Predictors of Negative Pressure Ventilation Response in Pediatric Acute Respiratory Failure.

BACKGROUND: Use of negative pressure ventilation is neither well described nor widespread in pediatric critical care; existing data are from small, specialized populations. We sought to describe a general population of critically ill subjects with acute respiratory failure supported with negative pressure ventilation to find predictors of response or failure. METHODS: We conducted a retrospective cohort study of subjects 0-18 y old admitted to a single (non-cardiac) pediatric ICU who received acute respiratory failure support via negative pressure ventilation from May 2015 through May 2016.

Efficacy of positive vs negative pressure ventilation in unloading the respiratory muscles.

We compared the efficacy of positive pressure ventilation (PPV) vs negative pressure ventilation (NPV) in providing ventilatory muscle rest for five normal subjects and six patients with chronic obstructive pulmonary disease (COPD). All participants underwent measurement of transdiaphragmatic pressure (Pdi), pressure time integral of the diaphragm (PTI), integrated diaphragmatic electromyogram (iEMG), minute ventilation (Ve), tidal volume (Vt), and end-tidal CO2 (etCO2) during 15 minutes of PPV and NPV. For each subject, ventilator adjustments were made to obtain Ve similar to levels measured

Hemodynamic effects of continuous negative chest pressure ventilation in heart failure.

We have previously shown improved cardiac output (QT) with external continuous negative-pressure ventilation (CNPV) compared with continuous positive-pressure ventilation (CPPV) in dogs with low pressure pulmonary edema (1). The current study was done to determine if this effect was reversed in high pressure pulmonary edema. Seven supine, anesthetized dogs were fluid-loaded and treated with disopyramide (3.5 to 7.0 mg/kg) and propranolol (0.25 to 1.5 mg/kg). This produced a mean pulmonary wedge pressure (Ppaw) of 21.0 mm Hg on intermittent positive-pressure ventilation (IPPV). CPPV and CNPV we

Ventilatory and diaphragmatic EMG responses to negative-pressure ventilation in airflow obstruction.

To assess the responses of patients with chronic obstructive lung disease (COLD) to negative-pressure ventilation (NPV), we studied eight naive patients with moderate to severe COLD before (control) and during NPV with "low" (-10-cmH2O) and "high" (-30-cmH2O) pressure swings in a Drinker tank respirator. Tidal volume (VT) and minute ventilation (VE) were recorded from a Respitrace and diaphragmatic electromyogram (DEMG) from a bipolar esophageal electrode. During short, 5-min runs of "low" and "high" NPV, VT did not change and VE increased in a borderline significant way at -30-cmH2O NPV. Peak

Negative pressure ventilation in COPD patients with acute on chronic respiratory failure.

Negative pressure ventilators act by exposing the surface of the chest wall to subatmospheric pressure during inspiration. During negative pressure ventilation (NPV), tidal volume is related to the peak of the inspiratory negative pressure and the pressure waveform generated by the ventilator pump; for the same peak of negative pressure a square wave produces a greater tidal volume than a half sine wave. Several uncontrolled studies suggest that NPV may have a potential therapeutic role in the treatment of acute on chronic respiratory failure in patients with chronic obstructive pulmonary dise

Central alveolar hypoventilation (Ondine's curse) treated with negative pressure ventilation.

A 22 year old woman with central alveolar hypoventilation (CAH) underwent repeated nocturnal polysomnographic recordings to assess the efficacy of negative pressure ventilation (NPV) administered by pneumowrap. The NPV successfully prevented hypoxia during most of the recording time; however, a sudden severe hypoxic episode not related to an apnoea occurred during a recording night with NPV. This made home use advisable together with an ear oximeter fitted with an alarm.

Reintroduction of continuous negative pressure ventilation in neonates: two-year experience.

