Implementing housing first in rural areas: pathways Vermont.

The benefits of Pathways Housing First in addressing chronic homelessness for persons with severe mental illness have been well established. However, the implementation and effectiveness of such programs in rural areas has yet to be examined. We described the model's adaptations in Vermont, including the use of hybrid assertive community treatment-intensive case management teams, which consisted of service coordinators with geographically based caseloads (staff/client ratio of 1:20) and regional multidisciplinary specialists. The program's innovative and widespread inclusion of technology into

Outbreak of Powassan encephalitis--Maine and Vermont, 1999-2001.

Powassan (POW) virus, a North American tickborne flavivirus related to the Eastern Hemisphere's tickborne encephalitis viruses, was first isolated from a patient with encephalitis in 1958. During 1958-1998, 27 human POW encephalitis cases were reported from Canada and the northeastern United States. During September 1999-July 2001, four Maine and Vermont residents with encephalitis were found to be infected with POW virus. These persons were tested for other arbovirus infections found in the northeast after testing for West Nile virus (WNV) infection was negative. This report describes these f

Elective resection for colorectal cancer in Vermont: 1971--1975.

An analysis of 612 patients undergoing elective resection for colon and rectal cancer in Vermont during the period 1971 to 1975 indicates an increasing incidence of cancers of the cecum and ascending colon, an increased number of Duke's A cancers, a synchronous cancer rate of 6 percent, an overall operative mortality of 6.5 percent, a wide variance in postoperative stay and a relatively small experience per operating surgeon. Only one third of colorectal cancers can be visualized through the rigid sigmoidoscope. Better screening techniques that are simple, reliable, inexpensive and safe must b

Eastern equine encephalitis in moose (Alces americanus) in northeastern Vermont.

During fall 2010, 21 moose (Alces americanus) sera collected in northeastern Vermont were screened for eastern equine encephalitis virus (EEEV) antibodies using plaque reduction neutralization tests. Six (29%) were antibody positive. This is the first evidence of EEEV activity in Vermont, and the second report of EEEV antibodies in moose.

The methadone epidemic: methadone-related deaths on the rise in Vermont.

The prevalence of methadone-related overdose deaths is increasing worldwide and has been a topic of recent debate. Methadone-related deaths, to this point, have not been systematically reviewed in the state of Vermont. All of the methadone-related fatalities from 2001 to 2006 (total, 76 cases), which were examined by the Vermont Office of the Chief Medical Examiner were retrospectively reviewed. The mean age of the decedents was 36 years (range, 16-74 years), and 72% were male. The manners of death were classified as follows: 84% accident, 12% undetermined, and 4% suicide. The mean level of me

An outbreak of norovirus gastroenteritis at a swimming club--Vermont, 2004.

John Snow's historic investigation of a severe epidemic of cholera traced the cause of infection to a common water source. Today, 150 years later, waterborne diseases remain a public health problem, and similar investigations are used to identify the source of infection. On February 3, 2004, the Vermont Department of Health (VDH) was notified of an outbreak of acute gastroenteritis among children whose only common exposure was attendance at a swimming club the previous weekend (January 31-February 1). This report summarizes the results of an investigation conducted by VDH and CDC, which determ

Establishment of an exposure level to tetrachloroethylene in ambient air in Vermont.

Where environmental contaminants pose potential health hazards, health departments are involved in complex and often controversial situations. Often the rapid formation of a threshold exposure level is required to protect public health. A decision making process was implemented in Vermont when it became necessary to have an interim ambient air exposure level to test for tetrachloroethylene contamination in the water, air, and soil of a community. Contamination of public and private drinking water and ambient air in schools and homes was discovered as a result of uncontrolled waste disposal fro

Mortality in Vermont granite workers and its association with silica exposure.

To assess mortality in Vermont granite workers and examine relationships between silica exposure and mortality from lung cancer, kidney cancer, non-malignant kidney disease, silicosis and other non-malignant respiratory disease. Methods Workers employed between 1947 and 1998 were identified. Exposures were estimated using a job-exposure matrix. Mortality was assessed through 2004 and standardised mortality ratios (SMRs) were computed. Associations between mortality and exposure to silica were assessed by nested case-control analyses using conditional logistic regression. Results 7052 workers h

Vermont granite mortality study: an update with an emphasis on lung cancer.

