-treatment, 3 months, and 6 months. Patients were c

Society for Vascular Surgery femoral runoff score is associated with limb-based patency after aortofemoral bypass.

OBJECTIVE: Despite advancements, aortofemoral bypass (AFB) remains the most durable option for aortoiliac occlusive disease. Although runoff has been shown to be associated with AFB patency, the association of the Society for Vascular Surgery (SVS) thigh runoff scoring system with patency has not been assessed. The aim of the present study was to evaluate the association between the SVS runoff scoring system and limb-based primary patency after AFB. METHODS: Institutional data for patients undergoing AFB with preoperative runoff imaging available from 2000 to 2017 were queried. Runoff scores

Bioinspired, Anticoagulative, 19 F MRI-Visualizable Bilayer Hydrogel Tubes as High Patency Small-Diameter Vascular Grafts.

The clinical patency of small-diameter vascular grafts (SDVGs) (ID < 6 mm) is limited, with the formation of mural thrombi being a major threat of this limitation. Herein, a bilayered hydrogel tube based on the essential structure of native blood vessels is developed by optimizing the relation between vascular functions and the molecular structure of hydrogels. The inner layer of the SDVGs comprises a zwitterionic fluorinated hydrogel, avoiding the formation of thromboinflammation-induced mural thrombi. Furthermore, the position and morphology of the SDVGs can be visualized via 19 F/1 H magnet

Patency of saphenous vein coronary artery bypass grafts from the vascular prosthesis of the ascending aorta.

BACKGROUND: Patients who have Stanford type A aortic dissection with impaired coronary arteries or who have aneurysms from the ascending aorta to the aortic arch with coronary artery disease need coronary artery bypass grafting (CABG) with tube graft replacement of the ascending aorta simultaneously. When vein grafts are used for CABG in these patients, the proximal anastomoses of vein grafts are attached to the prosthetic tube graft of the ascending aorta. However, the validity of proximal anastomoses of vein grafts to the prosthetic tube graft of the ascending aorta has not been confirmed.

Patency of arteriovenous fistulas in hemodialysis in French Guiana: influence of vascular flow, climate and clinical factors.

INTRODUCTION: Arteriovenous fistula (AVF) is the preferred access for hemodialysis. Achieving one-year patency is crucial to avoid dependence on central venous catheters, which carry significant risks of infection and thrombosis. In French Guiana, a territory marked by precariousness, isolation, and a tropical climate, local determinants of patency are poorly documented. This study aims to evaluate AVF patency at 6 weeks and one year and to identify associated factors. METHODS: This is a single-center retrospective study including all AVFs created at the Cayenne Hospital Center between 2018 an

Case Report: Vascular access in paediatric haemodialysis patients-creating and maintaining the patency of an arteriovenous fistula.

INTRODUCTION: Paediatric vascular access is a demanding field. The need for a multidisciplinary team is mandatory in children with end-stage kidney disease (ESKD). Central venous catheters (CVCs) remain the preferred option worldwide. Recent emerging data demonstrated the benefits of using arteriovenous fistulas (AVFs) in the paediatric population for long-term vascular access. The small vessel size in children represents a surgical challenge for vascular access. CASE PRESENTATION: We report three cases from our haemodialysis department and the difficulty in maintaining permanent vascular acc

Percutaneous crossover rotational atherectomy recanalization: A case about health status and patency in a multimorbid vascular patient.

INTRODUCTION: Endovascular treatment of the common femoral artery (CFA) and its branches is often challenging. Sometimes, stent placement cannot be avoided. Furthermore, stent placement in this area carries several risks for complications. We present a challenging case in which we used a rotational atherectomy device percutaneously in cross-over-technique to recanalize the external iliac artery in combination with the femoral bifurcation, all in one session - and - without the need for a femoral stent in a multimorbid patient. We also tried to gain more insights in the patient's perspective an

Arteriovenous fistula creation using the Optiflow™ vascular anastomotic connector: the OPEN (Optiflow PatEncy and MaturatioN) study.

PURPOSE: Arteriovenous fistulas (AVFs) are the preferred form of vascular access for hemodialysis. However, non-maturation and patency are major clinical problems. The Optiflow™ device is an implantable anastomotic connector used to standardize the creation of an AVF. Studies have suggested that the geometry of the anastomosis and experience of the surgeon impact patency and maturation rates. The Optiflow serves as a surgical template whereby the geometry and flow path of the anastomosis are predetermined. This prospective study was intended to evaluate maturation, patency and safety of the Op

A machine learning approach for identification of vascular access patency in hemodialysis patients using photoplethysmography: A pilot study.

INTRODUCTION: Vascular access (VA) is essential for patients with hemodialysis, and its dysfunction is a major complication that can reduce quality of life or even threaten life. VA patency is not only difficult to predict on an individual basis, but also challenging to predict in real-time. To overcome this challenge, this study aimed to develop a machine learning approach to predict 6-month primary patency (PP) using photoplethysmography (PPG) signals acquired from the tips of both index fingers. MATERIALS AND METHODS: PPG signals were obtained from hemodialysis patients who received an art
