Because of its wide range of presentations, its highly variable mode of onset, its numerous causes, and its unpredictable outcome, cerebral venous thrombosis (CVT) remains a diagnostic and therapeutic challenge. Treatment of CVT consists primarily of symptomatic treatment of seizures and intracranial hypertension, antithrombotics, and etiologic treatment whenever possible. Heparin remains the first line of treatment for CVT; although its systematic use remains debated, recent studies have confirmed its safety even in patients with large hemorrhagic infarctions. INTRODUCTION: Cerebral venous thrombosis (CVT) is a type of pathology which has many predisposing factors and forms of presentation, which make diagnosis and prognosis difficult. The objective of this paper is to review, and bring up to date, different aspects of knowledge concerning: aetiology, clinical features, diagnosis and treatment.
MAIN FINDINGS: The proportion of cases of unknown aetiology is between 20-35% in spite of new causes being described. Magnetic resonance and recently, angiographic resonance are the diagnostic methods of choice, although conventional angiography is still nec

Venous thromboembolism (VTE) is a significant cause of morbidity and mortality in hospitalized patients. For many years, stasis in the deep veins of the legs has been recognized as being a necessary but not a sufficient cause of thrombosis. The recent demonstration of the high prevalence of a mutation at Arg506 in the factor V gene ('factor V Leiden') in patients presenting with VTE has emphasized the protective role of activated protein C in neutralizing thrombin, and has highlighted the importance of genetic factors in the pathogenesis of venous thrombosis. For many if not most patients, the

Sixteen patients with mesenteric venous thrombosis were reviewed retrospectively during a period from 1983 to 1987. Twelve patients had progressive abdominal pain, three had gastrointestinal bleeding, and one had general malaise. Seven of these 16 patients had previous deep-vein thrombosis. After negative routine gastrointestinal and hepatobiliary evaluation, 11 patients underwent an infusion computerized tomographic scan. Of these, 10 had superior mesenteric vein thrombosis; three of these 10 patients had portal vein thrombosis. Selective arteriography was done in two patients because of gast

In neonates, thrombosis beginning in small renal veins and progressing to larger veins is best termed renal venous thrombosis (RVT) since the renal vein is not usually concerned. RVT occurs dominantly in the new-born and affects males more often than females (2:1). Hyperosmolality, maternal prediabetes and angiocardiography contribute to the occurrence or RVT. Early signs and symptoms are largely non-specific with the most reliable being the presence of haematuria (49%) or a palpably enlarged and hard kidney (60%). A falling platelet count, raised FDP level of falling plasminogen level support

OBJECTIVE: Although superficial venous reflux is an important determinant of post-thrombotic skin changes, the origin of this reflux is unknown. The purpose of this study was to evaluate the frequency and etiologic mechanisms of superficial venous reflux after acute deep venous thrombosis (DVT).
METHODS: Patients with a documented acute lower extremity DVT were asked to return for serial venous duplex ultrasound examinations at 1 day, 1 week, 1 month, every 3 months for the first year, and every year thereafter. Reflux in the greater saphenous vein (GSV) and lesser saphenous vein (LSV) was as

The incidence of thrombotic disorders in neonates and children is increasing with advances in diagnostic modalities, supportive care, and management of many health conditions. The developing coagulation system, need for intensive care, including catheterization, and co-morbid conditions are responsible for the relatively high risk of thrombosis in neonates compared to older children. This review addresses the advances over the last 3 years in neonatal thrombosis, with a focus on epidemiology, cerebral sinovenous thrombosis (CSVT), renal vein thrombosis (RVT), and portal vein thrombosis (PVT).

Venous thrombo-embolic disease (VTE) is a frequent pathology, responsible for around 10,000 deaths each year in France. Post-thrombotic sequellae is also a main problem related to the disease. The epidemiology of acquired and genetic risk factors of VTE is better understood and has allowed to show that in many cases it is a multifactorial disease. The association of several environmental or genetic and their analysis in the physiopathology of the disease and in the risk of recurrency is important in order to allow a better prevention of the disease and of its recurrencies.

Lung embolism is one of the most common causes of death during pregnancy and the puerperium. It is usually due to thrombosis of the deep leg and pelvic veins. As a result of modern imaging methods, it can be reliably diagnosed at an early stage. In some cases, special haemostaseological tests can identify congenital or acquired defects. This allows an initial assessment of the individual risk of thrombosis to be made. Body weight-dependent anticoagulation therapy with heparin is the major medication-based treatment. Compression treatment can be administered if accepted by the patient.

