Zolpidem for the Management of Catatonia: A Systematic Review.

BACKGROUND: Catatonia is a psychomotor syndrome associated with neurotransmitter disturbances, common in both psychiatric and medical settings. Hypoactivity of the GABAA receptor is one of the predominant theories behind the pathophysiology of catatonia, affecting both motor functioning and emotional regulation. Benzodiazepines such as lorazepam are considered the first-line treatment for catatonia. However, up to 27% of catatonia cases fail to respond to benzodiazepines alone. Zolpidem, which can be used as a challenge, monotherapy, or augmentation agent, serves as a promising pharmacological

The Use of Zolpidem in Catatonia: A Case Report.

Catatonia is a complex neuropsychiatric syndrome characterised by abnormal psychomotor disturbance, which poses a diagnostic and treatment challenge to clinicians. It is a life-threatening condition in its severe form, termed malignant and characterised by hyperthermia and autonomic disturbances. Early recognition and treatment are important in its management. A screening instrument, such as the Bush-Francis Catatonia Rating Scale (BFCRS), can help the clinician make a prompt diagnosis. Only a few published cases describe zolpidem being used to treat this condition, with lorazepam and electroc

Potential benefits of zolpidem in disorders of consciousness.

It has been suggested that zolpidem may arouse patients with decreased level of consciousness. Zolpidem may partially or even completely reverse abnormal cell metabolism following brain damage. In this article, available evidences regarding effects of zolpidem on disorders of consciousness were reviewed. Areas covered: A literature review was conducted using PubMed, Scopus, Medline, Cochrane Central Register of Controlled Trials, Cochrane Database of Systematic Reviews and Google Scholar as online databases. Search Keywords were 'vegetative state', 'minimally conscious state', 'semi-comatose',

[Dependence on zolpidem: a report of two cases].

Zolpidem is a sedative and hypnotic drug belonging to imidazopyridine family. Zolpidem facilitates GABAA function more selectively than benzodiazepines, and produces a selective hypnotic effect. In comparison with benzodiazepines this mechanism could be reduce liability to induce dependence. Recently, some cases of zolpidem abuse and dependence have been published. The Authors report 2 cases of addiction to high dose of zolpidem and compare them with others described in the literature. Both patients had been reknown drug addicts before their first prescription of zolpidem and a borderline pers

Zolpidem and Gender: Are Women Really At Risk?

BACKGROUND: In 2013 the Food and Drug Administration (FDA) claimed the existence of new data showing women to be at risk for excessive daytime sedation and impaired driving proficiency following bedtime doses of zolpidem. The putative explanation was the reduced metabolic clearance of zolpidem and higher morning blood concentrations in women compared to men. The FDA acted to reduce the recommended dosage for women down to 50% of the dose for men. No other regulatory agency worldwide has taken similar action. METHODS: Gender effects on zolpidem pharmacokinetics, pharmacodynamics, adverse effec

Hallucinations with zolpidem and fluoxetine in an impaired driver.

A 54-year-old male was observed driving erratically. The subject displayed both horizontal and vertical nystagmus and poor balance on field sobriety tests (FST's). Further observations included slow movements, breathing, and speech, extremely poor coordination, lack of convergence, and pupils with slow reaction to light. The subject related he was seeing roadway lines doubled and felt a hallucinogenic effect. Breath alcohol results were negative. Blood tests found zolpidem and fluoxetine in addition to hydrocodone at therapeutic levels. This case is presented as an example of hallucinogenic ef

Acute zolpidem poisoning--analysis of 344 cases.

Zolpidem is a new short acting hypnotic agent, first launched in France in 1988. Three hundred forty-four cases of intentional acute overdoses are reviewed retrospectively. Patients were predominantly female (70%) in their third or fourth decade. Ingested doses of zolpidem ranged between 10 and 1400 mg (one pack or less in 80%). Half of the patients ingested other substances (psychotropic drugs and alcohol) concomitantly. Signs of intoxication were observed in two thirds of the population but could be attributed to zolpidem in only 105 cases: drowsiness (N = 89) occurred at doses of 140 to 440

Zolpidem, Complex Sleep-Related Behaviour and Volition.

