Traumatic amputations. A vietnam experience.

Ninety-two acute combat amputations were performed in 70 casualities of the Vietnam conflict. In the 67 surviving patients with 83 stumps, primary closure was done in 24 limbs (30%) and delayed closure in 11 more. Closed cases were observed 2 weeks and no failures noted. In selected patients early suture of war amputations can be practiced with a satisfactory result.

EIA effectiveness in Vietnam: key stakeholder perceptions.

In line with its rapidly transforming economy, thousands of development proposals undergo Environmental Impact Assessment (EIA) in Vietnam each year. Since its inception in 1993, Vietnam's EIA system has undergone numerous amendments via a suite of legislative reforms to the Law on Environmental Protection (LEP) and its associated Circulars and Decrees, a testament to the Government's will to improve environmental performance. Here we evaluate the effectiveness of Vietnam's EIA system through a unique empirical study focusing on those engaged in the EIA system undertaken in Hanoi in 2016 compr

Bilateral transfemoral/transtibial amputations due to battle injuries: a comparison of Vietnam veterans with Iraq and Afghanistan servicemembers.

BACKGROUND: Multiple limb loss from combat injuries has increased as a proportion of all combat-wounded amputees. Bilateral lower-extremity limb loss is the most common, with bilateral transfemoral amputations being the most common subgroup followed by bilateral amputations consisting of a single transfemoral amputation and a single transtibial amputation (TFTT). With improvements in rehabilitation and prostheses, we believe it is important to ascertain how TFTT amputees from the present conflicts compare to those from the Vietnam War. QUESTIONS/PURPOSES: We compared self-reported (1) health

Death and injury rates of U.S. military personnel in Iraq.

In the first 6.5 years of Operation Iraqi Freedom (OIF), U.S. military casualties exceeded 3,400 hostile deaths, 800 nonhostile deaths (due to disease, nonbattle injury, and other causes), and over 31,000 troops wounded in action. Casualty rates in Iraq have been considerably lower that during the Vietnam conflict, and a greater proportion of troops wounded in Iraq survive their wounds. Before the surge in troop levels that began in early 2007, the survival rate was 90.4% in Iraq as compared to 86.5% in Vietnam. Wounded-in-action rates increased during the first few months of the surge, but de

Design and measurement in a study of war exposure, health, and aging: protocol for the Vietnam health and aging study.

BACKGROUND: Survivors of war throughout the world experience illnesses and injuries that are crucial to understand, given the ongoing treatment and adaptation they demand. In developing countries like Vietnam, where population aging and chronic disease burdens are rapidly rising, aging populations have seen a disproportionate share of armed conflict and related casualties. This paper describes the Vietnam Health and Aging Study (VHAS), a unique resource for investigating mechanisms of association between diverse exposures to armed conflict during the Vietnam War and multiple dimensions of olde

[The medical treatment of Kim Phúc at the BG Unfallklinik Ludwigshafen].

The Vietnam War was a military conflict in Vietnam during the Cold War that followed the First Indochina War. This war was fought between North Vietnam, supported by its communist allies, and the government of South Vietnam, supported by the USA and other anti-communist countries. Kim Phúc is the child depicted in the Pulitzer Prize winning photograph taken on June 8, 1972 by AP photographer Nick Út. The iconic photo shows her at about nine years of age running naked on a road amid the chaos after being severely burned by a napalm attack. After 14 months of hospital stay and 17 surgical proced

Can we solve the mysteries of the National Vietnam Veterans Readjustment Study?

The National Vietnam Veterans Readjustment Study (NVVRS) researchers reported that 30.9% of all men who served in that conflict developed posttraumatic stress disorder (PTSD) even though only about 15% had been assigned to combat units. Scholars, mainly historians, have questioned the accuracy of the PTSD prevalence rate. The purpose of this article is to evaluate the merits of several hypotheses adduced to explain the high apparent PTSD prevalence in the NVVRS. Empirical and conceptual analysis suggests that malingering is unlikely to account for many cases. Also, deployment to Vietnam in the

Lymphatic filariasis in soldiers exposed in INDOPACOM.

