An opioid-withholding human laboratory paradigm during opioid agonist treatment for opioid use disorder and chronic pain: Phase- and dose-dependent effects of cannabidiol.

Even during opioid agonist treatment (OAT) for opioid use disorder (OUD), chronic pain remains common and unrelieved, as opioids impair endogenous pain modulation. Cannabidiol (CBD) may represent a non-opioid adjunct, but its effects among persons with co-occurring OUD and chronic pain receiving OAT are unknown. We conducted a randomized, double-blind, placebo-controlled crossover study evaluating acute oral CBD (400, 800, 1200 mg) effects on pain modulation, craving, and cognition among 23 participants (11 female) with co-occurring OUD and chronic pain receiving methadone (mean dose 85.7; SD:

Decision aids for life-sustaining treatment, including withholding or withdrawal and end-of-life care in emergency and intensive care settings: a scoping review protocol.

INTRODUCTION: Decisions regarding life-sustaining treatment and end-of-life care are among the most ethically complex and emotionally challenging decisions encountered in emergency departments and intensive care units. Such decisions frequently involve uncertainty regarding prognosis, limited time for deliberation and situations in which patients may lack decision-making capacity, requiring family members or surrogate decision-makers to participate in the decision-making process. Shared decision-making (SDM) has increasingly been recommended as an approach to align treatment decisions with pat

Factors influencing self-decision in withholding or withdrawal of life-sustaining treatments among older patients admitted to general hospital: A retrospective case-control study.

PURPOSE: This study aimed to identify factors influencing older patients' self-decision in withholding or withdrawing life-sustaining treatment (LST) during hospitalization. METHODS: For this retrospective case-control study (self-decision versus family-decision groups), we used electronic medical records. The sample included 624 older patients who died after completing the LST implementation form. 249 (39.9%) patients were in the self-decision group versus 375 (60.1%) patients in the family-decision group. We performed chi-square tests, t-tests, and binary logistic regression. RESULTS: Rece

Evaluating the impact of a standardised intervention for announcing decisions of withholding and withdrawing life-sustaining treatments on the stress of relatives in emergency departments (DISCUSS): protocol for a stepped-wedge randomised controlled trial.

INTRODUCTION: The decisions of withholding or withdrawing life-sustaining treatments are difficult to make in the context of emergency departments (EDs) because most patients are unable to communicate. Relatives are thus asked to participate in the decision-making process, although they are unprepared to face such situations. We therefore aimed to develop a standardised intervention for announcing decisions of withholding or withdrawing life-sustaining treatments in EDs and assess the efficacy of the intervention on the stress of relatives. METHODS AND ANALYSIS: The DISCUSS trial is a multice

Characterization of patients with atrial fibrillation not treated with oral anticoagulants.

OBJECTIVE: An underuse of oral anticoagulants (OAC) in patients with atrial fibrillation (AF) has been suggested, as only 50% of all patients with AF receive OAC treatment. Whether this is due to contraindications, lack of an indication to treat, or an expression of underuse is sparsely investigated. This study therefore aimed to characterize individuals without OAC treatment in a real-life population of patients with AF. DESIGN: Retrospective cross-sectional study. The medical records were scrutinized in order to identify the type of AF, risk factors for embolism and bleeding, and other fact

Perioperative management of SGLT2 inhibitors: balancing euglycaemic ketoacidosis risk and cardiorenal benefits.

Sodium-glucose cotransporter 2 (SGLT2) inhibitors are established therapies for type 2 diabetes, heart failure, and chronic kidney disease, creating a perioperative need to balance prevention of euglycaemic diabetic ketoacidosis (euDKA) against cardiorenal protection. Fasting, surgical stress, infection, reduced carbohydrate intake, insulin dose reduction, and hypovolaemia can amplify glycosuria and ketogenesis and precipitate euDKA. Most guidelines recommend withholding SGLT2 inhibitors for 3-4 days before elective surgery, although recommendations vary by agent, procedure, fasting duration,

No-enema therapy for idiopathic constipation and encopresis.

