Analysis of water and electrolyte imbalance in a patient with adipsic hypernatremia associated with subfornical organ-targeting antibody.

Patients with adipsic hypernatremia present with chronic hypernatremia because of defects in thirst sensation and dysregulated salt appetite, without demonstrable hypothalamic structural lesions. The involvement of autoantibodies directed against the sodium channel, Nax in the subfornical organ (SFO) has recently been reported. However, the pathophysiology of water and electrolyte imbalance underlying the disease has yet to be elucidated. We describe the case of a 5-year-old boy who complained of headaches and vomiting that gradually worsened. Brain magnetic resonance imaging detected no abnor

[Etiology of water and electrolyte metabolism imbalance following the rupture of cerebral aneurysms--with special reference to preoperative condition].

One hundred fourteen patients with ruptured cerebral aneurysms were reviewed in regard to the incidence and etiological factors of preoperative disturbances of water and electrolyte metabolism. Patients with inadequate salt intake, evidence of renal disease, cardiac failure or excessive diuretic therapy were excluded. Twenty-five (21.9%) patients developed water and electrolyte disturbances. Hyponatremia (less than 130 mEq/l) occurred in 18 (15.8%) of 114 patients. The majority of those patients with hyponatremia showed laboratory findings and/or clinical features suggesting the syndrome of in

Effect of sodium administration on fluid balance and sodium balance in health and the perioperative setting. Extended summary with additional insights from the MIHMoSA and TOPMAST studies.

PURPOSE: We aimed to provide an extended analysis of the physiological handling of of the sodium burden induced by maintenance fluids. MATERIALS AND METHODS: We revisited two studies that demonstrated, in healthy volunteers and in surgical patients, that maintenance fluids with 154 mmol/L of sodium lead to a more positive fluid balance than a regimen containing 54 mmol/L. We report different unpublished data on the renal handling of the imposed sodium burdens with specific attention to the resulting fluid and sodium balances. RESULTS: The kidneys adapt to the sodium-rich fluids not only by a

Trauma-related electrolyte disturbances: From resuscitation to rhabdomyolysis.

Traumatic injury results in drastic changes to a patient's normal physiology. The hormonal stress response, as well as some treatment strategies, lead to significant disruptions in electrolyte homeostasis that are important for clinicians to understand. In addition, advances in fluid resuscitation and modern transfusion practices have led to their own unique set of consequences, which we are just beginning to appreciate. Special attention is placed on rhabdomyolysis, as this distinct entity represents an extreme example of injury induced electrolyte derangements. This review describes the phys

[Acid-base and electrolyte correlations in blood and fluid in alcoholic delirium (author's transl)].

The gas and electrolyte homeostatic changes in arterialized capillary blood were studied in 51 patients with acute alcoholic psychosis. In 29 patients the blood and fluid data were correlated. It was established that acute alcohol delirium is mostly accompanied by alkalosis of metabolic, respiratory, or mixed origin. Irrespective of the approximate uniformity of blood and fluid changes, there is a distinctly expressed imbalance between the different parameters. After subsidence of psychotic symptoms (7-10 days after admittance) fluid acid-base fluctuations were to some extent still observable.

Fluid and electrolyte disturbances in patients with long-established ileostomies.

Salt and water depletion has been noted in 10 patients with long-established ileostomies. Marked renal conservation of sodium and water was present in a number of apparently healthy ileostomy patients. The sodium and potassium concentrations of ileostomy material showed little variation when these patients were in good health but there was a decrease in the sodium concentration and an increase in the potassium concentration during episodes of salt depletion. Greater attention needs to be paid to the fluid and electrolyte requirements of ileostomy patients after they have left hospital.

Fluid and Electrolyte Management in the Neonate: Potassium and Phosphorus.