Some military organizations in the U.S. Indo-Pacific Command (INDOPACOM) give returning soldiers presumptive treatment for filariasis. As there have been few clinical cases in recent decades, the historical basis for this chemotherapy was reviewed. During the Second World War, U.S. Marines stationed on Polynesian islands such as Tonga, Samoa, and Fiji experienced clinical lymphatic filariasis. Although thousands of both U.S. and Australian soldiers served in New Guinea, few, if any, cases of lymphatic filariasis were ascribed to Melanesia. While the French Army reported dozens of cases of fila

The legacy of war: an epidemiological study of cluster weapon and land mine accidents in Quang Tri Province, Vietnam.

The study examines the epidemiology of cluster weapon and land mine accidents in Quang Tri Province since the end of the Vietnam War. The province is located just south of the demarcation line and was the province most affected during the war. In 2009, a cross sectional household study was conducted in all nine districts of the province. During the study period of 1975-2009, 7,030 persons in the study area were exposed to unexploded ordnances (UXO) or land mine accidents, or 1.1% of the provincial population. There were 2,620 fatalities and 4,410 accident survivors. The study documents that th

War Trauma and Strength: A Qualitative Study of Participants in the Vietnamese Insights into Cognitive Aging Program.

OBJECTIVES: Vietnamese Americans are the fourth largest Asian subgroup in the United States, but little qualitative work has been done on their war and migration experiences. In this study, we delineated the war-related traumas and strength in participants of the Vietnamese Insights into Cognitive Aging Program (VIP). METHODS: VIP is a longitudinal study of 548 Vietnamese Americans aged 65+ years living in Northern California. During Visit 1, a semi-structured interview was conducted on participants' life and experiences in Vietnam and immigration to the United States. Using data at one of th

Clinical Nursing in Times of War and a Pandemic.

Nurses who served in the Vietnam War and those who cared for patients during the 2020 COVID-19 pandemic surge may be separated by decades, but they share the experience of caring for large numbers of critically injured, ill, or dying patients while risking their own lives. This article reviews the findings of two studies, one conducted in 1986 based on interviews with Vietnam War nurses, and one conducted in 2020 based on interviews with nurses during COVID-19, and explores the idea that professional nurses from different generations have similar thoughts when their lives are in danger and pat

Melioidosis and Activation from Latency: The "Time Bomb" Has Not Occurred.

Burkholderia pseudomallei, the causative agent of melioidosis has long been considered able to exist in a latent form. Seropositivity among U.S. soldiers returning from the Vietnam conflict led to melioidosis being dubbed "the Vietnamese time bomb." Cases assigned to "(re)activation from latency" over 30 years of the Darwin Prospective Melioidosis Study (DPMS) were reviewed and reassessed and additional cases from DPMS years 31-34 were added. Historical reports of melioidosis attributed to activation from latency were reviewed. Some earlier DPMS cases and most historical cases described as act

Moral transgression during the Vietnam War: a path analysis of the psychological impact of veterans' involvement in wartime atrocities.

BACKGROUND AND OBJECTIVES: Involvement in wartime combat often conveys a number of deleterious outcomes, including posttraumatic stress disorder (PTSD), depression, hostility, aggression, and suicidal ideation. Less studied is the effect of engagement in wartime atrocities, including witnessing and perpetrating abusive violence. DESIGN AND METHODS: This study employed path analysis to examine the direct effects of involvement in wartime atrocities on hostility, aggression, depression, and suicidal ideation independent of combat exposure, as well as the indirect effects via guilt and PTSD symp

Palmoplantar keratoses and Bowen's disease in a Vietnam veteran: Could Agent Blue be implicated?

