Prevention of venous thromboembolism.

Patients with clinical conditions such as surgery, trauma, and acute medical illness have a transiently increased risk of venous thromboembolism and merit consideration for adequate thromboprophylaxis. The choice of an appropriate pharmacologic or physical means of prophylaxis should be made taking into account both the thrombotic and bleeding risk associated with patient-related factors and the type of surgery or other disease state involved. A large number of randomized clinical trials, meta-analyses, and guidelines developed by scientific societies worldwide have addressed this issue and ha

Treatment of venous thromboembolism.

Deep vein thrombosis (DVT) and pulmonary embolism (PE) are two manifestations of the same disorder, venous thromboembolism, and low-molecular weight heparin is the treatment of choice for both DVT and PE. Alternatively, intravenous adjusted-dose unfractionated heparin can be used in hemodynamically unstable patients with massive PE. Secondary thromboprophylaxis with vitamin K-antagonists (VKA) should be started as soon as the diagnosis is confirmed. The dose of VKA should be adjusted to a target international normalized ratio (INR) of 2.5. For most patients with PE, thrombolysis is not recomme

Venous thromboembolism after trauma.

PURPOSE OF REVIEW: The trauma population is at increased risk of venous thromboembolic disease, a potentially preventable cause of mortality and morbidity. Although the association between trauma and venous thromboembolism has been recognized for more than a century, there is still great variability in the clinical practices with respect to prophylaxis. This thorough review of recent literature aims to clarify the incidence and risk factors for deep venous thrombosis and pulmonary embolism after trauma, review options and recommendations for detection of deep venous thrombosis and pulmonary em

Treatment of venous thromboembolism.

Venous thromboembolism (VTE), comprised of pulmonary embolism (PE) and deep vein thrombosis (DVT), is a disease entity with a significant morbidity and mortality. Anticoagulation, initially with intravenous heparin and followed with long term warfarin treatment is the traditional therapy for VTE. Low molecular weight heparin, (LMWH) has a greater bioavailability than unfractionated heparin and can be administered subcutaneously. LMWH has resulted in shorter hospitalizations, reduced inicidents of major bleeding complications, and has moved the treatment of VTE for selected patients to the out-

Epidemiology of venous thromboembolism.

This review of the epidemiology of venous thromboembolism includes estimates of incidence and prevalence of venous thrombosis and its sequelae, a discussion geographical, annual and seasonal variations and data concerning possible risk factors. Selection of patients at increased risk for development of deep venous thrombosis or pulmonary embolism for specific diagnostic screening or for prophylactic therapy with low-dose heparin may be a more effective approach to lowering morbidity and mortality from this disease.

[Venous thromboembolism : therapeutic strategies].

Venous thromboembolism is one of the leading causes of morbidity and mortality. It is an insidious and potentially life-threatening disease with a clinical course characterized by a risk of recurrence and long-term sequelae, such as chronic thromboembolic pulmonary hypertension and post-thrombotic syndrome. Anticoagulant therapy is the cornerstone of venous thromboembolic disease management. This article summarizes current knowledge on the duration of anticoagulant therapy and available treatment strategies, based on recently published studies.

Venous Thromboembolism in Pregnancy.

Pregnancy considerably increases the risk of venous thromboembolism (VTE), encompassing deep vein thrombosis and pulmonary embolism. The treatment of these disorders poses unique diagnostic and therapeutic challenges, particularly in balancing maternal and fetal safety. Interventional radiology plays a critical role in the management of pregnancy-associated VTE through advanced imaging techniques and minimally invasive procedures such as catheter-directed therapy and inferior vena cava filter placement. This review explores the epidemiology, pathophysiology, and management strategies for VTE i

[Contraception and venous thromboembolism].

Combined oral contraceptives remain in 2020 the most used contraceptive method in Switzerland and Europe, and are found in about half of venous thromboembolism (VTE) occurring in women aged up to 50 years. In this narrative review, we describe the determinants of the VTE risk, related to the types of oral contraceptives and to genetic or acquired risk factors of users, while summarizing several current recommendations of prescription for contraceptives. The complex management of contraception at the time of VTE should be discussed with patients, in order to minimize the risks of undesired preg

Introduction: Obstetric venous thromboembolism.

How to best reduce maternal risk from obstetric venous thromboembolism (VTE) is a relatively controversial topic. In comparison, for other leading causes of maternal mortality and severe morbidity such as obstetric hemorrhage and hypertension, there is general agreement on recommendations. While obstetric VTE poses a unique epidemiological and public health challenge, a number of recommendations related to care improvement and patient safety can be made. This edition of Seminars in Perinatology focuses on (i) overview of clinical research and epidemiology that can serve as a basis for informed

Controversies in Venous Thromboembolism.

