Thalamic Nuclear Volumes in Fetal Alcohol Spectrum Disorders: From Adolescence to Middle-Age Twenty Years Later.

BACKGROUND: Midline orofacial and brain structures, including the multinucleated thalamus, may be differentially sensitive to prenatal alcohol exposure and vulnerable to accelerated aging. METHODS: Two sets of magnetic resonance imaging (MRI) data separated by 20 years are reported for control individuals, individuals with fetal alcohol syndrome (FAS), and nondysmorphic individuals with heavy fetal alcohol exposure (FAE). MRI1 included 179 participants, with 69 participants reassessed at MRI2. Segmentation produced estimates of bilateral thalamic volume and 10 bilateral nuclei, which were agg

Arterial Supply of the Thalamus: A Comprehensive Review.

The thalamus is a deep cerebral structure that is crucial for proper neurological functioning as it transmits signals from nearly all pathways in the body. Insult to the thalamus can, therefore, result in complex syndromes involving sensation, cognition, executive function, fine motor control, emotion, and arousal, to name a few. Specific territories in the thalamus that are supplied by deep cerebral arteries have been shown to correlate with clinical symptoms. The aim of this review is to enhance our understanding of the arterial anatomy of the thalamus and the complications that can arise fr

Thalamic connections of the caudal part of the posterior parietal cortex differ from the rostral part in galagos (Otolemur garnettii).

In this study, thalamic connections of the caudal part of the posterior parietal cortex (PPCc) are described and compared to connections of the rostral part of PPC (PPCr) in strepsirrhine galagos. PPC of galagos is divided into two parts, PPCr and PPCc, based on the responsiveness to electrical stimulation. Stimulation of PPC with long trains of electrical pulses evokes different types of ethologically relevant movements from different subregions ("domains") of PPCr, while it fails to evoke any movements from PPCc. Anatomical tracers were placed in both dorsal and ventral divisions of PPCc to

Leksell Radiosurgery for Movement Disorders.

Tremor is the most prevalent movement disorder in adults. Patients who are refractory to medical management can explore surgical intervention. Deep-brain stimulation (DBS) and radiofrequency thalamotomy (RFT) are surgical procedures for intractable tremor that target the ventralis intermedius (VIM) nucleus to relieve contralateral tremor. For patients who are not candidates for surgical procedures, stereotactic radiosurgery (SRS) is a minimally invasive management option for tremor relief. SRS has been used for the elderly, those considered high surgical risk for other surgical procedures, tho

Multi-atlas thalamic nuclei segmentation on standard T1-weighed MRI with application to normal aging.

Specific thalamic nuclei are implicated in healthy aging and age-related neurodegenerative diseases. However, few methods are available for robust automated segmentation of thalamic nuclei. The threefold aims of this study were to validate the use of a modified thalamic nuclei segmentation method on standard T1 MRI data, to apply this method to quantify age-related volume declines, and to test functional meaningfulness by predicting performance on motor testing. A modified version of THalamus Optimized Multi-Atlas Segmentation (THOMAS) generated 22 unilateral thalamic nuclei. For validation, w

Lack of Therapeutic Benefit of Ventral Intermediate Nucleus Thalamotomy for Tremor-Like Myoclonus in Two Cases of Benign Adult Familial Myoclonus Epilepsy.

INTRODUCTION: Benign adult familial myoclonus epilepsy (BAFME) is an autosomal dominant disorder characterized by adult-onset cortical tremor and infrequent generalized seizures. Treatment options for managing involuntary movements in BAFME remain limited. CASE PRESENTATION: Here, we present 2 cases involving individuals with BAFME who underwent Vim thalamotomy for tremor-like myoclonus. Despite the intervention, neither patient experienced any improvement in their symptoms. CONCLUSION: These findings suggest that the Vim may not contribute to the pathophysiology of tremor-like myoclonus in

Surgical treatment of essential tremor.

Surgical treatment for essential tremor (ET) has been used since the early 1950s. Initially, different areas were targeted for tremor control. However, the optimal target was eventually determined to be the ventralis intermedius (VIM) nucleus of the thalamus. Thalamotomy improves contralateral tremor in more than 90% of patients. Long-term studies of thalamotomy indicate that the benefits continue in most patients. Persistent morbidity associated with thalamotomy, which occurs in less than 10% of patients, includes dysarthria, dysequilibrium, weakness, and cognitive impairment. Bilateral thala

Deep brain stimulation of the Vim nucleus of the thalamus for the treatment of tremor.

The surgical treatment of tremor has evolved considerably in the past few years. Of the several conditions associated with severe tremor, the most common are Parkinson's disease (PD) and essential tremor (ET). Levodopa therapy reduced drastically the number of patients with PD who require surgery because of inadequate control. However, there remains a small number of "tremor dominant" PD patients for whom surgical treatment is often the best option. The ventralis intermediate nucleus (Vim) of the thalamus has been the preferred surgical target for the treatment of parkinsonian tremor for many

Volumetric Assessment and Graph Theoretical Analysis of Thalamic Nuclei in Essential Tremor.