Agent Blue was an arsenical herbicide used extensively in the Vietnam War. Arsenic is one of the known causes of acquired palmoplantar keratoderma (PPK). The most common manifestation of arsenic exposure in susceptible individuals is bilateral palmoplantar hyperkeratosis. We report a 67-year-old man with no known prior exposure to arsenic in the USA or family history of PPK who developed multiple squamous cell carcinoma in situ (SCCIS) and palmoplantar hyperkeratotic lesions beginning 23 years after service in Vietnam. The SCCIS were located on the trunk and extremities in both sun-exposed and

Do appraisals of military service indicate current distress in aging Vietnam War combat veterans?

OBJECTIVE: Appraisals of military service, both desirable and undesirable, assessed via Elder and Clipp's (1989) scale, are associated with psychological distress in veterans. Aging combat veterans (CV) are at increased risk for posttraumatic stress disorder and other psychological disorders yet may underreport symptoms and not seek treatment that could be beneficial. It is unknown whether desirable and undesirable appraisals of military service are associated with mental health outcomes above and beyond typical risk and protective factors, such as age, education, and combat exposure. Therefor

Cancer patterns in Hmong in Minnesota, 2000 to 2012.

BACKGROUND: Minnesota has the second largest Hmong population in the United States. The objective of the current study was to estimate the cancer incidence among Hmong individuals in Minnesota between 2000 and 2012 to determine targets for screening and interventions. METHODS: Cancer cases in Minnesota between 2000 and 2012 were obtained from the Minnesota Cancer Surveillance System, and proportional incidence ratios (PIRs) were calculated. The 2000 and 2010 US Census reports were used to obtain total population estimates. Age-adjusted cancer incidence rates (AAR) and 95% confidence intervals

Impact of a methadone maintenance therapy pilot in Vietnam and its role in a scaled-up response.

BACKGROUND: As a dual response to the HIV epidemic and the high level of injecting drug use in Vietnam, the Ministry of Health (MOH) initiated a pilot methadone maintenance therapy (MMT) program in Hai Phong and Ho Chi Minh City (HCMC) in early 2009. The objectives of the pilot were to provide evidence on whether MMT could be successfully implemented in Vietnam and scaled up to other localities. METHODS: A prospective study was conducted among 965 opiate drug users admitted to the pilot. Data on demographic characteristics, sexual behaviors, substance use behaviors (including heroin use), and

Diffuse Large B-cell Lymphoma Associated With Exposure to Agent Orange.

Agent Orange (AO) is a toxic herbicide carcinogen mixture of 2,3,7,8-tetrachlorodibenzo-p-dioxin (TCDD) that was used extensively during the Vietnam conflict of the 1960s and 1970s. We present a case of an elderly veteran who presented with a progressively enlarging right neck mass. During active combat in Vietnam, he experienced extensive exposure to AO. He was eventually diagnosed with stage I diffuse large B-cell lymphoma (DLBCL) with locally advanced/bulky disease. The patient was deemed higher risk for systemic disease given the skin breakthrough at presentation. Additional labs revealed

Drugs, Vietnam, and the Vietnam veteran: an overview.

Highlights are presented on the issue of drug use among servicemen in Vietnam and its aftereffects. Two stages of Vietnam drug use are identified-a period of increasing marijuana use followed by the 1970 influx of highly potent heroin to which 1/5 of the enlisted troops were addicted at some time during their tour. The major contributing factors appear to be: (1) the need of troops in stressful combat situations for self-medication, escape, and hedonistic indulgence; (2) the relaxation of taboos against drug use in the United States; and (3) the availability of illicit drugs at low cost, which

Recent trends in the treatment of posttraumatic stress disorder and other mental disorders in the VHA.

OBJECTIVE: This study proposed to evaluate Veterans Health Administration (VHA) specialty mental health care workload for treating posttraumatic stress disorder (PTSD) and other mental disorders between 2005 and 2010 in comparison with results from 1997 to 2005. The 2005-2010 time frame represents a period of increased utilization of services by recently returning veterans and of program expansion within VHA. METHODS: VHA administrative databases were queried for all veterans receiving specialty mental health treatment annually between 2005 and 2010. Veterans were categorized by military serv
