Prepregnancy weight change associated with high gestational weight gain.

OBJECTIVE: Gestational weight gain (GWG) above recommendations is a risk factor for adverse maternal, perinatal, and long-term outcomes. This study hypothesized that prepregnancy weight gain may portend excess GWG. METHODS: Among 1,126 women (51% of whom were of Black race) in the Coronary Artery Risk Development in Young Adults (CARDIA) study with post-baseline births, the prepregnancy annual rate of BMI change per woman was estimated (slope; 5 years before pregnancy) and was related to the risk of GWG above Institute of Medicine recommendations using mixed-effects models (binary) and GWG z

[GESTATIONAL WEIGHT GAIN AND LONG-TERM POSTPARTUM WEIGHT RETENTION].

Pregnant women have been told that gaining weight during pregnancy is important for maternal and perinatal health outcomes. However, today it is known that excessive weight gain during pregnancy is associated with adverse health outcomes. This article presents the connection between gestational weight gain and long-term postpartum weight retention as well as relevant epidemiology, predictors for postpartum weight retention and implications of its occurrence. In addition, in this review we raise possible prevention and intervention strategies for preventing weight retention following delivery.

Gestational Weight Gain: Achieving a Healthier Weight Between Pregnancies.

Rates of obesity continue to be a cause of morbidity and mortality, requiring intervention. Excessive gestational weight gain is related to postpartum weight retention and subsequent development of obesity, which translates into higher risk of adverse maternal and neonatal outcomes in future pregnancies and long-term excess cardiovascular disease and cancer for the mothers. Limiting gestational weight gain to within recommended limits prevents postpartum weight retention. This article provides an overview of methods and practices aimed at helping women achieve a healthy weight between pregnanc

Maternal Gestational and Postdelivery Weight Gain and Child Weight.

BACKGROUND: Maternal gestational weight gain (GWG) is a risk factor for the development of overweight in her child. It is unknown whether GWG programs the child's health or whether GWG indicates a shared familial lifestyle during childhood. To disentangle these influences, we studied the association of GWG and postdelivery maternal weight change simultaneously with child's weight development. METHODS: We used data from 3367 children participating in a birth cohort that started in 1996 in the Netherlands. Weight and height were self-reported. GWG was categorized as "inadequate," "adequate," an

Weight, weight gain, and survival after breast cancer diagnosis.

PURPOSE: To determine whether weight prior to diagnosis and weight gain after diagnosis are predictive of breast cancer survival. METHODS: Patients included 5,204 Nurses' Health Study participants diagnosed with incident, invasive, nonmetastatic breast cancer between 1976 and 2000; 860 total deaths, 533 breast cancer deaths, and 681 recurrences (defined as secondary lung, brain, bone, or liver cancer, and death from breast cancer) accrued to 2002. We computed the change in body mass index (BMI) from before to the first BMI reported > or = 12 months after the date of diagnosis. Cox proportiona

Lifestyle behaviors, metabolic disturbances, and weight gain in psychiatric inpatients treated with weight gain-associated medication.

Many psychiatric patients suffer from overweight/obesity and subsequent metabolic disturbances, where psychotropic medication is one of the main contributors. However, the magnitude of weight gain ranges individually, which leads to questioning the role of other contributors like lifestyle factors. The present study investigated several lifestyle factors among psychiatric inpatients, their relation to biological factors, and their predictive capability for weight gain during treatment. Using a naturalistic observational study design, psychiatric inpatients of all diagnoses were followed for 4

Optimal weight gain in triplet pregnancies.

OBJECTIVE: To identify appropriate weight gain in triplet gestations, which may aid in reducing the risk of perinatal morbidity within this high-risk cohort. METHODS: This retrospective cohort study evaluated all non-anomalous triplet pregnancies between 23 and 40 weeks' gestation resulting in live births at five tertiary-care medical centers between 1991 and 2011. Subjects were divided by pre-pregnancy BMI into underweight, normal-weight, overweight, and obese groups, and then stratified by low (<0.5 lb/week), moderate (0.5-1.49 lbs/week), or excess weight gain (≥1.5 lbs/week). Maternal obesity and gestational weight gain.

The aim of the study was to analyse gestational weight gain (GWG) according to body mass index (BMI) category and to explore the relationship between GWG and pregnancy complications. Women were recruited in the 1st trimester. Weight and height were measured and BMI calculated. Weight was measured at 38 weeks' gestation and GWG calculated. Clinical details were obtained prospectively. Of the 604 women recruited, 45.5% were primigravidas and 25.2% were obese. The overall mean GWG was 11.6 kg (SD 6.0). In obese women, the mean GWG was 10.4 kg (SD 7.5) compared with 12.6 kg (SD 5.7) in the normal

Prenatal weight gain: who is counseled?