INTRODUCTION: Essential tremor (ET) is a neurological disorder primarily characterized by upper limb action tremor. It is widely recognized that the thalamus is implicated in ET pathophysiology, playing a central role in treatment approaches. This study aimed to explore thalamic morphology, assessing macrostructural changes and intrinsic thalamic networks in ET patients. METHODS: A total of 109 ET (41 with and 68 without resting tremor) and 81 healthy controls (HC) were enrolled in the study. An automatic probabilistic segmentation of thalamic nuclei was employed on T1-weighted MRI images usi

Nigrothalamic projections in the monkey demonstrated by autoradiographic technics.

In spite of repeated demonstrations by degeneration technics, nigrothalamic fibers have been regarded with some skepticism. Attempts were made to trace nigral efferent projections in the monkey by autoradiographic technics. Tritiated amino acids (L-leucine, L-lysine and L-proline), injected into portions of the substantia nigra (SN), labeled cells in four regions, designated as, (1) rostrolateral, (2) caudolateral, (3) rostromedial and (4) central. Rostrolateral nigral neurons transported radioactive label preferentially and abundantly to thalamic nuclei; localized isotope was found in parts o

Early life stress causes refractoriness to haloperidol-induced catalepsy.

The use of classic antipsychotic drugs is limited by the occurrence of extrapyramidal motor symptoms, which are caused by dopamine (DA) receptor blockade in the neostriatum. We examined the impact of early-life stress on haloperidol-induced catalepsy using the rat model of prenatal restraint stress (PRS). Adult "PRS rats," i.e., the offspring of mothers exposed to restraint stress during pregnancy, were resistant to catalepsy induced by haloperidol (0.5-5 mg/kg i.p.) or raclopride (2 mg/kg s.c.). Resistance to catalepsy in PRS rats did not depend on reductions in blood or striatal levels, as c

Assessing the risk of central post-stroke pain of thalamic origin by lesion mapping.

Central post-stroke pain of thalamic origin is an extremely distressing and often refractory disorder. There are no well-established predictors for pain development after thalamic stroke, and the role of different thalamic nuclei is unclear. Here, we used structural magnetic resonance imaging to identify the thalamic nuclei, specifically implicated in the generation of central post-stroke pain of thalamic origin. Lesions of 10 patients with central post-stroke pain of thalamic origin and 10 control patients with thalamic strokes without pain were identified as volumes of interest on magnetic r

Progressive thalamic nuclear atrophy in blepharospasm and blepharospasm-oromandibular dystonia.

The thalamus is considered a key region in the neuromechanisms of blepharospasm. However, previous studies considered it as a single, homogeneous structure, disregarding potentially useful information about distinct thalamic nuclei. Herein, we aimed to examine (i) whether grey matter volume differs across thalamic subregions/nuclei in patients with blepharospasm and blepharospasm-oromandibular dystonia; (ii) causal relationships among abnormal thalamic nuclei; and (iii) whether these abnormal features can be used as neuroimaging biomarkers to distinguish patients with blepharospasm from blepha

Regional structural abnormalities in thalamus in idiopathic cervical dystonia.

BACKGROUND: The thalamus has a central role in the pathophysiology of idiopathic cervical dystonia (iCD); however, the nature of alterations occurring within this structure remain largely elusive. Using a structural magnetic resonance imaging (MRI) approach, we examined whether abnormalities differ across thalamic subregions/nuclei in patients with iCD. METHODS: Structural MRI data were collected from 37 patients with iCD and 37 healthy controls (HCs). Automatic parcellation of 25 thalamic nuclei in each hemisphere was performed based on the FreeSurfer program. Differences in thalamic nuclei

Thalamic afferents to the motor cortex in the cat. A horseradish peroxidase study.

Thalamic projections to the motor cortex have been studied in the cat by injecting small amounts of horseradish peroxidase in different parts of areas 4 and 6. The nuclei which more abundantly project to the motor cortex are the ventral anterior-ventral lateral complex, the ventral medial and the posterior complex. These projections are topographically organized. Other projections arise from the ventral postero-lateral, ventral postero-inferior, central lateral, central medial, medial dorsal, lateral posterior-pulvinar, suprageniculate and centrum medianum-parafascicular nuclei.

Organization of connections between the thalamic reticular and the anterior thalamic nuclei in the rat.

The thalamic reticular nucleus (TRN) receives topographically organized input from specific sensory nuclei such as the lateral geniculate nucleus. The present study shows this in the rat. However, the pattern of thalamic connections to the limbic reticular sector is unknown. Injecting biocytin into the ventral parts of anteroventral and anteromedial nuclei labeled neurons and axons in the rostral TRN. Filled axon collaterals and their terminals occupied a rectangular sheet in a plane close to the horizontal, and were confined to the inner zone (the medial portion) of the limbic TRN. Retrograde

The cerebellum contributes to generalized seizures by altering activity in the ventral posteromedial nucleus.

Thalamo-cortical networks are central to seizures, yet it is unclear how these circuits initiate seizures. We test whether a facial region of the thalamus, the ventral posteromedial nucleus (VPM), is a source of generalized, convulsive motor seizures and if convergent VPM input drives the behavior. To address this question, we devise an in vivo optogenetic mouse model to elicit convulsive motor seizures by driving these inputs and perform single-unit recordings during awake, convulsive seizures to define the local activity of thalamic neurons before, during, and after seizure onset. We find dy