Idiopathic constipation and encopresis of childhood are thought to occur when children volitionally withhold stool. Withholding may be prompted by social pressures or by episodes of painful defecation. Repetitive withholding may result in colonic dilatation and colorectal dysfunction. Therapy involves removal of impacted stool, stool softening, and behavioral therapy. The use of enemas in this therapy is widespread but may be counterproductive. A retrospective review of patients treated without enemas revealed 45 patients whose course could be followed for six months. Ninety-eight percent of t

Insulin withholding for weight control in women with diabetes.

OBJECTIVE: To provide a description of the clinical characteristics that distinguish individuals who withhold insulin for weight control from those who do not. Some individuals with insulin-dependent diabetes mellitus (IDDM) control their weight by withholding insulin and purging excessive calories. This process places patients at risk for developing severe hyperglycemia, diabetic ketoacidosis, and increases the risk of long-term complications of diabetes. RESEARCH DESIGN AND METHODS: Forty-two women with IDDM, ages 16-40, were interviewed and divided into two groups: insulin withholders (IWs

Can curative or life-sustaining treatment be withheld or withdrawn? The opinions and views of Indian palliative-care nurses and physicians.

INTRODUCTION: Decisions to withdraw or withhold curative or life-sustaining treatment can have a huge impact on the symptoms which the palliative-care team has to control. Palliative-care patients and their relatives may also turn to palliative-care physicians and nurses for advice regarding these treatments. We wanted to assess Indian palliative-care nurses and physicians' attitudes towards withholding and withdrawal of curative or life-sustaining treatment. METHOD: From May to September 2008, we interviewed 14 physicians and 13 nurses working in different palliative-care programmes in New D

Withholding Oral Anticoagulation at Discharge and Post-Discharge Stroke and Bleeding Vulnerability in a Predominantly Older Cohort Hospitalized With Acute Heart Failure and Atrial Fibrillation.

AIM: In older adults hospitalized with acute heart failure and atrial fibrillation, bleeding concern often drives withholding of oral anticoagulation at discharge. Whether withholding marks a genuinely lower bleeding-risk phenotype or residual clinical vulnerability is uncertain. We examined its association with post-discharge ischemic stroke, major bleeding, and death in a predominantly older cohort. METHODS: We analyzed a registry of patients hospitalized with acute heart failure and atrial fibrillation. In a discharge-based analysis of patients discharged alive and free of stroke or major

A Literature Review of the Therapeutic Perspectives of Sodium-Glucose Cotransporter-2 (SGLT2) Inhibitor-Induced Euglycemic Diabetic Ketoacidosis.

Euglycemic diabetic ketoacidosis (DKA), a side effect of sodium-glucose cotransporter-2 (SGLT2) inhibitors, is a triad of high metabolic anion gap acidosis, raised serum and urine ketones, and serum glucose <250 mg/dl. SGLT2 inhibitors cause a carbohydrate deficit by glucosuria, which leads to an increased glucagon/insulin ratio, the metabolic shift from glucose to lipid utilization causing ketogenesis, and hence euglycemic DKA. Additional factors like the ketogenic diet, illness, surgery, and pregnancy contribute to precipitating these episodes. Keywords search included "Euglycemic DKA and SG

Delaying initiation of diuretics in critically ill patients with recent vasopressor use and high positive fluid balance.

BACKGROUND: Fluid overload is associated with poor outcomes. Clinicians might be reluctant to initiate diuretic therapy for patients with recent vasopressor use. We estimated the effect on 30-day mortality of withholding or delaying diuretics after vasopressor use in patients with probable fluid overload. METHODS: This was a retrospective cohort study of adults admitted to ICUs of an academic medical centre between 2008 and 2012. Using a database of time-stamped patient records, we followed individuals from the time they first required vasopressor support and had >5 L cumulative positive flui

Impact of routine follow-up examinations on life expectancy in ovarian cancer patients: a simulation study.

