Successful resuscitation after multiple injuries in the wilderness.

BACKGROUND: Survival of blunt trauma associated with hypothermic and hemorrhagic cardiac arrest in wilderness areas is extremely rare. CASE REPORT: We describe a case of a 19-year-old female college sophomore who, while glissading down Mt. Adams, had a 400-pound boulder strike her back and left pelvis, propelling her 40 feet down the mountain to land face down in the snow at 7000 feet. It took 4 h from the time of injury until the arrival of the helicopter at our Emergency Department and Trauma Center. The patient lost vital signs en route and had no CO(2) production. A cardiothoracic surgeon

Pediatric wilderness recreational deaths in western Washington State.

STUDY OBJECTIVE: To determine the causes and characteristics of pediatric recreational wilderness deaths. METHODS: All deaths of children between 12 months and 20 years of age involving a wilderness recreational activity in 5 western Washington counties between 1987 and 1996 were identified by medical examiners' logs. Univariate analysis was used to examine variables such as age, gender, activity, mechanism of injury, adult presence, blood alcohol level, safety equipment, and mode of evacuation. RESULTS: Of 40 cases meeting inclusion criteria, 90% involved male subjects and 83% of victims we

Practical Considerations for Participating in Wilderness Activities During Pregnancy.

IntroductionPregnant individuals benefit both psychologically and physically by participating in outdoor activities.MethodsThe authors conducted a narrative review of the available literature to address unique pregnancy considerations for the wilderness adventurer. There is limited evidence behind many opinion-based recommendations on participation in wilderness activities in pregnancy.ResultsAs a result, remote or adventurous activities may be considered unsafe or restricted without significant medical literature to support these opinions. The authors reviewed the literature for the best avai

First-Trimester Pregnancy: Considerations for Wilderness and Remote Travel.

Women increasingly participate in outdoor activities in wilderness and remote environments. We performed a literature review to address diagnostic and therapeutic considerations during first-trimester pregnancy for remote multiday travel. Pretrip planning for pregnant patients traveling outside access to advanced medical care should include performing a transvaginal ultrasound to confirm pregnancy location and checking D rhesus status. We discuss the risk of potential travel-related infections and recommended vaccinations prior to departure based on destination. Immediate evacuation to definit

Lyme Disease: What the Wilderness Provider Needs to Know.

Lyme disease is a multisystem tickborne illness caused by the spirochete Borrelia burgdorferi and is the most common vectorborne disease in the United States. Prognosis after initiation of appropriate antibiotic therapy is typically good if treated early. Wilderness providers caring for patients who live in or travel to high-incidence Lyme disease areas should be aware of the basic biology, epidemiology, clinical manifestations, and treatment of Lyme disease.

Wicked problems in practice: Responding to ecological transformation in wilderness.

Congressionally designated wilderness offers the highest level of land protection in the USA, comparable to category 1b wilderness as defined by the International Union for Conservation of Nature, yet wilderness areas face impacts of climate change. Researchers and managers increasingly recognize the immense challenge of stewarding wilderness in a changing climate. Although responding to climate change is considered a wicked problem, this framing has not been applied to wilderness contexts. We move theory into practice by using wicked problem framing and four illustrative cases to explore how

Wilderness Falls: An Analysis and Comparison of Rock Climbers and Nonclimbers.

BACKGROUND: Wilderness travel and rock climbing are increasingly popular. Urban falls from height have been reported from trauma centers; however, there have been no trauma center reports of rock climbing or wilderness falls (WFs) in the United States. We sought to describe the injury pattern of WF and to determine whether rock climbers represent a distinct pattern of injury. MATERIALS AND METHODS: The trauma database from our level 1 trauma center was queried from 1/1/06 to 12/31/16 for the diagnosis of "fall from cliff" (ICD9: e884.1, ICD10: w15.xx). Demographics, injury characteristics, an

Against the Current: Wilder Penfield, the Frontal Lobes and Psychosurgery.

Psychosurgery refers to the surgical interruption of the white matter fibres joining the frontal cortex to the remainder of the cortical mantle and to the thalamus, in an attempt to mitigate the manifestations of psychosis. It reached its heyday following World War Two and was abandoned with the introduction of major tranquilisers such as chlorpromazine. Wilder Penfield, unlike most of his contemporaries, had a jaundiced view of psychosurgery. This paper addresses Penfield's early experience with experimental, penetrating brain trauma and with the surgical resection of frontal, epileptogenic l

Skull Anatomy in "Saint Jerome in the Wilderness" by Leonardo da Vinci.