Continuous negative pressure ventilation utilizes subatmospheric pressure around the thorax to improve oxygenation. It has not been routinely used since the mid-1970s. We treated 37 infants with the combination of continuous negative pressure (CNP) and intermittent mandatory ventilation (IMV), after failing to attain a PaO2 of greater than or equal to 50 torr on IMV alone. Lung diseases included pulmonary interstitial emphysema (PIE), respiratory distress syndrome (RDS), and pulmonary artery hypertension (PAH) due either to meconium aspiration syndrome (MAS) or other causes (non-MAS). Negative pressure ventilation versus conventional mechanical ventilation in the treatment of acute respiratory failure in COPD patients.

This case-control study was aimed to evaluate the effectiveness of negative pressure ventilation (NPV) versus conventional mechanical ventilation (CMV) for the treatment of acute respiratory failure (ARF) in patients with chronic obstructive pulmonary disease (COPD) admitted to a respiratory intermediate intensive care unit (RIICU) and four general intensive care units (ICU). Twenty-six COPD patients in ARF admitted in 1994-95 to RIICU and treated with NPV (cases) were matched according to age (+/-5 yrs), sex, causes triggering ARF, Acute Physiology and Chronic Health Evaluation (APACHE) II sc

[Change of intracranial pressure in neurosurgical patients by hyperventilation, positive negative pressure ventilation and PEEP (author's transl)].

The effects of a hyperventilation, positive negative pressure ventilation (PNPV) and ventilation using positive endexpiratory pressure (PEEP), on intracranial pressure (ICP) was measured in 24 patients suffering from different neurosurgical disorders. The patients were given a basic anaesthesia including muscle relaxation. The investigations showed, that hyperventilation, followed by a PCO2 of 26 and 30 torr, clearly lowered the intracranial pressure. The pressure drop was much more pronounced when the preexisting ICP was high than when it was low, due to the form of the compliance curve of th

Novel use of negative pressure ventilation for weaning of venovenous extracorporeal membrane oxygenation.

Negative pressure ventilation (NPV) has been used to assist patients with respiratory failure. Venovenous extracorporeal membrane oxygenation (V-V ECMO) supports patients with acute lung injury who do not respond to conventional therapy. Some patients remain on prolonged ECMO support without access to lung transplantation. We report a 40-year-old woman cannulated for V-V ECMO due to refractory hypoxemia (PaO2/FiO2 of 58) on hospital day 20 for ARDS from bacteremia and native mitral valve endocarditis. After 108 days, she continued to require ventilator and ECMO support. Applying a negative-pre

Outcome after neonatal continuous negative-pressure ventilation: follow-up assessment.

BACKGROUND: A previous randomised trial of continuous negative extrathoracic pressure (CNEP) versus standard treatment for newborn infants with respiratory distress syndrome raised public concerns about mortality and neonatal morbidity. We studied the outcome in late childhood of children entered into the trial to establish whether there were long-term sequelae attributable to either mode of ventilation. METHODS: Outpatient assessment of neurological outcome, cognitive function, and disability was done by a paediatrician and a psychologist using standardised tests. 133 of 205 survivors from t

Negative pressure ventilation as haemodynamic rescue following surgery for congenital heart disease.

A low cardiac output state is an important cause of morbidity and mortality following repair of tetralogy of Fallot (ToF). This is often refractory to conventional measures. The cardiac output of these patients is highly dependent on diastolic pulmonary arterial flow which is enhanced during spontaneous respiration, but much reduced by intermittent positive pressure ventilation (IPPV). We report the successful use of negative pressure ventilation (NPV) as haemodynamic therapy in three children with a low output secondary to restrictive right ventricular (RV) physiology following ToF repair. Upper airway obstruction induced by negative-pressure ventilation in awake healthy subjects.

