Is microvascular compression of the vestibulocochlear nerve a cause of unilateral hearing loss?

OBJECTIVES: We sought to confirm earlier findings in the literature that microvascular compression of the vestibulocochlear nerve might cause unilateral sensorineural hearing loss. We measured the length and width of the internal auditory canal (IAC) to investigate a possible association between a narrow porus, the presence of an anterior inferior cerebellar artery (AICA) loop, and the development of a microvascular compression syndrome. METHODS: We performed a prospective blinded analysis of 167 magnetic resonance imaging scans of the cerebellopontine angle. The presence of an AICA loop was

[Vascular compression syndrome of the vestibulocochlear nerve--otolaryngologic and radiologic diagnosis].

Vascular compression syndrome is the term used to classify a group of conditions though to be caused by the compression of cranial nerve by vessel. In the most cases the contact of vascular loop formed by the anterior inferior cerebellar artery (AICA) with the eight and facial nerve correlated with unilateral auditory symptoms or hemifacial spasms. The vascular compression syndrome of vestibulocochlear nerve in 8 patients treated in I ENT Clinic of Silesian Academy and MRI Department in Katowice was observed. All patients were otologic findings such as a tone audiometry, ENG, ABR and radiologi

Assessment of vestibulocochlear organ function in patients meeting radiologic criteria of vascular compression syndrome of vestibulocochlear nerve--diagnosis of disabling positional vertigo.

BACKGROUND: This study sought to assess the vestibulo-cochlear organ in patients meeting radiologic criteria of vascular compression syndrome (VCS) of the eighth cranial nerve. MATERIAL/METHODS: The authors performed a retrospective analysis of 34 patients (18 women, 16 men; mean age, 49 years) treated in between 2000 and 2007, with VCS of the eighth cranial nerve by MRI. Contrasted magnetic resonance imaging identified an anterior inferior cerebellar artery vascular loop adhering to the vestibule-cochlear nerve in all 34 cases. All patients were given pure tone audiometry, distortion product

Automated facial-vestibulocochlear nerve complex identification based on data-driven tractography clustering.

Small size and intricate anatomical environment are the main difficulties facing tractography of the facial-vestibulocochlear nerve complex (FVN), and lead to challenges in fiber orientation distribution (FOD) modeling, fiber tracking, region-of-interest selection, and fiber filtering. Experts need rich experience in anatomy and tractography, as well as substantial labor costs, to identify the FVN. Thus, we present a pipeline to identify the FVN automatically, in what we believe is the first study of the automated identification of the FVN. First, we created an FVN template. Forty high-resolut

[Macroscopic and microscopic changes of the vestibulocochlear nerve after Gamma Knife treatment].

We report on a case in which macroscopic and microscopic changes of the vestibulocochlear nerve could be observed after radiosurgery of an intrameatal vestibular schwannoma. This case shows for the first time a morphological correlate for undesirable effects after radiosurgical treatment of a vestibular schwannoma and indicates that despite a certain distance to the actual tumor, degenerative changes in neural structures can be expected.

Magnetic resonance imaging scans for vestibulocochlear nerve tumours: what is actually found?

BACKGROUND: Approximately 20 per cent of adult patients presenting to otolaryngology clinics have symptoms pertaining to the inner ear. These are investigated with magnetic resonance imaging (MRI) internal auditory meatus scans. This study analysed all findings from 736 sequential MRI internal auditory meatus scans performed on 731 patients over a one year period. METHODS: Six hundred and seventy-two patients were included for analysis. Of these, 419 (62.35 per cent) had normal MRI scans, 221 (32.90 per cent) had MRI findings thought to be unrelated to their presenting symptoms, 32 (4.76 per

Neurosyphilis as a cause of facial and vestibulocochlear nerve dysfunction: MR imaging features.

The prevalence of syphilis increased for several decades before the mid-1990s in the United States, particularly in the southern states. We report a case of neurosyphilis causing bilateral facial and vestibulocochlear nerve dysfunction in which the diagnosis was not initially suspected based on the patient's demographics and history. The MR imaging features helped to make the diagnosis in this case and to exclude other possible causes of multiple cranial nerve dysfunction in this patient. Hearing loss associated with neurosyphilis is one of the few treatable forms of progressive hearing loss,

[An autopsy case of meningeal carcinomatosis with vestibulocochlear nerve disturbance as the first manifestation].

A 54-year-old man initially complained of frontal headache, right ear pain and tinnitus in May, 1985. This was followed by right facial palsy and hearing loss, and he was admitted to our hospital. Physical findings revealed right trigeminal nerve disturbance, left facial nerve palsy and bulbar palsy. The spinal fluid showed pleocytosis, increased protein, decreased glucose, markedly increased carcinoembryonic antigen and adenocarcinoma cells. Gastric carcinoma was revealed by an upper GI series. He was treated with chemotherapy. However, he die in August, 1985. Nodular metastases were discover

Diffusion MRI of the facial-vestibulocochlear nerve complex: a prospective clinical validation study.

