Optimal allocation and operation of sewer monitoring sites for wastewater-based disease surveillance: A methodological proposal.

Wastewater-based epidemiology (WBE) is drawing increasing attention as a promising tool for an early warning of emerging infectious diseases such as COVID-19. This study demonstrated the utility of a spatial bisection method (SBM) and a global optimization algorithm (i.e., genetic algorithm, GA), to support better designing and operating a WBE program for disease surveillance and source identification. The performances of SBM and GA were compared in determining the optimal locations of sewer monitoring manholes to minimize the difference among the effective spatial monitoring scales of the sel

Evaluating the Pattern of Kava (Piper methysticum) Consumption in Australia by Wastewater-Based Epidemiology.

INTRODUCTION: Kava (Piper methysticum) is a plant native to the Pacific Islands, used in the preparation of a beverage consumed during ceremonies. The plant has gained popularity in other countries as a supplement for the treatment of anxiety and insomnia. Nevertheless, products containing P. methysticum were associated with liver damage, which led to the kava ban in several jurisdictions across the world, including Australia. However, in 2019, the Australian government started relaxing import restrictions as part of the kava pilot. It is therefore of interest to establish a baseline of kava c

Revealing the hidden burden: wastewater-based epidemiology for underreported and emerging infectious diseases in communities.

Wastewater-based epidemiology (WBE) has become a transformative tool for infectious disease surveillance, providing population-level insights that complement and extend traditional case-based reporting. This review examines the expanding role of WBE in identifying and characterizing underreported, novel, and emerging human pathogens. Evidence reveals that wastewater analysis consistently detects enteric, respiratory, and neglected pathogens that are often missed by clinical systems, thereby revealing the hidden burden of infection within communities. Sequencing-based studies have identified nu

Wastewater-based epidemiology of influenza viruses: a systematic review.

INTRODUCTION: Wastewater-based epidemiology (WBE) has emerged as a valuable public health tool for monitoring the circulation of many pathogens, including influenza viruses (IVs). The general aim of this study is to systematically retrieve and summarize evidence on the use of WBE for supporting influenza surveillance. Specific objectives are: (i) to map influenza monitoring activities using WBE; (ii) to assess the performance of viral recovery methods; (iii) to explore association with clinical data; (iv) to evaluate the feasibility of typing/subtyping IVs directly from wastewater. METHODS: W

Monitoring Multiple Sexually Transmitted Pathogens Through Wastewater Surveillance.

Wastewater-based epidemiology (WBE) offers a promising tool for sexually transmitted infection (STI) surveillance, especially in settings where underdiagnosis or social stigma complicates conventional reporting. To assess its utility, we conducted a year-long study examining six STIs, Chlamydia trachomatis, Treponema pallidum, Neisseria gonorrhoeae, human immunodeficiency virus (HIV), hepatitis C virus (HCV), and herpes simplex virus (HSV), in weekly composite samples from the primary influent of a small-sized Midwestern wastewater treatment plant. Pathogen detection and quantification were pe

Evaluating acute gastroenteritis-causing pathogen reduction in wastewater and the applicability of river water for wastewater-based epidemiology in the Kathmandu Valley, Nepal.

Rapid urbanization and population growth without the implementation of proper waste management are capable of contaminating water sources, which can lead to acute gastroenteritis. This study examined the detection and reduction of five gastroenteritis-causing enteropathogens, Salmonella, Campylobacter coli, Campylobacter jejuni, Clostridium perfringens, and genogroup IV norovirus, and one respiratory pathogen, influenza A virus, in two municipal wastewater treatment plants (WWTP) using an oxidation ditch system (WWTP A; n = 20) and a stabilization pond system (WWTP B; n = 18) in the Kathmandu

Making waves: toward systems-level interpretation of hormonal and endogenous biomarkers in wastewater-based epidemiology.

Wastewater-based epidemiology (WBE) has proven invaluable for population health monitoring, most notably during the COVID-19 pandemic. Yet current WBE largely relies on exogenous markers such as drugs, pathogens, and their metabolites, limiting surveillance to what communities are exposed to. We argue for expanding WBE towards endogenous biomarkers, particularly hormones, which provide insights into physiological stress, metabolic function, and endocrine activity. Hormone-based WBE offers new opportunities to capture population-level biological responses to societal and environmental stressors

Field evaluation of diffusive gradients in thin-film passive samplers for wastewater-based epidemiology.

There is a need for a simple water sampling technique to enable routine monitoring of community drug consumption through wastewater-based epidemiology (WBE). This study investigates the potential use of diffusive gradients in thin films to sample organic compounds (o-DGT) for WBE. Three types of resin gels (HLB, XAD 18, and XDA-1) within o-DGT samplers each were deployed in triplicate at the inlets of two sewage treatment plants of Southern Asian cities. The target compounds included 15 illicit drugs and 18 antibiotics. A comprehensive evaluation was undertaken regarding each resin's ability t

The sanitary sewer unit hydrograph model: A comprehensive tool for wastewater flow modeling and inflow-infiltration simulations.

