It has been asserted that speed alone is an effective indicator of the degree of gait abnormality. To determine the validity of this assertion, relationships between velocity and 18 other temporal gait parameters were determined in 25 patients with a first hemispheric stroke resulting in hemiplegia or hemiparesis of at least one month duration. Gait characteristics were recorded using footswitchs connected to a portable computerized monitoring device. Velocity was found to be significantly correlated with cadence, mean cycle duration, mean cycle length, hemiplegic limb stance phase duration, n

"Self-selected walking speed" was measured in 18 patients with hemiparesis after stroke and compared with that in a healthy control group. The average speed of the patient group for a 30 m walk was 0.78 m/s (SD 0.24) and of the control group 1.69 m/s (SD 0.05). The intraindividual variation for the patients was 0.004 m/s and the interindividual variation 0.37 m/s. For the controls, the corresponding values were 0.000 m/s and 0.003 m/s, respectively. "Self-selected walking speed" correlated significantly with "functional walking test" (r = 0.91) and, "number of steps" (r=0.97) as well as with s

OBJECTIVE: To determine the degree to which self-selected walking speed (SSWS), maximal walking speed (MWS), and walking speed reserve (WSR) are associated with fall status among community-dwelling older adults.
DESIGN: WS and 1-year falls history data were collected on 217 community-dwelling older adults (median age = 82, range 65-93 years) at a local outpatient PT clinic and local retirement communities and senior centers. WSR was calculated as a difference (WSRdiff = MWS - SSWS) and ratio (WSRratio = MWS/SSWS).
RESULTS: SSWS (P < 0.001), MWS (P < 0.001), and WSRdiff (P < 0.01) were associ

BACKGROUND AND PURPOSE: Stroke survivors often exhibit reduced walking efficiency, reflected by an increased energy cost of walking (Cw). Although Cw has been associated with walking performance, its relationship with impairment severity and independence in activities of daily living (ADL) remains unclear, particularly across real-life walking conditions. This study aimed to examine the association between impairment severity and Cw across real-life walking tasks. A secondary objective was to assess the association between Cw and independence in ADL.
METHODS: This secondary analysis used data

Human locomotion is characterized by remarkable versatility, enabling smooth speed modulation, acceleration, deceleration, and transitions between gaits. This versatility is thought to emerge from interactions between the musculoskeletal system, spinal circuits—including reflexes and central pattern generators—and supraspinal control. The relative contributions of these components remain a subject of ongoing debate. In this study, we explore the potential role of reflex modulation in achieving the observed flexibility of human locomotion. Using a neuromusculoskeletal model controlled by reflex

BACKGROUND: Walking speed is reduced with aging. However, it is not certain whether the reduced walking speed is associated with physical and coordination fitness. This study explores the physical and coordination determinants of the walking speed decline in older women.
METHODS: One-hundred-eighty-seven active older women (72.2 ± 6.8 years) were asked to perform a 10-m walk test (self-selected and maximal walking speed) and a battery of the Senior fitness test: lower body strength, lower body flexibility, agility/dynamic balance, and aerobic endurance. Two parameters characterized the walkin

Dynamic joint stiffness (or simply "stiffness") is a customization criteria used to tune mechanical properties of orthotic and prosthetic devices. This study examines metatarsophalangeal (MTP) joint stiffness during the toe-rocker phase of barefoot walking and establishes baseline characteristics of MTP joint stiffness. Ten healthy individuals walked at 4 speeds (0.4, 0.6, 0.8, and 1.0 statures·s-1) over level ground. MTP sagittal plane joint angles and moments were calculated during the toe-rocker phase of stance. Least-squares linear regressions were conducted on the MTP moment versus angle

Walking ability is significantly lower in hemodialysis patients compared to healthy people. Decreased walking ability characterized by slow walking speed is associated with adverse clinical events, but determinants of decreased walking speed in hemodialysis patients are unknown. The purpose of this study was to identify factors associated with slow walking speed in ambulatory hemodialysis patients. Subjects were 122 outpatients (64 men, 58 women; mean age, 68 years) undergoing hemodialysis. Clinical characteristics including comorbidities, motor function (strength, flexibility, and balance), a

