During a 5-year period between 1973 and 1977, 87 patients underwent vasovasostomy to correct postvasectomy sterility. Sixty-one patients had single-layer macroscopic reanastomoses and 26 patients had two-layer microscopic reanastomoses. Patency rates of 90% and 96% and pregnancy rates of 46% and 54% for the macroscopic and microscopic techniques were achieved.

BACKGROUND: Vasovasostomy (VV) is often pursued by men seeking natural conception after vasectomy. Microsurgical VV is associated with high patency rates (~95%) and moderate pregnancy rates (~40%), according to existing literature. However, long-term outcome data remain limited. This review evaluates patency and pregnancy rates at ≥12 months following microsurgical VV.
MATERIALS AND METHODS: A comprehensive literature search was conducted via PubMed using the terms "vasovasostomy," "patency rate," and "pregnancy rate." Studies were included if they reported patency or pregnancy outcomes ≥12 m

There are no major short- or long-term changes in pituitary testicular axis due to vasectomy. The response of the prostate and epididymis to endogenous may be altered after vasectomy. However, the secretory functions of the two organs return to normal after vasovasostomy. The effects of vasectomy and vasovasostomy on the pituitary gonadal axis and on the accessory sex organs' function are reported.

Vasovasostomy success rates improved with the application of the operative microscope in 1975. The robotic platform offers potential advantages including: a stable, ergonomic, scalable control system with three-dimensional visualization and magnification, the elimination of physiological tremor, and simultaneous control of three instruments and a camera. A previous publication revealed a fellowship-trained microsurgeon (PKK) could transition to robot-assisted microsurgical vasovasostomy (RAVV) with comparable outcomes. The objective of this current study was to evaluate the learning curve for

Optimal vision and ergonomics are essential factors contributing to the achievement of good results during microsurgery. The three-dimensional (3D) digital image microscope system with a better 3D depth of field can release strain on the surgeon's neck and back, which can improve outcomes in microsurgery. We report a randomized prospective study of vasoepididymostomy and vasovasostomy using a 3D digital image microscope system (3D-DIM) in rats. A total of 16 adult male rats were randomly divided into two groups of 8 each: the standard operating microscope (SOM) group and the 3D-DIM group. The

PURPOSE: After undergoing vasectomy approximately 4% to 6% of men change their minds and desire more children. In the past they had 2 options: 1) vasectomy reversal and 2) sperm retrieval and intracytoplasmic sperm injection. However, in our practices we began to receive requests for another option: reconstructive microsurgery after failed percutaneous epididymal sperm aspiration.
MATERIALS AND METHODS: In this report we combined our collective experience with 8 patients who requested reconstructive microsurgery after failed percutaneous epididymal sperm aspiration and intracytoplasmic sperm

PURPOSE: Microsurgical vasovasostomy and vasoepididymostomy remain technically challenging procedures. Refinements in technique have continually improved patency and pregnancy rates for the 2 procedures in experienced hands. Advances in surgical robotics produced the Da Vinci robot (Intuitive Surgical, Inc., Sunnyvale, California) with motion reduction and no tremor, features that may improve outcomes in microsurgery. We report a randomized prospective study of vasoepididymostomy and vasovasostomy using the Da Vinci robot in rats.

Seminal duct obstruction may result in obstructive azoospermia (OA) and severe oligoasthenoteratozoospermia (OAT) (<0.5 million/mL) (Nordhoff et al., 2015). Cases of partial OA and OAT can be treated effectively by microsurgical anastomosis (Goldstein and Kim, 2013) to obtain successful surgical reversal. However, microsurgical vasovasostomy (VV) (Dickey et al., 2015) and vasoepididymostomy (VE) (Peng et al., 2017) are not suitable for patients with atypical OA and poor epididymis conditions or unpredictable obstruction of the distal vas deferens. For those patients, cross anastomosis may be a

Sperm agglutinins emerging from the reopened part of the vas deferens were found in the seminal fluid of 10 out of 29 men after vasovasostomy. There was a strong association between the presence of these antibodies and the failure to impregnate over a period of 14 to 33 months. Of 20 couples, 11 pregnancies resulted among the 13 women whose men had no sperm agglutinins in the seminal fluid, but only 1 among the 7 whose men had such antibodies, 2p = 0.0044. The presence of sperm agglutinins in the seminal fluid was always associated with their presence in the serum, usually in a higher titre. P

WS represents the standard procedure of choice for the treatment of obstructive azoospermia following vasectomy. However, recently, ICSI has been suggested by some to represent the solution for all cases of male factor infertility regardless of its etiology based on its success rates. Therefore, we compared VVS to MESA/TESE and ICSI in terms of pregnancy, complications, and costs. Between 1/93 and 6/98 157 VVS was performed microsurgically using the 2-layer technique in 157 patients following prior vasectomy. Between 9/94 and 9/97 69 couples underwent MESA/ICSI for epididymal obstruction not a