Numerous controversies exist in the diagnosis of venous thromboembolism despite all the guidelines that are currently available. Screening of malignancy in venous thromboembolism has been a debated issue, as it has not been shown to change mortality. Calf vein thrombosis is also a controversial topic, but evidence points to risk stratifying those patients. Overdiagnosis, most notably from the finding of subsegmental pulmonary embolism, ultimately requires evaluating the risk and benefit for each patient. In addition, treatment of upper extremity deep vein thrombosis is a challenging scenario t

[Venous thromboembolism during pregnancy].

Pulmonary embolism is one of the most common cause of maternal death in developed countries. Pregnancy is associated with a hypercoagulable state, increased especially in patients with thrombophilia. The post-partum period is the period carrying the highest risk of venous thromboembolism, especially after caesarean delivery. The diagnosis is essential, applying strategies validated in the non-pregnant population, as none of the diagnostic tests is contra-indicated during pregnancy. These strategies use a combination of empirical evaluation of clinical probability, D-Dimer measurement. In case

Epidemiology of venous thromboembolism.

Thrombosis can affect any venous circulation. Venous thromboembolism (VTE) includes deep-vein thrombosis of the leg or pelvis, and its complication, pulmonary embolism. VTE is a fairly common disease, particularly in older age, and is associated with reduced survival, substantial health-care costs, and a high rate of recurrence. VTE is a complex (multifactorial) disease, involving interactions between acquired or inherited predispositions to thrombosis and various risk factors. Major risk factors for incident VTE include hospitalization for surgery or acute illness, active cancer, neurological

Treatment of venous thromboembolism.

IMPORTANCE: Venous thromboembolism (VTE), comprising deep vein thrombosis (DVT) and pulmonary embolism (PE), is a common, potentially lethal condition with acute morbidity. OBJECTIVE: To review the etiology of VTE and the 3 phases of VTE treatment: acute (first 5-10 days), long-term (from end of acute treatment to 3-6 months), and extended (beyond 3-6 months). EVIDENCE REVIEW: Cochrane reviews, meta-analyses, and randomized controlled trials, as well as other clinical trials for topics not covered by the former, were reviewed. Literature searches using broad terms were used to find meta-anal

[Treatment of venous thromboembolism].

The main objectives in the treatment of venous thromboembolism are to prevent clot extension and pulmonary embolism, to reduce mortality and to prevent recurrent thromboembolic events as well as postthrombotic disorders. Initial and effective anticoagulation with heparin, preferably with low molecular weight heparin (LMWH), or with fondaparinux is the most important measure. Unfractioned heparin (UFH) is as effective as LMWH, but requires coagulation-monitoring and is associated with a higher risk of heparin-induced thrombocytopenia. In patients with renal insufficiency direct determination of

Venous thromboembolism: the intricacies.

Venous thromboembolism (VTE) has been a subject of great interest of late. Since Rudolph Virchow described the famous Virchow's triad in 1856, there have been rapid strides in the understanding of the pathogenesis and factors responsible for it. Discovery of various thrombophilic factors, both primary and acquired, in the last 40 years has revolutionized prognostication and management of this potentially life-threatening condition due to its associated complication of pulmonary thromboembolism. Detailed genetic mapping and linkage analyses have been underlining the fact that VTE is a multifact

Venous thromboembolism in pregnancy.

The purpose of this review is to summarize the epidemiology of venous thromboembolism (VTE) in pregnancy and describe strategies used to prevent and treat it. The main reason for the increased risk of VTE in pregnancy is hypercoagulability. The hypercoagulability of pregnancy, which has likely evolved to protect women from the bleeding challenges of miscarriage and childbirth, is present as early as the first trimester and so is the increased risk of VTE. Other risk factors include a history of thrombosis, inherited and acquired thrombophilia, certain medical conditions, and complications of p

Venous thromboembolism in women.

The risk of venous thromboembolism (VTE) varies throughout a woman's life and is associated primarily with underlying hormonal exposure. Alteration in hemostatic mechanisms, including resistance to activated protein C, may explain this altered risk. Initially, development of VTE with the use of contraception in young adulthood may reveal inherited thrombophilia. Pregnancy, and particularly the post-partum period, likely confer the greatest risk of VTE, but the absolute risk is small. Guidelines for prevention of VTE during pregnancy are based on personal or family history of VTE, and known inh