A commonly prescribed hypnotic, Zolpidem, has been linked to accidental death, driving offences and a very uncommon legal defence – automatism. The deaths and prima facie criminal behaviour that have triggered legal concern and considerable publicity have occurred while the person was in a sleep-like state and apparently acting involuntarily after ingesting the drug. Australian courts have had a mixed response to these claims, and have closely examined the expert evidence that is essential to establishing the link between the conduct and the medication. Accepting that a connection has been est

Acute zolpidem overdose--report of two cases.

This report describes two cases of acute zolpidem overdose. The decedent in the first case was a 36-year-old female found dead in bed in her secured home. She had a history of psychiatric illness, including paranoid disorder, depression with panic episodes, and post-traumatic stress disorder. She was treated with risperidone and sertraline. Nine months prior to her death, the decedent was also prescribed zolpidem (Ambien). The postmortem examination revealed white foam within the larynx and upper trachea, which is indicative of pulmonary edema. Toxicological analyses of the urine showed the pr

Fatal zolpidem poisoning due to its intravenous self-injection: Postmortem distribution/redistribution of zolpidem and its predominant metabolite zolpidem phenyl-4-carboxylic acid in body fluids and solid tissues in an autopsy case.

We experienced a curious fatal case, in which a male in his 20s self-administered zolpidem intravenously. The victim was found dead lying on floor of his apartment room, with a tourniquet band and new injection marks on his right forearm. Nearby the body, a medical disposal syringe containing small-volume solution dissolving crushed zolpidem tablets was found. The postmortem interval was estimated at about two days. The direct cause of his death was judged as asphyxia due to the aspiration of stomach contents into the trachea and bronchi. The specimens dealt with were body fluids and solid tis

The Association of Zolpidem and Suicidality in Psychiatric Outpatients With Insomnia.

OBJECTIVE: Zolpidem is widely prescribed to psychiatric outpatients for the management of insomnia due to its rapid and potent efficacy. However, concerns persist regarding its potential association with suicidal tendencies. While studies have suggested that Z-drugs may influence suicidal behavior in the general population, the specific association of Z-drug use and suicide risk in psychiatric outpatients with insomnia remains unclear. This study aims to investigate the association of zolpidem use and suicidal tendencies in this population. METHODS: This retrospective study analyzed the medic

Benzodiazepine and Zolpidem-Induced Bradycardia: A Case Report.

Benzodiazepines and zolpidem are commonly used in suicide attempts. They have previously been thought to have very few side effects outside of central nervous system depression, and are considered relatively safe even in overdoses. Cardiovascular manifestations of these overdoses are exceedingly rare. We depict a case where an unknown woman appearing to be in her 50s presented to our emergency department somnolent after a large ingestion of alprazolam, clonazepam, and zolpidem. During the evaluation, the patient became bradycardic to 35 beats per minute on the monitor, which responded to atrop

Antipsychotic-like effects of zolpidem in Wistar rats.

Aside from their use in the treatment of anxiety disorders and insomnia, benzodiazepines and other GABAA receptor positive modulators are widely used as add-on treatments in schizophrenic and non-schizophrenic psychoses. However, there is relatively little direct clinical or pre-clinical evidence for antipsychotic effects of GABAergic medications. Previous studies have indicated that zolpidem, a GABAergic drug acting preferentially at α1-containing GABAA receptors, may produce catalepsy through interactions with dopaminergic neurotransmission. The aim of the present study was to investigate ef

Zolpidem is a potent anticonvulsant in adult and aged mice.

Zolpidem is a widely used hypnotic drug acting via benzodiazepine binding sites on GABA(A) receptors. Several studies suggested that zolpidem might have better anticonvulsant potency than previously thought. To compare the sedative and anticonvulsant potency of this drug, we studied in male mice the influence of zolpidem (0.1-10 mg/kg i.p.) on ambulatory locomotor activity (a measure of sedation) and on the threshold for myoclonus, clonic and tonic seizures, as well as death, in response to i.v. infusion of pentylenetetrazole (PTZ, 4.4 mg/min). Because older people take zolpidem more often tha

Modafinil and zolpidem use by emergency medicine residents.

OBJECTIVES: The objective was to assess the prevalence and patterns of modafinil and zolpidem use among emergency medicine (EM) residents and describe side effects resulting from use. METHODS: A voluntary, anonymous survey was distributed in February 2006 to EM residents nationally in the context of the national American Board of Emergency Medicine in-training examination. Data regarding frequency and timing of modafinil and zolpidem use were collected, as well as demographic information, reasons for use, side effects, and perceived dependence. RESULTS: A total of 133 of 134 residency progra
