The Occupational Safety and Health Administration at 50: Protecting Workers in a Changing Economy.

The passage of the Occupational Safety and Health Act of 1970 brought unprecedented changes in US workplaces, and the activities of the Occupational Safety and Health Administration (OSHA) have contributed to a significant reduction in work-related deaths, injuries, and illnesses. Despite this, millions of workers are injured annually, and thousands killed. To reduce the toll, OSHA needs greater resources, a new standard-setting process, increased civil and criminal penalties, full coverage for all workers, and stronger whistleblower protections. Workers should not be injured or made sick by th

A meta-ethnography of staff experiences of raising concerns about quality and safety in residential aged care.

BACKGROUND AND OBJECTIVES: Speaking up about quality and safety issues can protect older people and support learning in health and social care systems. In aged care, staff work in diverse roles to deliver care to residents with complex care needs. This meta-ethnography aimed to synthesise evidence on staff experiences of raising concerns about quality and safety in long-term residential aged care. RESEARCH DESIGN AND METHODS: We included qualitative studies examining staff experiences of speaking up, incident reporting, and whistleblowing. Five databases were searched. The seven phases of met

Seven sins in publishing (but who's counting…).

UNLABELLED: 'Oh what a tangled web we weave, When first we practise to deceive.’ Sir Walter Scott (Marmion, 1808) Think of scientific misconduct in the UK and Malcolm Pearce – one of the most high-profile cases – comes immediately to mind. Malcolm Pearce was an assistant editor of the British Journal of Obstetrics and Gynaecology, and a senior lecturer at St George's Medical School, when two fraudulent papers were published in the journal. A whistleblower at the hospital was the catalyst for an investigation that led to Pearce being fired, found guilty of serious professional misconduct by the

The Reverse Risk Register Approach to Understanding Systemic Factors in Risk Analysis and Management Case Studies.

Although recent developments have improved clarity on the characteristics of high-quality risk science, there is recognition that many of those characteristics relate to specific tasks taken by the risk analyst. However, the effectiveness of risk analysis and management often relies on both internal risk team activities and other external systemic factors. This article leverages reports on past risk events to develop new knowledge on systemic factors that have frequently appeared in connection with major risk events, including whistleblower dynamics, lack of accountability, financial aspects,

Predicting food safety problems among catering service operators using longitudinal regulatory data.

Full-coverage on-site inspection of catering service operators is difficult to sustain when regulated entities are numerous and risk is unevenly distributed. Using administrative regulatory data from Shanghai, China (2020-2024), we constructed a longitudinal dataset comprising 46,614 catering service operators and 150,467 inspection intervals. Machine-learning models were developed to predict whether at least one problem would be detected at the subsequent inspection, and model predictions were interpreted using SHAP. We further examined cross-period persistence and transition among categories

Emergence of Lyme disease as a social problem: analysis of discourse using the media content.

BACKGROUND: Chronic Lyme disease (LD) is a matter of debate worldwide and has emerged as a social problem. We aim to analyze the media content and describe the transformation process of a collective pain into a social problem in France. METHODS: Using social science methodology, a corpus of articles from 20 newspapers and videos from seven major TV stations from 1987 to 2017 were analyzed for discourse content. The speaking times and the frequency of interventions between doctors supporting the official guidelines and those against them were compared using the Mann-Whitney test and the Chi-sq

Sexual misconduct in prison: What factors affect whether incarcerated women will report abuses committed by prison staff?

More than 80,000 prisoners each year are sexually victimized during incarceration, but only about 8% report victimization to correctional authorities. Complicating reporting is the fact that half of the perpetrators are staff members. Given the restrictive and highly regulated prison environment, studies that examine reporting behaviors are difficult to conduct and to date information available relied on those who have reported or hypothetical victimization studies. This study uses an ecological framework and archival data from a class action lawsuit of sexual misconduct to determine predictor

Parents' champions vs. vested interests: who do parents believe about MMR? A qualitative study.

BACKGROUND: Despite the Government acting quickly to reassure parents about MMR safety following the publication of the 1998 paper by Wakefield and colleagues, MMR uptake declined. One of the reasons suggested for this decline is a loss of public trust in politicians and health professionals. The purpose of this analysis was to examine parents' views on the role the media, politicians and health professionals have played in providing credible evidence about MMR safety. METHODS: A qualitative focus group study conducted with parents living in Central Scotland. Eighteen focus groups were conduc

COVID-19 has united patients and providers against institutional betrayal in health care: A battle to be heard, believed, and protected.

In this commentary, researchers, health care consumers, and medical providers reflect on institutional betrayal during the COVID-19 pandemic in American and Canadian health care systems. Examples of institutional betrayal experienced by patients and their family members, as well as medical providers, are described. Although such examples may be more evident to the general public during the current pandemic, they do not represent new problems. (PsycInfo Database Record (c) 2020 APA, all rights reserved).

People matter: tomorrow's workforce for tomorrow's world.

The focus of any health service, now and into the future, should be people delivering safe, quality care to people; care that covers not just diagnosis and treatment, but the whole experience that patients and their carers have of the service. Workforce development, the process by which the current and future workforce is planned and trained, must be related to current and future patterns of service delivery and take account of financial reality. It cannot exist in isolation. Despite employing 1.3 million people, upon whom up to 70% of its budget is spent, the NHS has been curiously relaxed ab