BACKGROUND: Because prenatal counseling is associated with adherence to weight gain guidelines, we sought to identify patient-level characteristics associated with the receipt of counseling on weight gain, nutrition, and exercise during prenatal care. METHODS: We performed a secondary data analysis on a cohort of women enrolled in a prenatal counseling intervention study. We controlled for study group assignment (intervention versus usual care) as well as patient characteristics in a multivariable analysis. We performed three separate multivariable analyses for predictors of provider-patient

Dietary restraint and gestational weight gain.

OBJECTIVE: To determine whether a history of preconceptional dieting and restrained eating was related to higher weight gains in pregnancy. DESIGN: Dieting practices were assessed among a prospective cohort of pregnant women using the Revised Restraint Scale. Women were classified on three separate subscales as restrained eaters, dieters, and weight cyclers. SUBJECTS: Participants included 1,223 women in the Pregnancy, Infection, and Nutrition Study. MAIN OUTCOME MEASURES: Total gestational weight gain and adequacy of weight gain (ratio of observed/expected weight gain based on Institute of

[Antipsychotic-induced weight gain--pharmacogenetic studies].

Drug-naive patients with schizophrenia often present metabolic abnormalities and obesity. Weight gain may be the side effect of treatment with many antipsychotic drugs. Genetic effects, besides many other factors, are known to influence obesity in patients with schizophrenia treated with antipsychotics. Numerous studies of several genes' polymorphisms have been performed. -759C/T polymorphism of 5HT2C gene attracted most attention. In 5 independent studies of this polymorphism the association between T allele with the lower AP-induced weight gain was detected. No associations could be detected

Obesity, weight gain, and ovarian cancer.

OBJECTIVE: To investigate how adipose tissue alters endogenous hormone levels and may affect events at the ovarian tissue level. METHODS: We assessed current weight, weight at age 18, and adult weight change in relation to ovarian cancer risk among 109,445 participants in the Nurses' Health Study. Women reported ovarian cancer risk factors and new ovarian cancer diagnoses in biennial mailed questionnaires from 1976 to 1996. Height and weight were queried in 1976, current weight was updated biennially, and weight at age 18 was ascertained in 1980. During 20 years of follow-up and 1,703,474 per

Weight gain-induced blood pressure elevation.

This study was conducted to evaluate the mechanisms in weight gain-induced blood pressure (BP) elevation focusing, in particular, on the contributions of sympathetic nervous system activity, fasting plasma insulin, and leptin to BP levels. The study design was longitudinal with a cohort of 1897 men. BP, pulse rate, body mass index (BMI), fasting plasma norepinephrine (NE), insulin, and leptin were measured at 6 and 12 months in those 172 lean normotensive, 79 obese normotensive, 64 lean untreated hypertensive, and 38 obese untreated hypertensive men whose BMI increased >10% during the first 6

Excessive weight gain after liver transplantation.

Twenty-eight patients (19 females, 9 males) were evaluated pre- and posttransplant to determine the frequency and find predictors of excessive weight gain after orthotopic liver transplant. Posttransplant, 21 patients gained and 7 patients lost weight as compared with their pretransplant dry weight. The majority of weight gain occurred between 2 and 16 months; 64.3% of patients (18/28 pts.) became overweight. All patients overweight prior to transplant (11 pts.) were more overweight posttransplant (P less than 0.005). Overweight and nonoverweight patients were similar in age, female predominan

Maternal weight gain and preterm delivery.

This study examined the relationship between maternal weight gain and preterm delivery in 2163 women who participated in the Prenatal Nutrition Project at the University of California, San Diego between 1978-1986. Multivariate analysis of the data indicated that the risk of spontaneous preterm birth increased 60% in women with a low rate of weight gain (less than 0.27 kg/week) compared with those with an average rate (0.27-0.52 kg/week). Women with a low rate of gain were more than twice as likely to experience a preterm delivery as those with a high gain (higher than 0.52 kg/week); the odds r

Weight gain in HIV-infected patients.

Since its emergence, HIV has been linked to metabolic alterations with an impact on the distribution of fat and the weight of people living with HIV. While extreme weight loss and processes such as lipodystrophy were of concern at first, in recent years, and with the appearance of increasingly effective and better tolerated drugs, an abnormal weight gain is paradoxically taking place among people living with HIV. Although this weight gain is a multifactorial process in which lifestyle habits, physical exercise or diet have a great impact, antiretroviral treatment has been recently considered a

Weight gain with antidepressants and lithium.

Undesired weight gain is a common complaint of patients receiving pharmacological treatment for major affective disorders. It has been found to jeopardize patient compliance and may pose additional health hazards. A review of the literature on weight gain associated with tricyclic antidepressants, monoamine oxidase inhibitors, and lithium was carried out with the aim of deriving practical management strategies. Tricyclic antidepressants were found to stimulate appetite, carbohydrate craving, and a dose-dependent continuous weight gain of 0.57 to 1.37 kg per month of treatment. Proposed mechani