Maintenance of electrolyte balance in the neonate is challenging in the context of illness or prematurity. Imbalances in potassium and phosphorus can occur in neonates, ranging from benign and clinically insignificant to those that can be life-threatening. An individualized approach to fluid and electrolyte management is based on the neonate's gestational age, day of life, maintenance needs, deficits, and ongoing losses. This article reviews normal and abnormal potassium and phosphorus values along with management strategies. Using a case scenario, the reader can apply concepts outlined in thi

Acid-base and electrolyte abnormalities observed in patients receiving cardiovascular drugs.

Cardiovascular drugs can cause a variety of acid-base and electrolyte abnormalities that need to be considered when clinicians manage the large number of patients who receive these agents. Diuretic-induced metabolic alkalosis is the most common acid-base disorder observed and is associated with hypokalemia. Drug-induced hyperkalemia is the most important cause of increased potassium levels in everyday clinical practice. Multifactorial-origin diuretic-induced hyponatremia is mostly due to thiazides and should be carefully managed. This review focuses on the pathogenetic mechanisms as well as on

Fluid status and its management in Japanese peritoneal dialysis patients.

This article reviews published studies related to fluid status of Japanese peritoneal dialysis (PD) patients and its impact on technique and patient survival. In addition, some specifics related to clinical background that potentially influence fluid status are described. According to a multicenter survey conducted in Japan, nearly 25% of Japanese PD patients are overhydrated. Available data indicate that a high salt diet may conceivably play an important role in the pathogenesis of fluid overload in Japanese PD patients, and it in turn negatively impacts patient prognosis. Because of the gene

Intravenous nutrition. A clinical evaluation of a 50 per cent dextrose in water solution, containing 1 mg. of hydrocortisone per 100 ml.

A 50 per cent dextrose in water solution, containing 1 mg. of hydrocortisone per 100 ml., was used successfully in 70 patients for intravenous nutritional maintenance and repletion. There were no adverse systemic effects during or following 216 infusions. The only undesirable local reaction was the rare occurrence of pain in the arm when the concentrated solutions were given too rapidly. Glycosuria was minimal if the infusion rate did not exceed 0.85 gm. of glucose per kilogram of body weight per hour, particularly if 50 units of insulin were added to each 550 ml. bottle of 50 per cent dextros

A case of prolonged hyperemesis resulting in hepatorenal failure, foetal distress and neonatal mortality.

Hyperemesis gravidarum is defined as a condition emerging during pregnancy, is prominent with severe nausea and vomiting, a water-electrolyte imbalance and liver-kidney function disorder and causes more than 5% loss of body weight. In cases of severe hyperemesis gravidarum, maternal morbidities such as water-electrolyte imbalance, hepatorenal failure, Wernicke's encephalopathy, splenic avulsion, oesophageal rupture, pneumothorax have been reported. We present a case of hyperemesis gravidarum, which continued until the third trimester and developed secondary hepatorenal failure, a water-electro…

Electrolyte Inertia In Chronic Kidney Disease.

Electrolyte imbalances are a common and challenging aspect of managing chronic kidney disease (CKD). Despite advances in treatment, a phenomenon known as "electrolyte inertia"- the inertness to correcting abnormal electrolyte levels- may persist in CKD patients. This inertia can result from various factors which include delayed therapeutic intervention, the complexity of multiple coexisting comorbidities, and the judicious balanced therapeutic approach necessitated by the multidyselectrolytaemia in CKD management. Through the concept of "electrolyte inertia," we explore the underlying causes f

Fluid Homeostasis and Diuretic Therapy in the Neonate.

Mutations in the gap junction β2 (GJB2) gene, which encodes connexin 26, are the leading cause of genetic deafness. These mutations are characterized by the degeneration and fragmentation of gap junctions and gap junction plaques (GJPs) composed of connexin 26. Dominant-negative mutations of GJB2, such as R75W, cause syndromic hearing loss and palmoplantar keratoderma. We previously reported that the R75W mutation, a single-base substitution where C is replaced by T, causes fragmentation of GJPs. Therefore, an adenine base editor (ABE), which enables A-to-G base conversions, can potentially be
