Modeling metacognition and executive functions in the metacognitive wisconsin card sorting test using the neuropsychological digital-twin method.

Executive functions rely on goal-directed manipulation of representations, while metacognition reflects the evaluation and control of one’s own representations. Several studies have examined these processes separately, but none formalise the neuro-representational computations underlying their interaction during goal-directed behaviour. This gap prevents comprehensive frameworks and extended model-based neuropsychological investigations. Here we address these issues by introducing a neuropsychological digital-twin method - a translational modelling framework that integrates clinical and experi

Selective neuronal damage and Wisconsin Card Sorting Test performance in atherosclerotic occlusive disease of the major cerebral artery.

BACKGROUND: In atherosclerotic internal carotid artery (ICA) or middle cerebral artery (MCA) disease, selective neuronal damage can be detected as a decrease in central benzodiazepine receptors (BZRs) in the normal-appearing cerebral cortex. This study aimed to determine whether a decrease in the BZRs in the non-infarcted cerebral cortex is associated with poor performance on the Wisconsin Card Sorting Test (WCST), which assesses executive functions. METHODS: The authors measured the BZRs using positron emission tomography and (11)C-flumazenil in 60 non-disabled patients with unilateral ather

Wisconsin Card Sorting Test synchronizes the prefrontal, temporal and posterior association cortex in different frequency ranges and extensions.

Current findings show some brain regions consistently related to performance of the Wisconsin Card Sorting Test (WCST). An increase of local cerebral blood flow or metabolic demands has been detected in those regions. Functional integration of the neuronal circuits that subserve the task performance, based upon the identification of the oscillations and their distributed cerebral sources, has not been accomplished previously. The event-related tonic oscillations within a period of 2,000 msec after the stimulus onset and the probable neural substrate were evaluated in healthy volunteers by vari

Pinpointing the deficit in executive functions in adolescents with dyslexia performing the Wisconsin card sorting test: an ERP study.

Adolescents with dyslexia exhibit well-established impairments in executive abilities. The Wisconsin card sorting test (WCST) is an executive test that yields surprisingly inconsistent results with this population. The current study aimed to shed light on the contradictory findings in the literature regarding the performance levels by individuals with dyslexia in WCST. We used a computerized-WCST (named the 'Madrid-Card Sorting Test') assessing executive functions using the Event-Related Potentials (ERPs) methodology. Adolescents with dyslexia exhibited a higher error rate and slower reaction

Wisconsin Card Sorting Test performance over time in schizophrenia. Preliminary evidence from clinical follow-up and neuroleptic reduction studies.

Two groups of schizophrenic patients took the Wisconsin Card Sorting Test (WCST) in separate follow-up studies; a naturalistic clinical follow-up (n = 12, study 1) and a neuroleptic reduction study (n = 10, study 2). 11 of 22 subjects (50%) performed well on the task at one or both time points. In each study no group differences were found between time 1 and time 2 performance on three WCST variables. However, correlational analyses revealed considerable within-subject variation on the WCST in the study 1 sample, which was composed of younger and more acute patients than those in study 2. This

Executive dysfunction and cognitive disorganization in schizophrenia: Case-control and cluster analyses of a Korean Wisconsin Card Sorting Test cohort.

OBJECTIVE: Executive dysfunction is a hallmark cognitive deficit in schizophrenia, yet phenotypic heterogeneity within the disorder complicates clinical translation. This study aimed to identify discrete executive profiles within a large Korean schizophrenia cohort using the Wisconsin Card Sorting Test (WCST) and to evaluate their clinical and functional correlates. METHOD: We analyzed WCST performance in 276 individuals with schizophrenia and 361 healthy controls. A principal component analysis (PCA)-derived executive composite was validated against chart-reviewed clinical variables in a pat

Effects of Diabetic Ketoacidosis on Executive Function in Children With Type 1 Diabetes: Evidence From Wisconsin Card Sorting Test Performance.

OBJECTIVE: Diabetic ketoacidosis (DKA) in patients with type 1 diabetes mellitus (T1DM) is known to affect memory function, but little is known about its impact on executive function. This study aimed to determine whether a history of DKA was associated with changes in executive function in children with T1DM. METHODS: The sample consisted of 99 patients with T1DM with histories of DKA, 82 patients with T1DM without DKA, and 100 healthy controls aged 7 to 18 years. Neuropsychological function and emotion assessments were performed in all participants. The Wisconsin Card Sorting Test (WCST) wa

Diagnostic Utility of the Shortened Version of the Wisconsin Card Sorting Test in Patients With Sporadic Late Onset Alzheimer Disease.