Sanitary sewer systems are critical urban water infrastructure that protect both human and environmental health. Their design, operation, and monitoring require novel modeling techniques that capture dominant processes while allowing for computationally efficient simulations. Open water flow in sewers and rivers are intrinsically similar processes. With this in mind, we formulated a new parsimonious model inspired by the Width Function Instantaneous Unit Hydrograph (WFIUH) approach, widely used to predict rainfall-runoff relationships in watersheds, to a sanitary sewer system consisting of nea

Tracing the transmission of mpox through wastewater surveillance in Southeast Asia.

High population density and tourism in Southeast Asia increase the risk of mpox due to frequent interpersonal contacts. Our wastewater surveillance in six Southeast Asian countries revealed positive signals for Monkeypox virus (MPXV) DNA, indicating local transmission. This alerts clinicians and helps allocate resources like testing, vaccines and therapeutics in resource-limited countries.

A mechanistic classification framework for RT-qPCR inhibitors in wastewater-based epidemiology.

Reverse transcription quantitative PCR (RT-qPCR) is widely used in wastewater-based epidemiology (WBE) but is frequently compromised by matrix-associated inhibition. While mitigation strategies abound, a systematic framework for diagnosing RT-qPCR inhibition in wastewater remains lacking. Here, a mechanistic classification framework for RT-qPCR inhibitors in wastewater is established based on two orthogonal dimensions: kinetic effect (linear vs. exponential inhibition) and molecular target (nucleic acid sequestrators vs. enzyme activity inhibitors). This framework enables diagnosis of the domi

Norovirus detection in wastewater and its correlation with human gastroenteritis: a systematic review and meta-analysis.

Norovirus (NoV) is a major cause of sporadic cases and outbreaks of acute gastroenteritis (AGE), thereby imposing threat to health globally. It is unclear how quantitation of wastewater NoV reflects the incidence of human AGE infections; therefore, we conducted this systematic review and meta-analysis of published NoV wastewater surveillance studies. A literature search was performed, and all studies on NoV wastewater surveillance were identified. Quantitative results were evaluated. The results showed that the overall detection rate of NoV in wastewater was 82.10% (95% confidence interval [CI

From health check-ups to "City sewage health scan": Multidimensional chemical analysis for wastewater-based epidemiology in Hong Kong.

Wastewater-based epidemiology (WBE) can provide a multidimensional "health check-up" for community-wide health. We conducted a "city sewage health scan" in Hong Kong by analyzing 401 time-proportional composite samples from 12 sewersheds over five weeks, classifying 117 biomarkers, referencing the local drug formulary and WHO ICD-11, into 45 features and seven health categories (infectious, endocrine, central nervous system, cardiovascular, respiratory, pain-related, and lifestyle). Our findings contextualized Hong Kong's longevity, showing that Hong Kong's biomarker levels are moderate global

Representation of urban and rural contexts in the application of wastewater surveillance for antimicrobial resistance: A systematic review.

Wastewater surveillance (WWS) can reveal community health trends through targeted monitoring of sewage. Given concerns regarding the global dissemination of drug-resistant infections, there is growing integration of antimicrobial resistance (AMR) in WWS infrastructure. However, most WWS has focused on highly urbanized sewersheds in resource-rich areas, which may exacerbate disparities in disease burdens and clinical datasets. This work presents both a systematic and scoping review of current published WWS efforts targeting antimicrobial resistance (AMR), with the aim to quantify and compare ap

Rapidly developed, optimized, and applied wastewater surveillance system for real-time monitoring of low-incidence, high-impact MPOX outbreak.

Recent MPOX viral resurgences have mobilized public health agencies around the world. Recognizing the significant risk of MPOX outbreaks, large-scale human testing, and immunization campaigns have been initiated by local, national, and global public health authorities. Recently, traditional clinical surveillance campaigns for MPOX have been complemented with wastewater surveillance (WWS), building on the effectiveness of existing wastewater programs that were built to monitor SARS-CoV-2 and recently expanded to include influenza and respiratory syncytial virus surveillance in wastewaters. High Detection Frequency of Enteric Pathogens: Insight from Wastewater-Based Epidemiology (WBE) Surveillance Approach in Dakar, Senegal.

Despite the importance of wastewater environmental monitoring in disease prevention and response strategies, its use remains poorly documented in Senegal. In addition, there is more onsite sanitation than sewer networks in Dakar, and open drains channel for rainwater are also used as clandestine wastewater discharge into the sea. This study aimed to assess the presence of specific pathogens in wastewater, faecal sludge, and bathing water (the sea). Samples were taken at treatment plants, an open drain, and in the receiving environment (the sea) from June to December 2023. Total nucleic acid wa

A critical review of wastewater-based epidemiology as a tool to evaluate the unintentional human exposure to potentially harmful chemicals.

Wastewater-based epidemiology (WBE) is a powerful tool to gather epidemiological insights at the community level, providing objective data on population exposure to harmful substances. A considerable portion of the human exposure to these potentially harmful chemicals occurs unintentionally, unlike substances such as pharmaceuticals, illicit drugs, or alcohol. In this context, this comprehensive review analyzes WBE studies focused on classes of organic chemicals to which humans are unintentionally exposed, namely organophosphorus flame retardants, per- and polyfluoroalkyl substances (PFAS), be