OBJECTIVE: The clinical benefit of routine follow-up in patients treated for ovarian cancer is subject to debate. In this study, the magnitude of the potential survival benefit of routine examinations was evaluated by Markov modeling. METHODS: The clinical course of ovarian cancer was simulated using a 4-state nonstationary Markov model. Risk of recurrence and mortality probabilities were derived from individual patient data and Statistics Netherlands. The life expectancy was simulated for 3 follow-up scenarios: a current, withholding (all recurrences detected symptomatically), and perfect fo

Withholding the Heimlich Maneuver: Ethical Considerations.

The topic of withholding the Heimlich maneuver as part of a do-not-attempt-to-resuscitate (DNAR) order or an advance directive has not been widely discussed in the clinical ethics literature. This discussion addresses a request by family members to withhold the Heimlich maneuver from a patient in a long-term care facility. A request to forgo the Heimlich maneuver seems to have prima facie categorical similarities to justifications for withholding life-saving treatments such as cardiopulmonary resuscitation (CPR). Further examination reveals significant distinctions. Such distinctions call into

Why Do Cancer Patients Decide to Withdraw Their Treatment? A Systematic Review and Meta-Synthesis.

BACKGROUND: Discontinuation of treatment by cancer patients is a complex decision driven by many factors that are not yet fully understood. Using a qualitative approach is the best way to explore this complexity in greater depth. Therefore, this study aimed to explore and synthesize the factors that influence the decision-making of cancer patients regarding withdrawal from treatment. METHODS: This is a qualitative systematic review and meta-synthesis, conducted using the six-step meta-synthesis method developed by Sandelowski and Barroso in 2007. Initially, a comprehensive systematic search w

Parental Refusal of Surgery in an Infant With Tricuspid Atresia.

We present a case of a fetal diagnosis of tricuspid atresia (TA). The pregnant woman and her husband requested that the baby be treated with only palliative care. The cardiologist did not think it would be appropriate to withhold life-prolonging surgery once the infant was born. The neonatologist argued that outcomes for TA are similar to those for hypoplastic left heart syndrome, and the standard practice at the institution was to allow parents to choose surgery or end-of-life care for those infants. The team requested an ethics consultation to assist in determining whether forgoing life-prol

Renin-Angiotensin-Aldosterone System Optimization for Acute Decompensated Heart Failure Patients (ROAD-HF): Rationale and Design.

INTRODUCTION: The long-term benefits of angiotensin-converting enzyme inhibitors (ACEIs) and angiotensin receptor blockers (ARBs) on outcomes in patients with chronic congestive heart failure are well-known, making them one of the most widely prescribed medications. However, the administration of ACEIs/ARBs in acute decompensated heart failure (ADHF) can increase the risk of morbidity and mortality secondary to worsening renal function (WRF). A decrease in estimated glomerular filtration rate (eGFR) during the treatment of ADHF has been associated with an increase in mortality proportional to

End-of-life care in Moroccan ICUs: ethical challenges, practices, and perspectives of intensivists.

BACKGROUND: End-of-life (EOL) care practices, particularly the withholding and withdrawing (W/W) of life-sustaining treatments (LSTs), remain underexplored in North Africa. This study examined factors that influence EOL practices in Morocco. METHODS: A nationwide online survey was conducted over one month (from July to August 2023) among Moroccan intensivists, assessing their perspectives on W/W LSTs, decision-making processes, and influencing factors, including ethical, cultural, and religious considerations. Univariable analyses were performed to screen for potential associations, followed

Effects of two-dose ceftiofur treatment for metritis on the temporal dynamics of antimicrobial resistance among fecal Escherichia coli in Holstein-Friesian dairy cows.

A pair-matched longitudinal study conducted on three dairy farms in the U.S. High-Plains explored the temporal effects of two-dose ceftiofur crystalline-free acid (CCFA) treatment for metritis on third-generation cephalosporin (3GC) resistance among enteric E. coli in Holstein-Friesian cows. The current 13-day slaughter withholding period does not account for rising populations of third-generation cephalosporin (3GC) resistant bacteria in feces of animals following CCFA treatment. A total of 124 matched-pairs of cows were enrolled in the study. Cows diagnosed with postpartum metritis received