OBJECTIVES: Surgical planning of vestibular schwannoma surgery would benefit greatly from a robust method of delineating the facial-vestibulocochlear nerve complex with respect to the tumour. This study aimed to optimise a multi-shell readout-segmented diffusion-weighted imaging (rs-DWI) protocol and develop a novel post-processing pipeline to delineate the facial-vestibulocochlear complex within the skull base region, evaluating its accuracy intraoperatively using neuronavigation and tracked electrophysiological recordings. METHODS: In a prospective study of five healthy volunteers and five

Microsurgical relationships of the anterior inferior cerebellar artery and the facial-vestibulocochlear nerve complex.

The relationship of the anterior inferior cerebellar artery (AICA) to the facial (7th) and vestibulocochlear (8th) nerves was studied using 3x to 20x magnification in 50 cerebellopontine angles (CPAs) from 25 adult cadavers. The AICA originated from the basilar artery as a single (72% of the CPAs), duplicate (26%), or triplicate (2%) artery. Each of the 50 CPAs had one or more arterial trunks that coursed in close proximity to the 7th and 8th cranial nerves and thus were said to be nerve-related. The nerve-related arterial trunks were divided into three segments based on their relationship to

Criteria for preservation of vestibulocochlear nerve function during microsurgical removal of acoustic neurinomas.

A careful examination of the shape, location and course of the 8th cranial nerve in medium-sized and large tumours exhibits three distinct variants. In all cases where cochlear function was preserved, the type III variant (12%) in the cranial nerve tumour relationship was observed. The author's experience clearly indicates that, if a tumour is resected in toto, anatomical nerve continuity cannot be preserved in the type I and II variants (48%, respect. 40%). However, in the type I and II variants the patients invariably had preoperative hearing loss. The important criteria which must be consid

Papilledema With Facial and Vestibulocochlear Nerve Involvement in Idiopathic Intracranial Hypertension: An Unusual Case.

INTRODUCTION: Idiopathic intracranial hypertension (IIH) is defined by elevated intracranial pressure in the absence of an intracranial mass or cerebrospinal fluid abnormality and is commonly associated with papilledema. Although cranial nerve involvement may occur, simultaneous facial and vestibulocochlear nerve dysfunction is rare and may lead to diagnostic confusion. CASE PRESENTATION: A 42-year-old obese woman with metabolic comorbidities developed acute right-sided peripheral facial palsy preceded by facial pain and accompanied by severe headache, pulsatile tinnitus, and blurred vision.

Solitary Metastasis to the Facial/Vestibulocochlear Nerve Complex: Case Report and Review of the Literature.

BACKGROUND: Distant metastasis of mucinous adenocarcinoma from the gastrointestinal tract, ovaries, pancreas, lungs, breast, or urogenital system is a well-described entity. Mucinous adenocarcinomas from different primary sites are histologically identical with gland cells producing a copious amount of mucin. This report describes a very rare solitary metastasis of a mucinous adenocarcinoma of unknown origin to the facial/vestibulocochlear nerve complex in the cerebellopontine angle. CASE DESCRIPTION: A 71-year-old woman presented with several month history of progressive neurological decline

Reduced size of the cochlear branch of the vestibulocochlear nerve in a child with sensorineural hearing loss.

A 12-year-old female patient presented with unilateral sensorineural hearing loss. Distortion-product otoacoustic emission testing failed to reveal any measurable emissions in the affected side. MR imaging did not reveal labyrinthine malformation. Three-dimensional Fourier transformation-constructive interference in steady-state MR images showed a thin cochlear branch. We speculated that mumps infection or developmental malformation caused the unilateral sensorineural hearing loss.

Microstructural changes of the vestibulocochlear nerve in patients with Ménière's disease using diffusion tensor imaging.

OBJECTIVE: To evaluate the microstructural changes of the vestibulocochlear nerve in patients with Ménière's disease. METHODS: A total of 26 subjects, 13 patients with MD and 13 healthy controls, underwent diffusion tensor imaging (DTI) on a 3T scanner. The independent sample t-test was used to compare the differences in fractional anisotropy (FA) and apparent diffusion coefficient (ADC) between the two groups. A Pearson correlation was used between DTI and the dizziness handicap inventory (DHI) scores. RESULTS: There was a significant decrease in FA and an increase in ADC of the vestibuloco

Acute vertigo and sensorineural hearing loss from infarction of the vestibulocochlear nerve: A case report.

RATIONALE: Acute unilateral audiovestibulopathy is a common neurotological syndrome. Differential diagnoses of acute unilateral audiovestibulopathy include viral infection, vascular insults, and tumors. Regarding vascular causes, ischemic stroke in the anterior inferior cerebellar artery (AICA) territory is known to be the leading cause of acute audiovestibular loss. Previous reports of AICA infarction with audiovestibulopathy failed to demonstrate magnetic resonance imaging (MRI)-positive vestibulocochlear infarctions. Only 1 report demonstrated acute infarction involving the vestibulocochlea

Is there a functioning vestibulocochlear nerve? Cochlear implantation in a child with symmetrical auditory findings but asymmetric imaging.

The finding of an abnormally narrow internal auditory meatus during the assessment of a child for cochlear implantation raises the possibility that the meatus may not contain the normal number of nerves. Even with currently available MRI techniques it may be extremely difficult to decide whether or not to offer cochlear implant in such cases. We present a child of 4 1/2 years, assessed for cochlear implantation. MRI and CT imaging suggested aplasia of one vestibulocochlear nerve and hypoplasia of the other. However, audiological tests showed clear responses to sound in both ears and functional