To more understand the influence of the walking speed on the spinal joint force distribution, a three-dimensional biomechanical model was used to estimate the spine loads during human gait with three different walking speeds. This previously developed and validated model included a dynamic external model and an internal model with forces of disc, 8 major muscles, 2 ligaments and 2 facet joints at L5/S1 level. A linear optimization method was used to solve the internal model to estimate the L5/S1 spinal joint force distribution. The results of five young male subjects showed that the mean peak

BACKGROUND: Usual walking speed may predict cardiovascular disease in type 2 diabetes, but it is not routinely assessed in UK primary care.
OBJECTIVE: To examine the feasibility of three methods of assessing walking speed in patients with diabetes in general practice: timed walking, accelerometry, and treadmill walking.
METHODS: We approached consecutive mobile patients attending a general practice diabetes clinic. Consenting participants performed a timed 28-m walk (4 × 7 m) at their usual pace in the surgery corridor while wearing an accelerometer, followed by a 3-minute treadmill walk. Me

Pedestrian crossings in the UK and US require people to walk at 1.2 m/s to cross the road in time; however a large proportion of older people do not walk this fast, potentially discouraging walking or putting older people at risk of injury. We use longitudinal data to investigate changes in walking speed, and ability to cross the road in time, at older ages. 31,015 walking speed measurements were taken from 10,249 men and women aged 60+ years in waves 1-7 of the English Longitudinal Study of Ageing (2002-2014). Growth curve analyses were used to model how walking speed changes with increasing

QUESTIONS: In people with Parkinson's disease, what is the effect of adding external cueing (ie, visual, auditory or somatosensorial cueing) to walking training compared with walking training alone in terms of walking, mobility, balance, fear of falling and freezing? Are any benefits carried over to participation or maintained beyond the intervention period?
DESIGN: Systematic review of randomised trials with meta-analysis.
PARTICIPANTS: Ambulatory adults with Parkinson's disease.
INTERVENTION: Walking training with external cueing compared with walking training without external cueing.

This study investigated the effects of walking speed and direction on gait coordination dynamics in healthy young adults, focusing on intralimb and interlimb coordination during forward (FW) and backward (BW) walking. Twenty participants walked on a treadmill under six speed conditions in both FW and BW. Kinematic data were collected to compute spatiotemporal gait parameters, continuous relative phase (CRP), and phase coordination index (PCI) as measures of segmental coordination and bilateral timing. Repeated-measures ANOVAs were used to assess the effects of speed independently for each walk

BACKGROUND: Walking speed is a reliable marker of sarcopenia and a strong predictor of mortality, but its relationship with cancer incidence remains largely unexplored. We aimed to investigate the association between walking speed and the risk of any cancer and five common cancers, including lung, breast, colorectum, prostate, and stomach, and to explore potential mediation by biomarkers of inflammation, and lipid and glucose metabolism.
METHODS: The primary analysis was conducted in 431 598 participants from the UK Biobank (mean age 56.3 [SD 8.1] years at baseline), and the generalizability

Exposure to noise in everyday urban life is considered to be an environmental stressor. A specific outcome of reactions to environmental stress is a fast pace of life that also includes a faster pedestrian walking speed. The present study examined the effect of listening to annoying acoustical stimuli (traffic noise) compared with relaxation sounds (forest birdsong) on walking speed in a real outdoor urban environment. The participants (N = 83) walked along an urban route of 1.8 km. They listened to either traffic noise or forest birdsong, or they walked without listening to any acoustical sti

The metabolic energy that human walking requires can vary by more than 10-fold, depending on the speed, surface gradient, and load carried. Although the mechanical factors determining economy are generally considered to be numerous and complex, we tested a minimum mechanics hypothesis that only three variables are needed for broad, accurate prediction: speed, surface grade, and total gravitational load. We first measured steady-state rates of oxygen uptake in 20 healthy adult subjects during unloaded treadmill trials from 0.4 to 1.6 m/s on six gradients: -6, -3, 0, 3, 6, and 9°. Next, we teste

Lesions to the diabetic foot have various causes. However, there is broad consensus that excessive plantar pressure plays a major role in the chain of events leading to ulcerations and gangrenes. During walking, on the other hand, peak values of plantar pressure are likely to increase with velocity even in therapeutic shoes. Therefore, the question arises whether a moderate velocity should be recommended to diabetic patients to reduce the risk of foot lesions. In this study, two velocities were compared for different types of therapeutic footwear. The velocities selected were considered modera