Negative-pressure ventilation (NPV) induces sleep-related upper airway obstruction. However, the precise mechanism and site of upper airway obstruction during NPV have not been worked out. We studied seven awake healthy volunteers (23-30 yr old) in an Emerson tank respirator. Subjects had the head outside the iron lung and breathed through a pneumotachograph, which yielded the airflow (V) signal. Supraglottic pressure (Psg) was measured with a catheter with the tip at the retroepiglottic level. Diaphragmatic electromyograms (EMGdi) were obtained from an esophageal bipolar electrode. Intermittent negative pressure ventilation in the treatment of respiratory failure in progressive neuromuscular disease.

Five men with degenerative neuromuscular diseases (three with amyotrophic lateral sclerosis [ALS] and two with Duchenne's muscular dystrophy [DMD]) who had respiratory failure were treated with intermittent negative pressure ventilation (NPV). One patient with ALS in severe acute respiratory failure was successfully treated with NPV alone. This patient and two other ALS patients in chronic respiratory failure with PaCO2 elevation stabilized or improved their vital capacity (VC) and lowered their PaCO2 after 5 to 11 weeks of therapy. Finally, intermittent NPV was used to replace 24-hour positiv

Historical and Potential Modern Uses of Negative-Pressure Ventilation for Critically Ill Patients.

Negative-pressure ventilation (NPV) is a form of noninvasive respiratory support in which an external subatmospheric pressure is applied to the thorax to facilitate lung expansion. Although largely supplanted by positive-pressure ventilation (PPV) in modern-day practice, NPV has garnered renewed interest as a potential noninvasive adjunct or alternative to PPV. Appropriate patient selection would be key, particularly in the ICU setting, where NPV is generally contraindicated in patients with severe upper airway obstruction, high oxygenation requirements, or absent airway reflexes. Respiratory outcome in late childhood after neonatal continuous negative pressure ventilation.

BACKGROUND: The outcome in late childhood for children entered into a randomised trial of continuous negative extrathoracic pressure (CNEP) versus standard respiratory management for the treatment of neonatal respiratory distress was studied. In the original trial, there were advantages in the duration of oxygen and the prevalence of chronic lung disease for those assigned to receive CNEP. AIM: To determine whether the above differences had persisted into childhood. METHODS: Outpatient evaluation of children by a paediatrician using Spirometry (Vitalograph Spirometer 2120, Ennis, Ireland) an

Negative-pressure ventilation improves cardiac output after right heart surgery.

BACKGROUND: A low cardiac output state can complicate the postoperative course of patients undergoing Fontan-type operations and tetralogy of Fallot repair. METHODS AND RESULTS: We investigated the effect of negative-pressure ventilation on cardiac output in 11 children in the early postoperative period after right heart surgery. All patients were initially ventilated with volume-cycled intermittent positive-pressure ventilation, and negative-pressure ventilation was delivered with the Hayek external high-frequency oscillator. Cardiac output was calculated by the direct Fick method, oxygen co

Effects of positive and negative pressure ventilation on cerebral blood volume of newborn infants.

The effects of intermittent positive airway and continuous negative extrathoracic pressure ventilation on cerebral blood volume in preterm infants were studied using near infrared spectroscopy. In 12 infants continuous negative extrathoracic pressure caused a median decrease in cerebral blood volume of 0.14 ml/100 ml brain (95% confidence intervals (CI) 0.035-0.280) compared with no respiratory support. Oxygenated and deoxygenated haemoglobin also decreased, implying increased venous drainage as the main effect. In 17 infants intermittent positive pressure ventilation also caused a median redu

Reversal of negative pressure ventilation-induced lower esophageal sphincter dysfunction with metoclopramide.

We have previously demonstrated that lower esophageal sphincter (LES) dysfunction is induced in healthy volunteers placed in negative pressure body ventilators. This is important, because regurgitation of gastric contents and peptic esophagitis are frequent complications of the use of such ventilators. The present study was conducted to determine whether LES dysfunction during the use of these ventilators also occurs in patients with chronic respiratory failure, and whether this dysfunction can be pharmacologically reversed. Seven patients with documented chronic respiratory failure due to COP
