Poor prognosis of ovarian cancer with large cell neuroendocrine carcinoma: case report and review of published works.

Large cell neuroendocrine carcinoma (LCNEC) is well-reported to result in unfavorable prognoses in many organ cancers while being rarely reported in gynecologic cancer, especially ovarian and endometrial cancers. Here we report a case of ovarian cancer with LCNEC which spread to distant organs within 1 year of primary surgery despite the fact that the post-surgical stage was Ia. The case received platinum-based chemotherapy as an adjuvant therapy after her curative surgery. However, LCNEC in the case was resistant to the chemotherapy. In our review of published works, ovarian cancer cases with

The contribution of Dr. Mary Walker towards myasthenia gravis and periodic paralysis whilst working in poor law hospitals in London.

Dr. Mary Walker discovered in 1934 that physostigmine and Prostigmin temporarily restored muscle function in patients with myasthenia gravis. In the next five years, Dr. Walker and colleagues provided clinical evidence for the weakness of myasthenia gravis being caused by a "disturbance of transmission of excitation from motor nerve to voluntary muscle presumably caused by a deficiency of acetylcholine. Physostigmine (or Prostigmin) compensated for the lack of acetylcholine by delaying its destruction." Dr. Walker and colleagues also described the association between familial periodic paralysi

Evaluating current deficit theories of poor reading: role of phonological processing, naming speed, balance automaticity, rapid verbal perception and working memory.

To clarify the nature of cognitive deficits experienced by poor readers, 9-10-yr.-old poor readers were matched against 9 chronological age and 9 younger reading age-matched controls screened and selected from regular classrooms. Poor readers performed significantly more poorly than chronological age-matched peers on digit naming speed, spoonerisms, and nonsense word reading. Poor readers were also significantly poorer than reading age-matched controls on nonword reading but were significantly better than reading age-matched controls on postural stability. Analyses of effect sizes were consist

The Effect of Shift Work and Poor Sleep on Self-Reported Skin Conditions: A Survey of Call Center Agents in the Philippines.

Night shift workers may have a disrupted circadian rhythm, which may contribute to the development of skin disease. The purpose of this study was to determine whether there is a significant difference in the prevalence and severity of self-reported skin disease between "regular" day shift workers compared to "graveyard" night shift workers. We conducted surveys from 630 call center agents in Manila, the Philippines, and they were analyzed regarding demographics, medical history, dermatologic history, lifestyle, and sleep. No difference was found in the prevalence of skin disease between shifts

Lumbar Discography Is Associated With Poor Return to Work Status Following Lumbar Fusion Surgery in a Workers' Compensation Setting.

Lumbar discography (LD) is used to guide surgical decision making in patients with degenerative disc disease (DDD). Its safety and diagnostic accuracy are under contention. This study evaluates LD's efficacy within the workers' compensation (WC) population. Multivariate logistic regression analysis was used to determine the impact that undergoing LD before lumbar fusion for DDD had on return to work (RTW) rates among 1407 WC subjects. Discography was negatively associated with RTW status (p = .042; OR 0.76); 22.2% (142/641) of LD subjects met the RTW criteria, compared with 29.6% (227/766) of

Engaging the AQ10 to Predict Professional Burnout or Poor Work-Related Psychological Wellbeing Among Anglican Clergy in Wales.

The ten-item Autism Spectrum Quotient (AQ10) is a self-report instrument originally designed to identify referrals for professional diagnosis for Autism Spectrum Disorders (ASD). Recent studies suggest that this instrument may also be tapping more generalised affective disorders. Working with this interpretation, this study examines the predictive power of the AQ10 to account for additional variance, after personal and personality factors have been taken into account, on the two scales of the Francis Burnout Inventory. Data provided by 220 Anglican clergy serving in Wales demonstrated that 8.6

E-health systems for management of MDR-TB in resource-poor environments: a decade of experience and recommendations for future work.

INTRODUCTION: Multi-drug resistant TB (MDR-TB) is a complex infectious disease that is a growing threat to global health. It requires lengthy treatment with multiple drugs and specialized laboratory testing. To effectively scale up treatment to thousands of patients requires good information systems to support clinical care, reporting, drug forecasting, supply chain management and monitoring. METHODS: Over the last decade we have developed the PIH-EMR electronic medical record system, and subsequently OpenMRS-TB, to support the treatment of MDR-TB in Peru, Haiti, Pakistan, and other resource-

Workplace bullying: individual hostility, poor work environment or both? Exploring competing explanatory models in a single longitudinal study.

PURPOSE: A central focus of research and literature on workplace bullying is the importance of explanatory factors such as individual dispositions (i.e., the vulnerability hypothesis) and work environment factors (i.e., the work environment hypothesis). Although several studies address the importance of the two approaches, as well as their individual and combined effects, the unique contribution of each of the competing approaches remain unexplored in a single longitudinal study. METHODS: Based on Affective Events Theory, we explore the contribution of work environment and individual hostilit

A Comprehensive Evaluation of Feasibility and Acceptability of a Nurse-Managed Health Clinic for Homeless and Working Poor Populations: A 3-Year Study.

Background/Objectives: Homeless populations have higher rates of chronic illness and mortality than more advantaged peers but have low primary care engagement. Nurse-managed clinics emerged as a possible solution to increase healthcare access for marginalized populations. This paper presents a comprehensive evaluation of feasibility (conceptualized as patient recruitment and retention) and acceptability (conceptualized as patient satisfaction) of a nurse-managed primary care clinic tailored to people experiencing homelessness and poverty. Methods: This is a three-year retrospective chart revie

Poor Joint Work in the Lower Limbs during a Tennis Forehand Groundstroke after a Cross-Over Step Inhibits an Increase in the Racket Speed.

A forward run-up and stepping are used to accelerate hitting tools or throwing objects in sports. This study aimed to investigate the effect of a forward cross-over step on the speed of a hitting tool by analyzing the joint work and mechanical energy of the whole body and the hitting tool using inverse dynamics. Thirteen advanced tennis players performed forehand groundstrokes at maximum effort with and without a forward cross-over step. From the whole body plus racket perspective, the body-weight-normalized mechanical energy at the start of the hitting motion increased by 1.74 ± 0.42 J·kg-1 d
