Trunk Kinematics of Experienced Riders and Novice Riders During Rising Trot on a Riding Simulator.

Asymmetry of horses and humans is widely acknowledged, but the influence of one upon the other during horse riding is poorly understood. Riding simulators are popular for education of beginners and analysis of rider biomechanics. This study compares trunk kinematics and saddle forces of 10 experienced riders (ER) and 10 novice riders (NR) performing rising trot on a simulator. Markers were placed on the 4th lumbar (L4) and 7th cervical (C7) spinous processes, and both acromion processes. Displacements in three axes of motion were tracked using 10 high-speed video cameras sampling at 240 Hz. Di

Arthroscopic rotator cuff repair with and without subacromial decompression is safe and effective: a clinical study.

BACKGROUND: Subacromial decompression, that consists of the release of the coracoid-acromial ligament, subacromial bursectomy and anterior-inferior acromioplasty, has traditionally been performed in the management of this pathology. However, the purpose of subacromial decompression procedure is not clearly explained. Our reaserch aimed to analyse the differences among the outcomes of arthroscopic rotator cuff repair (RCR) made with suture anchors, with or without the subacromial decompression procedure. METHODS: 116 shoulders of 107 patients affected by rotator cuff (RC) tear were treated wit

Potential Utility of SPECT/CT for Early Detection of Scapular Stress Reaction Following Reverse Total Shoulder Arthroplasty.

Scapular stress fracture (SSF) after reverse total shoulder arthroplasty is a unique and common complication that may have negative impact for long-term outcomes. Scapular stress reaction (SSR), characterized by pain and tenderness without radiographic evidence of fracture, can be precursor of SSF. We believe that early detection using SPECT/CT with proper management for SSRs including acromion, scapular spine, and coracoid process is crucial for better prognosis without additional complications by preventing SSFs. Therefore, we present 3 typical cases with SSR following reverse total shoulder

Local pressures in the subacromial space.

We recorded pressures in the subacromial space with various degrees of humeral abduction. The recordings were made during open surgery and under general anaesthesia using a 2-mm-thick piezo-electric pressure transducer. The pressures were recorded in 14 patients with shoulder impingement syndrome (Neer's stage II) and in eight patients with acromioclavicular dislocation serving as controls. The pressures were higher in the impingement group than in the control group. In both groups the highest pressures were recorded antero-laterally under acromion. In patients with impingement syndrome, the p

Risk for Fracture with Acromioclavicular Joint Reconstruction and Strategies for Mitigation.

Acromioclavicular (AC) joint injuries are a common cause of shoulder pain, especially among athletes. Surgical reconstruction of the AC joint can lead to complications such as fracture of the coracoid process, clavicle or acromion, which can negatively affect the patient's outcome. The purpose of this review is to discuss the risk factors for fractures associated with AC joint reconstruction, as well as the strategies that can be used to mitigate this risk. Risk factors for fractures include low mineral density, coracoid/clavicle drilling, larger holes in the coracoid, and the number of tunnel

Shoulder impingement syndrome: radiographic evaluation.

The impingement syndrome is an important source of shoulder pain that occurs when the bony and soft-tissue structures of the superior aspect of the shoulder encroach upon the coracoacromial ligamentous arch during abduction of the arm. Subacromial bursitis, bicipital tendinitis, and rotator cuff disruptions are common sequelae of this abnormality. The presence of bony excrescences arising from the anteroinferior aspect of the acromion and of flattening and sclerosis of the greater tuberosity of the humerus is an important plain radiographic sign of the impingement syndrome. Long tendon of the biceps brachii: sites of predilection for degenerative lesions.

The extreme stress on the long head of the biceps tendon is defined by its specific anatomic course, with its proximity to the rotator cuff, the intertubercular groove and the acromion. We studied the morphologic and cross-sectional anatomy of the long head of the biceps tendon in 104 cadavers to analyze degenerative lesions. Degenerative changes in the tendon (disorganized collagen fibers and large mucoid deposits) occurred mainly in the distal bicipital groove and near the origin of the tendon from the superior part of the glenoid labrum. The results of histologic examination identified site

MRI of trauma and sports-related injuries of tendons and ligaments. Part I: Spine and upper extremities.

MRI is valuable in evaluation of tendons and ligaments in sports-related injuries. Tendons and ligaments show dark signal intensity on all MR sequences. Alteration in size, shape, continuity, and signal intensity of these structures can be well visualized with MRI. Impingement syndrome is the leading cause of atrophy and degeneration of the rotator cuff. Shape of the acromion, presence of subacromial spur, and involvement in certain sport activities that lead to shoulder instability may cause impingement of the rotator cuff. This article systematically reviews the injuries of tendons and ligam

Establishing safe zones to avoid nerve injury in the posterior minimally invasive plate osteosynthesis for humerus fractures: a magnetic resonance imaging study.

BACKGROUND: Safety zones to avoid nerve injury at proximal incision of posterior minimally invasive plate osteosynthesis for humerus fracture have been scarcely studied. The purpose of this study was to describe the location of axillary and radial nerves (RN) in magnetic resonance imaging to establish safety zones. METHODS: Fifty-two magnetic resonance imaging studies of the entire humerus were reviewed. The mean age was 50.6 ± 12.1 years, with 37 female patients. The distance of the axillary nerve (AN; distal portion, humeral midpoint) and RN (medial border, midpoint, and lateral border of t

[Arthroscopic revision of the sub-acromial space in impingement syndrome].

Degenerative lesions of the subacromial structures can cause chronic anterior pain and disability of the shoulder. They are most frequently found in patients whose arms are repeatedly exposed to force while raised above head level, e.g. during tennis, volleyball or swimming. This is often due to impingement of the supraspinatus tendon, the subacromial bursa or the biceps tendon against the anterior edge and lower surface of the anterior third of the acromion, the coracoacromial ligament and, in some cases, the acromioclavicular joint. Open anterior acromioplasty as described by Neer is therefo

Zone-specific acromial cortical thickening after reverse shoulder arthroplasty.

PURPOSE: Acromial and scapular spine fractures after reverse shoulder arthroplasty (RSA) can impair post-operative outcomes; however, quantitative assessment of pre-fracture cortical changes remains challenging. This study evaluated longitudinal changes in inferior acromial cortical thickness after RSA using a computed tomography-based (CT-based) acromial coronal view. METHODS: This multicentre retrospective cohort study included 44 patients who underwent RSA. The acromion was divided into three zones: I (lateral acromion), II (acromial base), and III (scapular spine). Inferior cortical thick