BACKGROUND: The classic version of the Wisconsin Card Sorting Test (WCST) consists of correctly sorting 128 cards according to changing sorting criteria. Its application is costly in terms of the time employed, with all the negative consequences this entails (decrease in motivation, frustration, and fatigue). METHOD: The main objective of this study was to test the usefulness of the shortened version of the WCST as compared to the full test by analyzing the equivalence between the two decks comprising the full 128-card version on a sample of patients diagnosed with sporadic late onset Alzheim

An Item Response Theory Analysis of the Wisconsin Card Sorting Test in Normal Aging, Alzheimer's Disease and Parkinson's Disease: Neurophysiological Approach.

The Wisconsin Card Sorting Test (WCST) is widely used to assess executive function, specifically mental flexibility. Item Response Theory (IRT) has several advantages over classical test theory, including the invariance of the measure. This study aimed to apply IRT to study the psychometric properties of WCST in control subjects and patients with idiopathic Parkinson's disease (PD) and Alzheimer's disease (AD). The sample consisted of 86 controls, 77 Parkinson's disease subjects, and 155 AD subjects. The Rasch model was applied to binary data from the conversion to percentiles adjusted for age

Learning proficiency on the Wisconsin Card Sorting Test in people with serious mental illness: what are the cognitive characteristics of good learners?

Although it is widely accepted that schizophrenia and other serious mental illnesses (SMI) are associated with neurocognitive difficulties, there is great variability in neurocognitive functioning across individuals. In recent years, a growing number of schizophrenia studies have utilized the concept of learning potential to explore individual variation in cognition. Learning potential refers to the ability to benefit from instruction and is measured by assessing test performance before and after training. The present study was intended to explore the cognitive characteristics associated with

Performance and brain activity during the Wisconsin Card Sorting Test in adolescents with obsessive-compulsive disorder and adolescents with weight-restored anorexia nervosa.

Anorexia nervosa (AN) and obsessive-compulsive disorder (OCD) both show a peak age of onset during adolescence and share a number of phenotypic features, such as rigid rule-bound behavior and perseverative thinking. There is evidence of difficulties with set shifting or task switching in adults with each disorder, but evidence in adolescents is limited. Furthermore, no studies have previously directly compared AN and OCD on this cognitive process or examined comparative neural correlates. This study provides exploratory analyses to address this gap by measuring brain activity with functional m

The set-shifting profiles of anorexia nervosa and autism spectrum disorder using the Wisconsin Card Sorting Test: a systematic review and meta-analysis.

Difficulties in set-shifting are commonly reported in both autism spectrum disorder (ASD) and anorexia nervosa (AN) populations. Despite this, it is not known whether this cognitive profile persists across different ages, or whether the profiles seen in ASD and AN are comparable. This systematic review and meta-analyses aimed to compare the set-shifting profiles, as measured by the Wisconsin Card Sorting Test (WCST) in adults and younger people with either ASD or AN, relative to healthy controls (HCs) and to statistically compare performance on the WCST between ASD and AN. In all, 24 studies o

Differences in performance and learning proficiency on the Wisconsin Card Sorting Test in schizophrenia: do they reflect distinct neurocognitive subtypes with distinct functional profiles?

The aim of this study was two-fold: 1) to determine whether a priori subtyping of patients with schizophrenia based on both overall performance on a measure of executive-function, the Wisconsin Card Sorting Test (WCST), and ability to learn the task with expanded instruction, could be confirmed with other, independent measures of executive-function and learning, and, if so, 2) to determine whether these groups have different neurocognitive profiles and show differences in functional capacity. Fifty-four outpatients with schizophrenia were divided by WCST performance into three groups: intact e

Auditory and visual working memory performance in patients with frontal lobe damage and in schizophrenic patients with low scores on the Wisconsin Card Sorting Test.

Impaired performance in the Wisconsin Card Sorting Test (WCST) has frequently been postulated to be one typical feature indicating frontal dysfunction in patients with schizophrenia. From a functional point of view, impairments were attributed to a dysfunction of working memory. The present study compares the performance of groups of schizophrenic patients, groups of patients with acquired brain damage as well as normal controls on tasks involving visual and auditory working memory. Modified versions of Sternberg tasks were used varying the physical attributes of the material to be rehearsed i
