Civilian gunshot wounds to the genitourinary tract: incidence, anatomic distribution, associated injuries, and outcomes.

OBJECTIVES: Gunshot wounds (GSW) affecting the genitourinary (GU) system in civilians are uncommon. This study describes the incidence, anatomic distribution, demographics, associated injuries, management, and outcomes after civilian GU GSW. METHODS: A Level 1 Trauma Center Registry was used to retrospectively identify all patients who sustained GU GSW (January 1997-December 2008). Patient information was abstracted from the Registry, medical, and autopsy records. Multivariate regression detected significant factors associated with mortality. RESULTS: Of 2941 civilian GSW patients, 309 (10.5

Self-inflicted submental and transoral gunshot wounds that produce nonfatal brain injuries: management and prognosis.

OBJECT: Penetrating brain injuries caused by self-inflicted gunshot wounds are very often fatal and survivors suffer serious disabilities. Recognition of a possibly more favorable prognosis for a specific type of injury, the submental or transoral handgun or low-energy rifle wound, prompted the authors to review their experience with patients who had attempted suicide in this manner. METHODS: The records of 11 consecutive patients seen over a 10-year period (1992-2001) were retrospectively reviewed. Handguns were used by eight patients and .22 caliber rifles by the others. The patients presen

Penetrating heart wounds repaired without cardiopulmonary bypass. Evaluation and follow-up of recent war injuries.

Penetrating cardiac injuries requires urgent management. Between September 1991 and June 1992, 10 patients with penetrating cardiac injuries sustained in war were treated at our hospital in Croatia, which does not have cardiopulmonary bypass facilities. Seven of these patients survived cardiorrhaphy and were discharged from the hospital, subject to follow-up. In 5 of the survivors, the injuries were inflicted by fragments from explosive devices; in 1 survivor, by a bullet; and in 1 survivor, by a rib fragment. The left ventricle was lacerated in 4 patients, the right ventricle in 2, the left a

[Surgical treatment of penetrating chest wounds with injuries to the internal thoracic and intercostal arteries].

Injuries of the internal thoracic and intercostal arteries are observed rather frequently in penetrating wounds of the thorax. In intrapleural hemorrhage the only correct tactics is the urgent surgical intervention. Its efficiency is dependent on the terms of admittance of the patient to a specialized clinic, preparedness of different services (reanimation, x-ray analysis, operation room) for urgent help, on qualification of surgeons and careful management during the postoperative period.

Wound ballistics and blast injuries.

Wounds due to gunshot and explosions, while usually observed during battlefield combat, are no longer an exceptional occurrence in civilian practice in France. The principles of wound ballistics are based on the interaction between the projectile and the human body as well as the transfer of energy from the projectile to tissues. The treatment of ballistic wounds relies on several principles: extremity wound debridement and absence of initial closure, complementary medical treatment, routine immobilization, revision surgery and secondary closure. Victims of explosions usually present with a co

[Complications of gunshot wounds and blunt injuries of the spine and spinal cord in early period].

UNLABELLED: Combat injuries to the spine and spinal cord refer to rare, but some of the most severe. Currently, there is a need to improve the treatment and rehabilitation of patients with injuries to the spine and spinal cord. However, the complicated course of the wound infection can become a critical factor influencing the treatment outcome. OBJECTIVE: To analyze the structure of complications in the early period and determine the factors that have the greatest influence on the incidence of their development in gunshot wounds and closed injuries to the spine and spinal cord under modern co

Management of vertebral artery injuries in civilian cervical gunshot wounds: A case series and literature review.

Cervical gunshot injuries, although rare, carry a significant risk of vertebral artery injuries (VAIs), which can lead to complex neurological and vascular complications. Effective management requires individualized strategies tailored to the clinical presentation and associated injuries. This study presents three cases of civilian cervical gunshot wounds involving VAIs and reviews the literature to highlight optimal management approaches. Three male patients, aged 24, 41, and 44, presented with cervical gunshot wounds and varying degrees of spinal cord and vertebral artery injuries. Two patie

Antibiotic therapy in gunshot wound injuries.

Protocols for antibiotic prophylaxis in the treatment of fractures caused by gunshots have not been delineated clearly in the literature to date. The current review of the literature reveals that antibiotic therapy for treatment of these fractures is predicated on the muzzle velocity of the weapon used to inflict the fracture. General consensus has been reached regarding the requirement of at least 24 hours of intravenous antibiotic treatment in fractures caused by high-velocity weapons in conjunction with the appropriate wound and fracture care. Similarly, in fractures caused by shotguns, tho

A Penetrating Facial Wound With Burn Injury.

A patient presented with a complex penetrating facial wound by high temperature steel. The hot steel penetrated right temple, ethmoid bone, and maxillary sinus, and then exited from his left cheek. He kept his right eye but lost his sight. For functional and esthetic considerations, treatments were provided in a staged procedure. First, the debridement was performed under the endoscope. The dead bone was removed, broken teeth were extracted, and necrotic tissue was cleaned. The exposed wounds were cleaned, and dressing was changed daily until the exudation was widely reduced. Then, a 50-mL exp

Timing of surgical intervention in peripheral nerve injuries from gunshot wounds: Management and review of the literature.

BACKGROUND: Gunshot wounds (GSWs) can result in various peripheral nerve injuries (PNIs), ranging from direct nerve transection to neuropraxia caused by the ballistic shockwave mechanism. PNIs from GSWs can be treated with either early or delayed intervention, with the literature supporting both approaches and sparking a debate between early and delayed intervention for PNIs from GSWs. Here, we present a case that underwent delayed exploration of the right common peroneal nerve after GSW and a literature review comparing early versus delayed intervention for PNIs from GSWs. CASE DESCRIPTION:

Survivors of self-inflicted gunshot wounds to the head: characterization of ocular injuries and health care costs.

IMPORTANCE: Suicides and attempted suicides are major public health issues in the United States and around the world. Self-inflicted gunshot wounds (SIGSWs) are a common method of attempting suicide, the head being the most commonly injured body region; however, the literature lacks an overview of the orbital and ocular injuries as well as outcomes associated with SIGSWs. OBJECTIVES: To characterize the ocular and orbital injuries and outcomes of patients presenting with SIGSWs and to examine the cost associated with these injuries. DESIGN, SETTING, AND PARTICIPANTS: Retrospective medical re

Ex-situ identification of vertebral artery injuries from stab wounds through contrast-enhanced fluoroscopy and micro-CT.

Vertebral artery injuries (VAIs) are rarely observed in forensic practice and can result from either penetrating injuries or blunt force trauma to the neck. The identification of such injuries represents a pivotal challenge for forensic pathologists, due to the protected anatomical course of these vessels, particularly in their extracranial tract. Over the years, several approaches have been proposed to address this issue, including vessel gross examination through different dissection techniques, the water injection method and, more recently, post-mortem computed tomography angiography (PMCTA
