Surface electrocardiographic clues suggesting presence of a nodofascicular Mahaim fiber.

Standard electrocardiograms from 87 consecutive patients with tachycardia of left bundle branch block configuration were analyzed retrospectively for features that might be characteristic of tachycardia utilizing a nodofascicular Mahaim fiber. The study group consisted of 13 patients with nodofascicular tachycardia, 34 with supraventricular tachycardia and aberrant conduction over the His-Purkinje system, 22 with ventricular tachycardia and 18 with antidromic tachycardia utilizing a right-sided accessory atrioventricular pathway. Six variables present during tachycardia of left bundle branch b

Successful trans-septal ablation of a left concealed accessory pathway in a patient receiving surgical mitral valve repair and mechanical aortic valve replacement.

We presented a case of severe aortic regurgitation and moderate mitral regurgitation s/p aortic valve replacement and mitral valve repair. Deterioration of tachyarrhythmia attacks was noted. In EP study, left lateral accessory pathway with orthodromic atrioventricular reentrant tachycardia was identified. We successfully ablated the accessory pathway by trans-septal approach. Even though trans-septal approach currently is a daily routine of invasive interventional electrophysiologists, in this case, we want to emphasize and illustrate the distance between true mitral annulus and coronary sinus

Impact of a Novel Pre-Procedural ECG Schematic for Accessory Pathway Localization on Procedural Outcomes of Radiofrequency Catheter Ablation in Typical Wolff-Parkinson-White Syndrome.

BACKGROUND/AIMS: This study aimed to evaluate the clinical efficacy, procedural metrics, and safety outcomes of radiofrequency catheter ablation (RFCA) for typical Wolff - Parkinson - White (WPW) syndrome utilizing a pre-procedural 12-lead surface electrocardiogram (ECG) schematic for accessory pathway (AP) localization in a Vietnamese cohort. METHODS: A prospective, descriptive study was performed on 109 consecutive patients with typical WPW syndrome undergoing electrophysiological study (EPS) and RFCA. A stepwise 12-lead ECG algorithm (the new schematic) was applied to predict the AP anatom

Double ventricular response in dual AV nodal pathways mimicking interpolated premature beat.

Double ventricular response in dual atrioventricular (AV) nodal pathways can result in nonreentrant supraventricular tachycardia. Since this condition was first described in 1979, around 20 cases have been reported. Here, we present the case of a patient with a confirmed diagnosis of double ventricular response in dual AV nodal pathways resembling an interpolated premature beat who underwent successful radiofrequency ablation of the slow pathway.

A case for ambidextrous doctors.

We report the case of a 30-year-old man with situs inversus totalis, recurrent orthodromic reciprocal tachycardia, and the Wolff-Parkinson-White syndrome. He underwent, in our department, radiofrequency ablation of an accessory pathway (AP) located in the lateral mitral atrioventricular ring. Ablation of the AP was carried out successfully through a patent foramen ovale under fluoroscopic guidance, in a right anterior oblique projection with a 30° tilt and in anteroposterior views. We also used a mirror reversal of electrocardiogram (ECG) leads to better judge the site of the AP by using exist

Percutaneous catheter ablation of arrhythmias in children.

BACKGROUND: Supraventricular tachycardias (SVTs) are the most common arrhythmias in children, with Wolff-Parkinson-White (WPW) syndrome the most prevalent entity. Percutaneous catheter ablation is now the standard of care for SVT in pediatric patients, although specific techniques are required due to their age. OBJECTIVE: To evaluate the results of percutaneous catheter ablation of arrhythmias in children at our institution. METHODS: This was a retrospective study of the clinical records of patients aged 18 years who underwent electrophysiologic study and ablation between January 2002 and De

Effect of isoproterenol on accessory pathways without overt retrograde conduction.

INTRODUCTION: Absence of overt retrograde accessory pathway conduction may be related to low resting sympathetic tone in patients with apparent unidirectional anterogradely conducting accessory pathways (UACAP). METHODS AND RESULTS: To test this hypothesis, we studied the effect of isoproterenol on accessory pathway function and tachycardia induction in 18 patients (12 men and 6 women, ages 34 +/- 16 years [mean +/- SD]) with UACAP. After baseline study in the drug-free state, electrphysiologic testing was repeated during infusion of isoproterenol (0.5 to 1.5 micrograms/min, titrated to incre

Novel pacing maneuvers for mapping the upstream insertion of unidirectional accessory pathways.

BACKGROUND: Anatomic characterization of unidirectional accessory pathways (APs) is inherently limited to the localization of the downstream insertion site. The inability to define the full anatomic course of unidirectional pathways can limit the safety and effectiveness of ablation in the setting of complex pathways, slanted pathways, unstable catheter positioning at downstream insertions, or insertions near the conduction system. OBJECTIVE: We aimed to develop novel pacing maneuvers to localize upstream insertions of unidirectional APs. METHODS: Two methods were evaluated: localizing the s

Accessory pathway ablation during atrial fibrillation in Ebstein anomaly.

An 84-year-old woman with type B Wolff-Parkinson-White (WPW) with Ebstein anomaly was admitted with heart failure. She had rapid wide QRS tachycardia due to accessory pathway (AP) conduction associated with atrial fibrillation (AF). Since transesophageal echocardiography before catheter ablation showed a left atrial thrombus, ablation was performed using a 3D mapping system under AF. After marking the functional tricuspid anulus with intra-cardiac echocardiography, 3D intra-cardiac electrogram visualization (ripple map) during AF enabled clear identification of location of the AP. After ablati

Radiofrequency catheter ablation of accessory pathways in patients with Ebstein's anomaly: At 8 years of follow-up.