As a result of a detailed study of "Saint Jerome in the Wilderness", we found that Leonardo described the skull in this work in an original way. A portion of the skull's face is visible in St Jerome's chest and abdomen projection. This image shows the orbit, the frontal bone, the nasal aperture and the zygomatic process. In our opinion, Leonardo described the skull in the painting with his usual originality.

Management of Live Insects in the External Auditory Canal: A Wilderness Perspective.

A live insect within the external auditory canal is an unpleasant possibility during wilderness recreation. To our knowledge, no study has attempted to quantify the risk of this event occurring in the wilderness. However, such events anecdotally seem to occur with some regularity in a variety of climates. Most cases are benign, but a small subset of patients can develop complications including infection, hearing loss, and vestibular complaints related to the foreign body. In the emergency department or clinic, removal of the insect is a simple procedure in most circumstances; however, the mate

Emerging Tickborne Viral Infections: What Wilderness Medicine Providers Need to Know.

Ticks are versatile vectors of infectious diseases and transmit a broad range of pathogens, including bacteria, viruses, and parasites. Ticks harbor pathogens without infection and share pathogens with other ticks while feeding together on a host. The primary objective of this review is to identify tickborne viral pathogens in the United States, focusing on emerging pathogens. Additional objectives include describing the epidemiology of tick-transmitted viruses, identifying the most common tick vectors of viral pathogens in the United States, identifying the most common tick-transmitted viruse

Rescue blankets as multifunctional rescue equipment in alpine and wilderness emergencies: a commentary.

Emergency applications of rescue blankets go far beyond protection from hypothermia. In this review alternative applicabilities of these remarkable multifunctional tools were highlighted. Newly fabricated rescue blankets prove impressive robustness. The high tensile strength along with its low weight enable further applications, e.g. immobilization of injured extremities, splinting, wound dressing, a makeshift chest seal in sucking chest wounds, amongst others. Furthermore, the foil can be used as a vapour barrier, as eye protection and it can even be used to construct a stopgap bivouac sack,

Ethical, Legal, and Administrative Considerations for Preparticipation Evaluation for Wilderness Sports and Adventures.

Preparticipation evaluations (PPEs) are common in team, organized, or traditional sports but not common in wilderness sports or adventures. Regarding ethical, legal, and administrative considerations, the same principles can be used as in traditional sports. Clinicians should be trained to perform such a PPE to avoid missing essential components and to maximize the quality of the PPE. In general, participants' privacy should be observed; office-based settings may be best for professional and billing purposes, and adequate documentation of a complete evaluation, including clearance issues, shou

Bandages & Brews: Evaluation of an Open-Access Community Wilderness Medicine Education Intervention.

IntroductionOutdoor recreation in the Pacific Northwest frequently exposes individuals to wilderness environments where delays to definitive care can increase morbidity and mortality. Limited access to formal wilderness medicine training may contribute to preventable emergencies. We developed Bandages & Brews, a free community-based wilderness medicine event designed to improve preparedness among recreational participants.MethodsBandages & Brews was held in Portland, Oregon, in March 2026. Participants rotated through four hands-on stations: first aid kits and trail logistics, splinting, bleed

Air Force Pararescue Experience in Wilderness Medicine: Relevance for the Deployed Environment.

In the realm of military special operations, the practice of medicine extends beyond the conventional boundaries of healthcare. Operating within remote and often hostile environments demands a unique fusion of medical expertise, tactical acumen, and adaptability. In this report we review three distinct pararescue missions executed by the 212th Rescue Squadron in which the complexities of wilderness medicine and operational demands are examined. The relationship of Special Operations Forces (SOF) medical skills to wilderness medicine is examined in each mission: wound care for soft tissue injur

Wilderness Dermatology: Bugs, Plants, and Other Nuisances That May Ruin Your Hike.

Spending time outdoors can be rewarding. However, exposure to the sun, insect bites, and plant exposures may result in a wide range of dermatologic manifestations. In this article, we describe potential cutaneous manifestations of common wilderness exposures in New England including photodermatoses from prolonged sun exposure, phytodermatoses from plant exposures, and arthropod-bite reactions from common insects (mosquitos, spiders, ticks, hymenoptera, mites and chiggers). The article will also address preventive and treatment strategies which may help physicians and their patients better prep