Chlamydia pneumoniae infection has been linked to atherosclerosis, but a possible relationship with venous thromboembolism (VTE) had not been sought. We determined circulating anti-C. pneumoniae antibody levels in patients with VTE. Fifty-four percent of the cases and 15.9% of the controls had specific IgG titers of at least 256 (p<0.0001). The crude odds ratio for VTE was 6.2 (95% CI, 3.8-10.1), and rose to 7.7 (4.5-13.2) after excluding subjects carrying the factor V Arg 506 Gln or factor II G 20210 A mutations. Other studies did not confirm such an association or found a lower odds ratio. T

Mechanical measures such as graduated compression stockings and intermittent compression boots are available for venous thrombosis prophylaxis, but compliance may be limited. Plantar venous pneumatic compression devices have attained widespread acceptance by both patients and nurses because of their comfort and compact size, but their track record for efficacy is poor. Inferior vena cava filters prevent pulmonary embolism, but do not halt the thrombotic process or prevent venous thrombosis. Pharmacologic prophylaxis traditionally has relied upon minidose unfractionated heparin; however, re-exa

Major studies addressing various aspects of the treatment of deep vein thrombosis are reviewed. It has lately been demonstrated that heparin should be dosed according to body weight and is preferably given as subcutaneous injections twice daily. Alternatively, low-molecular-weight heparins may be given in a fixed dose once daily, which does not require monitoring. Oral anticoagulation should be started concomitantly with heparin and targeted at an international normalized ratio of 2.0-3.0. This treatment should continue for a longer duration than previously accepted, in many cases for 6 months

It is imperative that the orthopedic surgeon recognize the magnitude of the problem of thromboembolic disease. It is the most common complication of trauma or surgical procedures. Because the clinical signs and symptoms of deep venous thrombosis are so variable, and since pulmonary embolism is often the first sign of deep venous thrombosis, we must continue to refine our techniques for screening and diagnosis. The 125-I fibrinogen localization of propagating venous thrombi in the legs is a very promising and sensitive screening procedure. With refinements, the Doppler effect flowmeter and impe

Pregnant women are at an increased risk of venous thromboembolism (VTE) and the consequences of an acute event in pregnancy can be debilitating, long-lasting or fatal. Screening for risk factors early in pregnancy and the provision of thromboprophylaxis are useful ways of preventing VTE in some women, but even when performed diligently, acute events are likely to remain common for the foreseeable future. It is therefore important for obstetric and non-obstetric clinicians to recognize the symptoms and signs of deep vein thrombosis and pulmonary embolism in pregnancy, to understand how a diagno

Cerebral sinus-venous thrombosis (CSVT) is a rare life-threatening disease with an estimated annual incidence of 3-4 cases per million in adults and 7 cases per million in neonates. Brain tumors, cerebral infections and traumas are local risk factors for CSVT, but the commonest encountered risk factors are oral contraceptive use, pregnancy and puerperium that make the disease predominant in female sex. In 15-20 % of patients, the disease remains unprovoked, i.e., occurring in the absence of predisposing factors. Thrombophilic abnormalities either inherited [deficiency of the natural anticoagul

Cerebral venous sinus thrombosis (CVST) is a rare and potentially deadly condition. Common etiologies include hypercoagulable diseases, low flow states, dehydration, adjacent infectious processes, oral contraceptives, hormonal replacement therapy, pregnancy, and puerperium. Symptoms include nausea, seizures, severe focal neurological deficits, coma, and headache (the most common presenting symptom). Anticoagulation is the mainstay of treatment for CVST. Transvenous clot lysis can be performed using injected thrombolytic agents and specialized catheters for clot retrieval.

Axillary-subclavian venous thrombosis (ASVT) is classified as either primary, which is often associated with active use of the upper extremities and underlying anatomic abnormalities, or secondary to recognized thrombotic risk factors. Although there is a risk of pulmonary embolism with both, late outcome varies with etiology. Primary ASVT occurs in young healthy patients, and up to 80% may have significant chronic symptoms. Secondary ASVT occurs in older patients with medical comorbidities, high mortality rates, and few late manifestations. Although anticoagulation is appropriate for most pat