BACKGROUND: Data regarding long-term follow-up of radiofrequency catheter ablation (RFCA) of accessory pathways (APs) in patients with Ebstein's anomaly (EA) are limited. The procedures are challenging due to multiple or wide APs. METHODS: Analysis was performed on clinical and periprocedural data of patients with EA referred to the centre in order to perform catheter ablation of AP. The group consisted of 22 patients (female 40.9%, mean age 33.6 ± 19.1 years). The follow-up utilized electrocardiogram and Holter monitoring. RESULTS: Twenty-two patients had 33 accessory pathways (8 patients h

[Immediate and long-term results of radiofrequency ablation of accessory atrioventricular pathways].

The aim of this study was to report the authors' experience of radiofrequency ablation of accessory atrioventricular pathways over a 10 year period (01-91 to 10-00), and the effect of the "learning curve" on the results. The data of 400 patients admitted to primo-ablation of a bundle of Kent was analysed retrospectively. A total of 481 ablations were performed (1.20 per patient). The cumulative global success in the 414 accessory pathways treated was 90.6%. The primary success rate increased from the 1st to the 4th quartile from 68 to 97% (p = 0.0001). The mean duration of fluoroscopy and numb

[Ablation of accessory pathways by radiofrequency current. Towards a simplified approach of Wolff-Parkinson-White syndrome?].

From December 1990 to April 1992, 41 consecutive patients (22 men and 19 women with an average age of 35 +/- 16 years -6-72) underwent ablation of accessory atrioventricular conduction pathways (Bundles of Kent) for poorly tolerated and/or medically resistant supraventricular tachycardias. In 33 cases, the arrhythmia was a paroxysmal SVT, in 7 cases atrial fibrillation, and in 1 patient incessant junctional tachycardia causing left ventricular dysfunction. The Wolff-Parkinson-White syndrome was apparent in 30 patients and concealed in 11 cases. The location of the Kent bundle was left lateral

Orthodromic Reciprocating Tachycardia Relying on Aberrant Conduction: The Need for a Larger Circuit.

Aberrant conduction during orthodromic reciprocating tachycardia (ORT) prolongs the ventriculoatrial conduction time, which can be essential for the maintenance of tachycardia in specific cases. We searched for ORT relying on aberrancy among 220 cases in our center. Three patients showed the phenomenon of aberrancy-dependent ORT. All accessory pathways were located at the lateral regions of the atrioventricular annulus. None of them had a baseline bundle branch block (BBB). Creating a functional BBB was necessary to induce the tachycardias. In two cases, termination of tachycardias was directl

Pathogenesis of cardiac conduction disorders in children genetic and histopathologic aspects.

Fetal dysrhythmias are usually transient. Abnormal fetal rates and rhythms during labor are "functional." Fetal dysrhythmias may be associated with congenital heart disease and fetal hydrops. Bradycardia is usually related to fetal distress; supraventricular tachycardia, atrial flutter, and atrial fibrillation may be associated with severe congestive heart failure. Ventricular fibrillation is rare in the fetus and infant and is usually associated with myocardial necrosis with perimembranous septal defect; the nonbranching atrioventricular (AV) bundle may have an aberrant position and result in

Radiofrequency ablation of a concealed nodoventricular Mahaim fiber guided by a discrete potential.

INTRODUCTION: We present the case of a 17-year-old woman who underwent an electrophysiological study and radiofrequency (RF) ablation of supraventricular tachycardia refractory to medical treatment. Two right-sided, concealed, nondecremental atrioventricular accessory pathways (AV-APs) involved in orthodromic circus movement tachycardias were identified. After RF ablation of both AV-APs, evidence of bidirectional dual AV nodal conduction was demonstrated and regular narrow complex tachycardia was induced. METHODS AND RESULTS: During the tachycardia, retrograde slow and fast AV nodal pathway c

Antidromic Nodoventricular Reciprocating Tachycardia With Left-Ventricular Insertion: Insights From High-Density Mapping.

We describe the case of a 38-year-old male with a long history of paroxysmal supraventricular tachycardia (PSVT) who was found to have typical slow-fast atrioventricular nodal reentrant tachycardia (AVNRT) and nodoventricular reentrant tachycardia (NVRT). During an electrophysiology study, a wide complex tachycardia with left bundle branch block (LBBB) morphology with V2 transition was induced, exhibiting ventriculo-atrial (VA) dissociation and characteristics suggesting an AV node-ventricle reentrant circuit. Careful mapping and pacing maneuvers supported the diagnosis of a nodoventricular re

Implantation of engineered conduction tissue in the rat heart.

Engineered conduction tissues (ECTs) are cardiac conduction tissues fabricated in vitro to allow for more precisely targeted in vivo transplantation therapy. The transplantation of ECTs may be ideal for the treatment of atrioventricular conduction block and could have a significant impact on the future application of biological pacemakers. However, there is little published information regarding the conduction function of ECTs in vivo. In the present study, ECTs were constructed by seeding cardiac progenitor cells (CPCs) into a collagen sponge and were then transplanted into animal hearts to d

Radiofrequency catheter ablation in familial paroxysmal supraventricular tachycardia due to accessory atrioventricular pathways.

Radiofrequency catheter ablation (RF-CA) has been widely used to cure paroxysmal supraventricular tachycardia (PSVT). However, its use has never been reported in familial PSVT caused by an accessory atrioventricular pathway (AP), which is known as one of the typical familial cardiovascular diseases. Two cases of using RF-CA for familial PSVT due to APs are presented, in a brother and sister, supporting a potential genetic role in the developmental failure to lose the atrioventricular connection during fetal life. The sister, a 24-year-old woman, had intermittent episodes of palpitation accompa