This mortality study extends the period of observation of an article published in 1988 of 5414 workers in Vermont granite sheds and quarries to assess whether previously reported reductions in silicosis and tuberculosis mortality were maintained. The relationship between lung cancer and quartz exposure is also examined by comparing mortality in workers hired before and after 1940, when dust controls were introduced and exposures were reduced by 80% to 90%. Before 1940, general stone shed air contained 20 million particles/cubic foot (mppcf) (approximately equivalent to 0.2 mg/m of quartz), and

Traditionality and cancer screening practices among American Indian women in Vermont.

The study purpose was to examine the relationship between cancer screening and traditionality, using a culturally specific survey instrument. American Indian (AI) women were randomly selected from the Vermont Breast Cancer Surveillance System (VBCSS), a statewide mammography database. The 13 items that assessed traditionality examined identity, cultural beliefs, customs, and health practices. The sample of 115 was predominately married, educated, middle class, insured, and rural. Significant associations were found between traditionality and past mammography and clinical breast examination (CB

Antecedents of neonatal encephalopathy in the Vermont Oxford Network Encephalopathy Registry.

BACKGROUND: Neonatal encephalopathy (NE) is a major predictor of death and long-term neurologic disability, but there are few studies of antecedents of NE. OBJECTIVES: To identify antecedents in a large registry of infants who had NE. METHODS: This was a maternal and infant record review of 4165 singleton neonates, gestational age of ≥ 36 weeks, meeting criteria for inclusion in the Vermont Oxford Network Neonatal Encephalopathy Registry. RESULTS: Clinically recognized seizures were the most prevalent condition (60%); 49% had a 5-minute Apgar score of ≤ 3 and 18% had a reduced level of cons

Characteristics, hospital charges, and effects of road transportation injuries in Vermont.

We studied 470 crash- and noncrash-related road transportation injuries to vehicle occupants, bicyclists, and pedestrians seen at a trauma center in Vermont. Among primary care patients, crash and noncrash groups had similar AIS and average disability at one year. Hospitalized patients had longer disability, but a majority of overall disability days and ongoing disability was found among nonhospitalized patients. Seventy-four percent of hospital bills were paid by insurance, 16% by government, 3% by self, with 7% unpaid. Limiting the sample only to hospitalized patients, or including referred

Legionnaires' disease: the epidemiology of two outbreaks in Burlington, Vermont, 1980.

Eighty-five cases of Legionnaires' disease were diagnosed in two major outbreaks at a large regional medical center in Burlington, Vermont, in the summer of 1980. Cases in both outbreaks were positive for Legionella pneumophila, serogroup 1 by culture, serology, or direct fluorescent antibody tests. All cases had spent time in the city of Burlington in the 10 days before the onset of symptoms. Cases in both outbreaks were both hospital- and community-acquired. A case-control study identified no common in-hospital exposure, including shower use, that was associated with illness. Cases without p

Supporting Student Health and Learning: Outcomes of Vermont School-Based Health Centers.

Since their inception in the 1990s, School-Based Health Centers (SBHCs) in Vermont have largely been conceived and sustained through the interest and investment of local communities. Vermont does not have a formal SBHC state policy, dedicated funding, or coordinated technical assistance. Recognizing the need to better understand and support SBHCs in our state, we conducted the first exploratory study of SBHC utilization in Vermont. Data were collected from six SBHCs operating in 16 different schools in Vermont over 2 weeks in the winter of 2025. We sought to determine reasons for seeking care

Vermont Primary Care Provider Perspectives on Psychedelics - A Cross-Sectional Study.

The medical utility of psychedelics has been the subject of significant scientific interest in recent years. While most of these substances remain Schedule I under the Controlled Substances Act, advancements in research have led states to consider expanding legal access, impacting primary care, where patients often seek mental health support and treatment. In July 2023 we conducted a cross-sectional survey of 770 Vermont primary care providers (PCPs) about their familiarity and concerns with psychedelics, as well as opinions on access and therapeutic value (response rate 17%). The Vermont Oxford Neonatal Encephalopathy Registry: rationale, methods, and initial results.

BACKGROUND: In 2006, the Vermont Oxford Network (VON) established the Neonatal Encephalopathy Registry (NER) to characterize infants born with neonatal encephalopathy, describe evaluations and medical treatments, monitor hypothermic therapy (HT) dissemination, define clinical research questions, and identify opportunities for improved care. METHODS: Eligible infants were ≥ 36 weeks with seizures, altered consciousness (stupor, coma) during the first 72 hours of life, a 5 minute Apgar score of ≤ 3, or receiving HT. Infants with central nervous system birth defects were excluded. RESULTS: From
