Zygomatic implants (ZIs) offer a robust solution for maxillary rehabilitation in patients with severe atrophy, oncological resection, or following trauma, where the use of conventional implants is not practical. However, because of their length, design, and placement, ZIs face mechanical challenges which can lead to complications such as fracture. This treatment details the novel, minimally invasive in situ repair of the fractured shaft of a ZI. The patient, diagnosed with carcinoma of the right maxillary sinus, was treated with hemimaxillectomy and conventional radiotherapy. She was restored

The endoscope has been used to visualise the upper face and brow in aesthetic facial surgery which is performed without a coronal incision. We applied these principles to repair zygomatic fractures with the aid of a 4 mm, 30 degree telescope inserted through a small temporal incision. Fracture sites at the zygomatic arch and the zygomaticofrontal suture were exposed and fixed with miniplates under endoscopic control. This technique was used in 15 consecutive patients including 2 with isolated zygomatic arch comminuted fractures and 13 having displaced zygomatic fractures with segmental, displa

OBJECTIVE: To determine if performing a superior cantholysis eases the surgical exposure, reduction, and rigid fixation of the zygomaticofrontal suture in the open repair of zygomatic complex (ZMC) fractures.
PATIENTS AND METHODS: Fifteen superior cantholysis procedures were used in 14 patients who presented with ZMC fractures requiring open reduction and internal fixation. Follow-up ranged from 6 to 18 months. Collected data included patient demographics, cause of fracture, fracture classification, associated facial injuries, methods of fracture exposure and reduction, type and location of f

Comminuted zygomatic arch fractures are a challenge to manage without complete exposure owing to their instability. The presence of facial nerve branches near the bone often means that these fractures are often treated by closed methods or left untreated. The disadvantages of such comminuted fracture left untreated cannot be overemphasised. Hence, there is a need of a surgical approach for exposure and fixation of such fractures without limitation of facial nerve injury. We propose and report a case of malunited comminuted fracture of zygomatic arch using facelift approach.

The prominent zygomatic bone is one of the most commonly fractured. Fracture with displacement of the bone results in a cosmetic and functional deformity. The fractured zygomatic bone is usually dislocated in an inferomedial and posterior direction, which results in a cosmetic deformity with loss of ipsilateral malar prominence, possible depression of the zygomatic arch, asymmetry of the bony orbital circumference, and possible enophthalmos. Fracture of the zygomatic bone may result in ocular, maxillary antral, and mandibular dysfunction; diplopia, restricted extraocular muscle movement, or in

BACKGROUND: M-shaped zygomatic arch fractures can usually be treated effectively through closed reduction. It consists of 2 fracture segments: the anterior zygomatic segment and the posterior temporal bone segment. In clinical practice, atypical M-shaped fractures are often encountered, in which the anterior and posterior fracture segments are discontinuous and separated. Closed reduction usually cannot achieve the desired anatomic reduction effect for this type of fracture.
METHODS: The preoperative design showed that the anatomic reduction of the posterior zygomatic arch fracture segment wa

OBJECTIVE: To explore the procedure and effectiveness of sequential reduction and fixation for zygomatic complex fractures.
METHODS: Between March 2004 and February 2012, 32 patients with zygomatic complex fractures were treated. There were 28 males and 4 females with a median age of 29 years (range, 17-55 years). Fractures were caused by traffic accident in 29 cases and by tumble in 3 cases. The time between injury and admission was 1-12 days (mean, 3 days) in 28 fresh fractures and 22-60 days (mean, 40 days) in 4 old fractures. All patients were diagnosed by clinical symptom and CT scan. Co

BACKGROUND: Anatomic reduction of the zygomatic arch, a key surgical landmark for midfacial width and projection, is essential for the treatment of combined fractures of the zygomaticomaxillary complex and zygomatic arch. Reduction control in surgery for this common facial fracture would be facilitated by intraoperative real-time assessment using widely available and reliable equipment. Although C-arm fluoroscopy is routinely used in the repair of orthopedic fractures, its use in the maxillofacial region, particularly for combined zygomatic fractures, has been scarcely reported.
METHODS: We p

The authors present their experience with transconjunctival approach to access the orbital floor, infraorbital rim and zygomatic-frontal and zygomatic temporal sutures in 40 patients with fractures in the orbital and zygomatic region. The preseptal and retroseptal approaches are described. Despite the complication rate of 12.5%, the esthetic results and simultaneous visualization of the infraorbital rim and lateral orbital rim supports the use of the transconjunctival approach.

Isolated zygomatic arch fractures represent about 10% of facial fractures. Most are easily reduced, but about 7% require fixation. Stabilization with plates, transcutaneous wiring, and various packing materials may involve additional morbidity and other drawbacks. A double balloon epistaxis catheter may be inserted beneath the arch fracture site and inflated for 5 to 7 days as a relatively simple, reliable alternative that involves minimal morbidity.

PURPOSE: With the increasing number of patients with facial trauma, orbital-zygomatic-maxillary (OZM) fracture combined with orbital wall fracture has become common. Such fractures always induce symptoms of enophthalmos, hypoglobus, and diplopia. This study was aimed to investigate the surgical technique and operative effect of late reconstruction of OZM fractures combined with orbital wall fractures.
METHODS: A retrospective analysis of the 46 patients (46 orbits) who were treated from January 2005 to December 2008 was performed. All cases had reconstruction of OZM fractures combined with or

OBJECTIVE: To document the neurosensory changes in the infraorbital nerve following zygomatic fractures managed in various ways.
STUDY DESIGN: Twenty-five patients were included in the study. Neurosensory function was assessed with calibrated nylon monofilaments, electrical stimulation, heat detection thresholds and response to pin prick in the infraorbital, supraorbital, and mental nerve regions. Patients were seen immediately post-trauma, then 1 and 6 months following surgery.
RESULTS: Nine fractures were caused by traffic accidents (TAs), 8 by falls, and 8 by a local blow in a physical di

The analysis of 120 patients with zygoma fractures showed disturbances of sensibility in the field of the N. infraorbitalis in 84% and pathological statements of eyes in 20%. The re-examinations of 76 of these patients 0.5-4.5 years postoperative showed that more than the half of these patients, who were treated by only the bone-retractor have a redislocation of the zygoma, and 49% of all cases have permanent damages of the N. infraorbitalis. The reason for the damage of the nerve is not only the compression of the nerve for a short time by the dislocated zygoma but also regular present additi

Fractures of the pediatric midface are infrequent, particularly in children in the primary dentition, due to the prominence of the upper face relative to the midface and mandible. With downward and forward growth of the face, there is an increasing frequency of midface injuries seen in children in the mixed and adult dentitions. Midface fracture patterns seen in young children are quite variable; those in children at or near skeletal maturity mimic patterns seen in adults. Non-displaced injuries can typically be managed with observation. Displaced fractures require treatment with appropriate r

OBJECTIVE: To report the case of a 58-year-old man with a zygomatic arch fracture, which was well localized and reduced using a surgical navigation system.
METHODS: In this clinical report, the authors suggest intraoperative surgical navigation systems are useful diagnostically and for localizing sites of zygomatic arch fractures.
RESULTS: The patient underwent successful closed reduction of zygomatic arch fractures using a surgical navigation system.
CONCLUSIONS: Surgical navigation is a useful tool for identifying the locations of zygomatic arch fractures and for guiding closed reduction.