OBJECTIVE: This retrospective study aimed to evaluate the effectiveness of low-dose prednisone as a rescue therapy for patients with deteriorating semen parameters following vasovasostomy.
MATERIALS AND METHODS: Electronic medical records were queried at the University of Miami with documented CPT code 55400 (Bilateral Vasovasostomy) between January 2016 and April 2023. Records were then reviewed to identify patients who demonstrated ≥50% decrease in semen parameters, specifically sperm concentration, motility and total motile sperm count. Patients who were treated with 6 weeks of low-dose pr

Obstruction of the vas deferens may occur after inguinal hernia repair with mesh and lead to infertility. In cases where natural conception is desired and after obtaining test results that suggest the presence of spermatogenesis, surgical reconstruction can be attempted but may be difficult. Several approaches have been reported, including the laparoscopic mobilisation of the pelvic vas deferens, as well as mobilisation and passage of the scrotal vas deferens intra-abdominally for robot-assisted vasovasostomy. We describe a novel approach that used the surgical robot and a no-touch technique t

Cross-sections of human spermatic cords and vasectomy specimens were prepared and the number and cross-sectional area of nerves were determined. On average, about one-half of all nerves in the near neighborhood of the vas deferens were resected during vasectomy. The total cross-sectional area of the nerves along the vasectomy specimens amounted to about one-half of the total area in the spermatic cord samples. The data support the hypothesis that removing nerves to the vas deferens during vasectomy could result in poor functional results after vasovasostomy, i.e., that powerful contraction of

PURPOSE: Vasovasostomy and vasoepididymostomy are technically challenging microsurgical reconstructive procedures necessary for men with obstructive azoospermia at the level of the vas deferens or epididymis. Patency rates following vasovasostomy or vasoepididymostomy have been widely described in the literature. However, few reports have discussed the timing of sperm return to the ejaculate after reconstruction as well as the proportion of men in whom late failure develops following vasovasostomy or vasoepididymostomy. Therefore, the objective of this article was to review the rates and predi

BACKGROUND: Microsurgical 2-layer vasovasostomy is a commonly used technique to treat vasal obstruction in male infertility, and optimizing the surgical approach has important clinical implications.
OBJECTIVE: To compare the differences between stent-assisted vasovasostomy and the conventional technique in terms of operative time, vasal patency rate, and reproductive function.
MATERIALS AND METHODS: This experimental study included 54 male Sprague-Dawley rats (8-9 wk, 300 ± 15 gr) and 12 female rats (7-8 wk, 250 ± 13 gr, used for co-housing experiments). The male rats were randomly assigned

This article reviews the outcomes for vasovasostomy (VV) when only sperm parts were present in the vasal fluid. Thirtyfour patients who underwent bilateral (31) or unilateral (3) VV had either sperm parts bilaterally or sperm parts on 1 side and intravasal azoospermia on the contralateral side. Two of the procedures (1 unilateral, 1 bilateral) were repeat procedures. Patient and partner age were 42 +/- 1.2 (range: 34-54 and 33 +/- 0.9 (range: 23-42) years, respectively. Follow-up was 10 +/- 1.8 months. The obstructive interval was 10 +/- 0.9 (range: 4-27) years. The patency rate was 76% (26/34

BACKGROUND: The use of the microsurgical vasovasostomy technique allows for greater precision and the use of extra-fine sutures, resulting in improved outcomes for vasectomy reversal procedures. The challenging nature of these microsurgical procedures has led to the investigation of robotic-assisted microsurgical vasovasostomy for decreased hand tremor and increased dexterity and hand-eye coordination. To date, no research has investigated vasovasostomy using the Symani Surgical System, a robotic device approved by the US Food and Drug Administration (FDA) for microsurgical procedures. This st

OBJECTIVE: To investigate the factors influencing the recovery from childhood inguinal herniorrhaphy (IH)-induced vas deferens obstruction following microscopic vasovasostomy.
METHODS: We retrospectively analyzed the clinical data on 41 cases of microscopic vasovasostomy for obstructive azoospermia in our hospital from July 2015 to September 2018. All the patients had a history of inguinal hernia treated by IH in the childhood. We performed scrotal ultrasonography, semen analysis and seminal plasma biochemistry to confirm vas deferens obstruction preoperatively. If sperm was observed for ≥2 t

BACKGROUND: In the era of improving assisted reproductive technology (ART), patients with obstructive azoospermia (OA) have 2 options: vasal repair or testicular sperm extraction with intracytoplasmic sperm injection. Vasal repair, including vasovasostomy (VV) and vasoepididymostomy (VE), is the only option that leads to natural conception.
METHODS: This article reviews the surgical techniques, outcomes, and predictors of postoperative patency and pregnancy, with a focus on articles that have reported over the last 10 years, using PubMed database searches.
MAIN FINDINGS: The reported mean pa
